Nurse practitioner management of common respiratory and genitourinary infections, using protocols.

Included in this study were all patients with symptoms of respiratory tract infection and female patients with symptoms of urinary tract and vaginal infections who sought care from a hospital-based outpatient department walk-in unit which was operated in two different modes: experimental (the "nurse-protocol mode")-in which a nurse practitioner guided by a protocol initially evaluated all such patients and independently managed many patients- and traditional-in which only physicians managed patients. Safety, effectiveness, efficiency, and cost of care rendered for these tracer conditions in the two modes were compared. No serious illnesses were overlooked by practitioners in either mode. Eighty-six percent of patients in the nurse-protocol mode and 73 percent of patients in the traditional mode reported good symptom relief. Patients reported equivalent satisfaction with care. The time physicians spent managing patients with these complaints was reduced by 91 percent from 15.5 minutes to 1.4 minutes per patient. Personnel costs were equivalent in the two modes. Costs of laboratory tests and medications ordered were 27 percent less in the nurse-protocol mode. The study indicated that care in the nurse-protocol mode was of equivalent quality, led to equivalent patient satisfaction, allowed a substantial reduction in physician time, and reduced overall costs of care.

Detection of etiological agent for cholera by PCR protocol.

BACKGROUND: PCR protocol for Vibrio cholerae, the causative agent of the diarrheal disease cholera has been described in this report. We report the detection of Vibrio species in drinking water samples in a duplex PCR reaction. The target loci used in the study were ctxA and tcpA. The sensitivity and efficiency of detection of this protocol can be applied in epidemic conditions, wherein monitoring of target organisms is very crucial. MATERIAL AND METHODS: Single step thermocycling programs have been reported for amplification of specific target loci. We have demonstrated that a gradient multi-step thermocycling program is more efficient in improving the sensitivity of detection by PCR. The method for preparation of template DNA from environmental sample uses filtration of water followed by harvesting the possible bacterial residue. This was further treated with proteinase K and heat to yield DNA compatible for PCR. The protocol was optimized for amplification of target loci from standard strains as well as from environmental water samples. RESULTS: The method can simultaneously detect two different loci of Vibrio cholerae in a single reaction. The sensitivity of detection achieved for the pathogen was 100 cells per reaction. The specificity of the primers was demonstrated by spiking the reaction with non-specific template. The developed protocol was successfully extended to environmental samples. CONCLUSIONS: The developed duplex PCR protocol allows the simultaneous detection of two genetic loci of the target pathogen from water samples. This enhances the efficiency of detection and provides a sensitive tool for the rapid detection of the pathogen that can be useful in epidemic conditions where the time factor involved in the identification of target pathogen is very crucial.

Newly developed encapsulation-dehydration protocol for plantcryopreservation.

A simplified and efficient encapsulation-dehydration protocol, which is a compromise between vitrification and encapsulation-dehydration, was presented for plant cryopreservation. In this protocol, during the encapsulation process, the apices precultured with 0.3 M sucrose for 16 h were simultaneously osmoprotected with a mixture of 02 M glycerol plus 0.4 M sucrose for 1 h. These encapsulated apices were directly dehydrated with dry silica gel prior to a plunge into LN without the pretreatment of 0.8M sucrose for 16 h. This protocol produced much higher rates of recovery growth in the three plant species tested (wasabi, chrysanthemum, and mint) than those cryopreserved by the conventional encapsulation-dehydration. This protocol also considerably reduced the time needed for the cryogenic procedure. Thus, this new protocol appears promising for cryopreservation of shoot apices and other explants.

Limitations of the obstetric group B Streptococcus protocol.

OBJECTIVE: To assess compliance with the Centers for Disease Control and Prevention (CDC) screening-based protocol for obstetric group B Streptococcus (GBS) and to determine an acceptable threshold for protocol failure. STUDY DESIGN: A retrospective chart review was carried out for all deliveries performed through the resident-run community clinic from January through June 1999. Compliance with the CDC protocol was assessed by reviewing collected data from patient charts and comparing it to CDC requirements. Data were collected regarding patient demographics, antenatal GBS status, gestational age at screening, time of rupture of the membranes, time the antibiotic was given and time of delivery. RESULTS: A total of 248 charts were reviewed. Elective cesarean deliveries were excluded (25 charts). Unknown culture status was found for 22 (9.9%) patients. Cultures were collected before 35 weeks' gestation in 39 (17.5%) patients and at > 37 weeks' gestation in 28 (12.6%) patients. Of those with known positive GBS status, 4 (7.0%) were not treated, and antibiotics were given less than four hours before delivery in 13 (24.5%) patients. Of those with unknown status, six (27.2%) were not treated. Overall, there was 70% compliance with the culture collection arm of the protocol and 87% compliance with the treatment arm. CONCLUSION: Fulfillment of CDC guidelines in this community setting is imperfect. There are several areas beyond physician control, including precipitous delivery and patient non-compliance. However, perhaps an acceptable threshold for limitations of adherence to the protocol can be reached.

