Hyperlipemia and myocardial infarction in polycythemia vera.

Hyperlipemia was found in 46 of 118 polycythemia vera patients and in 20 of a control group of 115 healthy subjects. Myocardial infarction in the polycythemia vera group occurred in 14 of the 46 hyperlipemic and in 17 of the remaining 72 nonhyperlipemic patients. Repeated phlebotomies may induce hyperlipemia; therefore, this form of treatment may be potentially dangerous in polycythemia vera patients who are already hyperlipemic.

Lovastatin versus bezafibrate for hyperlipemia treatment after heart transplantation.

BACKGROUND: Elevation in total and low-density lipoprotein cholesterol levels and a decrease in high-density lipoprotein cholesterol plasma concentrations are common in heart transplant recipients. The pathogenesis of this hyperlipemia after heart transplantation is complex. Currently available antilipemic agents are difficult to use because their adverse effects are potentiated by immunosuppressor treatment. The present investigation was carried out to test the safety and efficacy of lovastatin and bezafibrate in 18 patients with hyperlipemia after heart transplantation. METHODS: In this crossover study, after 3 months of dietary recommendations, the subjects were randomly assigned to an 8-week period of lovastatin treatment (10 mg/day) followed by an additional 8-week period of treatment with bezafibrate (400 mg/day) or vice versa. The two treatments were separated by an 8-week washout period. RESULTS: Both drugs reduced total and low-density lipoprotein cholesterol and apoprotein B concentrations. High-density lipoprotein cholesterol was only increased with bezafibrate. The total cholesterol/high-density lipoprotein cholesterol and low-density lipoprotein cholesterol/high-density lipoprotein cholesterol ratios were decreased under both treatments, but these changes were greater with bezafibrate. Apo AI levels increased with lovastatin. Bezafibrate produced a rise in high-density lipoprotein cholesterol and reduced total and very low-density lipoprotein triglycerides and very low-density lipoprotein cholesterol. Both drugs decreased intermediate density lipoprotein cholesterol and triglyceride levels, but the effect of bezafibrate on intermediate-density lipoprotein triglycerides was significantly greater. The two drugs were well tolerated and liver enzymes, creatine kinase, and renal function remained stable.

Effect of hyperlipemia on pulmonary diffusing capacity at rest and during exercise.

Since existing data suggest decreased pulmonary gas exchange at rest when hyperlipemic, the effect of alimentary hyperlipemia on steady-state pulmonary diffusing capacity for carbon monoxide (DLco) at rest and exercise was investigated. Twelve men were measured on two mornings after a 12-hr fast. One trial was performed with subjects in the fasting state, and the other three hours after ingestion of one gm fat per kg body weight. At each trial, venous blood was drawn at 0800 and again three hours later just before resting DLco measurement. The DLco was subsequently determined at a work intensity on the cycle ergometer of 49 watts, and then at an increased intensity sufficient to raise the heart rate to 150 bpm. Serum was examined spectrophotometrically to assess degree of lipemia as reflected by optical density. The DLco and serum optical density data were analyzed by ANOVA. Lipemia was significantly increased three hours after fat ingestion (p < 0.05). Although mean DLco, in ml/min/mmHg was lower following fat ingestion at both intensities of exercise (49W = 37.1 fasted; 34.5 fed: 150 bpm = 49.8 fasted; 44.6 fed), alimentary hyperlipemia did not significantly affect resting or exercise DLco.

[Anti-hyperlipemia and anti-oxidation effects of xue zhi ling].

92 cases with complete clinical data among 125 hyperlipemia patients were randomly divided into two groups: 51 cases in the Xue Zhi Ling treatment group and 41 cases in the control group with medication of panagin. The drugs were administered to all patients for 12 weeks and the blood lipid was then examined at the 4th, 8th, 12th week after medication respectively. In treatment group, there was the effect of lowering TC, TC-HDL/HDL and raising HDL at the 4th week (P < 0.05). However, there were no significant difference in above-mentioned parameters at the 4th, 8th and 12th week respectively. The experiment also showed that Xue Zhi Ling could reduce TG at the 8th and 12th week (P < 0.05). While there was no significant difference between that of the 8th and 12th weeks. The mean reduction of TC, TG and TC-HLD/HDL were 18.7%, 19.5% and 27.6% respectively, while the elevation of HDL in average was 17.4%. All of the lipid indexes in control group had no significant changes at any stage. In addition, it was shown that Xue Zhi Ling could decrease serum LPO at 12th week (P < 0.05). The results indicated that the Xue Zhi Ling has the effect of regulating the hyperlipemia and anti-oxidation.

