Hospital participation in community partnerships to improve health.

BACKGROUND: The core business of hospitals now requires, for both competitiveness and quality improvement reasons, that hospitals move beyond their physical and conceptual walls to form community partnerships. THE HOSPITAL'S ROLE AS A PARTNER IN COMMUNITY-BASED HEALTH IMPROVEMENT SYSTEMS: Hospitals, as organizations that are significant health care, social, and economic institutions in their communities, should play a leading role in mobilizing resources for such community-level health improvement efforts. MOVING OUTSIDE THE WALLS TO IMPROVE QUALITY: Three examples of extending hospital efforts into the community demonstrate that improvement of a problem involving hospital care can derive from a collaborative, community-based activity. In Boston, infection control--once a standard, strictly in-house procedure--has been forced by altered patterns of hospital use to become a largely community-based process. In Chicago, a variety of health care providers and community representatives have worked effectively to reduce mortality and morbidity in a single disease (asthma) model. In Akron, Ohio, Lifelink program hospitals, working together with community agencies and groups in a door-to-door neighborhood program, improved the effectiveness of prenatal care and the quality of birth outcomes. CONCLUSION: Efforts to work with community groups to improve health status should not be simply an optional do-good endeavor, as they have often been in the past, but rather an essential part of quality improvement and good business practice. Marketplace incentives will increasingly reward hospitals that are able to form successful community partnerships.

Alkylphenol and alkylphenol-ethoxylates in carp, water, and sediment from the Cuyahoga River, Ohio.

The occurrence of alkylphenol and alkylphenol ethoxylates (APEs) was determined over a 74-mile length of the Cuyahoga River, Ohio. Measurable levels of both the octyl and nonyl forms of these abundantly used nonionic surfactants were observed with the nonylphenol (NP) plus nonylphenol ethoxylates (NPEs) typically accounting for greater than 90% of the total APEs in each sample. For all media (water, fish, and sediment) the total NPE (NP + NPE) concentrations were higher in the more urbanized downstream section of the river. Maximum water and fish values were observed immediately downstream (2.1 miles) from the discharge of the Akron WWTP located 35.31 miles from the river mouth and the sediment maxima occurred at the most downstream site near Cleveland. The ranges in concentration fortotal NPEs and their ethoxylate (EO) makeup were as follows: 32-920 ug/kg wet wt (NP 0 to 2 EO) for carp; 0.13-1.0 ug/L (NP 0 to 3 EO) for water; and 250-1020 ug/kg dry wt (NP 0 to 5 EO) for sediment. When the higher ethoxymers (NP 6 to 17 EO) were added to these sediment totals, the average total estimated NPE concentrations were 1.3-1.8 times higher.

Effects of the administration of Lactobacilli on body growth and on the metabolic profile in growing Maltese goat kids.

The aim of this study was to evaluate the effect of some lactobacilli on body growth and on the metabolic-nutritional status in growing goat kids. Twenty growing Maltese goat kids (10 Control and 10 Bios) were studied. The animals of the Bios group received a concentrate including 1 g x kg(-1) of SEB Bovino (spray-dried), Akron S.r.l., Italy, with non bacterial components: gum arabic, soybean meal, silicate alum of magnesium, and with bacterial components: 10(11) cfu kg(-1) each of Lactobacillus acidophilus, Lactobacillus salivarius, Lactobacillus reuteri. Monthly, bio-metric and weight evaluations were carried out on each animal and individual blood samples were taken. The Bios group showed the highest body weight (Control 19 vs. Bios 23 kg P < 0.001), anamorphosis (Control 71 vs. Bios 78 P < 0.05) and body proportion (Control 35 vs. Bios 41 P < 0.001) indices; the lowest levels of Non Esterified Fatty Acids (Control 0.778 vs. Bios 0.403 mmol L(-1) P < 0.001), triglycerides (Control 0.21 vs. Bios 0.18 mmol L(-1) P < 0.05), urea (Control 8.83 vs. Bios 7.65 mmol L(-1) P < 0.05) and the highest levels of Alkaline Phosphatase (Control 270 vs. Bios 851 U L(-1) P < 0.01) and Creatine Kinase (Control 173 vs. Bios 285 U L(-1) P < 0.01). The results testify to the better metabolic activity of the Bios group which achieved, at the end of the trial (7 months old), about 99% of the morphological development of the adult, therefore an adequate structure for mating and going into production within the first year of life.