[Screening protocol of spouse abuse. Efficacy measure].

Screening protocols that can be used to identify domestic violence are lacking (Gendron, 1987), as are studies evaluating the effects of using these protocols. With the objective of screening domestic violence, the Saint-Hubert CLSC has developed a protocol by in collaboration with the Centre de recherche interdisciplinaire sur la violence familiale et la violence faite aux femmes (CRI-VIFF). Evaluation of the domestic violence screening protocol established at the Saint-Hubert CLSC indicates that the number of cases in which domestic violence was detected was 3 times higher after the screening protocol was put in place, and 15 times higher at the Saint-Hubert CLSC than at the CLSC used as a comparison site.

Prevention of non-steroidal antiinflammatory drugs-induced gastropathy: follow up of protocol adherence.

OBJECTIVE: To reach a consensus on and evaluate the compliance of the protocol for the prophylaxis of gastropathy induced by non-steroidal anti-inflammatory drugs with 20 mg omeprazole per day. PATIENTS AND METHOD: Three transversal studies have been carried out on patients admitted to our hospital before establishing the protocol, immediately after and 6 months later. RESULTS: Between 16 and 24% of patients admitted to our hospital take non-steroidal anti-inflammatory drugs. Before the application of the protocol 34.7% of prescriptions coincided with this use. After its establishment adherence by the prescribing doctors improved to 51.8% (c2 = 5.27, p = 0.02) and at 6 months it reached 51.4% adherence, an improvement that continued being significative (c2 = 5.74, p = 0.01). The main reason for non-compliance was the use of ranitidine instead of omeprazole. One of the most useful methods for improving adhesion has been meeting with the medical services that most use the anti-inflammatory drugs, focussing specifically on the use of the protocol. CONCLUSIONS: The follow-up of the protocol shows an improvement in compliance after the establishment stage which is maintained after 6 months. Adherence by the prescribing doctors to the treatment, nevertheless, is lower than had been hoped.

Nurse-protocol management of low back pain. Outcomes, patient satisfaction and efficiency of primary care.

To test the validity of a nurse-administered protocol for low back pain, a prospective trial of 419 patients was undertaken in a walk-in clinic. In all, 222 patients were randomly allocated to a "nurse-protocol group" in which they were evaluated by one of five nurses using the protocol; the nurses independently managed 53 percent of the patients and referred to a physician patients with potentially complex conditions. In addition, 197 patients in a randomly allocated control group were managed by one of 32 physicians. Care in the experimental and control groups was compared by follow-up telephone contact and by a four-month chart review. There was no significant difference in symptomatic relief or the development of serious disease in the two groups. Nurse-protocol patients expressed greater satisfaction with the care they had received; patient satisfaction correlated positively with symptom relief. In over 95 percent of the patients, there were noncomplex, nonserious, nonchronic conditions as the cause of back pain. We conclude that nurse-protocol management of this generally benign condition in a primary care setting is both effective and efficient.

[Pediatric serial urinary cystoureterography. Needs, nursing diagnosis, and care protocol].

The authors present the causes which lead to carrying out a Series of Urinal Tract Retrograde Cystography (CUMS) study in a child and they discuss some reasons which justify the elaboration of a nursing treatment protocol and the corresponding procedures in order to care for the child. To make a protocol for a procedure facilitates and enhances its execution. Nursing treatments, like any other activity, are susceptible to having a protocol and therefore can improve their quality. Nurses in the radio-diagnostic pediatrics ward at the University Hospital Reina Sofía in Cordoba are elaborating treatment protocols for every one of their radiological studies for the care of every child to whom we attend whom requires nursing care. In this article some protocols and procedures are commented on along with the reasons which justify their elaboration.

[Diagnostic value of a new myocardial perfusion agent, 99mTc-teboroxime (SQ30,217) in patients with coronary artery disease, utilizing a SPECT imaging protocol: a multicenter phase III clinical trial].

