[Preliminary study on relationship between syndrome differentiation of esophageal cancer and changes of exfoliated cells of tongue coating].

Exfoliated cells of tongue coating of 63 esophageal cancer (EC) cases have been investigated by applying exfoliate cytologic examination, which showed that abnormal cornified cells were commonly seen while they were less in the patients of EC than those in the healthy group, and their difference was significant (P < 0.05), It showed an increase mainly in the number of pro-cornified cells in the patient of the Qi-Yang Deficiency group, an increase mainly in completely cornified cells in the Yin Deficiency and an increase in both pro-cornified cells and completely cornified cells in Phlegn-Dampness and blood Stasis patients. The difference among four groups was significant (P < 0.05). The changes of exfoliated cells of tongue coating provided an objective index for the early diagnosis of EC.

[Relation of left and right axillary temperature with Yin-Yang and its guiding action on diagnosis and treatment of acupuncture and moxibustion].

Body temperature is one of the four physical signs and clinically, the axillary temperature is the most commonly used as the index of body temperature. The study on relation of the difference of the left and right axillary temperature with constitution types and diagnosis of diseases proves that the temperature of the left and right axillary is correlated with excess and insufficiency of Yin and Yang, namely, higher left axillary temperature is recognized as Yin-deficiency and Yang-excess, and higher right axillary temperature as Yang-deficiency and Yin-excess, and this can guide clinical syndrome differentiation treatment of acupuncture and moxibustion and individual diagnostic and therapeutic program can be established based on this.

[Cell-mediated immunity in chronic pyelonephritis].

The authors used monoclonal antibody (CD3, CD4, CD8) and indirect immunofluorescence technic to study peripheral blood lymphocyte surface markers in patients with chronic pyelonephritis (CPN) and in normal controls. In CPN patients, a significant decrease in CD3, a lower percentage of CD4 and significant higher percentage of CD8 leading to a decrease in the CD4/CD8 ratio were noted. No differences could be observed between Spleen-Kidney Yang deficiency and Kidney Yin deficiency. The overall picture shown by the CPN patients was compatible to a cell-mediated immune response defection. The CPN patients were then treated with Zi-Ling capsule (ZLC)/transfer factor (TF) and various antibiotics for three months, during which period the authors found a significant increase in CD3, CD4, but no significant change in CD8. Symptoms such as lumbago, fatigue, frequency of urination were improved. A significant decreased in CD4 after cessation of treatment with ZLC and TF was observed. These observations suggested that both ZLC and TF could also improve the immune function, however, they mainly affect CD4 but not CD8 cells, and the effect lasted for only a short period. In conclusion, ZLC/TF and antibiotics administered together would serve as a useful therapeutic measure to be recommended for patients with CPN.

[Determination of blood flow of the lower leg in patients with diabetes mellitus and the effects of treatment with the principle of vitalizing blood and solubilizing thrombus].

This paper reports the determination of blood flow of the lower leg in 50 cases of non-insulin dependent diabetes mellitus (NIDDM) using an XLJ-2 Bipolar Rheoencephalometry Impedance Rheogram. In patients with leg pain (but without obvious vascular pathological changes, 85 legs) the blood flow was decreased. In male patients the blood flow of the left leg (9 legs) was 3.28 +/- 0.47 ml/100ml.tissue.min (mean +/- S), while that in the right leg (11 legs) was 3.88 +/- 0.80; in females, the blood flow of the left leg (32 legs) was 2.72 +/- 0.8; while that in the right leg (33 legs) was 2.94 +/- 0.66. These figures were significantly (P less than 0.01-0.001) lower than those obtained from normals. In diabetic feet (15 painful legs) the decrease of blood flow of the lower leg was more apparent: it averaged 1.87 +/- 0.79 for the left leg (7 legs) and 2.66 +/- 0.87 for the right leg (5 legs) in male patients. The values were significantly different when compared with those of normals (P less than 0.001) or with those of the diabetic patients with leg pain (P less than 0.05). These results demonstrated that determination of the blood flow of the lower leg of diabetic patients might aid in early discovery of the abnormal changes of blood supply to the lower legs in diabetes mellitus and judge the degree of ischemia. Of the 50 cases of diabetics 32 were Qi-Yin deficiency with blood stasis while the remaining 18 cases were deficiency of both Yin and Yang with blood stasis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Syndrome differentiation in traditional Chinese medicine in chronic aplastic anemia; T-lymphocyte subpopulation in peripheral blood and erythrocyte C3b receptors].