The prevalence of thromboembolic events among women with extended bed rest prescribed as part of the treatment for premature labor or preterm premature rupture of membranes.

OBJECTIVE: This study was undertaken to determine the prevalence of thromboembolic events among women with extended bed rest prescribed as part of the treatment of premature labor or preterm premature rupture of membranes. STUDY DESIGN: A retrospective chart review was undertaken of all women who had bed rest of >/=3 days' duration prescribed as part of the treatment of premature labor or preterm premature rupture of membranes in the Akron General Medical Center Perinatal Unit during the period January 1, 1997-December 31, 1998. The prevalence of thromboembolic events in this population was determined. The charts of all additional gravid women with antepartum or postpartum deep vein thrombosis or pulmonary embolism diagnosed during the study period were also reviewed. The prevalence of these disorders among the pregnant population for whom extended bed rest was not prescribed as part of the treatment of premature labor or preterm premature rupture of membranes was also calculated. Statistical comparison of the prevalences in the 2 populations was undertaken by means of the chi(2) analysis with the Fisher exact test. RESULTS: There were 192 patients admitted during the study period who had extended bed rest prescribed as part of the treatment of premature labor or preterm premature rupture of membranes. Three of these women had thromboembolic events, for a prevalence of 15.6 cases per 1000 women. Five additional gravid women were admitted for the treatment of deep vein thrombosis or pulmonary embolism. There were 6164 deliveries among women not treated with extended bed rest for premature labor or preterm premature rupture of membranes during this period. Thus the prevalence of these phenomena among the remaining pregnant women was 0.8 cases per 1000 women. The prevalences of these disorders in the 2 populations were highly significantly different. CONCLUSION: The prevalence of thromboembolic events among women for whom extended bed rest is prescribed as part of the treatment of premature labor or preterm premature rupture of membranes is significantly increased with respect to that among gravid women who do not receive this therapy and is substantially higher than previously reported. If this finding is confirmed in other populations, it may be prudent to undertake further studies to determine whether this prevalence can be reduced.

A metropolitan experience with infrainguinal revascularization. Operative risk and late results in northeastern Ohio.

Despite being of fundamental importance, the late results of major arterial reconstruction rarely have been documented throughout a large metropolitan area. In this study of 932 patients entered into the computer registry of the Cleveland Vascular Society, 19 surgeons representing 13 community hospitals and referral centers in Cleveland and Akron report the intermediate-term outcome during a mean interval of 35 months after infrainguinal lower extremity revascularization performed in northeastern Ohio from 1978 through 1982. Operative risk (5%), the early amputation rate (7%), and actuarial 5-year survival (48% to 55%) for patients with rest pain or tissue necrosis were significantly worse (p less than 0.05) than comparable figures (0.6%, 0%, and 77%, respectively) for others who underwent procedures for disabling claudication. Although both materials had similar success above the knee, the cumulative 3-year patency rate of autogenous vein bypass to the distal popliteal (69% to 88%; p less than 0.05) and tibioperoneal arteries (43%; 0.05 less than p less than 0.1) was superior to the results of polytetrafluoroethylene grafts (32% to 50% and 19%, respectively). Moreover, polytetrafluoroethylene grafts required reoperations at three times the rate of vein grafts to maintain limb salvage.

Reevaluation of benzene exposure for the Pliofilm (rubberworker) cohort (1936-1976).

