[Ecology of syngamosis in a large scale farming area].

Within recent 20 years, Czechoslovak agriculture has undergone considerable changes due to the introduction of largescale farming. Our contribution presents the results of studies on chickens (Gallus gallus f. dom.) reared traditionally on runs in an area contaminated with the eggs of Syngamus trachea distributed with pheasant faeces. In addition to postmortem examination of 336 chicken tracheae, their faeces collected for one year from the runs were examined, but syngamosis was not found. Studies on small stocks of fowl in gardens close to the fields visited regularly by roosting, Syngamus-infected pheasants disclosed an occasional incidence of syngamosis in the chickens, none in guinea fowl, but frequent infection in young and adult turkeys. Another observation was made on a chicken stock reared inside a pheasantry. Although the incidence of infection of the pheasants with syngamosis was high throughout most of the year, an occasional infection only of the chickens was observed in three months. Postmortem examination of 254 tracheae of free-living birds disclosed that the incidence of syngamosis was highest in the pheasant. - Due to the introduction of large-scale farming, numerous biotopes formerly covered with trees and shrubs, were changed into arrable land. A considerable reduction occurred in the former high stand of partridges (Perdix perdix), while the pheasant (Phasianus colchicus) became widely distributed in agricultural areas. We studied the ecology of the pheasant in various biotopes by means of regular coprological examination without disturbance of the pheasants under consideration. Throughout most of the year the pheasant roosts on trees often growing in moist biotopes. In the morning and evening, the pheasant feeds regularly off the ground close to the roosting trees. From the spring onwards, throughout the summer any autumn, we observed at several sites a marked distribution of pheasants from the trees into the field where they roosted on the ground. The incidence of infection decreased considerably in pheasants, roosting on the ground of dry field biotopes, while in pheasants roosting in moist field biotopes, the incidence of infection increased considerably during the spring and remained at these levels throughout the summer until the autumn. A decrease in the incidence of infection was observed in late autumn. The incidence of infection was low during the winter and the beginning of the spring, but was present in biotopes with trees on which the pheasants from the surrounding areas concentrated. Free-living pheasants in a moist forest surrounded by fields with intensive large-scale cultivation showed a higher incidence of infection with syngamosis than pheasants from a forest pheasantry, in which rearing had to be discontinued for reasons of the death of chicks due to syngamosis. The incidence of infection increases with decreasing altitudes. The partridge (Perdix perdix) suffers less from syngamosis than does the pheasant from the same area...

The V89L polymorphism in the 5-alpha-reductase type 2 gene and risk of breast cancer.

Women with high androgen levels appear to be at increased risk for breast cancer. The 5-alpha-reductase type 2 enzyme (SRD5A2) is an important mediator of local androgen actions. The SRD5A2 gene contains a polymorphism leading to a valine to leucine change in codon 89 (V89L). The Leu allele has been associated with lower SRD5A2 activity and might be protective for breast cancer. At the same time, among breast cancer patients, the Leu allele has been associated with lower prostate-specific antigen expression, indicating poor prognosis. Within a cohort of breast cancer screening participants in the Netherlands (DOM-cohort) we examined whether the V89L polymorphism is associated with the risk and prognosis of breast cancer. We studied 295 postmenopausal breast cancer cases and a randomly selected reference group from the baseline cohort (n = 382). The genotype distribution in the reference group was: VV 52%; VL 40%; and LL 8%. Compared with women with the VV genotype, adjusted breast cancer rate ratios for women with VL and LL genotypes were 1.5 (95% confidence interval = 1.0-2.2) and 1.1 (95% confidence interval = 0.5-2.1), respectively. Compared with breast cancer patients with VV or VL genotypes, those with the LL genotype showed larger tumors (proportion with size > 2 cm is 26 versus 55%, respectively; P = 0.07), a higher frequency of positive lymph nodes (28 versus 55%, respectively; P = 0.09), and a higher tumor-node-metastasis stage (proportion with stage III/IV: 6 versus 25%, respectively; P = 0.04). The LL genotype is also associated with shorter survival than the VV and VL genotypes (P = 0.10). In conclusion, our findings do not provide evidence for an important role of the V89L polymorphism in the etiology of breast cancer. However, in breast cancer patients, the LL genotype may be associated with unfavorable prognosis.

