[Evaluation of the liver function in workers of the vinyl resin plant].

187 workers exposed to vinyl chloride (VC) were examined. The control group consisted of workers of the same plant but not exposed to vinyl chloride. In all workers biochemical liver function tests were performed. The results showed no damaging influence of VC on the liver cells membrane. A slight increase of the aminotransferases (ALAT, AspAT) activity was noticed only in alcohol drinking persons. The most important changes of the glutamyltranspeptidase activity were found. Increased activity of this enzyme was noticed in 84 persons. The results suggest the induction of the glutamyltranspeptidase (GGTP) activity by VC after exclusion of the effects of other agents like cholestase, barbiturates, ethyl alcohol, as well as the effects of carcinogenesis.

Microsomal liver function declines steadily after kidney grafting: a three to five year follow-up.

We have previously shown that the functioning hepatocyte mass (galactose elimination capacity, GEC) and microsomal liver functions (non-renal clearances of unbound prednisolone and cyclosporin A) are impaired in renal allograft recipients (N = 28) one month and one year after successful transplantation. To assess the natural history of these hepatic functional derangements, we reinvestigated 21 patients with stable renal function three to five years following grafting. GEC remained with 6.07 +/- 0.86 mg/min x kg significantly (P less than 0.001) below that in healthy controls (7.52 +/- 0.78 mg/min x kg), but did not significantly change during follow-up (5.93 +/- 0.96 and 6.26 +/- 0.94 mg/min x kg at 1 year and 1 month, respectively). In contrast, the non-renal clearance of unbound prednisolone declined steadily during follow-up averaging 4.98 +/- 0.71 ml/min x kg at three to five (compared to 5.83 +/- 1.51 and 6.80 +/- 1.73 ml/min x kg at one year and one month, respectively). These values were lower (P less than 0.01) than those observed in healthy control subjects (7.56 +/- 1.59 ml/min x kg). The total body clearance of cyclosporin A decreased similarly with time averaging 4.5 +/- 1.2 ml/min x kg at three to five years (compared to 4.9 +/- 1.2 and 5.9 +/- 2.1 ml/min x kg at 1 year and 1 month, respectively).(ABSTRACT TRUNCATED AT 250 WORDS)

Reproducibility of double indicator dilution measurements of intrathoracic blood volume compartments, extravascular lung water, and liver function.

STUDY OBJECTIVE: Arterial thermal dye dilution (TDDart) with the COLD system (Munich, Germany) allows measurement of cardiac index (CI), partial blood volumes, lung water, and liver function. The aim of the study was to determine agreement of TDDart measurements with pulmonary artery thermal dilution measurements (TDpa) and to assess the reproducibility of TDDart parameters. DESIGN: Prospective study. SETTING: ICU of a university hospital department of cardiac surgery. PATIENTS: Thirty consecutive patients after coronary artery bypass grafting. MEASUREMENTS AND RESULTS: Triplicate measurements of TDDart parameters were performed 1, 3, 6, 12, and 24 h postoperatively and coefficients of variation (CVs) were computed. At the 3-h point, additional fivefold TDDart measurements were done and compared with TDpa measurements. The coefficient of correlation for CI from TDDart vs TDpa was 0.96 (p<0.001), and the mean difference was 0.16 L/min/m2 (2.4%). The CVs of the TDDart and TDpa CI measurement were 7.2% and 5.9%; the CVs of other TDDart parameters were 4.6% (cardiac function index), 8.3% (global end-diastolic volume), 7.0% (intrathoracic blood volume), 7.6% (total blood volume), 7.4% (right ventricular end-diastolic volume), 7.4% (right heart end-diastolic volume), 11.3% (left heart end-diastolic volume [LHEDV]), 12.0% (right to left heart volume proportion [R/LHV]), 8.8% (pulmonary blood volume), 10.8% (extravascular lung water), 16.4% (plasma disappearance rate of dye), and 19.8% (dye clearance). The CV did not depend on Glasgow coma scale or on body temperature. CONCLUSION: The CVs of LHEDV and R/LHV are influenced by asynchronous TDDart and TDpa variation. The CVs of plasma disappearance and dye clearance are increased as the half-life of the dye is longer than the measurement sequence. All other parameters derived from TDDart and TDpa show a clinically sufficient reproducibility.

