Frequent aggression and attribution of hostile intent in people with mild to moderate intellectual disabilities: an empirical investigation.

We investigated whether aggressive individuals have an attributional bias of hostile intent compared to nonaggressive peers. We compared 43 frequently aggressive individuals who had mild to moderate intellectual disabilities with 46 nonaggressive controls on an attributional task. The aggressive participants attributed significantly more hostile intent to protagonists and indicated that they would respond more aggressively than did the controls to provocative scenes, but only when the threat was to themselves. Results suggest that differences in attribution of threat to self play a role in frequent aggression. These differences appear to be due, in part, to a positive bias of the nonaggressive participants on the self-referent scenes. Clinically, results highlight the importance of assessing and addressing aggressive individuals' interpersonal perceptions.

Religion and motion pictures' effects on reversing suicidal intentions: two case studies.

Case studies of two veterans contemplating suicide are described. Their suicidal intentions subsided as a result of a rekindling of their Christian faith. Although their Christian beliefs were awakened following their attendance at a motion picture, they both attributed the remission of their suicidal intentions to the practices of their Christian faith in their lives. Despite the favorable outcome, these findings are not generalizable to other cases.

A laboratory study of satisfaction effects on mood state, withdrawal intentions, and organizational citizenship behavior.

154 undergraduates role-played a restaurant server experiencing positive, neutral, or negative Job or Life Satisfaction. Positive Satisfaction resulted in ratings of more positive reported mood state, lower expected absenteeism and turnover intentions, and greater expected Organizational Citizenship Behavior than did Neutral Satisfaction and Negative Satisfaction. Life Satisfaction was associated with reported mood states and intentions similarly to Job Satisfaction, except Negative Job Satisfaction was more negatively influential on expectations of quitting than Negative Life Satisfaction. Both Job and Life Satisfaction may be antecedents to affect and its consequences in work settings.

The influence of spatial context and the role of intentionality in the interpretation of animacy from motion.

We present three experiments investigating how spatial context influences the attribution of animacy to a moving target. Each of our displays contained a moving object (the target) that might, depending on the way it moved, convey the impression that it was alive (animate). We investigated the mechanisms underlying this attribution by manipulating the nature of the spatial context surrounding the target. In Experiment 1, the context consisted of a simple static dot (the foil), whose position relative to the target's trajectory was manipulated. With some foil positions--for example, when the foil was lying along the path traveled by the target--animacy judgments were elevated relative to control foil locations, apparently because this context supported the impression that the target was "reacting to" or was in some other way mentally influenced by the foil. In Experiment 2, contexts consisted of a static oriented rectangle (the "paddle"). On some trials, the target collided with the paddle in a way that seemed to physically account for the target's motion pattern (in the sense of having imparted momentum to it); this condition reduced animacy ratings. Experiment 3 was similar, except that the paddles themselves were in motion; again, animacy attribution was suppressed when the target's motion seemed to have been caused by a collision with the paddle. Hence, animacy attributions can be either elevated or suppressed by the nature of the environment and the target's interaction with it. Animacy attribution tracks intentionality attribution; contrary to some earlier proposals, we conclude that attributing animacy involves, and may even require, attributing to the target some minimal mental capacity sufficient to endow the target with intentionality.

Spatial S-R compatibility effects in an intentional imitation task.

The active intermodal mapping hypothesis suggests that intentional imitation is mediated by a highly efficient, special-purpose mechanism of actor-centered movement encoding. In the present study, using methods from stimulus-response (S-R) compatibility research, we found no evidence to support this hypothesis. In two experiments, the performance of adult participants instructed to imitate actor-centered spatial properties of head, arm, and leg movements was affected by task-irrelevant, egocentric spatial cues. In Experiment 1, participants imitated using the same side of their bodies as did the model, and performance was less accurate when egocentric stimulus location was response incompatible than when it was response compatible. This effect was reversed in Experiment 2 when participants imitated using the opposite side of their bodies. These findings, in line with general process theories of imitation, imply that intentional imitation is mediated by the same processes that mediate responding to inanimate stimuli on the basis of arbitrary S-R mappings.

