[Occurence of M. africanum in the Federal Republic of Germany (author's transl)].

In the last years mycobacteria have been isolated in the Federal Republic of Germany identical or similar to "M. africanum". The identification of the species remains uncertain, but nevertheless these mycobacteria belongs to the group of tubercle bacilli and they are different from M. Tuberculosis and from M. bovis. It seems that these mycobacteria have not been isolated yet from white patients in other European countries.

[Medicodemographic problems in Russian Federation].

The authors of the article analyze the current health status of Russian population and underline the necessity to set priorities in the development of the country's public health system. Within a short period of time the mortality in this country has risen 1.5 times, and the birth rate have declined twice. By January 1, 2005, the population of Russian Federation had decreased by 10,361,300 people, as a result of natural depopulation. Mostly affected by the crisis is able-bodied population, the mortality among who have increased 2.5 times. The gap in the mortality between ours and other economically developed countries is now sevenfold. This means that six out of seven cases of death at the productive age in this country are absolutely preventable. The mortality among pensioners has not changed at all, i.e. people at this age seem to be reliably protected against the crisis. The birth rate has decreased abruptly; its summary coefficient has fallen down to 1.17 while 2.15 are necessary for simple reproduction. The proportion of out-of-wedlock children, as well as that of disabled children, displays a multiple increase. Male population in Russia demonstrates a catastrophic super mortality, the gap between the length of life among men and women being 13.5 years. In a number of regions men do not reach the age of 54.

[The effectiveness and ultilization of screening cytology for cervical cancer in the Federal Republic of Germany (author's transl)].

The mortality from invasive carcinoma of the uterine cervix in the Federal Republic of Germany decreased from 1970 to 1977 in the age group 35 to age 59, between 19% and 28% in an age standarized study. A correlation with the introduction of the universal screening cytology in 1971 is highly probable. The utilization of the screening program in the age group of 30 to 60 years is 40--50% of the eligible women. At 2 year intervals the utilization is 55%. The utilization be senior citizens is only 25%. The number of women screened in the most important age groups is higher than usually quoted in the German literature.

[Organizational technologies and management of the organs and institutions of the State Sanitary and Epidemiological Surveillance in the Russian Federation].

The authors offer a scientifically-based rationale for the system of management of the State Sanitary and Epidemiological Service and its institutions under modern socioeconomic conditions in Russia. Organizational models of the structure of State Sanitary and Epidemiological Surveillance Centers are offered and their functions determined for various levels of management. A concept of development and organizational structure of the service and its institutions is proposed, aimed at provision of sanitary and epidemiological well-being in the Russian Federation.

Nevada State plan; final approval determination. Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. Final State plan approval--Nevada.

This document amends OSHA's regulations to reflect the Assistant Secretary's decision granting final approval to the Nevada State plan. As a result of this affirmative determination under section 18(e) of the Occupational Safety and Health Act of 1970, Federal OSHA's standards and enforcement authority no longer apply to occupational safety and health issues covered by the Nevada plan, and authority for Federal concurrent jurisdiction is relinquished. Federal enforcement jurisdiction is retained over any private sector maritime employment, private sector employers on Indian land, and any contractors or subcontractors on any Federal establishment where the land is exclusive Federal jurisdiction. Federal jurisdiction remains in effect with respect to Federal government employers and employees. Federal OSHA will also retain authority for coverage of the United States Postal Service (USPS), including USPS employees, contract employees, and contractor-operated facilities engaged in USPS mail operations.

[Forensic medical evaluation of flaws in medical care rendered at public health institutions in the Russian Federation].

The authors analyze the flaws in medical care rendered at various public health institutions by the results of 2436 expert committee conclusions, verified at bureaus of forensic medical expert evaluations. The essence and causes of these defects at the hospital and prehospital stages of medical care, rendered by physicians of different specializations and nurses, are analyzed.

[Disability and rehabilitation problems in Russian Federation].

The authors considered theoretic topics, including terminology aspects, of disablement and rehabilitation. The results are up-to-date approaches and measures helping to solve disablement problem that remains among priorities in the state social policies.

[Prospects for development of functional diagnosis in antituberculosis institutions in the Russian Federation].

The author proposes a programme for improving the functionally diagnostic service of antituberculosis institutions, which envisages studies of major spirographic parameters, the forced expiratory flow-volume curve, blood gases and acid-alkali balance in all antituberculosis institutions, global plethysmography, examinations of the diffuse capacity of the lung and spiroergometry in the regional, republican dispensaries and research institutes of tuberculosis and phthisiopulmonology. Studies of lung function are considered to be a mandatory part of clinical examination of patients with tuberculosis in the diagnosis of lung disease, during treatment and after its completion.

[Alimentary iodine deficiency in the Federal Republic of Germany (author's transl)].

Urinary iodine excretion of 1945 schoolchildren aged 13 to 15-years from 24 cities throughout the Federal Republic of Germany (FRG) was measured. For comparison, urinary excretion of 5678 adults from 20 cities was calculated from the 24-hour radio-iodine uptake (RIU) by the method of Oddie. On the average the schoolchildren excreted 25.1 mug iodine per g creatinine: by WHO criteria this can be classified as a grade II iodine deficiency. There was a significant decline of urinary iodine excretion from North to South of the FRG. Urinary iodine excretion of children with thyroid enlargement (21.9 mug/g) was significantly less than that of normal children (26.1 mug/g creatinine; P less th that 0.0005). Urinary iodine excretion of adults averaged 25 to 35 mug/g creatinine, values calculated from RIU agreeing well with those determined chemically. Alimentary iodine intake was found to vary between 30 and 70 mug/d throughout the FRG, which is less than 30% of WHO recommended figures for optimal goitre prevention (150-200 mug iodine per day). Alimentary iodine deficiency has been demonstrated throughout the FRG, increasing from North to South in parallel with an increase in goitre rate. Goitre prevention through compulsory iodinization of salt is recommended.