Technetium-99m teboroxime (TEBO) is a new technetium-based myocardial perfusion imaging agent, which has high myocardial extraction and rapid myocardial washout. Its rapid myocardial washout may necessitate rapid imaging protocols. This study was designed to perform exercise/rest SPECT imaging protocols with rapid data acquisition (from 2.8 +/- 1.4 min to 11.2 +/- 4.1 min after injection) in 171 patients with coronary artery disease, 15 patients with cardiomyopathy and 8 patients with other heart disease. The rapid imaging protocols resulted in a high prevalence of good image quality. The diagnostic value of TEBO was recognized from the high concordance of image findings with thallium-201 (201Tl) scintigraphy, 88.2% in the exercise protocol and 89.9% in the rest protocol. Also, the sensitivity and specificity of TEBO for detecting coronary artery disease were comparable (86.6% and 53.9%, respectively) with those of 201Tl (90.7% and 46.2%, respectively). No toxic effects of TEBO were observed in subjective and objective clinical findings and blood examinations. TEBO was finally evaluated to be useful in 98.4% of the patients. Thus, this agent has good clinical potential for myocardial perfusion imaging.

A lesion detection observer study comparing 2-dimensional versus fully 3-dimensional whole-body PET imaging protocols.

UNLABELLED: We compared the impact of 2-dimensional (2D) and fully 3-dimensional (3D) acquisition modes on the performance of human observers in detecting and localizing tumors in whole-body (18)F-FDG images. METHODS: We selected protocols based on noise equivalent count (NEC) rates derived from a series of 2D and fully 3D whole-body patient and phantom acquisitions on a dual-mode PET scanner. The fully 3D peak NEC value for a standard 70-kg patient was achieved for an injected dose of approximately 444 MBq (12 mCi) assuming a 90-min delay before acquisition, whereas the 2D peak value was never reached. The protocols were therefore set to those corresponding to a 444-MBq injected dose in fully 3D and 2D and a 740-MBq (20 mCi) injected dose in 2D that was considered as the maximum allowable dose. We used a non-Monte Carlo simulator to generate multiple realizations of whole-body PET data based on the geometry of the mathematic cardiac torso phantom (MCAT) with accurate noise properties. Two-dimensional and fully 3D acquisition times were set to 5 min per bed position. Spherical 1-cm-diameter lesions (targets) with random locations and contrasts were distributed in different organs. The simulated 2D datasets were reconstructed using attenuation-weighted ordered-subsets expectation maximization ((AW)OSEM) and the fully 3D datasets were reconstructed with FORE+(AW)OSEM (FORE = Fourier rebinning). Five human observers located and ranked the targets using a volumetric display of the whole-body PET data to replicate the clinical practice. An alternate free-response operating characteristic (AFROC) analysis of the human observer reports was performed for each protocol and each organ separately. RESULTS: The 2D protocol corresponding to 740-MBq injected dose allowed the overall best detection performance. It was followed by the fully 3D acquisition at the peak fully 3D NEC rate from a 444-MBq injected dose. A 2D acquisition corresponding to a 444-MBq injected dose was ranked last. Differences in detection performance were organ specific. CONCLUSION: This study showed that, for this patient size and scanner type, the fully 3D acquisition mode allowed better or equivalent detection performance than the 2D mode for an injected dose corresponding to the peak fully 3D NEC rate. The 2D acquisition protocol combined with a higher injected dose resulted in the highest detectabilities.

Optimisation of the EpiDerm test protocol for the upcoming ECVAM validation study on in vitro skin irritation tests.

An ECVAM-funded prevalidation study (PV) was conducted during 1999 and 2000 to identify in vitro tests capable of reliably distinguishing between skin irritants (I) and non-irritants (NI) according to European Union risk phrases ("R38" or no classification). The tests evaluated were EpiDerm, EPISKIN, PREDISKIN, the non-perfused pig ear method, and the mouse skin integrity function test (SIFT). Whereas reproducibility of the two human skin model tests and SIFT was acceptable, none of the methods was deemed ready to enter a formal validation study due to their low predictivity. The ECVAM Skin Irritation Task Force therefore suggested improvements of protocols and prediction models for these tests. Furthermore, it was agreed that experience gained with the two human-skin models be shared, and a common protocol should be developed for EpiDerm and EPISKIN (Zuang et al., 2002). When we applied an improved EPISKIN protocol (Portes et al., 2002) to the EpiDerm model, an acceptable specificity (80%) was achieved, whereas the sensitivity (60%) was far too low. In 2003, the EPISKIN protocol was further refined by extension of the post-incubation period following chemical exposure. In the current study, we evaluated this EPISKIN refinement by applying it to EpiDerm. In addition, we developed technical improvements for the application of the test chemicals and rinsing procedure, which reduced the variability of results and increased the percentage of correct predictions. A set of twenty non-coded reference substances from the ECVAM prevalidation study phase III (Fentem et al., 2001) was tested with the final protocol in three independent runs. Both high sensitivity (80%) and high specificity (78%) were achieved, and the statistical probability of correct classifications was high, so that the test is now regarded ready for formal validation.