Intention to learn influences the word frequency effect in recall but not in recognition memory.

Watkins, LeCompte, and Kim (2000) suggested that the recall advantage for rare words in mixed lists is due to a compensatory study strategy that favors the rare words. They found the advantage was reversed when rare and common words were studied under incidental learning conditions. The present study investigated the possibility that the rare-word advantage in recognition memory is also the result of a compensatory study strategy. Experiment 1 replicated the findings of Watkins et al. that the rare-word advantage in recall is eliminated under incidental learning conditions. In contrast, Experiment 2 showed that the rare-word advantage in recognition memory is maintained under both intentional and incidental learning conditions. Experiment 3 replicated the results of Experiments 1 and 2 using different stimuli and a different orienting task. Finally, Experiment 4 showed that the rare-word advantage in recognition is maintained with pure lists. These findings show that the rare-word advantage in recognition memory is not the result of a compensatory study strategy. Instead, rare words are encoded more distinctively than common words, irrespective of participants' intention to remember them.

Prevalence and severity of intimate partner violence and associations with family functioning and alcohol abuse in psychiatric inpatients with suicidal intent.

BACKGROUND: Many medical settings have conducted screenings for domestic violence, but no study has assessed the prevalence and frequency of intimate partner violence (IPV) within the acute psychiatric inpatient population. METHOD: This descriptive, cross-sectional study was conducted in adult inpatient acute care units at a psychiatric hospital. Participants completed questionnaire-based assessments of recent and lifetime history of IPV, family functioning, and alcohol use. Recruited patients were aged between 18 and 65 years, were English-fluent and literate, had suicidal ideation, and had been living with an intimate partner for at least the past 6 months. Acutely psychotic patients and patients who were too agitated to complete the questionnaires were excluded. 110 patients completed the assessments. Interpersonal violence was assessed using the Revised Conflict Tactics Scale (CTS2), family functioning was measured using the Family Assessment Device, and alcohol use was assessed with the Alcohol Use Disorders Identification Test. Data were gathered from August 2004 through February 2005. RESULTS: Over 90% of suicidal inpatients reported IPV perpetration and victimization in their relationships in the past year, with the overwhelming majority reporting severe IPV. Male and female patients did not differ significantly on any CTS2 violence perpetration or victimization subscale (all p values >.05). Poor family functioning predicted physical violence victimization in both male and female suicidal inpatients, even after controlling for alcohol use and demographic characteristics. CONCLUSION: Psychiatric inpatients with suicidal ideation or intent would benefit from screening for IPV. Information about IPV and treatment options should be made available to psychiatric inpatients with suicidal intent. Attention to the family functioning of these patients is recommended.

[Seeking the solution at the heart of the problem: facing the intentionality of people suffering of borderline personality disorder (BPD)].

This essay takes its support on the study of concepts of intentionality and life projects and their use in the rehabilitation of people with borderline personality disorder (BPD). It considers that these people are dis-concerted and disconcerting and the pertinence of envisaging new ways of helping them reconstruct a sense of their existence. Inspired by constructivist approaches that recognize the constant interaction of the subject with his environment and the duality of things, this reflection leads to the necessity of exploring the intentionality of people with BPD and develop their reflexive capacity in order to allow the progressive emergence of a life project. This recovery process is part of an approach where hope and mobilization of the person's strengths take an important place.

Differential involvement of parietal and precentral regions in movement preparation and motor intention.