[A study from Navarra. Hyperlipidemia V. What is the best definition of hyperlipemia in childhood and adolescence?].

As part of an epidemiological study on cardiovascular risk factors among children and adolescents in Navarra, lipids and lipoproteins were analyzed in 5,829 children of both sexes, between 4-17 years of age, and selected at random from the school population in our community. In this article, we analyze the different definitions for lipid risk during childhood, whether based on percentile values, according to age and sex of the child, of cholesterol, LDL/cholesterol, or risk quotients (C/HDL, LDL/HDL), or even on the absolute values of all of these parameters. An appropriate definition for hyperlipemia during childhood, once we know the average variations in the levels of lipids and lipoproteins according to age and sex, as well as the variations of the lipid risk prevalence according to its definition, would be: 1. Previous screening according to cholesterol serum levels: Values higher than the 70th percentile for each group according to age and sex: or higher than 185 mg/dl for children age 4 to 12 and 170 mg/dl for children age 13 to 17. 2. To calculate the LDL/HDL quotient among those selected children included in the definition of hyperlipemia when the quotient is higher than the 85th percentile for the patients age and sex, or it is higher than 2.2.

[Epidemiological study on the association between oral contraceptive and female hyperlipemia and hypertension].

This paper analyzed the mean of serum lipid and blood pressure, prevalence rates and risks of hyperlipemia and hypertension in 999 healthy rural women who had been taking Chinese No. 1 contraceptive pill for 5-25 years. Data was compared with those in 1 065 control subjects wearing IUDs in a series of cohort studies. The results showed that the level of serum triglycerides (TG), total cholesterol (TC), high density lipoprotein (HDL) and hypertriglyceridemia prevalence rate after long-term administration of contraceptive pill were significantly higher than that in control subjects (P < 0.01), which hypercholesterolemia prevalence rate did not increase significantly. The mean of systolic pressure and diastolic pressure in taking-pill group were 6.57 mmHg and 3.24 mmHg (lmmH = 0.133322kPa) respectively, higher than that of control group, but still maintained in the normal range. The prevalence rates of systolic hypertension and diastolic hypertension were higher than that of control group (P < 0.01). The results of multiple regression analysis suggested that long-term administration of Chinese No. 1 contraceptive pill had no marked influence on female hyperlipemia and hypertension after dispelling the effects caused by confounding factors.

The successful treatment of a case of primary sterility resulting from Fredrickson's Type V hyperlipemia and panhypopituitarism.

Successful treatment of primary sterility in a woman having the rare association of panhypopituitarism with Fredrickson's Type V hyperlipemia is described. Replacement therapy with l-thyroxine, prednisone and a low fat diet cleared the patient's blood of the excessive chylomicrons and very low density lipoproteins. Ovulation was induced with human gonadotrophins, and triplets (two normal girls and a boy) were born.

Effect of the essential oils of garlic and onion on alimentary hyperlipemia.

SUMMARY: The effect of garlic and onion on alimentary hyperlipemia, induced by feeding 100 g butter, has been studied in 10 healthy subjects. The freshly extracted juice of 50 g of garlic or onion, as well as an equivalent amount of their ether-extracted essential oils, was administered randomly on four different days during a one-week period. Garlic and onion have a significant protective action against fat-induced increases in serum cholesterol and plasma fibrinogen and decreases in coagulation time and fibrinolytic activity. The essential oil fraction, which contains all the taste and odour, exactly duplicated the beneficial effects of whole garlic and onion. It is, therefore, concluded that the active principle of garlic and onion is the essential oil, which chemically is a combination of sulphur-containing compounds, mainly allyl propyl disulphide and diallyl disulphide.

Comparative effect of clofibrate, garlic and onion on alimentary hyperlipemia.

