Experimental induction of struvite uroliths in miniature schnauzer and beagle dogs.

Urease positive staphylococcal urinary tract infection was experimentally induced in 13 dogs. Eight dogs developed cystic and/or urethral struvite calculi in 2 to 8 weeks. No abnormalities in systemic cell mediated immunity were detected in dogs before or after the establishment of the urinary tract infection. Miniature schnauzers whose ancestors had developed stones seemed to be no more susceptible to experimental urinary tract infection and stone formation than miniature schnauzers or beagles whose ancestors did not develop stones.

Attapulgite in the treatment of acute diarrhoea: a double-blind placebo-controlled study.

A double-blind placebo-controlled study was carried out to assess the therapeutic effects of Attapulgite, a purified hydrated aluminium magnesium silicate, as an additional agent to oral rehydration solutions in 30 adults with mild-to-moderate acute diarrhoea where no bacterial or protozoal causes could be identified. The study subjects were give 600 mg tablets of Attapulgite or placebo in a random order and followed up for 72 hours. Attapulgite was observed to be significantly better than placebo in reducing the severity and duration of diarrhoea in terms of frequency of motion and consistency of stools (p < 0.01), severity of dehydration (p < 0.01) and in reducing the amount of ORS consumed (p < 0.05). Attapulgite was also well-tolerated. Attapulgite, therefore, merits attention as an additional safe agent besides ORS for the treatment of mild-to-moderate acute diarrhoea.

Low-cost device for avoiding bulk susceptibility artifacts in chemical-selective fat saturation MR of the head and neck.

An inexpensive, commercially available travel pillow filled with a suspension of attapulgite provided good suppression of inhomogeneous chemical-selective fat saturation in imaging of the head and neck. This device is assembled from readily available materials at a low cost and compares favorably with a commercially available perfluorochemical-filled device.

Ureteropelvic junction obstruction with a simultaneous renal calculus: long-term followup.

We reviewed 111 patients who presented with simultaneous renal calculi and a ureteropelvic junction obstruction. Of 34 patients with ureteropelvic junction obstruction and a coexisting struvite stone 62% had recurrent calculi. The use of antibiotics significantly affected the incidence of recurrent struvite calculi. In particular, if prolonged prophylactic antibiotics (greater than 3 months) were used 15% of the patients had recurrent stones, compared to 90% if only perioperative antibiotics (less than 15 days) were used (p < 0.001). Patients with ureteropelvic junction obstruction and coexisting nonstruvite calculi were treated by either observation alone (53) or metabolic evaluation with appropriate intervention (24). Metabolic evaluation of patients with nonstruvite calculi revealed that 76% had an identifiable metabolic abnormality, treatment of which significantly decreased the incidence of recurrent renal calculi: 17% of the patients on interventional therapy had recurrent stones compared to 55% treated by observation alone (p < 0.001).

Increased postprandial energy expenditure in obese women after peroral K- and Mg-phosphate.

Energy expenditure (EE) was studied in 15 obese (BMI > 27.5, median 29.2) and 15 lean (BMI < 22.9, median 21.7) healthy, postmenopausal women before and after peroral orange juice with or without K- and Mg-phosphate supplementation. The women, 56-58 years old, were studied in a double-blind, crossover experiment on two separate days. They received 100 ml of orange juice (10 g of carbohydrates) with or without a supplement of K+ (35 mmol), Mg2+ (17 mmol) and HPO4(2-) (39 mmol). Measurements were made before and 30 and 60 min after the stimuli. Intake of juice only had no effect on EE in either the lean or the obese subjects. When the minerals were added, EE (+6.3%; p < 0.001) increased significantly in the obese group with in a maximum 30 min after stimulus, while no such increase was seen in the lean group. The difference between the groups was significant (p < 0.05) and so was the difference in the obese group with or without minerals (p < 0.01). We conclude that an addition of potassium/magnesium-phosphate to glucose increases the postprandial thermogenesis in obese postmenopausal women, but not in lean ones.

Bacteriology of urinary tract stones.

