[An algorithm of computerized processing of molecular-genetic data for personality identification based on the outcome of events involving numerous victims].

Computerized systems of data processing are a high-efficiency tool for the storage and processing of big volumes of molecular genetic data as well as for the forensic medical identification of personality in events involving enormous unidentified bodies coming from zones of calamity or natural disaster. Described in the paper is a sequence of actions undertaken by the expert when the DNAdacto and mDNAbase soft tools are in use. The algorithm of computer-based expert analysis of typing of chromosome and mitochondrial DNA was tested at the 124th Central laboratory of medical-criminal identification of Russia's defense ministry; it was proven to be highly effective for identification made in events with numerous unidentified bodies.

Intraspinal synovial cyst causing sciatica.

Four patients who had low-back pain and sciatica were diagnosed as having a lumbar intraspinal extradural synovial cyst adjacent to a facet joint between the fourth and fifth lumbar vertebrae. The patients ranged in age from forty-nine to seventy-one years, and the symptoms and signs involved the fourth or fifth lumbar-nerve roots. Roentgenographically, degeneration of the intervertebral discs and facet joints was noted in every patient. Degenerative spondylolisthesis was also a frequent finding. Myelography and computed tomographic scans aided in diagnosis, revealing a soft-tissue lesion, occasionally rimmed with calcification, adjacent to the involved facet joint. The treatment was surgical excision of the cyst, as well as complete laminectomy if there was concomitant spinal stenosis. Follow-up, ranging from eighteen to twenty-five months, revealed complete resolution of the sciatica in all patients.

[Primary squamous cell carcinoma of the ureter: a case report].

A case of squamous cell carcinoma of ureter is presented. A 64-year-old male suffering from right lower abdominal pain and gross hematuria visited our hospital. Right hydronephrosis was found by ultrasound examination. Intravenous pyelography revealed a right non-functioning kidney. Abdominal computed tomographic scanning showed right hydroureteronephrosis and a soft-tissue density mass in the right lower ureter. Retrograde pyelography demonstrated a filling defect in the right lower ureter. Squamous cell carcinoma was suspected by cytological examination. On the basis of the above findings, right nephroureterectomy with partial cystectomy was performed. Pathohistological diagnosis was squamous cell carcinoma of the ureter, G3, INF gamma, pT3, pR0, pL1, pV1, pN1. No evidence of either tumor recurrence or metastasis was found for 6 months after the operation. Sixty-one cases of primary ureteral squamous cell carcinoma, including our case, were collected from the Japanese literature and characteristic clinical features of the tumor are discussed.

Fibrolipomatous hamartoma of the lung: a case report and review of the literature.

A fibrolipomatous hamartoma of the lung in a 57-year-old Japanese man is reported. Computed tomography revealed an abnormal mass with fatty and soft tissue densities in the upper portion of the right lung, indicating a liposarcoma. Upon resection, the tumor was found to consist of a 7.5 x 4.0 cm soft, yellowish mass continuous with a 3.0 x 1.5 cm polyp. The tumor was composed primarily of mature fibro-adipose tissue with some glandular and cartilaginous components, which identified it as a fibrolipomatous hamartoma. Follow-up studies of the patient for two years after surgery found no sign of malignancy. Thus, lipomatous or fibrolipomatous hamartoma should be considered as a possible diagnosis whenever atypical lipomatous lung tumors are encountered.

Unusual radiological manifestations of Lemierre's syndrome: a case report.

Lemierre's syndrome is an uncommon clinical entity characterized by oropharyngeal infection followed by septic thrombophlebitis of the jugular vein with embolization to the lungs and other organs. The organism is a gram-negative anaerobic bacterium, Fusobacterium necrophorum. We report a case of Lemierre's syndrome in an 8-year-old child who presented with septic arthritis of the left hip joint. Roentgenograms and computed tomography demonstrated gas in the joint and adjacent soft tissues, along with a dislocated hip. Sonography of the neck coupled with the colour Doppler technique did not reveal any abnormality in the jugular veins. A blood culture grew Fusobacterium necrophorum, confirming the diagnosis of Lemierre's syndrome.