The effect of clofibrate on the same subjects in similar test conditions were used as a control to verify the alleged beneficial effects from garlic and onion on alimentary hyperlipemia in normals and in cases with ischemic heart disease. The results showed that clofibrate checked the fat-induced (a) rises of serum triglyceride and plasma fibrinogen, and (b) falls of coagulation time (CT) and blood fibrinolytic activity (BFA). Only garlic had a clofibrate-like effect on CT but both garlic and onion checked the postprandial fall of BFA. Clofibrate, however, increased BFA even above the fasting level. Serum cholesterol and beta-lipoprotein were not appreciably affected by fat with or without any drug. Thus, surprisingly, the so-called beneficial effects of garlic and onion were not seen in subjects who had shown significant changes after clofibrate.

Plasma lipoproteins and cholesterol metabolism in Yoshida rats: an animal model of spontaneous hyperlipemia.

The purpose of this study was to characterize the lipoprotein profile and cholesterol metabolism in Yoshida rats, a strain of inbred genetically hyperlipemic animals. For comparison, Brown Norway rats were used as control animals. Plasma cholesterol and triglycerides were higher in Yoshida as compared to Brown Norway, the elevation of cholesterol being due to a rise in HDL fraction. Triglyceride distribution among lipoproteins showed an increase in VLDL fraction. Hyperlipemia was not related to diabetes, hypothyroidism or nephropathy. Plasma triglycerides production was increased in Yoshida rats, while lipoprotein and hepatic lipases were similar in the two groups. Hypercholesterolemia was associated with a defect of lipoprotein receptor activity and with elevated HMG-CoA reductase and cholesterol 7 alpha - hydroxylase; conversely ACAT activity was lower in Yoshida as compared to Brown Norway rats. Sterol fecal excretion was comparable in the two groups and hypercholesterolemia in Yoshida rats was not associated to an increase of cholesterol saturation of the bile. We suggest that lipoprotein overproduction is the main cause for hyperlipidemia in this strain of rats.

Gender differences in the development of hyperlipemia and atherosclerosis in hybrid hamsters.

In response to a diet enriched in saturated fat and cholesterol (CH), male Syrian hamsters develop hyperlipemia and changes of early atherosclerosis. However, it has not been determined if female hamsters are equally susceptible to an atherogenic diet. Male and female hamsters of the F1B hybrid strain (Bio Breeders, Fitchburg, MA) were fed either a chow diet or this diet (HiFat) with added saturated fat (10% coconut oil) and CH (0.05%) for up to 12 weeks. Female hamsters ate significantly more than males, and with the HiFat diet gained threefold more weight than males. However, with the HiFat diet, serum triglycerides (TGs) and CH were markedly increased only in male hamsters. Furthermore, only in males was there a significant increase in stainable fat in the aorta that corresponded to an increase in subintimal foam cells. In freely feeding males, the largest percentage increase in serum CH was in the TG-rich fraction of lipoproteins. After females were castrated, serum TG and CH levels increased to the same extent as in males. These studies demonstrate a profound gender difference in response to an atherogenic diet in these hamsters that has parallels to the lipid patterns of humans and their susceptibility to atherosclerosis.

Apolipoprotein C-II promoter T-->A substitution at position -190 affects on the transcription of the gene and its relationship to hyperlipemia.

A Chinese patient with severe hypertriglyceridemia was found to have similar clinical features to that of malignant hyperlipemia in infancy. DNA sequence analysis of the apoC-II gene from the patient's parents revealed a novel heterozygous mutation of T-->A substitution at position -190 base in the apoC-II promoter. We speculated that the patient was a homozygote of the same mutation that resulted in the deficiency of apoC-II. In vitro expression studies showed T-->A substitution in the apoC-II promoter leads to a decrease by approximately 20% in transcriptional activity compared with its counterpart that inserted the normal promoter. These results suggested that T-->A substitution at position -190 in the apoC-II gene promoter only partly affected transcriptional activity of the apoC-II promoter, leading to decrease of apoC-II expression in quantity.

A procedure for inducing sustained hyperlipemia in rats by administration of a surfactant.

Tyloxapol (Triton WR 1339) is a non-ionic detergent that inhibits lipoprotein lipase and thereby raises levels of serum lipids. It is used frequently for acute studies on lipids in rats but not for subacute or chronic studies. In the present work, we found that tyloxapol must be injected intravenously three times each week in order to have high and sustained levels of serum cholesterol and triglycerides for 1, 2, or 3 weeks. These results make it possible to extend the use of tyloxapol into chronic studies of hyperlipemia and vascular disease.

The pathogenesis of hyperlipemia induced by means of surface-active agents. II. Failure of exchange of cholesterol between the plasma and the liver in rabbits given triton WR 1339.

