ARTIST (Asian regional tobacco industry scientist team): Philip Morris' attempt to exert a scientific and regulatory agenda on Asia.

OBJECTIVE: To describe how the transnational tobacco industry has collaborated with local Asian tobacco monopolies and companies to promote a scientific and regulatory agenda. METHODS: Analysis of previously secret tobacco industry documents. RESULTS: Transnational tobacco companies began aggressively entering the Asia market in the 1980s, and the current tobacco industry in Asia is a mix of transnational and local monopolies or private companies. Tobacco industry documents demonstrate that, in 1996, Philip Morris led an organisation of scientific representatives from different tobacco companies called the Asian Regional Tobacco Industry Science Team (ARTIST), whose membership grew to include monopolies from Korea, China, Thailand, and Taiwan and a company from Indonesia. ARTIST was initially a vehicle for PM's strategies against anticipated calls for global smoke-free areas from a World Health Organization secondhand smoke study. ARTIST evolved through 2001 into a forum to present scientific and regulatory issues faced primarily by Philip Morris and other transnational tobacco companies. Philip Morris' goal for the organisation became to reach the external scientific and public health community and regulators in Asia. CONCLUSION: The Asian tobacco industry has changed from an environment of invasion by transnational tobacco companies to an environment of participation with Philip Morris' initiated activities. With this participation, tobacco control efforts in Asia face new challenges as Philip Morris promotes and integrates its scientific and regulatory agenda into the local Asian tobacco industry. As the local Asian tobacco monopolies and companies can have direct links with their governments, future implementation of effective tobacco control may be at odds with national priorities.

Implementation and evaluation of information desk services provided by library technical assistants.

Changes in the role of information services librarians and in the health care environment have required a rethinking of the provision of reference services at the University of Illinois at Chicago Library of the Health Sciences. This is a report of a new service offered after that analysis. An information desk staffed by twenty-five library technical assistants was established. Details of staff training, scheduling, and data gathering for this new service are provided. After eight months of operation, an evaluation of services provided by the Information Desk was conducted. A combination of evaluation methodologies, both quantitative and qualitative, has been used to determine overall staff performance. Results from analysis of service statistics, structured observations of real-time services operations, and questionnaires distributed to information services librarians and to patrons are presented. The findings from this study are discussed in terms of comparison with similar studies in other libraries and identification of future research studies. The results confirm the value of the Information Desk and support the decision to continue this service model.

The future of Medicare. Their plans, your practice, Medicare reform, and how it will affect the practice of dermatology.

The future of Medicare will be determined by patient demands, government action, and physician reactions. The federal government has always had the authority to design the Medicare program, but in the past physicians have had control over its implementation. While it is impossible to accurately predict the future, past and present reform efforts shed considerable light on directions the Medicare program will take and the impact Medicare will have on the science and practice of dermatology. It is important to understand these historic trends and the changes they foster to position ourselves to avoid threats and benefit from opportunities that will arise for the science and practice of dermatology.

The information-processing approach to the human mind: Basics and beyond.

Cognitive psychology attempts to understand the nature of the human mind by using the information-processing approach. In this article, the fundamentals of the cognitive approach will be presented. It will be argued that the human mind can be described at three levels-computational, algorithmic-representational, and implementational-and that the cognitive approach has both important theoretical and practical/clinical implications. Finally, it will be suggested that the study of cognitive psychology can provide a foundation for other fields of social science, including the field of clinical psychology.

Pragmatics in analogical mapping.

Theories of analogical reasoning differ in the roles they ascribe to pragmatic factors as a source of constraints on analogical mappings. The multiconstraint theory as instantiated in the ACME model (Holyoak & Thagard, 1989a) claims that pragmatic constraints interact with structural and semantic constraints within the mapping stage itself, in addition to influencing pre-mapping and post-mapping stages. Participants in three experiments were asked to generate mappings between non-isomorphic analogs for which mappings for some elements were ambiguous on structural grounds. In all experiments, manipulations of participants' processing goals influenced their preferred mappings. At the same time, goal-irrelevant information contributed to many-to-one mappings (Experiments 1 and 2) and to the resolution of mappings that were ambiguous on the basis of goal-relevant information alone (Experiment 3). The qualitative pattern of results was successfully simulated using the ACME model, implementing the impact of processing goals as an inhibitory process of selective attention.

Effective use of critique and dialogue at scholarly conferences.