Facial infiltrating lipomatosis.

Facial infiltrating lipomatosis is a rare congenital disorder in which mature lipocytes invade adjacent tissue. The phenotypic features include soft-tissue and skeletal hypertrophy, premature dental eruption, and regional macrodontia. There is a high risk for regrowth after resection that is, perforce, subtotal. The etiology, natural history, optimal management, and relationship to other disorders of fatty overgrowth are unclear. In this study, the clinical features, radiographic findings, histopathology, and postoperative results were analyzed in 13 patients with facial infiltrating lipomatosis. The condition was diagnosed in infancy (eight male subjects, five female subjects) and characterized by enlargement of the cheek (n = 12) or chin (n = 1). Other findings included cutaneous capillary blush (n = 9), ipsilateral macroglossia (n = 8), and mucosal neuromas (n = 6). Most patients had early eruption of ipsilateral deciduous and permanent teeth (n = 12). Computed tomography and magnetic resonance imaging showed an infiltrated soft-tissue mass of fatty density (n = 13) and skeletal overgrowth (n = 9). Multiple resection was performed on six patients (mean number of operations per patient, 2.5; range, one to six operations); regrowth and/or worsening of the capillary stain occurred in all six patients. Because surgical removal of the mass is usually unsuccessful, specific management of this condition will require insight into its etiopathogenesis. Given the presence of mucosal neuromas and lipomatosis, this study included testing for the known mutations in three entities that are associated with these soft-tissue findings (Cowden syndrome, Bannayan-Riley-Ruvalcava syndrome, and multiple endocrine neoplasia type 2B). Results of DNA analyses for these germline mutations were negative. It is more likely that this disorder is caused by a somatic mutation involving a local increase in growth factor(s).

Nucleolar organizer regions in soft tissue tumours.

The diagnostic perspectives of the combination of silver NOR staining method with TV image analysis were tested in 138 soft tissue tumours. In addition to counting the dots per cell, we also measured the summed areas of dots within the nucleus. The so called "malignancy factor" (MF), i.e. the number of dots multiplied by summed areas of dots in one nucleus, was calculated. Tumours were divided into four groups: 1) benign, 2) benign with diagnostic difficulty, 3) intermediate malignant and 4) malignant. The number of dots, the summed area of dots and the MF were separately evaluated by the "t" test. Groups 2 and 3 were not distinguishable at all (p. - 0.788-0.863), while the others showed significant differences in the former values (p less than 3.5 x 10(-6) - 10(-6). When the number of dots was considered in itself, some overlaps occurred between the fourth and the other groups. With the determination of MF, however, no overlap was encountered at all. MF seems to be superior to traditional dot counting when malignant, intermediate and benign soft tissue tumours are to be distinguished.

Fluid-fluid levels in cavernous hemangioma of soft tissue.

Five cases of cavernous hemangioma with fluid-fluid levels on magnetic resonance imaging and/or computed tomography are reported. The signal characteristics were those of blood and histological analysis of the fluid-fluid levels showed that they were blood-filled cavities in the tumor. Although this finding itself is not specific, it may help in confirming the diagnosis of cavernous hemangioma.

[The evaluation of computer-aided simulating system in orthognathic surgery]

OBJECTIVE: The purpose of this study is to evaluate the reliabillty of computer-aided simulating system for orthognathic system. METHODS: 24 cases with dentofacial deformities were undergone treatment of orthognathic surgery.The presurgical cephalograms and profiles were input to the computer and were digitized.The simulated profiles and cephalometric analysis were obtained by the simulating system,and computer with the actual posttreatment cephalograms. RESULTS: The prodicted and actual posttreatment soft tissue line drawings were quite similar in all areas except for the lower lip region. CONCLUSION: In all cases,the actual images were judged more esthetic than the predicted images. It allayed the fears that the computer generated predictions were unrealistically and ideal.