Ice massage. Effects on exercise-induced muscle damage.

AIM: The purpose of this study was to examine the effects of ice massage on the signs and symptoms associated with exercise-induced muscle damage. METHODS: Nine recreationally resistance trained males performed an exercise protocol designed to induce muscle damage on 2 separate occasions; this was performed on the dominant or non-dominant arm in a random cross over design. The protocol consisted of 3 sets of 10 repetitions of single arm biceps curls, at 70% of a pre-determined one repetition maximum (1RM), with the eccentric phase of the contraction extended to 7 seconds. Subjects were also randomly assigned to an ice massage group or control group in the cross over design and received treatments immediately post-exercise, 24 hours and 48 hours post-exercise. 1RM, plasma creatine kinase (CK), muscle soreness (DOMS), limb girth and range of motion (ROM) were measured pre, immediately post, 24 hours, 48 hours and 72 hours post-exercise. RESULTS: Significant time effects were observed for all dependent variables (p<0.05), though no significant group effects were observed. A group by time interaction was found for CK (p<0.05), which at 72 hours post-exercise was significantly lower in the ice massage group (p<0.05). CONCLUSION: These results indicate that although ice massage reduces the appearance of CK it has no other effect on signs and symptoms associated with exercise-induced muscle damage.

[Causes of urinary tract infection in pregnant women].

Data of etiology characteristics of uroinfections with pregnant women are presented treated at University Hospital of Obstetrics and Gynecology "Maichin dom" in three periods. Gram negative species of microorganisms (E. coli, proteus, Klebsiella) are the agens of uroinfections in 95.4% of cases in the period 1988/89. After 10-year period gram (-) are the uroinfection in 66.1% of the cases. The growing etiologycal importance of Gram positive species of microorganisms (S. aureus, Enterococcus, GBS) is 33,9% in the period 1997/98 and it is preserved in june 2001/may 2003 too. In cases of long lasting hospitalization or after repeated treatment with antibiotics the probability grows that the agents of uroinfections be polyresistant strains. In the period june 2001/may 2003 in 7% of the cases polyresistant E. coli are isolated, in 2.6% MRSaureus, in 7.6% Ampicillin - R Enterococcus spp. That suggest exact identification of the agen and therapy according to sensitivity to antibiotics.

[The significance of parameters of the acid-base status, blood gas and blood lactate level for the adequate resuscitation and prognosis in newborns with very low birth weight].

UNLABELLED: The aim of this prospective study was to optimize the resuscitation guidelines for VLBW and ELBW premature newborns, to improve their life prognosis and to minimize the permanent complications of the perinatal asphyxia. RESULTS: The newborns were divided in 2 groups: main group--28 ELBW and 55 VLBW infants born from 01.10.01 to 30.06.02; and control group--52 ELBW and 78 VLBW infants, born in year 2000 in "Maichin dom". More intensive resuscitation was given to the VLBW newborns in the mean group--58% of them were intubated and ventilated in Delivery room (DR) infants compared with 40% from the controls. In the ELBW groups the rate of the assisted ventilation (AV) remains high (85% and 82%) but more often we insufflated Surfactant for prophylaxis of the RDS (71% compared with 52% in the controls). This, as well as the precisely monitoring of the early postnatal adaptation--pulsoxymetry, parameters of acid-base status (ABS) and homeostasis, resulted in significantly reduction of the mortality rate in ELBW infants from 46% to 18%. We found a reduction of the severe neurological injuries and high degree retinopathy of prematurity too. We evaluated the lactate values and the ABS parameters in blood from umbilical artery (u.a.) and from arterial blood 1 hour after birth. We found out that the low lactate levels in u.a. didn't correlate with the degree of the metabolic acidosis (pH and BE). Correlation between high lactate levels in u.a, low pH and BE were found predominantly in newborns with significant intrauterine retardation and chronic placental insufficiency. Levels > 5.0 mmol/l 1h after birth and the persistence of the metabolic acidosis despite an adequate resuscitation determinated in principle bad outcome and severe complications. CONCLUSIONS: The adequate DR resuscitation under strict monitoring of the cardio-pulmonary adaptation and the prophylactic Surfactant application improve the survival rate and the prognosis of the ELBW newborns. The persistence of high lactate levels in combination with a metabolic acidosis and intrauterine retardation are bad prognostic criteria for outcome.