It has been reported that up to half of renal stones and associated urine specimens have been positive on culture, and that up to 50% of such stones contain magnesium ammonium phosphate. In a prospective study using infrared and wet chemical analysis, we found positive cultures in only 7 of 132 renal, 5 of 105 ureteral and 6 of 21 bladder stones obtained surgically and handled with sterility. Of the culture positive calculi only 43% from the kidney, none from the ureter and 50% from the bladder contained detectable magnesium ammonium phosphate. However, magnesium ammonium phosphate was detectable in 20% of renal, 2% of ureteral and 27% of bladder stones with negative cultures. Of the culture positive renal and ureteral calculi 42% were predominantly calcium phosphate and 17% were predominantly calcium oxalate. For culture negative stones 25% and 51% from the kidney, and 15% and 82% from the ureter were composed of predominantly calcium phosphate and calcium oxalate, respectively. Among the culture positive stones, related positive urine cultures were noted in 100% of the renal, 20% of the ureteral and 50% of the bladder cases, compared to 26%, 10% and 27%, respectively, of culture negative calculi. The same organism was found in the stone and urine in only 38% of the cases. The lower frequency of positive urine cultures, of stones with magnesium ammonium phosphate, and especially of culture positive renal and ureteral stones (5%) than in previous reports suggests that stone culture may be of less value than indicated previously, except for bladder calculi and large renal stones, such as the branched type.

Antiinflammatory effect of flavonoids in the natural juice from Aronia melanocarpa, rutin and rutin-magnesium complex on an experimental model of inflammation induced by histamine and serotonin.

The antiinflammatory effects of the anthocyane flavonoids in the natural juice from Aronia melanocarpa and of rutin-magnesium complex, the water-soluble derivative of rutin were studied in comparison with rutin. Two experimental models of inflammation were used. Inflammation of rat hind paw was induced either by 0.5% solution of histamine or by 0.01% solution of serotonin. The swelling of the rat paw was measured oncometrically by a pletismometer. The results showed that the anthocyane flavonoids from the natural juice of Aronia melanocarpa exerted more pronounced effects as compared to rutin in both models of inflammation. The rutin-magnesium complex did not exhibit any antiinflammatory activity against histamine-induced inflammation. Its effects against serotonin-induced inflammation were comparable to those of rutin.

Effects of Tamm-Horsfall glycoprotein and albumin on struvite crystal growth in urine of cats.

Tamm-Horsfall protein (THP) was isolated and purified, using pooled urine collections from clinically normal cats. The effects of feline THP and purified feline serum albumin on urine chemical and struvite crystal variables were compared, using an in vitro crystal growth system and 24-hour samples of pooled urine obtained from 4 cats. Urine samples were placed in wells of cell culture plates, increasing concentrations of ammonium hydroxide were added to adjacent wells to stimulate struvite crystal growth, and the plates were incubated at 37 C. The effect of albumin and THP on crystal growth in sample wells was compared with that in control wells, without protein addition, in the same plate. Crystal growth was assessed by determination of number of crystals and supersaturation index, a scale of crystal habit at different degrees of supersaturation, by use of direct visualization with an inverted microscope. Albumin addition did not have significant effect on either crystal number or supersaturation, compared with controls. Addition of THP significantly (P < 0.05) increased crystal number and supersaturation index. It was concluded that THP significantly (P < 0.05) promoted growth of struvite crystals in feline urine, and thus, may have a role in feline struvite uroliths and struvite urethral plug formation.

Effects of choreito consumption on struvite crystal growth in urine of cats.

The effect of a dietary supplement, choreito, on in vitro struvite crystal growth in feline urine was evaluated. Adult specific-pathogen-free cats (4 females, 4 males) considered to be clinically normal on the basis of physical examination findings and normal results of CBC, serum biochemical analyses, and urinalyses obtained before the beginning of the study were used. Before 24-hour urine sample collections were made, cats were fed a commercial canned diet with 0 or 500 mg of choreito supplement/kg of body weight for at least 2 weeks in a cross-over design with 4 cats/treatment. Filtered urine samples were analyzed for urine pH, specific gravity, osmolality, and urine electrolytes. The struvite activity product was calculated, using a statistical software program that calculates urine saturation. Urine samples were placed in wells of cell culture plates, increasing concentrations of ammonium hydroxide were added to adjacent wells to stimulate struvite crystal growth, and the plates were incubated at 37 C. Crystal growth was assessed by determination of number of crystals and supersaturation index by direct visualization, using an inverted microscope. Supplementation of the diet with choreito (at this concentration) did not change urine pH, specific gravity, osmolality, urine electrolyte composition, or calculated struvite activity product. However, supplementation significantly (P < 0.05) reduced crystal number and supersaturation index. These results indicate that direct observation of struvite crystal formation in whole urine may more accurately predict the effects of treatments to prevent or treat struvite urolithiasis than do calculations based on electrolyte concentration that do not account for the effect of urine macromolecules.(ABSTRACT TRUNCATED AT 250 WORDS)

Evaluation of Musa (Paradisiaca Linn. cultivar)--"Puttubale" stem juice for antilithiatic activity in albino rats.

