Strategic cycling: shaking complacency in healthcare strategic planning.

As the conditions affecting business and healthcare organizations in the United States have become more turbulent and uncertain, strategic planning has decreased in popularity. Strategic planning is criticized for stiffling creative responses to the new marketplace and for fostering compartmentalized organizations, adherence to outmoded strategies, tunnel vision in strategy formulation, and overemphasis on planning to the detriment of implementation. However, effective strategic planning can be a force for mobilizing all the constituents of an organization, creating discipline in pursuit of a goal, broadening an organization's perspective, improving communication among disciplines, and motivating the organization's workforce. It is worthwhile for healthcare organizations to preserve these benefits of strategic planning at the same time recognizing the many sources of turbulence and uncertainty in the healthcare environment. A model of "strategic cycling" is presented to address the perceived shortcomings of traditional strategic planning in a dynamic environment. The cycling model facilitates continuous assessment of the organization's mission/values/vision and primary strategies based on feedback from benchmark analysis, shareholder impact, and progress in strategy implementation. Multiple scenarios and contingency plans are developed in recognition of the uncertain future. The model represents a compromise between abandoning strategic planning and the traditional, linear model of planning based on progress through predetermined stages to a masterpiece plan.

Exercise and pregnancy in primary care.

An increasing number of women exercise throughout pregnancy. New evidence illustrates that moderate intensity exercise in healthy pregnant women, with certain considerations, does not increase adverse pregnancy risks. Exercise during pregnancy may actually decrease pregnancy-associated discomforts and improve maternal fitness and well-being. By following the American College of Obstetricians and Gynecologists recommendations, clinicians can confidently prescribe exercise to women during preconception, pregnancy, and the postpartum period.

Alpha 1-antitrypsin deficiency and the impact of nursing interventions and treatment with intravenous therapy. An overview.

The primary function of alpha1-antitrypsin is to protect the alveoli in the lung from harmful destruction from proteolytic enzymes, which prevent optimal elastic recoil of the lungs and destroy the lungs. Insufficient serum levels of alpha1-antitrypsin eventually lead to early onset of emphysema in the third, fourth, or fifth decade of life. Treatment of alpha1-antitrypsin deficiency by intravenous administration of an enzyme inhibitor known as alpha1-proteinase inhibitor, a human-derived blood product, can be administered to help replace the enzymes required to maintain lung function. Early detection, nursing intervention, and clinical management slow the progression of this hereditary disease.

[Long-term follow-up of patients with congenital heart defects].

There are about 500 new cases of congenital heart disease per year in Norway. Modern diagnostic skills, surgical techniques and follow-up programs have contributed to higher survival rates among patients. Based on international experience, 85-90 per cent of these children will survive into adulthood. Half will suffer from conditions which should be followed up by cardiologists. This article is based upon recommendations on long-term follow-up of patients with congenital heart disease.

[Research in general practice. Plea for organized and high quality clinical research by general practice physicians in Switzerland].

The clinical research in the practitioner's office is indispensable for the teaching of the medicine of first recourse. On the basis of the analysis of 109 publications, the authors propose to structure this research under the shape of investigator's networks while guaranteeing an interface between the primary care physicians and the academic specialists. To realise this network, an institutional support is necessary.