Flexible goal-oriented behavior relies on spatial coordinate transformations and motor control mechanisms, but also on the capability to take advantage of contextual information for steering the sensorimotor machinery. Although accurate performance of a sensorimotor task requires parietal and frontal regions, their differential contribution and functional relationship with other associative regions remains obscure. We have used event-related functional magnetic resonance imaging to measure human cerebral activity associated with motor cognitive processes in the context of delayed performance of an associative visuomotor task. Movement instruction (specified by visual cues) and motor performance (specified by an auditory cue) were separated by a variable delay period. By manipulating the predictive value of the instruction cue, we distinguished delay-related activity influenced by response probabilities (motor preparation and motor inhibition) from delay-related activity unaffected by the likelihood of providing a motor response (motor intention). We found delay-related activity distributed across a cerebral network involving not only frontal circuitry, but also extrastriate and mediotemporal regions. Areas showing motor intentions and preparatory responses were spatially intermingled. Posterior parietal cortex deviated from this pattern, showing delay-related activity regardless of movement probability, but no specific preparatory responses. These results suggest that posterior parietal cortex and dorsal precentral cortex play different strategic roles in handling associative visuomotor problems. While parietal regions cover a range of potential responses defined by the task setting, precentral regions focus on a likely movement. Temporo-prefrontal regions might incorporate contextual information in the visuomotor process by defining potential and probable responses on the basis of the task contingencies.

"Hey John": signals conveying communicative intention toward the self activate brain regions associated with "mentalizing," regardless of modality.

Successful communication between two people depends first on the recognition of the intention to communicate. Such intentions may be conveyed by signals directed at the self, such as calling a person's name or making eye contact. In this study we use functional magnetic resonance imaging to show that the perception of these two signals, which differ in modality and sensory channel, activate common brain regions: the paracingulate cortex and temporal poles bilaterally. These regions are part of a network that has been consistently activated when people are asked to think about the mental states of others. Activation of this network is independent of arousal as measured by changes in pupil diameter.

Intent of therapy in metastatic breast cancer with isolated ipsilateral supraclavicular lymph node spread--a therapeutic dilemma.

AIMS OF THE STUDY: Isolated ipsilateral supraclavicular lymph nodal (IISCLN) metastasis was considered part of locally advanced breast cancer (LABC) previously but the recent staging system categorised this group to metastatic disease (stage IV). There is no clear consensus on intent of therapy in patients with IISCLN. A retrospective comparative analysis of IISCLN and LABC patients treated with a curative multimodality approach was performed to evolve guidelines on treatment approach in patients with IISCLN spread. METHODOLOGY: A total number of 670 patients with breast cancer treated in the Department of Surgical Oncology, IRCH, AIIMS, during the period between January, 1993 to December, 2000 were studied retrospectively. Out of these 16 (2.4%) patients with cytology proven isolated metastasis to ipsilateral SCLN without any other distant disease and 299 LABC patients were analysed. All patients received neoadjuvant anthracycline based chemotherapy, surgery, postoperative radiotherapy (50 Gy) including supraclavicular fossa. The relapse patterns and survival in both the groups were compared. RESULTS: At a median follow up of 36 months (range 9-72 months) the total recurrence in the IISCLN group was 31% and in the LABC group was 26%, the systemic recurrence was equal at 25% in each group while the locoregional recurrence was 12% and 7% in the IISCLN and LABC groups respectively. The overall survival (OS) was 81% and 91% whereas the disease-free survival (DFS) was 12% and 7% in the IISCLN and LABC groups, respectively. CONCLUSION: Intent of therapy in metastatic breast cancer is palliative. However, patients with IISCLN metastasis have a relatively less aggressive biologic behaviour as compared to patients with spread to other distant sites. Results of the current study shows that the relapse patterns and survival of IISCLN group and LABC are comparable. Hence, despite being staged as metastatic disease, patients with IISCLN spread should be treated with combined modality approach for prolonged survival.

False memories of having said the unsaid: on the importance of a prior intention to speak.

Previous research found that adults who participated in a mock debate often later reported having made a point that they had not. In an extension of that study, the present research not only replicated that finding but also investigated the possibility that such errors were the result of the subjects having been induced into intending to make the unmade point. Specifically, the present results show that the occurrence of the inducing stimulus probably did produce such intentions and that the stimulus was also important to the production of subsequent memory errors. Finally, it is argued that the latter errors are most parsimoniously understood as being caused by distorted memories of the intentions demonstrated by the former result.