CT angiography in complex upper extremity reconstruction.

Computed tomography angiography is a new technique that provides high-resolution, three-dimensional vascular imaging as well as excellent bone and soft tissue spatial relationships. The purpose of this study was to examine the use of computed tomography angiography in planning upper extremity reconstruction. Seventeen computed tomography angiograms were obtained in 14 patients over a 20-month period. All studies were obtained on an outpatient basis with contrast administered through a peripheral vein. All the studies demonstrated the pertinent anatomy and the intraoperative findings were as demonstrated in all cases. Information from two studies significantly altered pre-operative planning. The average charge for computed tomography angiography was 1,140 dollars, compared to 3,900 dollars for traditional angiography.

Magnetic resonance imaging of maxillary cancer--possibility of detecting bone destruction.

The purpose of this study was to evaluate the detectability of bone destruction of maxillary cancer with magnetic resonance imaging (MRI) using 14 cases of squamous cell carcinoma of the upper jaw. The detectability of bone destruction including the degree of spread to adjacent soft tissues was evaluated and compared to that of clinical examination, computed tomography (CT) and conventional X-ray films. MRI could show bone destruction of each bony part almost equally with CT, but differentiation among simple bone defects, bone expansion and bone destruction was difficult on MRI. The pattern of bone destruction of alveolus that could be detected on conventional X-ray examinations, could not be assessed on either CT or MRI. Soft tissue infiltration of the tumour was more clearly detected on MRI compared with CT and conventional X-ray films.

CT findings of plexiform neurofibromatosis involving the ileum and its mesentery.

We present a case of plexiform neurofibromatosis involving the ileum and its mesentery. Computed tomography (CT) scans showed a cluster of small soft-tissue density nodules, which represented a cross-sectional image of the enlarged peripheral nerves and wall thickening of the distal ileum. Trapped fat tissue was demonstrated between these enlarged nerves. Histopathologic studies of the resected specimen correlated well with CT findings.

Virtual reality for dermatologic surgery: virtually a reality in the 21st century.

In the 20th century, virtual reality has predominantly played a role in training pilots and in the entertainment industry. Despite much publicity, virtual reality did not live up to its perceived potential. During the past decade, it has also been applied for medical uses, particularly as training simulators, for minimally invasive surgery. Because of advances in computer technology, virtual reality is on the cusp of becoming an effective medical educational tool. At the University of Washington, we are developing a virtual reality soft tissue surgery simulator. Based on fast finite element modeling and using a personal computer, this device can simulate three-dimensional human skin deformations with real-time tactile feedback. Although there are many cutaneous biomechanical challenges to solve, it will eventually provide more realistic dermatologic surgery training for medical students and residents than the currently used models.

Computed tomography of the hindfoot.

This investigation used computed tomography (CT) to study the normal anatomy of the hindfoot and to demonstrate abnormalities in patients with rheumatoid arthritis, pes planovalgus , old trauma, and soft-tissue masses. The hindfeet of 10 normal volunteers and 17 patients were scanned in the coronal and axial transverse planes. The articular surfaces of the tibia, talus, and calcaneus were assessed, as were the relationships of each bone to the others. Normal subjects had markedly symmetrical hindfeet . The "heel valgus angle" (relating the calcaneus to the tibia) was 5.2 +/- 1.6 degrees (mean +/- standard error of the mean). The "sustentacular angle" (indicating the angle of elevation of the sustentaculum tali at the midsubtalar joint) was 18.3 +/- 1.3 degrees. The "medial offset" of the talar head with respect to the calcaneus (measured at the anterior subtalar joint) averaged 5.2 +/- 1.8 mm. Patients with rheumatoid arthritis had erosions involving both sides of the subtalar joint, leading to talocalcaneal alignment abnormalities. In patients with pes planovalgus the heel valgus angle was greater and the elevation of sustentaculum tali lower in the foot that was most severely affected. Bony talocalcaneal fusions, fractures, and the erosions of pigmented villonodular synovitis were detected on CT. The pattern of postoperative bone healing after attempted subtalar fusion could be demonstrated. We conclude that CT is a useful tool for pre- and postoperative evaluation of the hindfoot.

