The use of phosphorus 32 in studies on Plasmodium gallinaceum. I. The development of a method for the quantitative determination of parasite growth and development in vitro.

A quantitative method for following parasite growth in vitro is described. The method is based on the determination of the degree of incorporation of phosphorus into the nucleic acids, in particular desoxypentose nucleic acids, of the parasitized cell. Phosphorus incorporation is measured by utilizing the radioactive isotope, P(32). The degree of phosphorus incorporation into DNA was found to be closely correlated with estimates of parasite growth based on stained film examinations of intact cell preparations. The isotope was likewise found to be incorporated into the DNA of preparations in which the host cell had been disrupted by lysis and the extent of this incorporation also paralleled the impressions gained from smear examinations. It is suggested that this method provides a sensitive and objective method of studying parasite growth.

Nutritional effects on response of children in developing countries to respiratory tract pathogens: implications for vaccine development.

Malnutrition and respiratory disease commonly occur together, and the immunologic defects caused by malnutrition may affect clinical responses and the impact of vaccines. These defects include impaired maturation and function of T lymphocytes, diminished secretory immune responses, poor humoral antibody responses to polysaccharide antigens, deficient complement activity (especially the alternative pathway), and impaired antimicrobial mechanisms by phagocytic cells. In addition, protein-energy malnutrition often leads to very high serum levels of IgE as a result of altered T cell regulation of IgE production and may contribute to IgE-mediated pathologic responses to respiratory syncytial virus or parainfluenza virus infection. Correction of these deficits may improve host response to infection and enhance vaccine-related protection. Both vitamin A and iron deficiency states have been epidemiologically associated with increased morbidity due to respiratory infection. The impact of preventive therapy with either vitamin A or iron is still uncertain, but such therapy has the potential for diminishing the incidence of respiratory infections and their consequences.

Developing a conscious sedation program: from policy development through quality improvement.

Conscious sedation (CS) has rapidly become a popular method of anesthesia, administered to decrease the pain and anxiety associated with operative or invasive procedures while avoiding the potential complications of general anesthesia. Intravenous conscious sedation (IVCS) is most frequently administered, but the patient can also receive CS by oral, intramuscular, or rectal routes. Although the level of sedation using CS is not as deep as general anesthesia, the patient is potentially at risk for respiratory depression. CS provides many benefits. It also places an increased burden on healthcare providers to ensure safe and competent care when this form of anesthesia is used.

Models for psychosocial services in the developed and developing world.

This paper is based on the assumption that psychosocial services can add important mental and social dimensions to the traditional concepts of help and medicine. Peer support which has been established world-wide for people with haemophilia and their families is invaluable and irreplaceable. However the role of the professional psychosocial worker is underutilized in most countries, mostly because of lack of financial resources. This paper will argue that skilled workers can assist individuals and groups to reach their full potential in a variety of creative and non-threatening ways. Psychosocial workers have strict codes of conduct and requirements relating to accreditation to ensure accountability in their work. They can work in a variety of individual roles and have a wide range of individual mandates within work settings.

Antibody repertoire development in fetal and neonatal piglets. XI. The thymic B-cell repertoire develops independently from that in blood and mesenteric lymph nodes.

The origin and function of thymic B cells is currently unresolved. In the present study we compared V(H) gene repertoire diversification in >3500 cloned VDJs (from 11 animals at three time-points, using three to five animals per time-point) that were expressed with immunoglobulin (Ig)M, IgD, IgG, IgA and IgE in thymus, mesenteric lymph nodes (MLN) and peripheral blood B cells (PBB) of newborn piglets and 5-week-old isolator piglets maintained germfree (GF) or colonized with Escherichia coli. The results showed that the repertoire expressed with IgM, IgD, IgG and IgA in PBB and MLN remained polyclonal, undiversified and unselected in piglets maintained GF for 5 weeks, that age and colonization resulted in significant repertoire diversification of IgG and IgA in the MLN and of IgG in blood, that the thymic B-cell repertoire was polyclonal, unaffected by colonization and showed no clonal selection in any isotype, and that the thymic IgA and IgE repertoires were more diverse at birth than the repertoire of any isotype in MLN or PBB. IgD was seldom recovered from the PBB of newborn piglets or at any time-point in thymus, but was recovered in the MLN of all 11 animals examined. The IgD and IgM repertoires in all tissues remained polyclonal and unselected, although V(H) usage by IgD transcripts did not always parallel that of IgM in the same tissue. Therefore, isotype-switched B cells in the thymic medulla cannot be accounted for by immigration of B cells diversified by colonization of the gut, and thymic B cells undergo switch recombination and repertoire diversification before birth without clonal selection.