UNLABELLED: In this paper, the Syndrome Differentiation in TCM and the determination on T-lymphocyte subpopulation and RBC C3b receptor were carried out by using indirect SPA assay and RBC C3b receptor ring test among 51 chronic aplastic anemia patients. RESULT: The OKT3, OKT4 and OKT4/OKT8 ratio in groups of Kidney-Yin Deficiency group were lower, while the OKT8 was higher than that of controls. In the group of both Kidney-Yin and Yang Deficiency, OKT4 and OKT4/OKT8 ratio were lower. and the value of OKT8 is higher than that of controls. The OKT8 in the Kidney-Yang Deficiency group was higher. and the ratio of OKT4/OKT8 was lower than that of controls. The OKT8 in the Kidney-Yin Deficiency group among three groups increased higher than that of other two groups. while the OKT4/OKT8 ratio reduced. The OKT4/OKT8 ratio in the group of both Kidney-Yin and Yang Deficiency was higher than that of Kidney-Yang Deficiency group. Among the three groups, the rosette rates of RBC C3b receptor were lower than controls. The above-mentioned data were statistically significant. These characteristic changes could be used as a reference of microcosmic Syndrome Differentiation and also a guide for how to apply the immunosuppressant and immunopotentiator.

[Treatment of 100 children with infantile nephrotic syndrome by integrated traditional Chinese medicine and Western medicine].

For ten years (1979-1989), 150 cases of infantile nephrotic syndrome were treated by integrated TCM and Western medicine (WM). The WM group (50 cases) was treated by conventional WM drugs (prednisone, CLB, etc) and the TCM-WM group (100 cases) was treated by the above-mentioned WM plus treatment according to Syndrome Differentiation with decoction of Chinese medicinal herbs. Wulin San, Zhenwu Tang and Zhibai Dihuang Tang were selected in treating Spleen-Qi Deficiency distressed by Dampness, both Spleen-Kidney Yang Deficiency and both Liver-Kidney Yin Deficiency Syndrome respectively. The total effective rate was 100%, and the efficacy of TCM-WM group was higher than that of WM group (P < 0.005). The efficacy of TCM-WM treatment of nephritic nephropathy was far better than that of simple nephropathy (P < 0.05). Also the efficacy of TCM-WM was superior in raising plasma albumin and reducing blood cholesterol than that of WM (P < 0.001). The cellular immunity function and plasma cortisol of TCM-WM group were significantly improved than that of WM, P < 0.01 and 0.001 respectively. The result showed that the curative effect of the TCM-WM group in treating nephrotic syndrome was much better than that of WM alone.

[Analysis on the prognostic factors in patients with large hepatocarcinoma treated by shentao ruangan pill and hydroxycamptothecine].

OBJECTIVE: To observe the effect of intervention therapy with Shentao Ruangan pill (SRP) and hydroxycamptothecine (HCPT) in treating 85 patients with middle-advanced large hepatocarcinoma, and to analyze the factors that could affect the prognosis. METHODS: Eighty-five patients were randomly divided into the treated group (n = 52) and the control group (n = 33). The treated group was treated by oral taking of SRP combined with local perfusion of HCPT through hepatic artery catheterization, while to the control group, the conventional therapy, transcatheter arterial chemoembolization (TACE) was conducted for control. The clinical efficacy of treatment in the two groups was evaluated by the change of tumor size, the factors related with prognosis were analyzed using Cox proportional hazards model and the analysis of survival conducted by Kaplan-Meier method. RESULTS: (1) The tumor size reducing rate in the treated group was 19.2% and the tumor size stabilizing rate was 82.7%, while those in the control group was 21.2% and 81.8% respectively, comparison of the criteria between the two groups showed insignificant difference (P > 0.05); (2) The median survival time, 0.5- year, 1- year and 2- year survival rate in the treated group was 326 days, 80.95%, 41.39% and 12.42% respectively, those in the control group was 262 days, 64.29%, 25.00% and 8.33% respectively, comparison between the two groups showed significant difference (P < 0.05); (3) Among the 3 TCM types in patients, the survival time and rates in patients of Gan-excess with Pi-deficiency type was similar to those in patients of Gan-heat with blood stasis type showing insignificant difference (P > 0.05), but as compared with those in patients of Gan-Shen Yin-deficiency type, the difference was significant (P < 0.05) ; (4) Beneficial factor to the prognosis were therapeutic method, that used in the treated group was superior to that used in the control group. The risk factors to the prognosis were TCM type, clinical stage and liver function. Patients of Gan-excess with Pi-deficiency type had the optimal prognosis, those of Gan-heat with blood stasis type the next and of Gan-Shen Yin-deficiency the worst. The later the clinical stage and the worse the Child-Pugh grade of liver function was, the worse the prognosis would be. CONCLUSION: (1) SRP combined with HCPT intervention treatment is superior to the simple TACE treatment in elevating patients' survival rate and time; (2) There are some relations between TCM types and prognosis; (3) Local Chinese drug therapy combined with systemic therapy could be one of the effective measures of non-operational therapy in treating large hepatocarcinoma.

