The extracted tooth pontic--provisional replacement during bone graft and implant healing.

When tooth extraction is required, a provisional restoration may be utilized as an interim prosthesis during bone graft and implant healing. The selection of provisional replacement of the anterior teeth following extraction may have a direct influence on the success of the definitive tooth replacement. This article describes a technique for using the extracted tooth or a denture tooth as an interim prosthesis during bone graft and implant healing. This method of provisionalization offers several advantages, including no adjacent tooth preparation, natural appearance, and retention of the papillae.

A 2-year electromyographic study of patients with an immediate complete upper and a partial lower denture.

A longitudinal electromyographic (EMG) investigation was performed on thirty subjects provided with an immediate complete upper and a partial lower denture. Electromyographic recordings in maximal clench, light tapping and postural position were obtained from the anterior temporal and masseter muscles before final extractions and 7 weeks, 6 months, 1 year and 2 years after denture insertion. The maximal clench activity of the jaw closing muscles showed an average tendency to decrease upon insertion of the dentures, but thereafter significant increases in mean voltages were observed during the first year of denture wear. During the second year no significant changes in EMG clenching force were noted. At the 2-year stage most of the muscles showed significantly greater clench activity than at the post-insertion stage, but did not exceed the pre-extraction level. The jaw muscle activity in light tapping showed no significant mean changes during the observation period. The postural activity tended to decrease during the period of denture wear, the decrease being significant for some of the muscles at the 2-year stage.

Immediate loading of dental implants in partially edentulous patients: a clinical report.

Immediate loading of dental implants offers attractive advantages for patient and prosthodontist alike. Current data support immediate loading of splinted screw-shaped implants in the parasymphysis region. This report presents a case of a partially edentulous patient successfully restored with a fixed implant prosthesis following an immediate loading protocol.

Immediate aesthetic provisionalization for the dental implant patient.

Providing an acceptable provisional restoration for the patient undergoing tooth extraction in conjunction with bone grafting and dental implant treatment can present a challenge for the restorative dentist. The unnatural appearance and function of the typical temporary is often unacceptable to the patient whose treatment requires long-term temporization. This article describes a technique for using heat-cured acrylics to provide immediate aesthetics in a provisional bridge for such patients.

Immediate prosthetic replacement following major maxillary surgery.

Treatment of the maxillectomy patient must embrace maxillofacial prosthetics at all stages of the patient's treatment, including the preoperative phase. Success can only be assured when full cooperation exists between the specialists concerned. Three case reports are offered as examples in which immediate prosthetic replacement was of considerable benefit, and the advantages of this service are discussed.

Use of stereolithographic models as diagnostic and restorative aids for predictable immediate loading of implants.

Implant dentistry has evolved into one of the most predictable treatment alternatives in all of medical science. Advances in the surgical and prosthetic components, implant designs and surface technologies, and imaging techniques have allowed for significant modifications to occur with respect to one- and two-stage surgical protocols, accelerating treatment times to the benefit of patient and clinician. This article presents a technique to improve surgical and restorative accuracy, allowing for predictable placement and immediate loading of implants through use of CT imaging, stereolithographic models, and CT-derived surgical templates.

Treatment of advanced periodontal destruction with immediately loaded implants and simultaneous bone augmentation: a case report.

BACKGROUND: Advanced periodontal destruction is often associated with extraction of the teeth. Oral rehabilitation in these cases may include the traditional prosthetic restoration or an implant-supported reconstruction. Immediately loaded implants present an alternative treatment modality using a bar in the anterior mandible, along with the placement of higher numbers of implants, which are connected with a fixed prosthetic reconstruction. METHODS: This clinical case report presents the surgical and prosthetic rehabilitation in a patient who had lost all teeth due to advanced periodontal bone destruction. Six implants were placed in the upper and lower jaw each. Some implants required guided bone regeneration in conjunction with autologous bone grafting. All of the implants were connected with their abutments, and a temporary fixed restoration was placed immediately after surgery. The final metalloceramic-fixed reconstruction was cemented after 6 weeks of loading. RESULTS: All of the implants were osseointegrated and showed no clinical signs of mobility or infection. Mobility values were evaluated during healing and were found to be reduced. Radiological findings showed a stable peri-implant bone level during the total 18-month loading observation period. CONCLUSIONS: This case report presents an alternative treatment concept for the oral rehabilitation in a patient with advanced periodontal destruction. The concept of immediate loading of implants might provide a better opportunity to meet patient needs than more traditional treatment modalities.

