[Hematologic characteristics of leishmaniasis].

INTRODUCTION: Leishmaniasis is a chronic infectious disease from the group of anthropozoonoses. It is caused by protozoa in the genus leishmania flagellate. There are five major foci of this disease in the world: India, Mediterranean countries, East Africa, South China and South America. Endemic regions in the Balkans are as follows: Montenegro, Macedonia, Herzegovina and the Morava's valley (1,2). Reservoirs of infection are infected humans and animals (dogs and rodents). Infection is transmitted by insects the most significant representative of which is a sandfly. The course of the disease may be acute, subacute and chronic and several forms are differentiated such as visceral, cutaneous and mucocutaneous. Signs of disease are elevated temperature, gastrointestinal disorders, splenomegaly and hepatomegaly and rarely generalized lymphadenomegaly. Laboratory findings point to pancytopenia. The diagnosis is established on the basis of parasitological findings in macrophages of the bone marrow and is confirmed by serologic tests (4,5). However, mortality is decreased to 5% after the application of 5-valent antimony and amphotericin B (6,7). CASE REPORT: A female patient aged 19 year from Novi Sad was admitted at the Clinic of Hematology due to unclear febrile state lasting 3 months accompanied by pancytopenia and enlarged spleen. The first discomforts were experienced in the second half of August in 1997 upon the patient's return from Sutomore. The disease started gradually with uncharacteristic manifestations. Firstly, discomforts developed in the region of the gastrointestinal tract and were characterized by loss of appetite, nausea, and vomiting in addition to drastic weight loss. Secondly, fatigue occurred during effort, later on at rest as well, accompanied by increased body temperature. Temperature increased twice a day and was followed by shuddering, fever, shivering and very often by nocturnal sweating. Antibiotics and antipyretics were used, but without fall of temperature. Subjective discomforts were increasingly pronounced, so that due to unclear febrile state and in addition to the present pancytopenia the patient was referred to hospital treatment and was therefore admitted at the Clinic of Hematology. Febrile state, tachycardia, a striking paleness of the skin and visible mucosa as well as splenomegaly were confirmed. Basic laboratory findings (Table 1) pointed to pancytopenia. Apart from anemia and mild thrombocytopenia, leukopenia with neutropenia, lymphocytosis and monocytosis were pronounced. Sternal puncture was the most significant diagnostic procedure on the basis of which the diagnosis was established. In hypocellular bone marrow in macrophages, both intra- and extracellular, protozoa were found in smaller and greater groups which resembled leishmaniasis (Figure 1). The diagnosis was confirmed by serologic tests to leishmaniasis, antibody titre was > 1:32. In order to exclude other infections and hematologic diseases, other diagnostic tests were performed (Table 1) and the findings were normal. After the establishment of diagnosis the patient was transferred to the Clinic of Infectious Diseases where the causal therapy with 5-valent antimony was introduced, parenterally. As early as the first week of therapy, the patient was afebrile, subjective discomforts disappeared, she regained appetite and put on weight. Also, the decrease of the spleen was observed as well as improvement in hematological findings. DISCUSSION AND CONCLUSION: This paper predominantly deals with hematological findings which are characteristic for leishmaniasis on the basis of which the diagnosis of this disease has been quickly established (1,2). Hematological findings were the key of diagnosis being confirmed by serologic and other tests. The disease is very rare in this region, so that there is a difficulty in recognizing it. In our case, the diagnosis was made on the basis of sternal puncture survey, because protozoa were found in hy

Bone marrow scintigraphy in hemopoietic depletion states.

