Rheumatic diseases may be connected to varying degrees with psychic factors. On the one hand, rheumatic diseases may produce psychic reactions, while on the other hand rheumatic symptoms--especially in extra-articular rheumatism--may be induced by psychosomatic hysteric pararheumatic diseases and depressive mechanisms. Special reference is made to psychosomatically induced extra-articular rheumatic diseases in which not only the complex genesis but also the clinical symptomatology are discussed. During therapy more attention should be directed to psychic factors than in the past, as only an intensive psycho and/or psychopharmacotherapy can influence the disease.
Identical twins with bronchial asthma were studied. One had the first attack of the disease in late adolescence, the other not until he was adult. Both were demonstrated by immunologic means to be sensitive to house dust and certain foods. Yet, of itself, the factor of exposure to a known allergen seemed not enough to precipitate clinical allergic reaction in either of them. It is believed that emotional stress is accompanied by physiologic changes which facilitate increased reactions to antigenic agents that in normal circumstances would not cause clinical disease. The twins were widely different with regard to emotional development and in their reaction to situations of stress. In both of them allergic manifestations were associated with periods of emotional conflict. The dissimilar clinical manifestations of allergy in these identical twins may be explained by differences in personality and therefore in reactions to stress situations.

The results of an empirical study on psychosomatic aspects of rheumatoid arthritis (RA) and fibromyalgia syndrome (FMS) are presented and discussed in the setting of previous concepts about rheumatoid arthritis. The patients investigated in the two disease groups represent an unselected population with regard to psychological abnormalities (from two internistic rheumatism outpatient clinics); the results can thus be considered to be representative and permit the following conclusions to be drawn: RA is not a psychosomatic disease in the strict sense; however, psychological factors must be adjudged to have a disease-modifying influence. FMS may be classified within the group of psychosomatic diseases, although it should be regarded as the final stage of a wide variety of different processes within the biological, psychological and social sphere.

Two types of vulnerability of coronary vessels damaged by atherosclerosis were proposed basing on results of a study of 94 patients with psychosomatic reactions accompanied with ischemia or myocardial infarction. First variant (35 patients) was characterized by stable course of the disease (high effort angina without coronary catastrophes, episodes of unstable angina, etc.) and selective sensitivity to definite key affect of high personal value. Second variant (49 patients) was distinguished by pronounced clinical manifestations (effort angina with progressive lowering of exercise tolerance). Cardiovascular system displays universal susceptibility to negative emotions of various intensity.

The difficulty of the diagnosis of psychosomatic disorders is known to be lying in the shortage of time, the inadequate training of physicians and in the similarity of psychosomatic symptoms are compared to those of solely organic diseases. The irritable stomach-even with hyperacidity--and the irritable colon with constipation or diarrhea are among the most frequent disorders caused by psychosomatic mechanisms. The duodenal ulcer and the ulcerative colitis belong to the true somatopsychic and psychosomatic diseases. Precise organic (diagnostic) clarification and differentiated drug therapy are necessary in addition to psychotherapeutic measures.