[The corse of pregnancy and delivery in patients with surgicaly corrected tetralogy of Fallot].

Twenty-eight pregnancies in 16 patients with surgically corrected tetralogy of Fallot between 1997 and 2002 at the SBALAG "Maichin dom", Sofia, were reviewed. Most of the patients were well and asymptomatic after surgery, excluding one patient who was functional class III. The patients had a total of twenty-one successful pregnancies and deliveries. There were seven abortions (33%) and two small-for-date babies (12%). None of the infants had any cardiac anomalies. The pregnancies were relatively well tolerated and there were no life-threatening cardiac complications. For a patient with some residual abnormalities after surgical correction of tetralogy of Fallot, it is important to reassess the cardiac state before pregnancy and, if once pregnant, to anticipate the possible occurrence of complications during labor. Compared to the general obstetric population, more women in the study group underwent instrumental vaginal delivery and probably best option was elective ventouse or forceps. In conclusion despite potential difficulties and complications associated with corrected tetralogy of Fallot, careful cardiac and obstetric management resulted in good maternal and fetal outcomes.

[Role of glycemic control and incidence of spontaneous abortions pregnant in women with type 1 diabetes mellitus].

UNLABELLED: Pregnant women with diabetes mellitus type 1 and poor metabolic control are with high frequency of spontaneous abortion. PURPOSE: To determine the correlation between the poor glycaemic control of diabetes in early pregnancy and the risk of spontaneous abortion. DESIGN OF STUDY: A prospective study over 116 women with type 1 diabetes mellitus who have been hospitalized in SHATOG "Maichin dom" during the period: January 1998-January 2004 have been done. The pregnant women have been divided in two groups. Pregnant women with pre-planned pregnancy n = 44 and pregnant women with unplanned pregnancy n = 72. The levels of pre- and postprandial blood glucose and those of glycosylated hemoglobin Hb A1c at first trimester have been measured. RESULTS: In 20 of all pregnant women (17.2%) spontaneous abortion have been observed. In those women with abortion the initial glycaemia has been significantly higher-preprandial glucose is 10.2 +/- 3.4 mmol/l and postprandial glucose is 12.2 +/- 4.2 mmol/l in contrast to those in women without abortions--preprandial glucose 6.6 +/- 1 mmol/l postprandial glucose is 7.9 +/- 1.9 mmol/l. The mean values of Hb A1c at first trimester have been higher in those women with abortions--10.1% +/- 1.5 in contrast to those without abortions 7.0% +/- 0.7. CONCLUSION: The pregnant women with diabetes type 1 and higher initial levels of blood glucose and glycosylated hemoglobin in the first trimester of pregnancy have a higher risk of spontaneous abortions. The rate of spontaneous abortions in diabetic women with pre-planned pregnancy and optimized metabolic control before conception is lower.

Synthetic- and bio-polymer use for runoff water quality management in irrigated agriculture.

Low concentrations of synthetic- or bio-polymers in irrigation water can nearly eliminate sediment, N, ortho- and total-P, DOM, pesticides, micro-organisms, and weed seed from runoff. These environmentally safe polymers are employed in various sensitive uses including food processing, animal feeds, and potable water purification. The most common synthetic polymer is anionic, high purity polyacrylamide (PAM), which typically provides 70-90% contaminant elimination. Excellent results are achieved adding only 10 ppm PAM to irrigation water, applying 1-2 kg ha(-1) per irrigation, costing 4 dollars - 12 dollars kg(-1). Biopolymers are less effective. Using twice or higher concentrations, existing biopolymers are approximately 60% effective as PAM, at 2-3 times the cost. A half million ha of US irrigated land use PAM for erosion control and runoff protection. The practice is spreading rapidly in the US and worldwide. Interest in development of biopolymer surrogates for PAM is high. If the supply of cheap natural gas (raw material for PAM synthesis) diminishes, industries may seek alternative polymers. Also "green" perceptions and preferences favor biopolymers for certain applications.