In this article, the authors offer a solution to a phenomenon identified as "silencing" among nurse colleagues during national, regional, and international scholarly conferences. Through an electronic anonymous survey, data were collected regarding perceptions of the structure and process at scholarly nursing conferences. The need for critique and dialogue while sharing research ideas or findings is identified as a means to encourage direct exchange at professional conferences. Based on an examination of the process of critique and dialogue, and theories that explain why honest and direct dialogue are sometimes subdued, the authors propose a model of constructive scholarly dialogue for conference participation. The goals of implementing this model are to make scholarly exchanges normative at nursing conferences, and to revise standard conference formats so that constructive critique and dialogue are encouraged actively. The likely outcomes include improved nursing science and professional development of nurses.

Effective Backpropagation Training with Variable Stepsize.

The issue of variable stepsize in the backpropagation training algorithm has been widely investigated and several techniques employing heuristic factors have been suggested to improve training time and reduce convergence to local minima. In this contribution, backpropagation training is based on a modified steepest descent method which allows variable stepsize. It is computationally efficient and posseses interesting convergence properties utilizing estimates of the Lipschitz constant without any additional computational cost. The algorithm has been implemented and tested on several problems and the results have been very satisfactory. Numerical evidence shows that the method is robust with good average performance on many classes of problems. Copyright 1996 Elsevier Science Ltd.

TIP, a T-cell factor identified using high-throughput screening increases survival in a graft-versus-host disease model.

A coordinated effort combining bioinformatic tools with high-throughput cell-based screening assays was implemented to identify novel factors involved in T-cell biology. We generated a unique library of cDNAs encoding predicted secreted and transmembrane domain-containing proteins generated by analyzing the Human Genome Sciences cDNA database with a combination of two algorithms that predict signal peptides. Supernatants from mammalian cells transiently transfected with this library were incubated with primary T cells and T-cell lines in several high-throughput assays. Here we describe the discovery of a T cell factor, TIP (T cell immunomodulatory protein), which does not show any homology to proteins with known function. Treatment of primary human and murine T cells with TIP in vitro resulted in the secretion of IFN-gamma, TNF-alpha, and IL-10, whereas in vivo TIP had a protective effect in a mouse acute graft-versus-host disease (GVHD) model. Therefore, combining functional genomics with high-throughput cell-based screening is a valuable and efficient approach to identifying immunomodulatory activities for novel proteins.

Making bootstrap statistical inferences: a tutorial.

Bootstrapping is a computer-intensive statistical technique in which extensive computational procedures are heavily dependent on modern high-speed digital computers. The payoff for such intensive computations is freedom from two major limiting factors that have dominated classical statistical theory since its beginning: the assumption that the data conform to a bell-shaped curve, and the need to focus on statistical measures whose theoretical properties can be analysed mathematically. The name "bootstrap" was derived from an old saying about pulling oneself up by one's bootstraps. In this case, bootstrapping means redrawing samples randomly from the original sample with replacement. The key idea, computations, advantages, limitations, and application potential of bootstrapping in the field of physical education and exercise science are introduced and illustrated using a set of national physical fitness testing data. Finally, an example of a bootstrapping application is provided. Through a step-by-step approach, the development and implementation of the bootstrap statistical inference are illustrated.

Genetically modified organisms: an analysis of the regulatory framework currently employed within the European Union.

BACKGROUND: Genetic engineering technology is starting to bring many commercial products to the market. These genetically modified organisms (GMOs) and their derived products are subject to topical debate as to their benefits and risks. The strengths and weaknesses of the regulatory framework that controls their development and application is central to the question of whether this technology poses significant risk to the public health during this critical phase of its evolution. METHODS: A critical review was carried out of the legal framework regulating the contained use, deliberate release and some aspects of consumer protection relevant to the control of GMOs in Europe and the United Kingdom. RESULTS: The current legal framework is failing to provide a speed of adaptation commensurate with the development of the science of genetic engineering; failing to properly respond to democratic control; failing to resolve significant conflict between the protection of free markets and protection of public health and the environment; and failing to implement obligations on biodiversity. CONCLUSION: The present legal framework must be replaced. Current European Union proposals for new standards of regulation are welcome, but provide only for further incremental change, and do not address some significant fundamental flaws in our current laws.

IMRT: delivery techniques and quality assurance.

Intensity modulated radiotherapy (IMRT) is a major development in the delivery of radiation therapy that has the potential to improve patient outcome by reducing morbidity or increasing local tumour control. Delivery techniques include those based on purpose built devices and treatment machines together with those utilizing the capabilities of computer controlled multileaf collimators which are more widely available. The complexity of IMRT techniques demands a high level of quality control both in the operation of the equipment and in the delivery of treatment to individual patients. The purpose of this paper is therefore to review the techniques available, concentrating on the use of multileaf collimators, and to consider the necessary quality control requirements for clinical application. It demonstrates that the technology is mature and sufficiently well understood so that IMRT can be safely implemented in the general clinical environment rather than being limited to application in the research environment.