The Pliofilm cohort is the most intensely studied group of workers chronically exposed to benzene. Information on this cohort has been the basis for regulations and/or guidelines for occupational and environmental exposure to benzene. Rinsky et al. (1986, 1987) and Crump and Allen (1984) developed different approaches for reconstructing the exposure history of each member of the group. The predicted levels of exposure, combined with the data on the incidence of disease, have been used to estimate benzene's carcinogenic potency. In this paper, recent information from worker interviews and historical records from the National Archives and elsewhere were used to evaluate the accuracy of prior exposure estimates and to develop better ones for the cohort. The following factors were accounted for: (1) uptake of benzene due to short-term, high-level exposure to vapors, (2) uptake due to background concentrations in the manufacturing building, (3) uptake due to contact with the skin, (4) morbidity and mortality data on workers in the Pliofilm process, (5) the installation of industrial hygiene engineering controls, (6) extraordinarily long work weeks during the 1940s, (7) data indicating that airborne concentrations of benzene were underestimated due to inaccurate monitoring devices and the lack of adequate field calibration mated due to inaccurate monitoring devices and the lack of adequate field calibration of these devices, and (8) likely effectiveness of respirators and gloves. Our estimates suggest that Crump and Allen (1984) overestimated the exposure of workers in some job classifications and underestimated others, and that Rinsky et al. (1981, 1986) almost certainly underestimated the exposure of nearly all workers. Airborne concentrations of benzene at the St. Marys facility during the years of its operation were found (on average) to be about half those of the two Akron facilities. Our analysis indicates that short-term, high-level exposure to benzene vapors and dermal exposure significantly increased (by about 25-50%) the total absorbed dose of benzene for some workers. One of the key findings was that, unlike prior analyses, the three facilities probably had significantly different airborne concentrations of benzene, especially during the 1940s and 1950s.

Ohio strains of a virus pathogenic for infant mice, Coxsackie group; simultaneous occurrence with poliomyelitis virus in patients with summer grippe.

Further evidence for the widespread occurrence of Coxsackie or C virus is presented in this paper. This virus is characterized by paralysis and myositis produced in infant mice. An epidemic of mild illnesses diagnosed as "non-paralytic poliomyelitis" and as "summer grippe" occurred during the summer of 1947 in Akron and Cincinnati, Ohio. From the pooled feces of such patients both poliomyelitis virus and C virus were obtained. From the samples of single patients, 3 immunologically related strains of C virus were isolated. One patient from whom virus was isolated had an illness which was diagnosed as non-paralytic poliomyelitis (with pleocytosis). Although poliomyelitis virus could not be isolated from this patient, previous tests (5) from similar non-paralytic patients in the same area revealed that 9 of 23 were infected with poliomyelitis virus. C virus could not be recovered from the stools of 3 of these poliomyelitis virus-infected patients. Four patients with an illness diagnosed as "summer grippe" were tested. Two harbored poliomyelitis virus of low virulence (4) as well as C virus, and two harbored poliomyelitis virus without any evidence of infection with C virus, either by virus isolation or by serological tests. The patients from whom C virus was isolated developed complement-fixing antibodies 4 weeks after onset. Neutralizing antibodies appeared within the first 5 days of disease (before complement-fixing antibodies) and increased in titer during the next 3 to 4 weeks. The patients from whom C virus could not be recovered failed to develop either neutralizing or complement-fixing antibodies. Other patients in the area were infected with C virus as indicated by their serological response to the agent.

Intercity spread of gonococci with decreased susceptibility to fluoroquinolones: a unique focus in the United States.

Patients and gonococcal isolates (n = 783) from five sexually transmitted disease clinics in Ohio and western Pennsylvania were studied to investigate the spread of gonococci with decreased fluoroquinolone susceptibility. Among patients with gonorrhea, rates of infection with strains with decreased fluoroquinolone susceptibility (MIC of 0.125-0.25 microg ciprofloxacin/mL) were 20% for Cleveland, 9% for Akron, 7% for Columbus, 1% for Toledo, and 0.5% for Pittsburgh. Persons infected with strains with decreased susceptibility were more likely than those with susceptible strains to be male and older; no significant differences in sex behaviors, residence of sex partners, or recent antibiotic use were detected. Prevalence of decreased susceptibility was not correlated with reported levels of community fluoroquinolone use. The Pro/IB-3 auxotype/serovar class accounted for 80% (44/55) of isolates with decreased susceptibility. Pro/IB-3 isolates from three cities had indistinguishable pulsed-field gel electrophoresis patterns, suggesting intercity spread of a clone.