The fresh juice of Musa stem (Puttubale) was tested for its antilithiatic activity. Zinc discs were implanted in the urinary bladder of albino rats to induce urolithiasis. The stones formed were mainly of magnesium ammonium phosphate with traces of calcium oxalate. Musa stem juice (3 mL/rat/day orally) was found to be effective in reducing the formation and also in dissolving the pre-formed stones.

The influence of applying magnesium preparation on the content of chosen ions of human plasma.

We have analyzed the influence of the administration of the "Calcimag" preparation, containing Mg+2 and Ca+2 ions, on the content of chosen blood plasma ions: Mg+2, Na+, K+, CI-, Ca+2 and phosphorus subject to the age of the volunteers. The preparation was administrated three times a day one tablet. The tendency to the decrease of magnesium concentration in plasma has been noticed subject to the age of the individuals. The administration of magnesium-calcium preparation caused statistically significant increase of magnesium in plasma for persons with hypomagnesemia and slight but characteristic increase for those with normomagnesemia.

Surface charge modifications do not affect the hydrolytic activity of membrane-bound pyrophosphatase of Rhodospirillum rubrum.

The surface charge of the membrane of chromatophores of Rhodospirillum rubrum was modified by two methods: fusion of liposomes with the membrane of the chromatophore by changing the pH and by incubating chromatophores in the presence of cationic or anionic detergents. The hydrolytic activity of membrane-bound pyrophosphatase, on surface charge modified chromatophores, did not change the Km of the enzyme for its substrate (Mg-PPi2-) nor the activation effect of free Mg2+ on the hydrolytic activity. This membrane enzyme is not regulated by surface charge.

[Treatment of staghorn calculi on the basis of composition and structure].

Most staghorn calculi are infection stones composed of struvite and/or carbonate apatite. Sometimes, cystine, uric acid, whewellite and brushite stones also assume a staghorn configuration when located in the kidney. It is very important in stone crushing to know the composition and architecture of the stones. Struvite stones show a concentric laminal structure and are fragile because of wide interstices of crystals and rich organic matrix. These stones usually contain many bacterial colonies in the interstices of crystals and bacteria break out of the stones when they are crushed. Therefore, perioperative administration of antibiotics is necessary for prevention of bacteremia and sepsis. Whewellite stones and uric acid stones have a smooth surface and reveal compact radial and laminal structure especially in the peripheral layer. They are very hard and are refractory to crushing, and the fragments are large. Cystine stones show a compact radial monomineral texture and are very hard. The fragments made by crushing are large. Therefore, combination therapy of stone crushing and irrigation of alkali solution may be useful for treatment of cystine stones as well as uric acid stones. Calcium phosphate stones, hydroxyapatite or brushite stones, are rare and are formed in hyperparathyroidism, Cushing syndrome and renal tubular acidosis. Hydroxyapatite stones are rich in matrix and fragile. Brushite stones reveal radiate structure and are hard. There is no general method of treatment for staghorn calculi but we should select the most reasonable method including open surgery for each case taking into consideration the stone composition, predisposing factors and possibility of stone residue and recurrence.

[The treatment of staghorn calculi].

Between May 1989 and November 1991, 19 staghorn calculi were treated by extracorporeal shock-wave lithotripsy (ESWL) with a Dornier MFL 5000 or Northgate SD-3. The 19 calculi were evaluated. Treatment was with monotherapy by ESWL in 9, combination percutaneous nephrolithotomy (PNL)-ESWL in 9, and nephrostomy in 1. Of the patients, 14 had a cross stent catheter pre-ESWL treatment to improve fragment evacuation. Radiologic follow up in 19 kidneys revealed that 57.9% were stone free. We arbitrarily separated our cases into 3 groups: struvite renal calculi, calcium carbonate calculi and others. Result of stone-free rate was 100% for stones consisting of struvite, and 14. 3% for stones consisting of calcium carbonate. In our opinion, the best indication of monotherapy by ESWL is for staghorn calculi, which consists of struvite, without marked dilation of pelvis and calyces.