[The frequency of mild and severe fetal malformations in diabetic women with high values of glycosilated hemoglobin in early pregnancy].

PURPOSE: To evaluate the correlation between maternal hyperglycemia in early pregnancy and the risk of fetal abnormalities in pregnant women with type 1 diabetes mellitus. STUDY DESIGN: A retrospective study over 124 pregnant women with diabetes mellitus type 1 hospitalized in High Risk Pregnancy Department--SHATOG "Maichin dom" has been done from January. 1998 to January 2004. The diabetic pregnant women were divided in two groups: first group pregnant women without malformations n = 105 and second group pregnant women with malformations n = 19. The pregnant women with fetal malformations were divided into two subgroups: with major malformations n = 13 and with minor malformations n = 6. The diabetic pregnant women were divided in classes according to Whites Classification: Class B - 38, Class C - 35; Class D - 39 and Class R/F - 12. The values of preprandial glucose, postprandial glucose and glycosilated hemoglobin has been measured at 13 week of gestation. RESULTS: 104 pregnancies of total 124 pregnancies were without abnormalities. The fetal malformations were observed in 19 (15.3%) of total 124 pregnancies. The rate of major abnormalities were - 13 (10.4%) and minor abnormalities were - 7 (5.6%). The highest rate of abnormalities there has been within the complicated diabetic women of class D - n = 7 (17.9 %) and class R/F n = 3 (25%). The initial values of preprandial glucose 9.54 (SD +/- 3.59) mmol/l and postprandiai glucose 10.52 (SD +/- 1.81) mmol/l between the women whit pregnancies with abnormalities were significantly higher then those values of preprandial glucose 7.39 (SD +/- 2.82) mmol/l (P - 0.021) and values of postprandial glucose 10.52 (SD +/- 1.81) mmol/l (P = 0.014) between the women without fetal malformations. The mean values of glycosilated hemoglobin were significantly higher HbA 1 c = 9. 01% (SD +/- 1.53) in pregnancies complicated with malformations than those values measured in pregnancies without fetal malformations 8.06% (SD +/- 1.64, P = 0.022). A positive correlation between the observed abnormalities and metabolic control in the early pregnancy exist. The values of Hbeta A1-c is significantly higher Hbeta A1-c - 9.9% (SD +/- 1.2) in pregnancies complicated with fetal malformations than those measured in pregnancies without malformations. Hbeta A1-c 8.2% (SD +/- 1.5) n = 125. Significant differences in the value of Hbeta A1-c between pregnancies with mild and those with severe abnormalities have not been established. A correlation between the levels of Hbeta A1-c in early pregnancy and the rate of the observed abnormalities exist. Within the values of Hbeta A1-c < 7.9%, the rate of malformations is 6.9%, Hbeta A1-c > 8.0% < 10%, the rate of malformations is 19.0% and within the values of Hbeta A1-c > 10%, the rate of the observed abnormalities is 31.5%. A logistic regression between the higher values of postprandial glucose and Hbeta A1-c values and the relative risk of congenital malformations has been observed. The relative risk is evaluated by odds ratio (OR) When the levels of Hbeta A1-c rise with 1% the relative risk of congenital malformations is evaluated by odds ratio OR = 2.02 (limited in 1.46 - 2.81 by 95% conf. interval) and when the levels of postprandial glucose rise with 1 mmol/l the relative risk OR = 1.21 (limited in 1.06 - 1.37: 95% conf. interval). CONCLUSION: Fetal abnormalities are more frequent in pregnant women with long lasting diabetes complicated with vasculopathy. Fetal abnormalities are associated with higher levels of Hbeta A1-c in the first trimester of pregnancy. In diabetic women who planed their pregnancy an optimal metabolic control must been established.