Rabbits subjected to subtotal hepatectomy failed to develop increased serum cholesterol levels following parenteral injection of triton WR 1339, the finding indicating that the liver is essential for the establishment of the hypercholesterolemia induced by surface-active agents. The cholesterol content of the livers of rabbits rendered hyperlipemic by means of triton remained unchanged both during the rapid rise of the serum cholesterol levels and during the return to normal values. By contrast, the cholesterol content of the livers of rabbits fed cholesterol rose progressively over a period of 5 weeks, concommittant with the increase in serum cholesterol levels. The findings provide support for the hypothesis that surface-active agents bring about hyperlipemia by altering the circulating lipoproteins in some manner so that they are retained in the circulating body fluids.

Triton hyperlipemia in dogs. I. In vitro effects of the detergent on serum lipoproteins and chylomcrons.

Triton WR-1339, a non-ionic detergent, added to canine serum or to ultracentrifugally separated lipoproteins, induced changes in the lipoproteins which were dependent upon concentration of detergent and class of lipoproteins. D 1.063 to 1.21 lipoprotein (alpha-LP) was especially sensitive to the action of triton. Addition of 2 mg. of triton to 1 mg. of alpha-LP (based on protein content), induced only slight changes in the electrophoretic and flotation characteristics of the lipoprotein. With a tenfold increase of the detergent (triton:alpha-LP, 20:1), the mixture, analyzed by starch gel and paper electrophoresis, yielded a tritonlipid complex which remained close to the origin, and a nearly lipid-free protein with electrophoretic mobility higher (starch gel) or lower (paper) than native alpha-LP. The splitting of the lipid and protein moieties of alpha-LP could not be clearly shown when the same mixture was analyzed by free boundary electrophoresis. Triton alone moved only slightly in an electrical field (paper, starch gel, Tiselius); it sedimented during ultracentrifugation at D 1.006 and D 1.063 and floated at D 1.21. D 1.006 to 1.063 lipoproteins (beta-LP), required larger amounts of triton to show changes. These were evident in 40 to 80:1 mixtures of triton and beta-LP. In starch gel and paper electrophoresis triton retained, in a position close to the origin, part of the lipids of beta-LP; the remaining beta-LP fraction, impoverished of lipids, had electrophoretic mobility similar to native beta-LP. The triton-lipid complex sedimented at D 1.063. After addition of triton to complexes [chylomicron-alpha-P-I(131)] or [lipomul-alpha-LP-1(131)], the electrophoretic and ultracentrifugal analyses of these mixtures revealed that the labeled protein was removed from the triglyceride component. Triton also prevented the occurrence of the interaction between lipomul and alpha-LP and the hydrolysis of both chylomicrons and lipomul-alpha-LP by lipoprotein lipase. It is postulated that, if the changes in lipoproteins and chylomicrons observed in vitro occur in vivo, they could account, at least in part, for the hyperlipemia which develops in animals following administration of triton.

Incidence of familial hyperlipemia.

Familial hyperlipemia is an inherited disease associated with early onset of coronary atherosclerosis. In a survey of a student population in Sweden, an estimated case incidence of 2 to 3 percent was discovered. This study also demonstrates that there is probably a heterogeneity of causes for the primary elevation of blood triglycerides in man.

Primary carcinoma of the colon and hyperlipemia: a paraneoplastic syndrome.

The human counterpart of the association of hyperlipidemia with cancer is rare, as compared with the relative frequency of the syndrome in experimental animals. A case is presented of adenocarcinoma of the colon with a non casual relationship between the presence and the progression of the tumor and hyperlipemia. Surgical resection and antiblastic chemotherapy moreover seemed to influence the metabolic anomaly.

[The prevalence of hypertension, diabetes mellitus and hyperlipemia among 1369 subjects for physical examination in Changsha].

A survey of the prevalence of hypertension (HT) and diabetes mellitus (DM) and hyperlipemia (HL) was conducted among 1369 subjects for health check in Changsha. The results showed that the prevalence of HT and DM and impaired glucose tolerance (IGT) and HL were 17.2% and 4.6% and 5.9% and 27.4%, respectively. The prevalence of HT and DM and IGT increased with age. The higher the body mass index was, the higher the prevalence of HT and DM and HL was. These findings indicate that the aging and over weight are important risk factors to the prevalence of HT, DM and HL.