[Intention, attitude, subjective norms and perception of control in Spanish adolescents about using Double Dutch].

With adolescence, boys and girls reach their sexual maturity and initiate their sexual encounters. They very frequently adopt risk behaviors as they tend not to use any contraceptive methods. Today's trend in relation to contraceptive methods is to recommend the combined utilization of condom and pill. In comparison to other methods, this last one, known as the Double Dutch (DD) method, has the advantage of preventing pregnancy and STD's, at the same time it increases the efficiency of contraception. Using the Theory of Planned Behavior the purpose of the study was to measure the intention, attitude, subjective norm and perceived behavioral control towards the utilization of the DD method among 15-16 year-old Spanish students and to determine the relationships among these variables. The results indicate that adolescents show favorable intention, attitude and subjective norm toward the utilization of the DD method. They seem to be more worried about pregnancy prevention than about STDs'prevention. Parents seem to be the most important reference people for adolescents, followed by their friends; the results show that at this age, adolescents tend to take into consideration these reference people's opinion.

Impact of a simple health education program about antenatal care on knowledge, attitudes, subjective norms and intention of pregnant women.

The aim of this study was to identify pregnant women practices and to assess the impact of simple health education program on knowledge attitudes, subjective norms and intentions to antenatal care of pregnant women The randomized post-test only control group design was used among 200 pregnant women attending Maternal and Child Health (MCH) centers. One hundred women represented the intervention group who received a simple health eduction program and another 100 women constituted the control group. The study demonstated that nearly the intervention and control group women were equal in sociodemographic characteristics as there was no statistically significant difference in any item studied. It was found that 92% of the intervention group women had high outcome belief that attending Ante-Natal Care (ANC) leads to early detection of dangerous signs of pregnancy compared to 77% of the control group. Also 88% of the intervention group women perceived the importance of early detection of dangerous signs compared to 75% of the control group. Also 95% of the intervention group women had high normative belief that their physician believe that they must go for check up during pregnancy compared to 87% of the control group. The study also clarified that 85% of the intervention group had high motivation to comply with their physicians compared to 73% of the control group. Only 43% of the control group had regular medical check up. The mean scores of indirect attitude, subjective norms, indirect subjective norms, intention and practice were highest for the intervention and control groups among those having more than 12 years of education. The mean score of indirect attitudes increased with increasing level of knowledge among both the intervention and control groups. The mean score of practice was highest among those having positive indirect attitudes among both the intervention and control group. The mean score of practice was highest among those having high indirect subjective norm either in the intervention or the control group.

Treatment of occipital acne keloidalis by excision followed by secondary intention healing.

OBJECTIVE: To demonstrate the principle of excision with healing by secondary intention in the treatment of occipital acne keloidalis, a chronic, inflammatory condition characterized by inflammation and hypertrophic scarring on the posterior aspect of the neck and the occipital aspect of the scalp. SETTING: Academic teaching institution. METHODS: Five consecutive patients with massive (> 10 x 15-cm) occipital keloids, all actively infected and growing, were treated by excision of the lesion deep to the dermis and hair follicles. Aggressive local wound care of the wound followed, with the end point being complete reepithelialization of the region. RESULTS: Complete wound closure took place in all patients within 6 to 10 weeks. No complications or recurrences occurred. Cosmetic results were judged as fair to good. Follow-up ranged from 2 months to 4 years. CONCLUSIONS: Successful treatment of occipital acne keloidalis with direct excision and secondary intention wound healing is demonstrated. While cosmetic results are not optimal, this form of treatment has produced stable scar beds without return of infection or keloids.

Frequent use of medical services. Patient reports of intentions to seek care.