[Characteristics of traditional Chinese medicine syndromes in both spontaneous and variant angina. An analysis of 21 cases].

An analysis of TCM syndromes is reported in 21 cases with both spontaneous and variant angina as compared with 147 cases with effort angina. The results showed that 3 characteristic features were present, which were as follows: the Biao-Shi syndrome of cold condensation was more than that of the control group in ratio of 42.86% to 3.40%; the Ben-Xu syndrome of Yang deficiency was more and that of Qi deficiency less than those in the control group, and they were in ratio of 33.33% to 6.12% and 33.33% to 72.11% respectively. An absolute reduction of blood supply resulted from coronary spontaneous spasm in both spontaneous and variant angina causes severe chest pain during attacks as a cold condensation type. Hyperfunction of parasympathetic nerves often occurring in coronary heart disease with Yang deficiency is liable to vasoconstriction of the large coronary arteries leading to episodes of both spontaneous and variant angina. The presence of less Qi deficiency type may be related to the less impairment of cardiac function resulted from the short course in these cases and only relatively mild state of an illness, even no marked lesion in coronary arteries in a part of patients with both spontaneous and variant angina. No significant difference in TCM syndromes occurred between spontaneous and variant angina. Both Yang and Yin deficiency, as the Ben-Xu syndromes, were more present in angina of cold condensation type.(ABSTRACT TRUNCATED AT 250 WORDS)

The liver in traditional Chinese medicine.

Medical thinkers in China visualized the liver in microcosmal and macrocosmal terms. An anatomical tradition did not exist, hence the liver was described grossly in broad outline. It was recognized as being functionally important in the movement of qi (vital energy) and storage of xue ('blood'). The liver corresponded to various phenomena in both the natural and social orders, according to the scheme of yin yang and five phases. These interrelationships provided the basis for the diagnosis and treatment of liver dysfunctions. The disorders fell into three general groups: (i) hepatic qi stasis; (ii) hepatic yang excess with yin deficiency; and (iii) hepatic yin insufficiency. The signs and symptoms represented the logical outcomes of the disturbed physiology. Acupuncture, moxibustion and herbal drugs were used to redress the imbalance of hepatic qi and yin-yang. The impact of Western medicine led traditional authors to recognize the hepatobiliary role in bile secretion and in jaundice. The exchange between the Western and Chinese medical traditions revealed that active agents were included in the Chinese formulary, such as glycyrrhizin, which has recently been shown to be beneficial in chronic viral hepatitis.

[Clinical study on therapeutic effects of treatment according to syndrome differentiation of traditional Chinese medicine combined with captopril on severe viral myocarditis complicated heart failure].

OBJECTIVE: To observe the therapeutic effect and mechanism of the treatment according to Syndrome Differentiation of TCM combined with captopril (CAP) on severe viral myocarditis (SVM) complicated heart failure (CHF). METHODS: One hundred and nine patients of SVM complicated CHF were randomly divided into the treated group (n = 72) and the control group (n = 37), the former was treated with TCM combined with CAP, while the latter was treated with dexamethasone and interferon. The TCM prescriptions were made depending on types of diseases by Syndrome Differentiation, i.e. Heart-Spleen deficiency type, Qi-Yin deficiency type and Spleen-Kidney Yang deficiency type. The efficacy of treatment was evaluated by the criteria including NYHA classification, myocardial enzymology, electrocardiogram, cardiac function and motorial toleration measured before and after treatment. RESULTS: The therapeutic effect of the treated group according to NYHA classification was obviously better than that of the control group; the creatine phosphokinase isoenzyme (CPK-MB), aspartate transaminase (AST), lactate dehydrogenase (LDH) content lowered in both groups, but more significantly lowered in the treated group than in the control group (P < 0.05, P < 0.01); the improvement of S-T segment of ECG in the treated group was better than that in the control (P < 0.01); also some parameters of heart function and motorial toleration were bettered in the treated group more significantly (P < 0.01). CONCLUSION: TCM treatment according to Syndrome Differentiation combined with CAP in treating SVM complicated CHF could elevate the clinical efficacy.