[Survey on the gerontological research in the Federal Republic of Germany (author's transl)].

This survey on the gerontological research shows the topical main accents of work in this field in the Federal Republic of Germany: Social services for aged, old-age-security, social and health situation, and older workers' problems are the most important items of the essay. Empirical examinations are dealt with separately. Moreover, there is a prospect on the different types of projects, research institutions and where they are distributed in the different areas and the different ways of financing this research.

Federal Claims Collection Act; claims collection and compromise--HCFA. Final rule with comment period.

These regulations implement the Federal Claims Collection Act, which provides authority to Federal agencies for collecting or compromising claims, or suspending or terminating collection action, as appropriate. This authority can be exercised only through agency regulations that conform to the regulations on claims collection (4 CFR Parts 101-105) issued jointly by the Comptroller General and the Attorney General. We intend these regulations to enable HCF to exercise full claims collection authority for all HCF programs, as delegated by the Secretary of HHS.

[Resistance of Haemophilus influenzae to antibiotics in the Federal Republic of Germany (author's transl)].

In a multi-center study 523 strains of H. influenzae and H. parainfluenzae isolated from patients in eight towns of the Federal Republic of Germany were tested for their sensitivity to 15 different chemotherapeutic agents. 1.7% of the strains were resistant to ampicillin, 1,5% to chloramphenicol and 2.5% to tetracycline. All ampicillin-resistant strains produced beta-lactamase. For the first time, multiple resistant strains were isolated: two against ampicillin, chloramphenicol and tetracycline; one in addition also against co-trimoxazole. It is likely that there will be a spread of such resistant strains.

[Responsibility, status and role of the medical physicist in the Federal Republic of Germany (author's transl)].

Pursuant to the regulations of the Roentgenverordnung (decree about X-ray examination and therapy), the Strahlenschutzverordnung (decree about radiological protection), and the Richtlinie Strahlenschutz in der Medizin (guide-line for radiological protection in medical practice), the responsible collaboration of a physicist on the diagnostic and therapeutic application of ionizing radiation is prescribed by law or, at least, seems reasonable with regard to the protection of the patient. This is why the authors describe the tasks and necessary qualifications of the physicist (competence as to radiological protection and professional admission) which are prescribed by law in the Federal Republic of Germany or which are desirable with respect to the performance of radioprotective measures during the application of ionizing radiation in medical practice. Furthermore, the authors summarize the results of a recently made inquiry about the state (e.g. education, professional experience, participation in medical care) and the role of the medical physicist (e.g. institutionalization of Medical Physics, educational work).

[Seroepidemiological investigations on the prevalence of rickettsial antibodies in man in the Federal Republic of Germany (author's transl)].

1,600 random serum samples from inhabitants of Hesse were investigated for rickettsial antibodies by complement fixation test. Antibodies were found in 19 samples (1,2%) against RMSF-group antigen with titers between 1:10 and 1:640. These results confirm recent reports on natural foci of such rickettsial infections in the Federal Republic of Germany. Antibodies against rickettsia of the Typhus-group or Coxiella burnetii could not be detected by the complement fixation reaction. None of the sera reacted with significant titer in the Weil-Felix-Test.

[Rehabilitation in France: a comparison with the Federal Republic of Germany (author's transl)].

The purpose of the following article is to present the rehabilitation efforts in France. It describes legal provisions, especially the French Rehabilitation Act of 1975, the "Loi d'orientation", with some of its most important rules of application, as well as some concrete individual measures. It describes approaches which differ from those in the Federal Republic of Germany. The article focuses particularly on the activities of the voluntary organisations and the efforts towards a barrier-free architecture.

[The role of the Federal Agency for Environmental Protection in the field of noise control].

The public efforts in noise abatement are shared among federal, cantonal, and local authorities. Since the acceptance of a new article in the Federal Constitution concerning the protection of the environment, the federal authorities have primarily the task to prepare and issue regulations, whilst the cantonal and local authorities have to enforce them. The Federal Office for Environmental Protection has to coordinate the activities at the federal level. The main present tasks are the preparation of new regulations, to act as an advisory board for other authorities, to analyse and evaluate the noise impact of installations and facilities, to inform the public, and to prepare the future education and training of the executive officials.

[The central bone tumor registry for the Federal Republic of Germany (author's transl)].

Among malignant tumors, bone tumors present a particularly acute problem. Bone tumors frequently occur in childhood; their etiology is completely obscure; early detection is still hardly possible, and the chances of success therefore leave much to be desired. A registry for bone tumors is being built up in the German Cancer Research Center in Heidelberg, which is to collect epidemiological data on all malignant and semimalignant primary bone tumors occurring in the Federal Republic. The lecturer gives an introduction to the registry and requests the cooperation of German surgeons in reporting pertinent cases.