Evaluation and interference study of hemoglobin A1c measured by turbidimetric inhibition immunoassay.

The technical performance of the turbidimetric immunoinhibition (TI) assay for hemoglobin (Hb) A1c (Tina-quant Hb A1c, Boehringer Mannheim, Indianapolis, Ind) was evaluated by using the BM/Hitachi 911 analyzer. Intra-assay imprecision was less than 2.7%, and interassay imprecision was less than 2.8% as measured by coefficient of variation. In 93 subjects with diabetes who did not have hemoglobin variants, results of the TI assay for Hb A1c correlated strongly with those obtained by using a high-performance liquid chromatography analyzer (Diamat, BioRad Laboratories, Hercules, Calif). Among 241 subjects who had or did not have hemoglobin variants, the TI assay for Hb A1c correlated strongly with results of affinity chromatography for total glycated hemoglobin (Glyc-Affin GHb, IsoLab, Akron, Ohio). We also studied the effect of various percentages of hemoglobin S, C, E, and F on the accuracy of the TI Hb A1c assay. Only high hemoglobin F percentages caused interference. More than 14 times as many samples can be analyzed per hour by using the TI Hb A1c assay than can be analyzed by using the HPLC assay. For high-volume reference laboratories, using the fully automated TI Hb A1c assay to monitor glycemic control in patients with diabetes may be preferable to using the conventional ion-exchange high-performance liquid chromatography Hb A1c assay because the TI assay measures Hb A1c more accurately in patients with diabetes who have hemoglobin variants, and it requires less time.

Histopathologic changes are not specific for diagnosis of gastric antral vascular ectasia (GAVE) syndrome: a review of the pathogenesis and a comparative image analysis morphometric study of GAVE syndrome and gastric hyperplastic polyps.

We studied the nonspecific nature of the histologic findings in the gastric antral vascular ectasia (GAVE) syndrome by using a morphometric comparison with common gastric lesions including hyperplastic polyps and gastritis. Five clinicopathologically confirmed cases of GAVE syndrome and 41 cases of gastric hyperplastic polyps were diagnosed during a 5-year interval at Summa Health Systems (Akron, Ohio). These cases, as well as 16 randomly selected cases of nonspecific gastritis and 9 normal gastric antral biopsy specimens, were evaluated. A semiquantitative comparison of the light microscopic findings believed to be essential in diagnosis of GAVE syndrome, including vascular hyperplasia, mucosal vascular ectasia, intravascular fibrin thrombi, and fibromuscular hyperplasia, was performed. Image analysis morphometric measures of the area ratio (vascular area/total biopsy area), mean vascular area, and number of ectatic vessels per square millimeter of tissue were performed on the CAS 200 Image Analyzer (Becton Dickinson, San Jose, Calif). By morphometric and statistical parametric analysis, several histopathologic variables, including area ratio, mean vascular area, mucosal vascular ectasia, and fibromuscular hyperplasia, did not confidently differentiate the histologic features of gastric hyperplastic polyp from those of GAVE syndrome, but did apparently differentiate GAVE syndrome from gastritis and normal gastric mucosa. The propensity of gastric hyperplastic polyps to undergo prolapse changes and prolapse as one proposed mechanism for development of the GAVE syndrome lesion probably accounts for this morphologic similarity. Specific diagnostic histopathologic changes probably do not exist for the GAVE syndrome.

First branchial cleft cysts presenting as parotid tumors.

Masses of the parotid gland region can be caused by a variety of different conditions, most commonly neoplasms of a benign or malignant nature. A rare cause for parotid tumors is the embryonologic remnant of first branchial cleft cysts. We reviewed branchial cysts for a 10-year period at the three teaching hospitals in Akron, OH. First branchial cleft cysts, or parotid lympho-epithelial cysts, were found to be a very rare condition presenting as parotid tumors. The surgeon should always be aware of these cysts as a possibility in the differential diagnosis of parotid masses in the infant and adult.