Bone marrow scintigraphy was performed in 29 patients with hemopoietic depletion states of various etiology. Two tracers were used for visualization of the marrow: 99mTc-sulphur-colloid and 111InCl3, some patients being examined by both indicators. 111InCl3 is bound on transferrin and is adsorbed on the surface of reticulocytes and erythroblasts). The scintillation camera PHO GAMMA SEARLE IV fitted with moving table and computer CLINCOM was used, so that whole body images were obtained. The comparison of all scans and marrow puncture smears was done. In patients with aplastic anemia with both hyperplastic or hypoplastic marrow good correlation of bone marrow scans and sternal puncture smears was found. In several cases the scintigraphic examination helped to establish a diagnosis of marrow depletion state. A peculiar disadvantage of the imaging method with either 99mTc-sulphur-colloid or 111InCl3 is that it shows the disorders in erythropoietic and reticuloendothelial cells, whereas the defects in myelopoietic cell series and platelet precursors are not provable. According to literature data, great attention is paid to the prognostic value of scintigraphic examination in aplastic anemia.

[Amniocentesis under permanent sonographic surveillance].

81 amniocenteses were carried out during the second trimenon for the purpose of prenatal diagnosis under permanent sonographic surveillance without any complications. Sonographic surveillance means that a sectorial scanner was used (RA1 and Combison 202 R) by the real-time process with a 110 degrees or 60 degrees sectorial field soundhead for closely observing the movements of the amniocentesis needle already in the subcutis. The progress of the needle through the individual layers can be observed continuously. As a matter of fact, the gradual progress of the needle from the extra-amnial to the intra-amnial area can be followed step by step and millimetre by millimetre. If the site of puncture is occupied by the foetus, the procedure can be stopped or interrupted. Depth of penetration of the needle into the amnion can be limited to a few millimetres. This puncture technique will usually eliminate anyl risk of injury to the foetus. Any danger of infection which may be attributed to the needle touching the soundhead or contact jelly, is also eliminated since the amniocentesis needle (sternal puncture needle 0.8 X 90 mm, preferably CSF puncture needle 20 G 3 1/2") is entered into the cutis at a distance of 8 cm from the soundhead. Hence, the only risk involved in amniocentesis which cannot be predicted in advance, would be haemorrhage due to injury to a vessel, mainly in the myometrium or, if transplacental puncture becomes necessary, a possible formation of a haematoma leading to abortion. This possible complication, however, had not been observed by us so far.

Treatment of hemorrhagic shock with intraosseous or intravenous infusion of hypertonic saline dextran solution.

The efficacy of intravenous or intraosseous infusion of 250 ml of 7.5% NaCl and 6% dextran 60 (H/H) was compared with intravenous Ringer's lactate (RL) for the initial treatment of patients with hemorrhagic shock due to upper gastrointestinal bleeding. 49 patients were randomly assigned to receive either H/H (n = 26) or RL (n = 23). In the first 16 patients with H/H and in all RL patients, solutions were infused by the intravenous route, while the intraosseous route through sternal puncture was chosen for the last 10 H/H subjects. H/H patients were analyzed together since no differences were noticed between the routes of infusion. The H/H group also received 2.3 +/- 0.7 liters of intravenous crystalloid solutions in the first hour and 4.4 +/- 0.1 liters in the 24-hour period, while RL received 3.3 +/- 0.7 and 7.3 +/- 2.4 liters, respectively. Blood pressure (BP) increased during the first 15 min in the H/H group (from 61 +/- 17/30 +/- 12 to 85 +/- 30/48 +/- 14 mm Hg) and thereafter, while remaining unchanged in the RL group (from 75 +/- 18/40 +/- 12 to 75 +/- 17/40 +/- 14 mm Hg; p less than 0.05). The differences between groups were significant throughout 24 h. Urine output and improvement of the Glasgow Coma Score were also higher in H/H patients than in the control group (p less than 0.05). There were 5 deaths in RL group and 1 in the H/H group. Sternal of peripheral vein infusion of 250 ml of 7.5% NaCl/6% dextran 60 is an effective initial treatment of hemorrhagic shock.

Porphobilinogen-synthase (delta-aminolevulinic acid dehydratase) deficiency in bone marrow cells of two patients with porphobilinogen-synthase defect acute porphyria.