OBJECTIVE: To examine differences between frequent and infrequent users in decisions to seek medical care based on symptom type and duration of experience of the symptom. DESIGN: Retrospective study. SETTING: University family practice clinic. PATIENTS: Fifty-eight of 130 patients returned the completed instruments, a response rate of 45%. MAIN OUTCOME MEASURES: The following two instruments were used: the Intentions to Seek Care questionnaire, which measures intention to see a physician, self-treat, or use medications for 11 different symptoms after three time periods--1 day, 3 days, and 1 week of symptom duration--and the SF-36 Health Survey measure. RESULTS: Initial analyses suggested that patients perceive three clusters of symptoms, which we have labeled as serious, mild, and psychological. Scale scores for these groups of symptoms yielded significant reliability and were used in subsequent analyses. Multivariate analysis of variance comparing source of care, symptom duration, and frequent vs infrequent user groups indicated that both user groups are more likely to seek care from physicians for symptoms that are serious. Analysis of mild and psychological symptoms yielded significant three-way interactions, with frequent users more likely than infrequent users to seek care from physicians at a 1-day duration. Such differences disappear by 7 days. Significantly lower scores were obtained on five of the eight SF-36 Health Survey dimensions for the infrequent users. CONCLUSIONS: Frequent users are more likely than infrequent users to report that they would seek care for minor symptoms. Infrequent users tend to self-treat for 3 to 7 days, after which time physician advice is sought. Efforts to teach patients optimal timing of use for different symptoms may be an effective intervention.

Secondary intention healing. The primary approach for management of selected wounds.

Secondary intention healing is an ancient and well-established method of wound management. Since the advent of various reconstructive techniques, it is often forgotten as a valuable alternative to immediate surgical reconstruction of wounds. In certain situations the cosmetic and functional results of secondary intention healing are equal to the results of more complex reconstructive surgery. We describe a case in which remarkable results were obtained from spontaneous healing of a facial wound.

Factors associated with the intent of firearm-related injuries in pediatric trauma patients.

OBJECTIVE: To examine the characteristics of unintentional and assaultive firearm-related pediatric injuries treated in trauma centers. DESIGN: Comparative analysis of patients 14 years or younger who were admitted to the trauma centers because of unintentional firearm-related injuries (n = 292) vs assaultive firearm-related injuries (n = 457). SETTING: Sixty-eight trauma centers or children's hospitals in the continental United States and Canada that reported data to the National Pediatric Trauma Registry from January 1, 1990, through December 31, 1994. MAIN OUTCOME MEASURES: Frequency distributions of firearm-related injuries in relation to intent and injury circumstances, odds ratios (ORs) on the intent of injury being assaultive, injury severity scales, and in-hospital fatality rates. RESULTS: The frequency of unintentional firearm-related injuries rose in the afternoons peaking between 4 and 5 PM; they predominantly occurred at home (89%). Assaultive firearm-related injuries peaked sharply between 8 and 9 PM and usually occurred on roads or in other public places (63%). About 3 times as many boys as girls were harmed in firearm-related injuries. Given a firearm-related injury resulting in admission to a trauma center, the adjusted OR of it being assaultive was 2.8 (95% confidence interval [CI], 1.6-4.6) if the victim was a girl, 4.9 (95% CI, 3.1-7.8) if the shooting occurred at night, 2.6 (95% CI, 1.6-4.2) if the shooting occurred on a weekday, and 21.1 (95% CI, 9.1-49.4) if the shooting occurred on a road. Injury patterns and severity were similar between patients with unintentional and assaultive injuries. Overall, 19% of the patients sustained head injuries, which contributed to 90% of the in-hospital deaths. CONCLUSIONS: Marked differences in injury circumstances exist between unintentional and assaultive firearm-related injuries among children. The late afternoon hours when many children have come home from school but their parents may still be working have the highest frequency of unintentional firearm-related injuries. The evening peak of assaultive injuries may be related to drug-related and gang-related violence. While it is important to reduce the access of firearms to children, school-based extracurricular and community-based social services should be considered in developing intervention programs.