Using ICNP for nurse electronic charts and protocols in rehabilitation divisions.

The aim of this work is to promote the use of standard terminology for nursing documentation and for nurse protocols' description. We implemented an ICNP browser and we integrated it into two different systems: the nursing electronic patient record and the nursing protocols editor. The former allows nurses to choose ICNP terms for filling the clinical chart, while the latter allows computerising protocols using ICNP terms for the actions specification. Since, in a nursing information system, it should be worth to integrate protocols with the electronic patient record, sharing the same terminology is a crucial issue. We also show how we tailored the information system to the different wards. For each ward, we devised the most frequent patient needs and nursing actions. These lists are used to customise the different interfaces. Finally, by testing the prototype using real-world documents (clinical charts and protocols), we carried out an evaluation of the terminology itself, about its completeness and usability.

Comparison of four hearing screening protocols.

This is the second in a series of three papers concerned with the early identification of hearing loss. In the first paper (Turner, 1991), a simple model that permits the calculation of the performance and cost of early identification protocols is presented. In this paper, the model is used to compare four protocols that differ only in their hearing screening strategies. The high-risk register and auditory brainstem response screening are used to construct the four screening protocols. The cost and performance of the four early identification protocols are calculated using base model parameters derived from the clinical literature. The protocols are evaluated separately for the intensive care nursery and the well-baby nursery.

Metastatic potential of mouse skin carcinomas produced by different protocols of chemical carcinogenesis.

Squamous cell carcinomas (SCC) of the mouse skin were produced by three different protocols of chemical carcinogenesis, i.e., complete carcinogenesis with 7,12-dimethylbenz(a)anthracene (DMBA) two-stage carcinogenesis with DMBA as initiator, 12-O-tetradecanoyl-phorbol-13-acetate (TPA) as promoter and three stage carcinogenesis with DMBA, TPA and N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) as third-stage agent or progressor. Tumors were sequentially studied at weeks 38-52 of treatment. Although no significant differences in the rate of appearance of gamma-glutamyl transpeptidase (GGT) could be seen, a larger number of SCC produced by complete carcinogenesis protocols were GGT-negative. This coincided with the higher grade of malignancy of these tumors as evaluated by histopathology. In general terms high-grade tumors were seen more frequently in the complete carcinogenesis experiment than in the other two protocols. SCC produced by complete carcinogenesis also exhibited a markedly higher DNA index than the SCC from the other experimental groups. All three protocols were very effective in producing late metastasizing tumors, and no significant differences could be established in the incidence of spontaneous lung metastasis. This shows that, contrary to general knowledge, if adequately observed for more than 40 weeks, SCC of the murine skin is able to metastasize in the lung in approximately 30% of cases. Nevertheless, complete carcinogenesis-induced SCC were usually of higher histological grade, a proportion of these were GGT-negative and produced more multiple or diffuse metastases than the tumors induced by the multistage protocols.

Pilot study to evaluate 3 hygiene protocols on the reduction of bacterial load on the hands of veterinary staff performing routine equine physical examinations.

Reduction factors (RFs) for bacterial counts on examiners' hands were compared when performing a standardized equine physical examination, followed by the use of one of 3 hand-hygiene protocols (washing with soap, ethanol gel application, and chlorohexidine-ethanol application). The mean RFs were 1.29 log10 and 1.44 log10 at 2 study sites for the alcohol-gel (62% ethyl alcohol active ingredient) protocols and 1.47 log10 and 1.94 log10 at 2 study sites for the chlorhexidine-alcohol (61% ethyl alcohol plus 1% chlorhexidine active ingredients) protocols, respectively. The RFs were significantly different (P < 0.0001) between the hand-washing group and the other 2 treatment groups (the alcohol-gel and the chlorhexidine-alcohol lotion). The use of alcohol-based gels or chlorhexidine-alcohol hand hygiene protocols must still be proven effective in equine practice settings, but in this study, these protocols were equivalent or superior to hand washing for reduction in bacterial load on the hands of people after they perform routine physical examinations.

