[Self-perception and oral health conditions in an elderly population].

This study assessed oral health conditions through clinical examination and self-perception. The sample consisted of 112 subjects 60 years of age or older in Rio Claro, São Paulo, Brazil, divided into two groups: G1, with dental insurance (n = 55) and G2, without dental insurance (n = 57). Clinical examinations were conducted according to WHO guidelines. Data on self-perceived oral health were collected using the GOHAI (Geriatric Oral Health Assessment Index). DMFT was 29.13, higher in G1 (p = 0.0091). Mean number of teeth currently present in both groups was 7.63, higher in G2 (p = 0.0131). Periodontal conditions showed a high percentage of null sextants (70.3%), where CPI revealed higher prevalence for calculus and PIP for pockets up to 3mm. The percentage of edentulous subjects was 45.5%, higher in G1 (p = 0.0142). Among edentulous subjects, 69.6% wore complete upper dentures and 42.9% complete lower dentures. Mean GOHAI was 33.61, qualifying self-perceived oral health status as positive, and the only difference was in the physical dimension, where G2 showed higher values (p = 0.0154). Self-perceived oral health was considered satisfactory, but this was not confirmed by clinical data from the two groups.

A two-year kinesiographic study of mandibular movement patterns in denture wearers.

Changes in functional jaw movements during a 2-year period of denture wear were investigated. The sample consisted of 27 partially edentulous subjects who were provided with an immediate complete maxillary denture and a removable partial mandibular denture. Recordings of habitual chewing of apple and empty open-close cycles were made with a mandibular kinesiograph. At the pretreatment stage, when the patients occluded on a residual anterior dentition, irregular patterns of movement were observed. After placement of the dentures, which restored posterior occlusion, the chewing and open-close patterns showed marked improvement and were characterized by significant mean decreases in sagittal and transversal range of movement. Relining of the maxillary dentures further stabilized the functional patterns. During the second year, significant reductions in vertical range of movement and lateral chewing excursions were observed, and were probably related to impaired retention of the complete upper denture.

The effect of mandibular overdenture to manage trauma to anterior maxillary residual ridge.

Residual alveolar ridge changes brought on by bone loss has always been one of the great problems related to denture prosthesis. The changes beneath denture bases has been investigated and the agreement on individual difference on the rate of bone resorption has been proven. Clinical experience and studies gave great knowledge of the prosthetic factors which influence bone resorption. It has been proved that excessive pressure, and shearing forces are the main causes of ridge changes. Very common is the problem associated with patient having only natural mandibular anterior teeth occluded with complete upper denture. The anterior maxillary region is the weakest area in the upper arch to resist stress when the lower natural anterior teeth occlude anterior to the basal support; trauma is inevitable.

Denture, Complete, Upper↗

[Forms of appearance of decubuti caused by complete upper and lower dentures].

One of the most usual forms of complaint after the insertion of complete dentures is decubitus. It is an ulcer caused by the mechanical harm during the wearing of complete or removable partial dentures. Its size is about that of a lentil. Its shape is usually round or oval but can also be longish in appearance. Its usual colour is grayish white or scarlet. In their research the authors concentrated on the places where these decubiti appear after the insertion of complete dentures. Also, they focused on the question whether new decubiti tend to appear after check-ups and how long the presence of the bedsores last. They took down the particulars of 61 patients (47 women and 14 men) wearing complete upper and lower dentures and of 122 patients with complete upper and lower dentures. The gender and age of the patients and the number of complete upper and lower dentures were also recorded. It is remarkable that 69% of the patients (70% of the women and 64% of the men) appearing at the first check-up turned up at the second one. At the third check-up, 13% of the patients (15% of the women and 7% of the men) showed up. During the first check-up, 87% of the dentures (meaning 106) had to be modified, while during the second one, this rate was 51% (meaning 61 dentures), and during the third one, it was 7% (9 dentures). Thus, the insertion is not the last stage of making complete dentures, for the short-term check-ups are very important phases, as well.

[Application of superplastic titanium alloy for denture base--Part 1. Adaptability of the titanium alloy denture base].

The adaptability of the upper complete denture base made from superplastic alloy 90Ti-6A1-4V was investigated by measuring the space between the master model and the denture base. The denture bases showed a high adaptability with the space less than 0.1 mm, whereas after curing resin for denture, it became worse especially with the heat-curing resin. In addition, the thinnest area of the denture base was observed at the middle of the plate with 38% reduction in thickness, and distortion of the base plate of 0.55mm thickness by curing resin was found to be nearly the same as that of type 304 stainless steel plate of 0.50 mm thickness.

Studies on mechanical properties of denture base material laminated with organic fiber reinforced plastics. Part I. Basic consideration of hybrid construction.

Acrylic resin is widely used as a denture base material, but the upper complete denture fails often during use. Therefore, the denture base made from acrylic resin should be reinforced in some way. Therefore, as a new method of reinforcement on the denture base, the authors devised a method to improve the strength and rigidity by laminating the palatal surface of the denture base with an organic fiber reinforced plastic, the so-called canapé construction. In this paper, as the basic experiment, the flexural strength, fatigue strength and the modulus of elasticity in bending of reinforced acrylic resin by the canapé construction were investigated. As the results, the flexural and fatigue strength are improved respectively 160% and 130% compared to the denture base acrylic resin. And the modulus of elasticity in bending is improved 80%. Therefore, it is considered that this new combination of materials using the canapé construction might be sufficiently applied to the denture base.

Reinforcement of maxillary dentures with silane-treated ultra high modulus polyethylene fibers.

Midline fractures appear to be the most common problem in maxillary complete dentures, and they can be prevented by reinforcement of the base material. In this clinical trial, complete upper dentures made, for patients having a history of midline fractures, that were reinforced with ultra-high modulus polyethylene fiber in woven form. This fiber was treated with a silane-coupling agent and sandwiched between acrylic dough. Laboratory procedures were conducted easily and conventionally, without any special equipment. At the end of 18 months, all of the dentures were well accepted and did not show any signs of fracture.