Designing interdisciplinary documentation for the continuum of care.

An increased emphasis on integrated care delivery and the need to access information across the care continuum led to an assessment and modification of the current documentation system at Summa Health System in Akron, Ohio. The goal was to achieve more complete and concise interdisciplinary charting. This article describes the process the two-hospital system developed to achieve integrated documentation reflecting the patient's progress toward team-defined outcomes. Steps in the evaluation and modification of the old system, lessons learned, and results/implications for quality improvement are shared.

Airway pressure measurement during high frequency oscillatory ventilation.

We developed a method to measure accurately pressures at the airway opening (Pao) and in the trachea (Ptr) in neonates during high frequency oscillatory ventilation (HFOV) from 15-30 Hz. All component parts of the pressure-measuring system were tested as a unit against a reference transducer in a closed chamber in which sinusoidal pressure waves could be generated. The resulting waveforms were displayed on an oscilloscope and measured. Ptr was measured through the intramural lumen of a Hi-Lo jet tracheal tube (National Catheter Co., Argyle, NY) opening 1 cm above the distal tip. Pressure readings from uncorrected waveforms indicated a discrepancy between measured and actual pressures, as high as 100% at frequencies of 100 Hz. When the resonance of the system was damped with a CorrecTORR (Norton Health Care Products, Akron, OH), the ratio of test to reference transducer output was flat +/- 5% from 0-160 Hz for the Pao system and flat +/- 4% from 0-100 Hz for the Ptr system. Ptr system accuracy was verified in an excised rabbit lung and the systems were used clinically in neonatal HFOV. We conclude that Pao and Ptr can be measured accurately during HFOV by this method.

Clinical nursing experience with cultured epidermal autografts.

In 1989, the Regional Burn Center of the Children's Hospital Medical Center of Akron, Ohio, switched to use of commercially produced cultured epidermal autograft (CEA) from cultured cells that had been grown in the institution's own laboratories. CEA has the advantage of producing the quantity of grafts that are needed from the original biopsy specimen. Clinical experience with 10 patients has demonstrated an average of 72% "take" with approximately 1309 grafts. Arms, legs, and anterior trunk are the preferred sites for coverage. Patient/family education is an integral component of the CEA regimen; a structured educational program begins immediately. The cooperation of both patient and family is often a critical factor in successful grafting. Usually one to two biopsy specimens from the groin or the axilla are sent for culturing. The patient receives standard burn care while cultured tissue is grown, which includes physical therapy and splinting. Early excision, temporary coverage with cadaver skin, topical antibiotics, graft protection, and pain control are keys to success. Physical therapy and splinting are delayed until after removal of the graft backing, which usually occurs 10 to 12 days after CEA is applied.

How to use a gel template for an exact facial flap transfer.

A variety of materials have been used as template materials to aid in the design of local facial flaps. The biggest criticism of these materials is that they do not conform sufficiently to complex defects. This report describes the use of a hydrogel sheet wound dressing (ClearSite, NDM, Akron, Ohio) as a template material. ClearSite appears to make an ideal template material because it is thin, pliable, transparent, and inexpensive. This gel template adequately conforms to irregular three-dimensional shapes. It has the added beneficial ability to "lift" marking pen lines, which can then be transferred as an exact replica of the defect size. A case is presented in which a ClearSite template directed the transfer of the exact amount of forehead tissue following excision of a complex congenital nevus of the nose. Use of the ClearSite template seems well suited to help in local facial flaps and is likely to simplify the design of many distant flaps as well.

Contemporary cardiac rehabilitation: patient characteristics and temporal trends over the past decade.

