Community reintegration of SCI persons: problems and perceptions.

A survey of 222 SCI patients treated by the Southeastern Michigan Spinal Cord Injury System was conducted to explore some key issues involving community reintegration problems. These patients had been discharged from the rehabilitation facility an average of 18 months. The 62 respondents (28%) did not differ from nonrespondents on demographic and health status variables and were considered representative of the total patient population. Important findings included the following: 1) 30% were noncompliant with prescribed medication, 2) 25% had not received all equipment prescribed during initial rehabilitation, 3) 45% indicated their current residence was not completely accessible, 4) 73% were dependent on others for transportation, 5) 8% were currently working, compared to 39% employed prior to injury 6) 47% indicated transportation problems as the main obstacle to their adjustment to SCI. The problems identified prevented the SCI patients from increasing their presence in the community, thereby preventing them from exposing obstacles and improving the opportunities available to them.

[Patients with craniocerebral injuries and their reintegration into vocational life. The effect of parameters independent of the accident on expert assessment].

A prospective study of 60 patients with head injuries who received both primary care and an assessment at our Department, the influence of various parameters on the assessment was investigated. Catamnestically, the insurance-relevant results actually achieved are shown. It can be seen that at a 40 per cent incapacitation, reintegration into the work-a-day world is hardly possible.

[Reintegration in residential facilities--a catamnestic study].

A rehabilitation programme at the University Clinic of Social Psychiatry, Berne, is described. The facility includes a half-way house and sheltered home in a suburb, where 20 psychiatric long-stay patients are trained for an independent life in the community. In a follow-up study 80 patients, mostly chronic schizophrenics, were examined in order to evaluate the outcome. After an average time-interval of 6 years only 14% were in a mental hospital, 31% lived in sheltered accommodations, and 55% lived independently in the community. 57% of the patients were working full- or part time and a further 20% worked in sheltered workshops. Apart from these very good results a careful assessment of the social circumstances reveals an ongoing impairment: 80% have only poor social relations and depend on psychiatric treatment and medication, 77% need invalidity rents and more than one third live or work in sheltered institutions. The study shows that psychiatric long-stay patients can reach a stable reintegration in the community. The ongoing impairment mainly affects the social relations and the financial resources, areas which are important for the quality of life.

Assessment of global function: The Reintegration to Normal Living Index.

This paper reports the development of an instrument, the Reintegration to Normal Living (RNL) Index, to assess global function status. Information to determine the components of the index was systematically collected through interviews with professionals, patients, and their significant others, promoting content validity. Psychometric properties of the index were assessed using three samples of patients with varied diagnoses from several settings. The index demonstrates high internal consistency and adequate interrater reliability (patient and significant other). It is responsive to changes in the clinical status of patients, particularly when the subscales of Daily Living and Perceptions of Self are considered. In terms of criterion validity, the index is somewhat related to work status and disease status. It also demonstrates construct validity, both convergent and discriminant, when assessed against a quality of life index and an index of psychologic well-being. The RNL index appears to assess global function and measures both the patient's perceptions of their own capabilities and objective indicators of physical, social, and psychologic performance.

[The limits of social reintegration after longterm psychiatric hospitalisation (author's transl)].

In cooperation with the University Clinic for Psychiatry, Innsbruck, the social psychiatric service of the regional government of South Tyrol set up a rehabilitation programm, under which 35 longterm hospitalized schizophrenic patients with an average stay in hospital of 12,9 years have been taken over by district institutions as outpatients. The object of our investigation was to study critically the working capability and the social relations of the 35 patients, the duration of postclinical catamneses is at least 3, at maximal 25 years. The degree of obtained reintegration is documented by the working situation and the number of social contacts. The obtained extent of resocialization and the proved positive social adaptation make it sensible to start a rehabilitation even with cronical patients with hospitalisation over decades: precondition is however a continual psychiatric care and control.

[Psycho-social reintegration of the urostoma patient].

In certain functional disorders of the urinary bladder or invasive carcinoma, cystectomy with urinary diversion to the abdominal wall (urostomy) is indicated and life-prolonging. In order to know what the consequences were for these patients, and if their life with a urostomy is still outwardly worth living, 66 of our urostomy patients have been surveyed. Only one patient is on a disablement pension because of the urostomy. The limitations on leisure are mostly the realms of sport and travel; married life, despite sexual problems, is comparatively little affected. The causes for the limitations are discussed. For most patients the urinary diversion brought an improvement in "life comfort", and, generally, it can be said that urostomy is not an disabling operation and psychosocial reintegration is possible.

[The integrated approach to social reintegration: beyond "case management" and psychosocial rehabilitation].

Recently in western countries mental health systems are, slowly but systematically, adopting case management (CM) as a system of service delivery. Nonetheless, case management remains undefined, poorly described and its characteristics uncharted and unresearched. Social reintegration is a comprehensive model that incorporates strategies of service delivery aimed at solving the problems encountered by CM in the maintenance, enhancement of clients quality of life and social functioning and, social and community acceptance are addressed and issues of costs and collaboration between institutions is explored in depth.

[Functional ability and quality of life in rheumatoid arthritis: use of the Functional Independence Measure and the Reintegration to Normal Living Index].

Functional disability and quality of life were evaluated using the Functional Independence Measure and the Reintegration to Normal Living Index, respectively, in 57 subjects (15 men and 42 women) with a mean disease duration of 15 years. Scores on both these nonspecific scales correlated with those obtained using instruments specifically designed for rheumatoid arthritis (ARA index, Lee index) and with a number of clinical parameters including patient age, disease duration and number of affected joints. Functional ability was correlated with quality of life in this study and in others performed using other evaluation tools. Nonspecific assessment scales are useful for comparing function and quality of life in various diseases.

Treating survivors of child sexual abuse: a strategy for reintegration.

Sexual abuse survivors often present with a complex and changing symptom picture in which posttraumatic reactions are superimposed on an underlying personality disorder, a combination that makes the establishment of a secure therapeutic relationship difficult. An eclectic blend of principles and techniques is recommended in order to address both aspects of the survivor's disturbance.

An Operationalized Multisymptomatic Model of Neuroses (OMMON): toward a reintegration of diagnosis and treatment in behaviour therapy.

Experimental results from a sample of 216 patients with four different "symptom-neuroses" show that 65% to 90% of these patients have different combinations of multiple symptomatology. With a background of these data, we present an Operationalized Multisymptomatic Model of Neuroses (OMMON), based on self-rating assessment of these patients on 4 symptom scales. Individual ratings on each scale are dichotomized into (+) or (-) results with regard to defined cut-off points and the model is derived from their 16 mathematically possible combinations. Subsequent analysis of these data (from a single test application) with our Varying Cut-Off Point Assessment (VACOPA) leads to hypotheses regarding causal symptom interactions and prediction of symptom changes over time, easily evaluated by repeated test application. In treatment research the model seems suitable to: (a) build more homogeneous diagnostic groups; (b) operationalize varying degrees of "neurotic" disturbance, from "normal" via "client" to "patient"--independently of existing illness theories; (c) support prognoses of individual developments within and without treatment; (d) specify treatment aims and optimal sequences of interventions; (e) monitor predicted outcome; (f) reconsider earlier apparently contradictory outcome studies; (g) evaluate theoretical concepts regarding "neurotic" symptom formations in neuroses, psychoses, and psychosomatic disturbances. For treatment purposes, OMMON should only be used together with "clinical" hypotheses; its prognostic potential can be increased by additional application of our Operationalized Multivariate Model of Motivation (OMMOM). All three diagnostic approaches may be used for mutual evaluation.
