The medical laboratory scientist in the clinical chemistry service laboratory.

The recent opening of an official register for medical laboratory scientists in South Africa has prompted an examination of the professional role, training and qualification of one particular group of scientists, namely clinical chemists working in hospital pathology departments. Lack of recognition of the potential contribution of these non-medical graduates towards improved health care, together with the lack of facilities for their professional advancement, has hitherto inhibited the growth and development of clinical chemistry in this country. An urgent need is the local establishment of a specialist postgraduate qualification open to the non-medical clinical chemist.

The regulation of medical laboratories in Australia.

Australia has had a system of compulsory accreditation and inspection of medical laboratories since 1986. This article details the regulations that have evolved over the years and the specific requirements of this system. The importance of separation of powers of various authorities, including standards, accreditation, payment, inspection, quality assurance, and education, is emphasized.

The role of the medical laboratory technologist in drinking and driving cases. Part 2: The use of hospital alcohol results as evidence and providing testimony in court.

Medical laboratory technologists routinely conduct alcohol analyses for medical purposes, however, in certain circumstances the results are used legally to determine a driver's Blood Alcohol Concentration. The technologist who performed the analysis may subsequently be required to provide evidence in court. The primary areas of interest to the court in these cases are: the type of swab utilized, the continuity of the blood sample, the method of analysis, the margin of error of the results, and the conversion of a serum alcohol concentration to a blood alcohol concentration in units as per the Criminal Code. Pre-trial preparation, courtroom procedures, and suggestions to enhance the technologist's credibility as a professional witness are outlined.

Medical laboratory audit: performance of twelve selected laboratories in metropolitan Enugu, Nigeria.

The abilities of 12 medical diagnostic laboratories situated in Enugu metropolis to estimate haemoglobin concentration accurately and precisely, by the cyanmethaemoglobin photometric method, were assessed. Nine (75%) of these laboratories showed good precision. Three (25%) were imprecise. As assessed by variance index (VI), only 5 (8.3%) of the entire haemoglobin results obtained from the participating laboratories showed good excellent accuracy (VI = 0 < or = 0.5). Conversely, 16 (26.7%), 9 (15%) and 30 (50%) of the entire haemoglobin results were satisfactory (VI > 0.5 - 1), acceptable (VI > 1 - 2) and rejectable (VI > 2) respectively. Furthermore, only 4 (33.3%) of the laboratories produced haemoglobin results that were both accurate and precise. Non-compliance with desirable practices that ensure quality of laboratory determinations were observed as possible contributing factor to this rather poor performance. The latter underscores the need for institution of external quality control laboratories in Nigeria.

Health and safety in medical laboratories.

There has been a large increase in the number of persons employed in medical laboratories in the last 25 years. These workers are exposed to a variety of infective agents in the course of their work, the most important being Mycobacterium tuberculosis, Salmonella typhi, Brucella spp., and serum hepatitis virus. Chemical and physical hazards include toxic chemicals, lacerations, skin disease, and possibly cancer. Current knowledge of safe working practice in laboratories leaves much to be desired and there is an urgent need for both internationally agreed codes of safe practice and the development of guidelines for the medical surveillance of laboratory workers. The World Health Organization is developing such guidelines in an attempt to protect the health of workers employed in the investigation of ill health in others.

Objective structured practical examination in pharmacology for medical laboratory technicians.

The objective structured practical examination (OSPE) is a useful evaluation method for testing psychomotor skills. Students for the degree in medical laboratory technology require to learn certain skills which will make them useful in any research or teaching laboratory in experimental pharmacology. We outline an OSPE which can be used for evaluating students in experimental pharmacology.