[Cesarean section for fetal distress--is the decision interval for delivery significant?].

UNLABELLED: The recommended Interval between the decision to perform emergency cesarean section and the delivery itself is 30 minutes. There is not enough clinical evidence in maintenance of this opinion. AIM: to study the interval from the decision of the cesarean section for fetal distress to delivery itself, as well as to analyze fetal and mother's outcome. METHODS: Prospective 13-month-study for the period 2003-2004 in Delivery room "Majchin Dom" included observation of 125 women. Cesarean section is classified in 3 Categories--1st--deemed highly hazardous for mother or/and fetus (crush); 2nd--not directly hazardous for mother or/and fetus (emergency); and 3dt--not direct mother or/and fetal distress, but the delivery is necessary in a short period of time (urgency). The Interval and the Categories are comparable with the presence or absence of fetal distress (Apgar < 7, pH < 7.20). RESULTS AND DISCUSSION: Positive results are found in the 1st category when the C-section is performed in 30 minutes. There are significant differences between the three Categories--the percentage of fetal distress is growing up comparable to prolongation of the time interval. Prolongation of the time interval for more than 60 minutes lead to poor fetal outcome.

Fluorescence characteristic changes of dissolved organic matter during municipal solid waste composting.

Dissolved organic matter(DOM) of municipal solid waste (MSW) consists of minerals, water, ash and humic substances, and is known to enhance plant growth. In this study, inoculating microbes (ZJ, MS) were used in municipal solid wastes composting, and composting implemented a industrialized technology. During composting, dissolved organic matter was extracted from the compost and purified. The spectral characteristics of dissolved organic matter was determined by fluorescence emission, excitation, and synchronous spectroscopy. Fluorescence emission, excitation, and synchronous spectra characterized by different relative fluorescent intensities and peaks over time. Fluorescence spectra were similar to that of fulvic acid in sewage sludge, indicating the presence of dissolved organic matter with aromatic structures and a high degree of molecular polymerization. Compared with the controls with no microbial inoculation, the microbe-inoculated treatments exhibited the increase of aromatic polycondensation, in the following order: MS + ZJ > ZJ > MS > CK.

[Post-operative complications following elective and emergency caesarean delivery].

AIM OF THE STUDY: 1) To analyze the frequency of complications after elective/planned [PCS] and emergency Cesarean section [ECS]; 2) To compare the types of complications in the two evaluated groups; 3) To analyze the possible risk factors for complications after elective and emergency procedures. METHODS: The study was retrospective, hospital-based one. Data regarding complications following Cesarean section [CS] that demanded transfer of the patients to The Clinic of High Infectious Risk, State University Hospital "Maichin Dom", Sofia and prolonged hospital stay (more than 7 days after the operation) were analyzed. The incidence of complications in 574 consecutive PCS and in 292 ECS was calculated. The type of the following complications was compared in the two groups: uterine infections (endo/mio/ metrophlebitis), wound infection, subfascial hematoma, residua, sepsis, pelvic thrombophlebitis. Statistical evaluation of the results was performed by Student's t-test with p<0.05 considered statistically significant. RESULTS: In 574 PCS the frequency of postoperative complications was 1,4% while in 292 ECS it was 2,05% (p>0.05). There was not significant difference in the distribution of the different types of postoperative complications in 34 cases with PCS and 33 cases with ECS. The percentage of patients with previous CS was significantly higher in the complicated cases with PCS compared to that with ECS. The two studied groups do not differ significantly regarding the type of skin incision, operator's qualification, blood loss, drainage of the subfascial space, accompanying diseases. CONCLUSIONS: Cesarean section constitutes a major surgical procedure characterised with morbidity even if performed as a planned procedure. The risk of complications seems to be higher in cases of repeated CS. Patients that demand CS without medical indications have to be informed and be aware of these facts.