Two male patients aged 23 and 25 years with intermittent acute, frequently repeated porphyria syndromes presented an almost total deficiency of porphobilinogen-synthase [(PBG-S); synonym: delta-aminolevulinic acid dehydratase] in peripheral erythrocytes. PBG-S was investigated in bone marrow cells obtained by sternal puncture. A minimal enzyme activity of less than 3% of controls was established. Specific activity and protoporphyrin concentration decreased considerably during the course of erythropoiesis. Both patients are homozygous gene carriers; their parents (father and mother) as well as most of their brothers and sisters are heterozygotes with a PBG-S deficiency of approximately 50% of controls. All people with PBG-S deficiency are especially endangered by alcohol intake and lead exposure, because alcohol and lead toxically inhibit PBG-S.

Solitary plasmacytoma of the spine: relationship of IGM to tumour progression and recurrence.

The authors report a retrospective study of 15 patients with solitary vertebral plasmacytoma. 15 patients were considered in this study on the basis of the following characteristics: 1) histologically confirmed plasmacytoma following surgical removal; 2) existence of a single vertebral lesion, documented by skeletal and MRI scan; 3) no signs, at diagnosis of disseminated disease by blood laboratory test, urine analysis, sternal puncture, iliac bone marrow biopsy, a total-body CT scan. The clinical course of the patients has been analysed on the basis of the following factors: age, sex, length of clinical history before diagnosis, site, presence/absence of the M component. The M component is an electrophoretically homogeneous immunoglobin. The most significant factors for predicting development of multiple myeloma proved to be the presence /absence of the M component at diagnosis and, to a lesser degree, the age of the patient. In the light of other reports too, it would seem that the presence of the M component at diagnosis is a reflection of aggressive biological and clinical tumour behaviour.

Experimental pancytopenia of the peripheral blood with granulocytopoietic bone marrow hyperplasia in the beagle-dog.

During subacute and chronic toxicity studies on beagle dogs with the platelet aggregation inhibitor UH-A 46 XX, a suboxicam derivative, a pancytopenia of the peripheral blood with granulocytopoietic bone marrow hyperplasia was induced. Our observations are based on the results of a 13 week routine experiment (control group and dosage groups 5-, 50- and 700 mg/kg per day UH-A 46 XX) and on a special open-ended reversibility study (control group and dosage groups 50- and 700 mg/kg). The reversibility study included closely monitored hematological controls, bone-marrow cytology from sternal punctures and plastic section histology of iliac crest biopsies. During this study, plasma levels of the substance were estimated. The reproducible syndrome described was similar in the dogs in both studies. As early as the 8th week, but mostly between the 9th and 12th experimental weeks, one third of the animals treated orally with 700 mg/kg b.wt. UH-A 46 XX per day developed an acute fever (10 out of 30 animals). Four died several days later. Dogs treated with 5 or 50 mg/kg UH-A 46 XX remained unaffected. In the diseased dogs in both studies, proven pancytopenia was associated with the occurrence of numerous hematopoietic, especially granulocytopoietic precursors. The hyperplastic bone marrow showed a massive increase in granulocytopoiesis with considerable maturation arrest. These processes were accompanied by suppression or reduction of erythropoiesis. In addition, histological findings in the animals that died showed extramedullary hematopoiesis in different organs. During the second study, the reversibility was investigated in sick animals. If substance administration was terminated when the illness had reached its peak, normalization occurred quickly within 2 to 3 weeks. The animals remained healthy until the end of the long term follow-up which varied from 9 to 10 months. The pathogenesis of pancytopenia caused by UH-A 46 XX seems to be a chemically induced hyperdestruction of the peripheral blood cells. This appraisal is supported by the clear degenerative alterations in the granulocytic cells of the peripheral blood. An additional inhibitory effect of UH-A 46 XX on cellular maturation processes of hematopoiesis can be assumed.

Minor dysplastic changes are frequently observed in the bone marrow aspirate in elderly patients without haematological disease.