BACKGROUND: Recent and dramatic therapeutic advances, aggressive care of the elderly, and a heightened awareness of secondary prevention have had a significant, albeit incompletely described, impact on cardiac rehabilitation (CR) programs. The authors did a retrospective analysis on 1,848 patients from their phase II CR program that were treated over the past decade. The authors sought to identify and analyze how advances in cardiovascular care might be related to temporal changes in demographics, medical comorbidities, coronary artery disease risk factors, and cardiovascular history among individuals with cardiac disease who have completed the phase II CR program over a 10-year period. METHODS: Cardiac rehabilitation records were reviewed from 1986 to 1996 at Akron City Hospital. The data were compiled prospectively by nurses and exercise physiologists and were subsequently analyzed. The charts reviewed were from 1,848 patients who completed outpatient phase II CR. RESULTS: The program began in 1986 with 53 patients completing CR and evolved to 309 in 1996. There has been an increase in the number of elderly patients referred to and completing the program. The number of participants older than 65 years of age increased from 28.3% in 1986 to 52.1% in 1996. Cardiac rehabilitation participants reflect the known demographics of patients with clinical coronary artery disease. Men outnumber women and, on average, the female participants are older than the male participants. There has been a statistically significant increase in medical comorbidities over the course of the study. Although the absolute number of patients entering CR after coronary artery bypass graft survey has remained fairly constant, there has been a dramatic increase in the absolute number of patients entering CR after percutaneous tansluminal coronary angioplasty (with the latter recently comprising a majority of CR participants). CONCLUSION: There is a significant trend in the attendance and completion of CR programs by older individuals, which suggests a greater awareness of patients and their physicians. The growth of the program is fueled by high-risk patients with more comorbidities who potentially are the group of patients able to obtain the greatest absolute benefit from CR.

The occupational determinants of chronic disabling pulmonary disease in rubber workers.

Chronic respiratory disease is among the three leading causes of premature retirement due to work disability. The association of occupational exposure and smoking history as potential determinants of pulmonary disability retirement were examined. A cohort of 4302 male, hourly rubber workers actively employed at the Akron plant of a large rubber company on January 1, 1964 was identified and followed for ten years. During the period, 1964-1973, 73 workers terminated gainful employment with a pulmonary disability retirement. Work and smoking histories for these cases and two age-race-sex matched control groups were ascertained. Cases spent significantly greater durations of time employed in curing preparation, curing and finishing and inspection work areas. Each of these areas involves exposures to particulate material and/or solvents. Significant risk of developing a pulmonary disability was associated with smoking and exposure to dust and fumes, generally, and talc and carbon black more specifically. Smoking and occupational exposures were found to interact in their association with pulmonary disability retirement among these rubber workers.

Retrospective comparison of nebulized levalbuterol and albuterol for adverse events in patients with acute airflow obstruction.

The objective of this study was to retrospectively compare the mean change in heart rate (HR) of patients with acute airflow obstruction treated with nebulized levalbuterol vs. albuterol. The study was conducted at the Akron General Medical Center, a 537-bed adult tertiary care teaching and research medical center. The participants were patients (> or = 18 years old) presenting to the emergency department with acute airflow obstruction. This was a retrospective chart review. Treatment groups received either levalbuterol (0.63 mg) or albuterol (2.5 mg). Respiratory care notes record HRs before and after nebulization of levalbuterol or albuterol. Primary analysis was conducted on days 1 and 3 of therapy to determine whether there is a difference between levalbuterol and albuterol with regard to mean change in HR with each treatment. In the primary analysis data, 35 subjects in each treatment group were compared. The mean age (+/- SD) was 65 +/- 16.4 and 68 +/- 16.5 for levalbuterol and albuterol, respectively. On day 1 of therapy, the difference in the mean change in HR with albuterol compared with levalbuterol was 1.0 bpm (95% CI, -1.6 to 3.7). On day 3, a statistically significant difference occurred in mean change in HR between treatment groups at 2.7 bpm (95% CI, 0.02 to 5.4). An increase in HR of 2.7 bpm by albuterol compared with levalbuterol on day 3 of therapy was the only significant finding among the analyses. However, this finding did not demonstrate dangerous elevations in HR following treatment with albuterol. Even the upper end of the confidence interval range at 5.4 bpm does not support a clinically significant difference in tachycardia with the pure isomer compared with the racemic mixture during acute airway obstruction.