The effects of arterialisation of the portal stump on liver function and hepatic haemodynamics in cirrhotic rats with a portacaval shunt.

Liver blood flow (xenon-133 clearance method) and wedged hepatic venous pressure were studied in cirrhotic rats immediately after and 3 weeks following portacaval shunting (PCS), PCS and arterialisation of the portal stump with the left gastric artery (PCS-ART) or sham operation. Liver weight and function were compared 3 weeks after operation. Liver blood flow and wedged hepatic venous pressure were significantly reduced immediately after and 3 weeks following PCS. PCS-ART maintained liver blood flow and wedged hepatic venous pressure within the pre-operative range and prevented the liver atrophy and deterioration in liver function observed in rats with PCS. The results suggest that arterialisation of the portal vein with an artery which does not significantly increase sinusoidal pressure may be of benefit in preventing the early undersirable sequelae of PCS in man.

The aminopyrine breath test for the evaluation of liver function in alcoholic patients: drug pharmacokinetics and environmental factors.

Drug pharmacokinetics and environmental factors contribute to the selection of an ideal drug substrate for the determination of liver function via the carbon dioxide breath test. An ideal drug should be rapidly absorbed, and have an hepatic extraction ratio between 0.2 and 0.5. Its metabolism should not be induced by ethanol or be affected by cigarette smoking. The relative promise of caffeine and methacetin are compared to aminopyrine.

Effects of troleandomycin and josamycin on thyroid hormone and steroid serum levels, liver function tests and microsomal monooxygenases in healthy volunteers: a double blind placebo-controlled study.

Serum TSH levels are moderately but significantly (P ANOVA: 0.05) decreased by troleandomycin (T; 1 g bid over a 10-day period) compared with josamycin (J) (same doses) and placebo (P) in healthy volunteers. T also significantly increases serum estradiol concentration (P ANOVA: 0.03). This effect may be related to a T-induced inhibition of some P450 monooxygenase isoenzymes and more specifically P 450 NF, determined in our study by a decrease in urinary excretion of 6-beta-hydroxy-cortisol. Troleandomycin and josamycin both show poor upper GI tolerance. Liver enzymes (SGOT, SGPT, alkaline phosphatase and gGT) are significantly altered by T compared with J and P (P ANOVA: 0.007, 0.001, 0.09 and 0.04 respectively). After J, liver function tests are very close to control values (placebo). Liver enzymes are significantly more altered by T than by J (P 0.004, 0.001 and 0.06 for SGOT, SGPT and gGT respectively). Using 6 volunteers in a latin-square designed study, some established effects of oral macrolides were confirmed (poor upper GI tolerance; liver toxicity of T). Some other effects of T were also elicited, which were either unknown (decrease in serum TSH) or expected but which had not previously been assessed in man (increase in serum estradiol; decreased urinary excretion of 6-beta-hydroxy-cortisol).

Occupational lead exposure and amino acid profiles and liver function tests in industrial workers.

