Safety in intraoperative trauma care.

Care of patients with penetrating trauma is risky because of increased prevalence of blood borne disease. Perioperative nurses have constant exposure to body fluids so they are at an increased risk of exposure. Personal safety has emerged as one of the primary workplace concerns of nurses. This report reviews intraoperative safety, Association of Operating Room Nurses and Occupational Safety and Health Administration standards, and universal precautions in practice.

The prevalence of perioperative nurse clinical judgments.

A clinical judgement about a patient situation precedes the selection of appropriate nursing actions and the identification of patient outcomes. The North American Nursing Diagnosis Association nomenclature (i.e., nursing diagnoses) is the accepted language for naming nurse's clinical judgements. Two hundred thirty-nine members of the Association of Operating Room Nurses, Inc, rated the frequency and treatment priority of 60 nursing diagnoses. They rated two diagnostic labels (i.e., risk for perioperative positioning injury, risk for infections occurring in more than 50% of the clinical judgments they make about perioperative patient situations that require immediate nursing action. These data reinforce perioperative nurses' primary role in protecting surgical patients from harm.

Latex sensitivity among perioperative nurses.

To estimate the prevalence, risk factors, and clinical spectrum of adverse reactions to natural rubber latex-containing products among a national sample of perioperative nurses, the investigators administered a self-reporting questionnaire to a convenience sample of 2,200 members of the Association of Operating Room Nurses, Inc. Of the 1,738 nurses who responded, 369 (21%) reported latex reactions. Multivariate analysis identified a history of allergies to cosmetic powders, other allergies, chronic illnesses, and regular use of latex gloves as independent risk factors for reactions to latex-containing products. Localized reactions, such as contact dermatitis, were the reactions reported most commonly, although severe systemic reactions also were described.

Nurses, surgeon not negligent in retained instrument suit. Kissinger v Turner.

Instrument counts are controversial. Though they are recommended on all procedures by the Association of Operating Room Nurses, the majority of hospitals aren't doing them, according to a survey by OR Manager. What happens if you don't have a policy, an instrument is left in, and the case goes to court? What is the role of professional recommendations? The health care law firm of Horty, Springer & Mattern discusses just such a case.

Development of the Conscious Sedation Scale: establishing content validity and reliability.

The purpose of this study was to determine content validity and reliability of the Consciousness Sedation Scale (CSS), an instrument developed as a documentation tool for patient response during invasive procedures using intravenous conscious sedation. Instrument items were developed to reflect goals of conscious sedation recommended by the Association of Operating Room Nurses. Content validity of the parameters and grades was established at Content Validity Index (CVI) .875. Cronbach's coefficient alpha of .69 indicated insufficient reliability of discriminatory items between grades. Further refinement of the tool was indicated.

Pre-preparation of the sterile instrument table for emergency cesarean section.

Cesarean section instrument tables are often not prepared in advance because of concern of contamination risk. The Association of Operating Room Nurses Standards decries the use of pre-preparation of surgical instrument tables because of this risk, although there are no scientific data to support this claim. We evaluated the contamination risk of pre-preparation of surgical instrument tables, prolonged table coverage and table uncovering using a specific technique referred to as the "sardine can roll." Colony counts were positive in only seven of 180 cultures (< or = 15 colonies per plate in each instance) from six tables evaluated after prolonged coverage or uncovering, or both. These data suggest that contamination risk is slight for the uncovering technique described herein and advance table preparation (24 hours or less, never recovered) is a reasonable clinical option in units in which table preparation reduces response time in emergent clinical situations, such as cesarean section for acute fetal distress.

A Project Alpha program for smaller chapters.

Smaller chapters cannot easily adapt to their groups the projects of larger chapters or programs at the national level. To address this problem, the Coosa Valley chapter of AORN, Rome, Ga, developed a program that offers perioperative experience to nursing students at a local college. Members of the chapter act as preceptors for the students and educate them before the students' preceptorships in the OR. This education seems to relieve some of the intimidation and fear related to the OR, and students in the program are more willing to participate in the program.