Immediate/early function of Brånemark System TiUnite implants in fresh extraction sockets in maxillae and posterior mandibles: an 18-month prospective clinical study.

BACKGROUND: The advantages of placing implants in fresh extraction sockets and putting them in immediate/early function are many. A predicable protocol opens the possibility of performing a single surgical procedure, giving the patient a temporary prosthesis immediately, and minimizing the shrinkage of hard tissue and soft tissue recession. PURPOSE: The aim of the present study was to develop a strict protocol for and to evaluate the feasibility of immediate/ early function on implants placed in fresh extraction sockets located in maxillae and posterior mandibles, including defects around the implants treated according to a regenerative procedure. MATERIALS AND METHODS: Nineteen patients were treated after tooth extraction according to an immediate function protocol and were observed for 18 months. Fifty Mk IV TiUnite (Nobel Biocare AB, Göteborg, Sweden) implants were installed in partially edentulous areas in maxillae (n = 17) and posterior mandibles (n = 5). Implants were installed directly into the alveoli, and the temporary prostheses were connected immediately after surgery (n = 11) or within 7 days, that is, an "early function" procedure (n = 11). Thirteen implants did not require any type of regenerative procedure, whereas the remaining 37 implants had filling with autogenous bone, 4 of which also had a resorbable membrane. Standardized intraoral radiographs were taken for evaluation of marginal bone level, and 38 of the implants were systematically checked by resonance frequency analysis. RESULTS: All patients were followed for 18 months, and none of the 50 implants failed. However, one implant showed signs of failure after 6 weeks, but once the occlusal load was removed, the implant regained its stability completely, no longer demonstrated symptoms, and could be used successfully for prosthetic rehabilitation. The mean value of the implant stability quotient was 60 at baseline (range 45-75) and 63 after 6 months (range 46-75). The marginal bone resorption was 0.9 mm (SD 1.1 mm; n = 48) 18 months after implant insertion (1 year after final prosthesis). CONCLUSION: The immediate placement of implants into fresh extraction sockets combined with immediate/early function procedures seems to be a safe and reliable procedure when using a strict protocol.

Bilaminar subepithelial connective tissue grafts for immediate implant placement and provisionalization in the esthetic zone.

Immediate implant placement and provisionalization has been considered as a preservative procedure when replacing failing teeth, especially in the esthetic zone. Nevertheless, an average facial gingival tissue recession of 1 mm is still common after one year of function. Furthermore, facial gingival recession of thin periodontal biotype seems to be more pronounced than that of thick biotype. Biotype conversion around both natural teeth and implants with subepithelial connective tissue graft has been advocated, and the resulting tissues appear to be more resistant to recession. A technique combining subepithelial connective tissue graft and immediate implant placement and provisionalization is devised to achieve a more stable peri-implant tissue in thin biotype situations. This article describes the surgical and prosthodontic approach of this procedure as well as its clinical rationale.

The use of transitional implants for immediate fixed temporary prostheses in cases of implant restorations.

While the original Brånemark implant protocol has continued to evolve, the avoidance of implant loading during osseointegration remains a prerequisite with all implant systems. Immediately loaded transitional implants have recently been developed to support the fabrication of a fixed provisional prosthesis that provides implant patients with improved aesthetics and function during the osseointegration period. In this manner, osseointegration can occur free from prosthetic and transmucosal loads. This article describes the use of transitional implants and presents a classification of three different case types.

Using irreversible hydrocolloid to evaluate preparations and fabricate temporary immediate provisional restorations.

Two unique techniques that use irreversible hydrocolloid and an Omnivac matrix have been presented. The first technique is used to evaluate the adequacy and uniformity of the tooth preparation. The second technique is used to create a TIPR for the patient while the PIPR is being fabricated. The TIPR-PIPR system can be used to facilitate proper diagnosis, planning, and treatment; and it results in improved patient comfort, dentist efficiency, and increased control over the final restoration.