The aim of the present study is to determine the effect of blood lead on the plasma levels of amino acids and serum liver enzymes in industrial workers in United Arab Emirates (UAE). This comparison study consisted of 100 industrial workers (exposed) and 100 non-industrial workers (non-exposed), matched for age, sex and nationality selected from Al-Ain, Abu-Dhabi Emirates. Industrial workers had higher proportion (19%) of smokers than non-industrial workers (11%) which was not considered to be statistically significantly different. Industrial workers had significantly higher mean of blood lead level (77.5 +/- 42.8 miccrog x dl(-1)) than non-industrial workers (19.8 +/- 12.3 microg x dl(-1)). The amino acid analysis showed higher values among industrial than non-industrial workers for histidine, isoleucine, leucine, threonine, lysine, valine, methionine and arginine (essential amino acids, (p<0.0001). Ornithine, taurine, glutamic acid, serine, glycine, proline and alanine (non-essential amino acids) showed significantly higher values in industrial when compared with the non-industrial workers (p<0.0001). Plasma liver function test, cardiac enzymes and renal function test were carried out on industrial and non-industrial workers. The results revealed alkaline phosphatase (p=0.012) and lactate dehydrogenase (p=0.029) were significantly higher in industrial than in non-industrial workers. On the basis of this study, it can be concluded that a substantial difference in amino acid profiles, blood lead and LFT between exposed and non-exposed was found. These results might be related to lead exposure and might have affects on the kidneys or liver.

Lipid modulation and liver function tests. A report of the Program on the Surgical Control of the Hyperlipidemias (POSCH).

BACKGROUND: Statin drugs are known to cause dose-dependent abnormalities in liver function tests (LFTs), with elevations three times the upper limits of normal of the aminotransferase enzymes in up to 2.5% of patients on the highest prescribable doses. The Program on the Surgical Control of the Hyperlipidemias (POSCH) trial employed no hypocholesterolaemic drugs and used a surgical procedure, partial ileal bypass, as the intervention modality. METHODS: Serum total bilirubin, alkaline phosphatase and serum glutamic-oxaloacetic transaminase (SGOT) (equivalent to aspartate aminotransferase [AST]), were the LFTs obtained in POSCH at baseline, 3 months, annually for 5 years, and at 7 or 10 years postrandomization. RESULTS: Abnormal values were found for total bilirubin in seven of 416 control group (CG) patients (1.68%) and in 34 of 373 intervention group (IG) patients (9.16%) (P = 0.001); for alkaline phosphatase, in 28 of 378 (7.41%) and in 41 of 326 (12.58%) (P = 0.0214); and for SGOT, in 102 of 412 (24.76%) and in 161 of 372 (43.28%) (P = 0.001). Values twice the upper limit of normal occurred in 1 CG and 1 IG patient for total bilirubin and for alkaline phosphatase, and in 11 CG and 7 IG patients for SGOT (NS). Values three times the upper limit of normal did not occur in any patient for total bilirubin or alkaline phosphatase, and occurred in 3 CG and 5 IG patients for SGOT (NS). CONCLUSIONS: In POSCH, the IG demonstrated statistically significant mild increases for total bilirubin, alkaline phosphatase and SGOT levels, with no significant increases in values twice or greater the upper limits of normal.

Acute basophilic leukemia presenting with abnormal liver function tests and the absence of blast cells in the peripheral blood.

Acute basophilic leukemia is an uncommon form of acute leukemia, rarely occurring as a de novo disease. We describe a case of de novo acute basophilic leukemia occurring in a 47-year-old man who presented with abnormal liver function tests in the absence of leukemic infiltration of the liver. We postulate that this presentation occurred as a consequence of pathophysiological features of the malignant basophilic blast cells.

Assessment of liver function in primary cultures of hepatocytes using diethoxy (5,6) chloromethylfluorescein and confocal laser scanning microscopy.

A method is presented which can be used to assess the function of hepatocytes in complex culture configurations without disrupting the integrity of the cell environment. It utilises a fluorescent probe for cytochrome P450 dependent mixed function oxidase (MFO) activity diethoxy (5,6) chloromethylfluorescein, and confocal laser scanning microscopy. The MFO activity of individual cells in primary cultures of intact hepatocytes can be detected in situ, and quantified by image analysis. This may be a valuable means of monitoring the effect of culture conditions on the function of bioartificial liver devices, and could be used to assess the need for effective oxygenation of cells, the influence of shear stress and of exposure to patient serum during clinical use.