MRI of facial skeleton and parapharyngeal space.

The diagnostic value of magnetic resonance imaging is examined in relation to computed tomography on the basis of 66 patients, who had pathological processes in the midfacial and nasopharyngeal space. The high power of contrast resolution in imaging soft tissue masses and the direct multiplanar capability of magnetic resonance offer advantages over computed tomography in respect of space occupying lesions in the median and lower parts of the craniofacial skull. Lymph nodes of more than 1 cm. in diameter were detected by both modalities. A disadvantage is the poorer detectability of tumour conditioned bone destruction at the base of the skull. In such cases, computed tomography remains the method of choice.

Value of computed tomography in management of acute laryngeal injury.

Blunt trauma to the larynx is an uncommon injury that results in a wide spectrum of damage to the endolaryngeal soft tissues as well as underlying cartilaginous skeleton. Recognition of these injuries is essential if long-term problems with compromised airway and voice quality are to be avoided. Although minor soft-tissue injuries do not mandate surgery, severe soft-tissue damage or cartilaginous fracture-dislocations require exploration and repair. Treatment is less clearly defined for the injured larynx with moderate damage. Frequently, the degree of soft-tissue injury is used to judge the likelihood of damage to the thyroid and cricoid cartilages. Computed tomography (CT) is an excellent noninvasive technique for examining the laryngeal skeleton. Laryngeal CT may be successfully used to determine the need for open exploration and repair in selected cases of blunt trauma to the larynx when clinical findings are equivocal for cartilaginous damage.

Hard-copy versus soft-copy image reading for detection of ureteral stones on abdominal radiography.

PURPOSE: The purpose of this study was to compare the usefulness of soft-copy images displayed on a cathode ray tube (CRT) with hard-copy images (film images) for detecting ureteral stones on abdominal radiography. MATERIALS AND METHODS: Five radiologists read images from 50 cases of ureteral stones and 50 normal cases diagnosed on the basis of intravenous urography and CT. For hard-copy reading, 10-bit images at 3,520x4,280 pixels obtained by computed radiography were printed on 14x17-inch films. For soft-copy reading, 8-bit images were displayed on a 17-inch monochrome monitor at 1,024x1,280 pixels. The study items were area under receiver operating characteristics (ROC) curve (Az), ureteral stone detection sensitivity and specificity, and reading time. RESULTS: For soft-copy and hard-copy images, the average Az values were 0.855 and 0.851, sensitivity was 62.8% and 62%, and specificity was 70.8% and 62.4%, respectively. There were no statistically significant differences between these values. Reading time was 106.6 min for soft-copy images, significantly longer than the 71.2 min for hard-copy images (p<0.05). CONCLUSION: Although soft-copy image reading time was longer than hard-copy image reading time, the ability to diagnose ureteral stones on abdominal radiography did not differ for soft- and hard-copy images.

A congenital malformation of the maxilla of a horse.

A 1-month-old male American Paint Horse was evaluated for a hard swelling on the right side of the maxillary region. On radiographs there was a large, expansile outpouching of the right maxillary bone between the second and third premolar teeth. Computed tomography further characterized the expansile lesion to have a soft tissue component and to originate in the region of a caudal maxillary tooth. Surgical reconstruction of the defect was unsuccessful and the animal was euthanetized. Based on failure to find histopathologic evidence of a neoplasm or cyst, the diagnosis was a congenital malformation.