Comparison of four different ovarian stimulation protocols in an in vitro fertilisation & embryo transfer programme.

Four different ovarian stimulation protocols were evaluated in an in vitro fertilisation and embryo transfer programme in 208 women (228 treatment cycles). In the rigid protocol (RP), 100 mg of clomiphene citrate (CC) was given from day 3 to day 7 of the menstrual cycle and 300 IU of human menopausal gonadotropin (hMG) was given from day 5 of the menstrual cycle. In the individualised protocol (IP) the same drugs and doses were used as in RP, but the day of initiation of CC depended on the length of the individual's menstrual cycle and hMG was administered from the last day of CC. In the programmed protocol (PP), ovarian function was suppressed with oral contraceptive pills (ethinyl estradiol 30 micrograms and norethisterone 1 mg) started on day 5 of the menstrual cycle for 45 to 70 days. Considering the last day of pill intake as day 0, CC was given for 5 days from day 5 and hMG (300 IU) from day 7. In the alternate day protocol (ADP), 100 mg of CC was administered from day 2 to day 6 and hMG (300 IU) was given on alternate days from day 2 to day 8 or day 10 of the cycle. In all the women, hCG (5000 IU) was administered when the diameter of at least 2 follicles was greater than or equal to 16 mm and estradiol levels were 300 pg/ml/dominant follicle. Patients not showing such a response were not treated further. The cardinal events of IVF-ET such as number of good responders, incidence of oocytes harvested, fertilised and embryos transferred per cycle were compared and it was concluded that the pregnancy rates were highest in women treated by the PP.

Protocol dependency of VO2max during arm cycle ergometry in males with quadriplegia.

The purpose of this study was to determine whether maximal oxygen uptake (VO2max) is protocol dependent during arm cycle ergometry (ACE) for quadriplegic males with spinal cord injuries (SCI). Twenty-four non-ambulatory subjects (aged 20-38 yr) with cervical SCI were divided into two groups based on wheelchair sports classification (IA group = 14; IB/IC group = 10). They underwent three different, continuous graded exercise tests spaced at least 1 wk apart on an electronically braked arm cycle ergometer. Following a 3-min, unloaded warm-up at 60 rpm, the work rate was increased 2, 4, or 6 W.min-1 for the IA group and 4, 6, or 8 W.min-1 for the IB/IC group. Ventilation and gas exchange were measured breath-by-breath with a SensorMedics 4400 computerized system. Repeated-measures ANOVA showed no significant difference among the three protocols for VO2max in the IA group (P greater than 0.05). The mean (+/- SD) VO2max values (ml.kg-1.min-1) were 10.8 (+/- 3.4), 11.0 (+/- 2.7), and 10.2 (+/- 2.9) for the 2, 4, and 6 W.min-1 protocols, respectively. In contrast, the IB/IC group showed a significant difference among the protocols for VO2max (P less than 0.05). The mean (+/- SD) VO2max values (ml.kg-1.min-1) were 16.8 (+/- 4.5), 15.3 (+/- 4.3), and 14.6 (+/- 4.3) for 4, 6, and 8 W.min-1, respectively. Post hoc analysis revealed a difference between the 4 and 8 W.min-1 protocols. Our results suggest that graded exercise testing of SCI persons with quadriplegia, using ACE, should employ work rate increments between 2-6 W.min-1 and that work rate increments of 8 W.min-1 or greater will underestimate VO2max.

[Anesthesia using propofol during surgery of strabismus in children. A comparison of two different protocols of induction and maintenance].

The purpose of this study is an investigation of two protocols using propofol as induction and maintenance agent in 100 children scheduled for strabismus surgery (4-8 year, ASA I, NYHA I). Protocol I; Propofol 6 mg.kg-1 in 60 s with fentanyl 2 micrograms.kg-1 and vecuronium bromide 0.08 mg.kg-1 for induction, followed by propofol 11 mg.kg-1 for maintenance; Protocol II; Propofol 3 mg.kg-1 in 20 s with fentanyl 3 micrograms.kg-1 for induction, followed by propofol 12 mg.kg-1.h-1 for maintenance. It appears that the use of protocol I offers significant advantages compared with protocol II: a better quality of induction with a lesser incidence of pain during injection of propofol; a better quality of maintenance with very infrequent bradycardia from oculocardiac reflectivity; and a better recovery with a greatly reduced frequency of nausea and vomiting.