Bone marrow aspirates were obtained by sternal puncture prior to sternotomy in 54 volunteers (40 males and 14 females) aged 60 years or more. All underwent surgery for cardiological diseases and had normal blood counts, without any haematological abnormalities. Quantitative examination of these bone marrow aspirates yielded reference ranges for each cell type similar to those obtained in younger adults. However, qualitative analysis revealed certain discrepancies: dysplastic changes were observed frequently, mainly in megakaryocytes and erythroblasts, with a normal growth pattern of haematopoietic progenitor cells. A low proportion of macrophages and mast cells were noted in 30% of the bone marrow aspirates. Lymphoid aggregates, seen in 13% of these samples, were generally of moderate size, contained few mast cells and were non-clonal on immunophenotypic analysis.

[False diagnosis of "osteoporosis" in diffuse multiple myeloma (author's transl)].

Multiple myeloma is very frequently diagnosed too late. In ten patients in whom the diagnosis was ultimately made in the course of one year, the early symptom of back or skeletal pain was at first most frequently misdiagnosed as osteoporosis and so treated. Such symptoms as loss of weight, increased sedimentation rate and increase in gamma-globulins should have led to further tests, for example sternal puncture, radiological survey of the entire skeleton, and immuno-electrophoresis.

[Visceral leishmaniasis (kala-azar), an important disease not only in the tropics (author's transl)].

A 39-year-old woman contracted visceral leishmaniasis on a 14-day visit to southern Italy and died of staphylococcal septicaemia before specific treatment could be effective, diagnostic efforts over four months having failed to make the diagnosis until four weeks before death. Eight further cases reported in German-speaking medical journals illustrate similar diagnostic difficulties. The disease is accompanied by septic temperatures, frequently with two daily spikes, hepato- and splenomegaly, normochromic anaemia and leucocytopenia. But diagnosis can only be made by specific tests. The organisms can be discovered in sternal puncture and organ biopsies only if the Giemsa stain is used. Complement-fixation reaction for leishmania should be tried repeatedly, in case the disease is suspected.

[Recurrent iritis as the initial symptom in plasmacytoma (IgG type)].

The author presents a case report on a 47-year-old man with recurrent serofibrinous iritis. As a result of intensive investigations an IgG-type myeloma was finally diagnosed with the aid of serum electrophoresis, immunoelectrophoresis, and sternal puncture. Apart from ocular symptoms of acute iritis all findings were normal. General clinical findings were also normal and the patient was subjective free of complaints.

Flow cytometric analysis of the DNA content of bovine and human bone marrow cells.

The defence against infection in high-yielding dairy cows is correlated with the number and function of circulating neutrophils and depends on their production in bone marrow. Therefore, the DNA content of isolated bone marrow cell suspensions from 7 calves, 7 cows and 14 humans was assayed by flow cytometry. Bovine sternal bone marrow samples were collected within 30 min of death, and human marrow samples were collected by sternal puncture and aspiration. Mononucleated cells were isolated by gradient centrifugation. In the bone marrow samples from calves and cows, 35 +/- 2.6% and 31.8 +/- 1.5% of the isolated bone marrow cells respectively were in the S/G2/M-phase. The difference between calves and cows was not significant. In the human samples, only 12 +/- 0.8% of the cells were in the S/G2/M-phase. A significant (P < 0.001) difference was observed between the two species. These results indicated that the proliferative, in activity of haematopoietic cells is significantly higher in cattle than in humans.

A new surgical method for repair of funnel chest.

We have devised a new surgical procedure for the repair of funnel chest deformity, which we have used in 148 of our 154 patients. The procedure involves resection of the deformed thoracic wall, including ribs and sternum, rib-sternal turnover, and anastomosis of the internal thoracic artery and veins. Several preoperative and postoperative examinations, such as bone biopsy, sternal puncture and indium chloride test, revealed definite signs of living tissue. This procedure results in minimal secondary deformity postoperatively and is indicated for adults as well as children.
