An intraspinal extradural foreign-body granuloma following surgical removal of herniated intervertebral disc is demonstrated by myelography, spinal computed tomography and transabdominal spinal sonography. The radiologic findings and differential diagnoses are discussed.
Lumbar surgery is currently benefiting from the development of anterior approach to the spine and the possibilities of body reconstruction by plate and bony graft. The anterior extraperitoneal approach enables access to the spine. The Fraser approach expose the lumbar and the lumbo sacral spine.
Demonstrating the clinical, sonographic and radiologic findings in three pediatric patients suffering from caudal spinal dysraphism the various diagnostic and therapeutic problems of neural tube defects are discussed. Important conclusions are as follows: (1) Early diagnosed and operated malformations of the caudal spine may cause neurologic, neurosurgical, urogenital, gastrointestinal and orthopedic complications even after uneventful years. Mostly the reason is a prevented ascensus of the conus medullaris. Therefore a longstanding follow-up of these patients is mandatory. (2) The patient's examiners have to look for cutaneous manifestations of spinal dysraphism even in the sacral region to avoid secondary damage. (3) Sonography today is a valuable diagnostic method not only applied for screening examinations but also to establish definite diagnoses in many cases. The application of other methods is therefore guided by sonographic findings.
Experience of treatment of 84 patients with radicular syndrome of lumbo-sacral localization revealed the efficacy of multifactorial effect on different structures of the nervous system responsible for generation and realization of pain impulsation. Treatment was carried out with consideration of pathogenetic factors forming the pain syndrome, namely, compression of the root, functional blocks, spinal reactive aseptic epiduritis, arterial dyscirculation, disorders of venous outflow, psychogenic factors.
In a female patient aged 68 years an extensive basal cell carcinoma with a proliferating tumour in its central part was observed. In view of the age of the patient, the extent and location of the lesion, and refusal to undergo surgical treatment combined treatment was used, with radiotherapy (5900 R, 70-45 KV, 10 mA) and compresses of 5-fluorouracil. The tumour became flat and epithelialization developed.
The authors report 4 cases of multiple tumors of the lumbo-sacral region with, on histology, a mixture of basal cell carcinomas and fibro-epithelial Pinkus tumours. They were struck by the fact that three of these four patients had received, in this region, radiotherapy numerous years previously. They discuss the role of this radiotherapy in the onset of the disease and the relative importance of this factor and of constitutional factors (Hamartomas). They recall the characteristics of multiple basal cell carcinomas and of Pinkus fibro-epithelial tumours, together with the close relationship between these two types of tumour.
The paper deals with the problem of pain in L-S segment of the vertebral column in Pelvic Congestion Syndrome-Pcs. The study covers two groups of women. The studied group comprised 63 seamstresses being employed in piece-work system, requiring great concentration of their attention and precision in producing the elements. That was accompanied by monotony and hypokinesis. The control group was made up of 22 women working in the same material-labour conditions, but in non-compulsory position. The analysis was performed by resorting to specially elaborated method of gynaecological examination elements of ergonomic studies and those of biomechanics. It has been shown that hypokinesis and piece-work claiming far advanced precision, as well as monotomy of the performed activity lead, in consequence, to congestion of organs in the small pelvis. In the investigated material, sacrodynia and hypogastric pain have been found to appear as the main symptoms involving women with Pcs. The material in both groups was selected by eliminating other causes likely to give rise to pain in lumbosacral segment of the vertebral column.