[Attitude of pregnant women towards prenatal screening for chromosomal and structural fetal anomalies between 11-14 weeks of gestation].

AIM: The aim of the present survey is to analyze the attitude of pregnant women towards the possibilities of prenatal screening and diagnosis between 11-14 weeks of gestation (w.g.). MATERIAL AND METHODS: Overall 109 pregnant women, hospitalized in SBALAG "Maichin dom", received written information about the possibilities for prenatal screening and diagnosis between 11-14 w.g. and 16-20 w.g., as well as a comprehensive description of the potential advantages and disadvantages of the different approaches/methods of screening/diagnosis. Consequently, the patients were asked to answer 13 closed and semi-closed questions in an anonymous enquiry. RESULTS: Overall 97,2% (106/109) of the women agreed to perform an ultrasound examination for fetal nuchal translucency measurement as a screening tool for chromosomal anomalies between 11-14 w.g. Furthermore, 82,6% (90/109) preferred the option of first trimester biochemical screening, rather than screening in the second trimester. On the other hand only 62,4% (68/109) accepted chorionic villous sampling between 11-14 w.g. in comparison to 83,5% (91/109) who agreed to perform an amniocentesis between 16-20 w.g. CONCLUSION: The vast majority of women prefer prenatal screening programs to be scheduled in the first, rather than in the second trimester. However, most women would still choose for a relatively safer second trimester invasive procedure instead of the option of earlier prenatal diagnosis.

[Optimization of prenatal screening for group B streptococcus].

The prophylaxis of endogen B streptococcal infections (GBS are part of the normal flora of the gastrointestinal and genital tract) in the presence of predisposed conditions for development of experience for every delivery (tissues' damage, time of delivery, loss of blood, obstetrics manipulations) has been a complicated objective. In the University Hospital of Obstet. Gynecol. "Maichin dom" have been delivered annually on the average 0,6-0,9/1000 born alive with GBS sepsis. The medium Granada shortens the duration for detection of GBS by 24 hours. On entrance of a pregnant woman the direct inoculations of cervico-vaginal secretions and the fast positive reactions of GBS can assist the therapeutical behavior of the mother as well as the newborn.

[Doppler velocimetry in patients with preeclampsia].

UNLABELLED: Doppler velocimetry is a fast and convenient method for monitoring of fetal well being in high risk pregnancies. Doppler velocimetry is more specific and sensitive method of fetal surveillance than cardiotocography. The study's objective is to prove Doppler's effectiveness in predicting perinatal outcome in preeclamptic pregnancies. A retrospective study is designed, including pregnant women, diagnosed with preeclampsia, admitted to Maternity hospital "Majchin dom" for ten years. Doppler study was made and results were correlated to Apgar score, umbilical artery pH and base excess of the neonate at delivery. A correlation between RI of umbilical artery, uterine arteries and Apgar score at 5th minute was demonstrated. In severe cases correlation between RI of umbilical artery and neonatal pH was found. CONCLUSION: Doppler velocimetry is an effective method of fetal surveillance in pregnancies, complicated by preeclampsia

[Effectiveness of antihypertensive medications in patients with preeclampsia].

The presented study is retrospective and prospective in design. It is based on the medical records of Maternity hospital "Majchin dom" and planned studies during the period 1994-2005. 544 preeclamptic patients with singleton pregnancies were included. 334 (61.3%) of them were diagnosed with mild preeclampsia and 210 (38.7%) had severe preeclampsia. Blood pressure was monitored at least four times daily, depending on the severity of the case with mechanical manometer. In special cases cardiopulsometry was applied. In order to compare the effectiveness of different antihypertensive medications, administered orally, either alone, or in combination (no antihypertensive medication, Methyldopa, Nifedipin, beta-blockers and combinations between them), patients were divided in seven groups. The mean values of systolic and diastolic blood pressure, measured before delivery were compared. No statistically significant difference between the groups was noticed. We were not also able to find statistically significant difference in blood pressure measurements before delivery, between patients with severe preeclampsia, treated with parenteral Nepresol or Chlofazolin.