[Quality assessment of medical-scientific research: in the future it should also be assessed on the basis of social impact].

Clinical research is the basis for the improvement of clinical care, yet assessments of the quality of research output in terms of the consequences for patient care are seldom acknowledged. However, contributions to guidelines, educational programmes and other products for clinicians indicate the clinical relevance of research and researchers. A recent report from the Royal Netherlands Academy of Arts and Science (Dutch acronym: KNAW) proposes a method to measure these aspects in addition to the scientific value of the output from research programmes. The transfer of research findings into clinical practice requires an implementation process. Measurements of the societal impact of research may lead to a better understanding of this process.

Overview of NASA's space radiation research program.

NASA is developing the knowledge required to accurately predict and to efficiently manage radiation risk in space. The strategy employed has three research components: (1) ground-based simulation of space radiation components to develop a science-based understanding of radiation risk; (2) space-based measurements of the radiation environment on planetary surfaces and interplanetary space, as well as use of space platforms to validate predictions; and, (3) implementation of countermeasures to mitigate risk. NASA intends to significantly expand its support of ground-based radiation research in line with completion of the Booster Applications Facility at Brookhaven National Laboratory, expected in summer of 2003. A joint research solicitation with the Department of Energy is under way and other interagency collaborations are being considered. In addition, a Space Radiation Initiative has been submitted by the Administration to Congress that would provide answers to most questions related to the International Space Station within the next 10 years.

[Present day psychosomatic medicine].

Article presents the development of psychosomatic medicine, respectively that of behavioral or anthropological medicine, stressing they do not belong to medical disciplines but they represent a way of thinking, which aims to introduce knowledge of humanistic sciences into the classical medicine and to change the orientation of contemporary medicine from the disease to the diseased person. Historically, the new approach represents a systemic course, which leads to the general theory of systems and to cybernetics. These are mathematical and logic sciences, which can be applied in every situation where complex manifestations have to be handled. Problems of human health and disease definitely represent such complexity. Systemic theories also develop as well as their implementation into the medicine. Formulation of the bio-psycho-social approach or the movement of family (systemic) therapy verifies the theory.

Environmental release of living modified organisms: current approaches and case studies.

Agricultural biotechnology is being rapidly adopted as evidenced by the acreage of genetically modified (GM) crops planted and tonnes of product (grain and fiber) harvested. Concurrent with this technological progress, is a growing concern that the worlds biological diversity is coming under increasing threat from human activities. As such, ecological risk assessment approaches are being developed for GM crop plants as international agreements regulating the transboundary movements of these products are being implemented. This paper reviews the ecological risk assessment approach that has been used to date to approve GM crops to date. The process has been case-by-case, using a comparative, science-based approach balancing the potential risks and benefits of the new technology versus those present with the currently accepted practices. The approach used to evaluate and approve these products is consistent with the conditions and requirements outlined in the Cartagena Protocol.

Job enrichment in job design.

For optimal operation in labor-intensive industries, such as foodservice, not only scientific management principles but also behavioral aspects (the people) must be considered in designing job content. Three psychologic states--work that is meaningful, responsibility for outcomes, and knowledge of outcomes--are critical in motivating people. These, in turn encompass the core dimensions of skill variety, task identity, task significance, autonomy, and feedback. Job enrichment and job enlargement--related but not identical means of expanding job content--when combined, offer the likelihood of redesigned jobs in the core dimensions. Effective implementation of a job enrichment program hinges on diagnosing problems in the work system, actual changes in the work, and systematic evaluation of the changes. The importance of the contribution of the behavioral sciences to management cannot be neglected.

IAIMS development at Harvard Medical School.

The long-range goal of this IAIMS development project is to achieve an Integrated Academic Information Management System for the Harvard Medical School, the Francis A. Countway Library of Medicine, and Harvard's affiliated institutions and their respective libraries. An "opportunistic, incremental" approach to planning has been devised. The projects selected for the initial phase are to implement an increasingly powerful electronic communications network, to encourage the use of a variety of bibliographic and information access techniques, and to begin an ambitious program of faculty and student education in computer science and its applications to medical education, medical care, and research. In addition, we will explore means to promote better collaboration among the separate computer science units in the various schools and hospitals. We believe that our planning approach will have relevance to other educational institutions where lack of strong central organizational control prevents a "top-down" approach to planning.
