Patients need medical and qualified surgery care for different purposes: to begin a diagnostic investigation, to monitor a medical therapy, to execute controls in view of or after a surgical operation, etc. Medical care becomes the heart of patient interest, the direct consequence is a lacking reflection about the purpose of nursery activity in this area and, therefore, on the method to be used to define the staff needed. On this particular subject is based the contribute of this article, considering it of particular interest, being the growing attention on the management of human resource in the health word.

Based on early concepts of quantitative evaluation of pilotage overload influence on pilot, the authors justified principles and criteria for biometric characterization of overload parameters and human response concerning the problem of manoeuvreable flight and pilot's occupational health preservation. The article presents quantitative dependencies between various overload parameters, occupational conditions and -effects, considering risk of disorders that determine potential unreliability of pilot in manoeuvreable flight and possible cumulative effects from dose viewpoint.

BACKGROUND: We wanted to explore the quality and importance of the collaboration between public health physicians and selected groups of professionals and politicians. MATERIAL AND METHODS: The study was a cross-sectional survey based on self-administered questionnaires with closed questions. Response came from 218 public health doctors who worked in the municipalities throughout the study period (1988-91), and from 98 doctors who left public health during the same period (81% and 86% of the study populations respectively). RESULTS: The doctors reported good and important collaboration with other health professionals, but less good and less important collaboration with other professionals and with politicians. With increased administrative distance between the doctor and other municipality staff, the perceived quality of the collaboration decreased. INTERPRETATION: Closer collaboration between public health doctors and important decision makers in the municipality promotes job satisfaction among doctors and may reduce turn-over.
OBJECTIVES: 1) To estimate the burden to diagnose and treat iatrogenic disorders out of all the activities of a referral hospital's medical ward. 2) To investigate about the main causes making the iatrogenic disorders arouse. DESIGN AND SUBJECTS: The entire series of iatrogenic disorder bearing patients (in- and out-), treated in the Institute of Medical Pathology of Ancona throughout a period of two years, were studied retrospectively. The low therapeutic index treatments (i.e. antiproliferative drugs, therapies under emergency, surgical amputations) were ruled out. The data to address the objectives were gathered. RESULTS AND CONCLUSIONS: 1) Highly probable iatrogenic disorders were diagnosed in 84/1620 patients (5.2%). 2) For this task 108/2763 medical performances were carried out (3.9%); 3) iatrogenic disorders were Slight in 20 (24%), Moderate in 44 (52%) and Severe in 20 (24%) patients. 4) Some facilitaters (poor monitoring of adverse drug reactions, primary disease incorrect treatment, poor accuracy of primary disease diagnosis, incoordination among therapists) were detected in 32, 25, 14, and 7%. 5) The most commonly occurring facilitaters were detected more frequently in Severe rather than Non Severe iatrogenic disorder bearing patients. 6) Severe iatrogenic disorders and the serious consequences of them, such as death and permanent serious harm, occur more often, when there are combinations of facilitaters; 7) Costs were minor in 69% and major in 31%.

The aim of this study was to investigate which factors could play a role on the work-load of the Alzheimer patients' caregivers. The work-load was measured by means of Zarit's scale and inserted as dependent variable in a regression model, where the following independent variables were considered: education and MMSE of patients, age, sex of both patients and caregivers. The relevant factors resulted patient's age and MMSE, being significantly (p < 0.05) inversely associated with work-load. This score reached his maximum when patient's MMSE ranged between 14 and 16 (medium impairment). However, a large variability was found in this interval, suggesting a possible effect of other factors. In addition, we observed a decrement of work-load when patients were older, being higher at 50 and lower after 72. This effect might be explained by the significant change (especially in terms of social and familial life) due to the illness when the patient is relatively young. When the patient is older, Alzheimer' disease had smaller effect on the caregiver's work-load.
