[An epidemiological study on erythromelalgia].

Erythromelalgia occurred in middle schools of many counties in Hubei province from February to March. A survey in 2 senior middle schools revealed that the total incidence of the disease was 13.1% (248/1892) the incidence of female was 30.7% (138/449) and that of male 7.9 (110/1443). The average course of the illness was 6.56 +/- 4.3 days. The study of meteorologic phenomenon, biological agents, nutrient etc shows that the factors of cold and humidity might play a leading role.

[Clinical evaluation of the control of antiplatelet therapy through platelet aggregation rate on polycythemia vera associated with thrombocytosis and erythromelalgia].

A 60-years-old woman with polycythemia vera with marked thrombocytosis and intolerable erythromelalgia was presented. A single dose of 400 mg aspirin was effective to improve the pain and cyanosis. And we studied the relationship between platelet aggregation rate and symptoms after administration of several antiplatelet drugs. A single dose of 100, 200, 400 and 800 mg aspirin, 25 mg indomethacin (Id), 200 mg OKY-046, and daily dose of 300 and 600 mg dipyridamole (Dp) and 300 mg ticlopidine (Tc) were given. Aspirin, Id and OKY-046 were effective for the improvement of finger pain. The complete inhibition of spontaneous aggregation (SPA) and aggregation by 2.0 micrograms/ml of collagen were well parallel with the improvement of symptoms. But duration of effect of LKY-046 were only 6 hours. Dp and Tc were not effective for the improvement of pain, had no relation with platelet aggregation rate. The concentration level of aspirin in vivo which suppresses the platelet aggregation induced by SPA and 2.0 micrograms/ml of collagen coincided well with the concentration level of this drug which suppresses the same platelet aggregation in vitro. It seems to be useful to suppress the platelet aggregation induced by SPA and 2.0 micrograms/ml of collagen with aspirin and Id for controlling the platelet aggregation induced circulatory disturbance in patient with thrombocytosis.

Erythromelalgia: case report and literature review.

Erythromelalgia, known also as erythalgia and erythermalgia, is a rare condition causing redness and burning sensation in the extremities, usually the lower. It may be primary or idiopathic, or secondary in association with other diseases, usually myeloproliferative disorders. The present report describes a patient in whom the condition was associated with astrocytoma, after having been misdiagnosed. Because of its frequent association with posture, its differential diagnosis is important to physiatrists.

Erythromelalgia.

Erythromelalgia is an extraordinary disorder of unknown etiology and pathophysiology that resembles the post-traumatic reflex dystrophy syndromes but has not been described previously in the orthopedic literature. Its distinctive triad of intense burning extremity pain associated with erythema and increased skin temperature are diagnostic. Primary or idiopathic and a secondary or associated form have been identified. The latter occurs in association with an underlying disease process, especially myeloproliferative disorders. Treatment with pharmacologic agents and surgery are ineffective except in the secondary group where treatment of the associated disorder generally results in a remission. Symptoms in the primary group can be minimized by appropriate environmental control with cooling and avoiding heat-producing situations that would raise skin temperature above a critical thermal threshold.

[The surgical treatment of erythromelalgia by the destruction of the inlet areas of the posterior spinal nerve roots].

The paper presents a 13-year-old patient suffering from erythromelalgia in whom arm pain and burning ceased for 3 years after unilateral stereotactic cryodestruction of the thalamic ventrolateral nuclei. Following a relapse, reoperation on the other side turned out to be ineffective. Then the patient underwent destruction of the inlet areas of the spinal dorsal roots by breaking the cervical pain-conducting tracts. Pain and other autonomic manifestations of the disease came to an end. The authors give an anatomic and neurophysiological rationale for surgery.

Adenosine for pain relief in a patient with intractable secondary erythromelalgia.

An unusual case is reported with severe erythromelalgia secondary to a sensorimotor polyneuropathy of immunological aetiology. The dominating symptoms were ongoing burning dysesthesia and pain in the legs, sustained thermal hyperalgesia and allodynia to pressure which produced intolerable pain on standing and walking. The primary pain-producing pathophysiology was apparently peripheral neurogenic inflammation with sensitization and excitation of nociceptors. The variable and progressive course prompted reassessments and successively amended multitargeted analgesic regimens. The most intensive bout of widespread pain and allodynia indicating secondary central sensitization was only controlled by adenosine treatment.
