Quantitative exercise thallium-201 rotational tomography for evaluation of patients with prior myocardial infarction.

The utility of stress-redistribution thallium-201 myocardial perfusion single-photon emission computed tomography (SPECT) in patients with a prior single myocardial infarction was studied in 66 patients who were tested by both SPECT and coronary angiography. SPECT was quantified by comparing the patients' circumferential count profiles to a previously established normal data base and by plotting the results onto a polar coordinate map that localized defects to the 3 major coronary artery territories. The pattern of reversibility of the quantitatively detected defects was assessed by consensus visual analysis. SPECT thallium-201 detected myocardial infarction in 62 of 66 patients (sensitivity = 94%). Sixty-five percent of the infarct zones showed some reversibility at 4-hour imaging which corresponded with angiographic evidence of flow to the infarct zones in 95.5% of cases. Late (18 to 24 hours) imaging, done in 26 patients, showed complete or partial reversibility of 29% of infarct zone segments which were nonreversible on 4-hour images. To improve specificity for detection of disease in coronary arteries supplying the non-infarct territories, new quantitative criteria were developed that took into consideration contiguity of defects with the infarct zone. Accuracy for detection of patients with multivessel coronary disease by quantitative thallium-201 SPECT was 86%, which was significantly higher than those of the clinical response to exercise (48%), the exercise electrocardiographic response (56%) or their combination (65%).(ABSTRACT TRUNCATED AT 250 WORDS)

Usefulness of myocardial contrast echocardiography in detecting the immediate changes in anterograde blood flow reserve after coronary angioplasty.

Myocardial contrast echocardiography has revealed that successful coronary angioplasty results in an immediate decrease in the amount of collateral blood flow to the perfusion bed supplied by the dilated vessel. This information could potentially be used with pharmacologic stress in the catheterization laboratory to also assess the improvement in coronary flow reserve after angioplasty. The immediate changes in area under the time intensity curve produced by a 1 ml slow injection of sonicated albumin immediately proximal to a stenosis before and after 14 angiographically successful angioplasties was studied in 12 patients. Area under the curve was assessed before and after an 8 mg selective injection of papaverine. The changes in area under the curve were correlated with percent improvement in epicardial area stenosis. Visually successful angioplasty resulted in > 30% improvement in area under the curve after papaverine in 9 of 14 studies. There was a significant correlation between improvement in area under the curve after papaverine and percent improvement in epicardial area stenosis (r = 0.75; p < 0.01). No patient had left ventricular wall motion abnormalities after papaverine before or after angioplasty. These changes suggest that quantitatively successful angioplasty results in decreased collateral blood flow to the involved myocardium during pharmacologic stress. These improvements in coronary flow reserve cannot be predicted by visual analysis of angioplasty results.

Rapid diagnosis of Legionnaires' disease by urinary antigen detection. Comparison of ELISA and radioimmunoassay.

An enzyme-linked immunosorbent assay was developed to detect urinary antigen excreted by patients with Legionnaires' disease. Of 47 patients tested, antigen was detected in 39. Of these 39 specimens, 35 gave clearly positive results by visual analysis; four others required spectrophotometric verification of positive results. Antigen was not detected in any of 178 urine specimens from patients with other pulmonary, bacteremic or urinary tract infections after a quick and simple confirmatory test. The assay required more time to perform than a previously described radioimmunoassay but was of equivalent sensitivity and specificity and did not require expensive equipment or contract with radioactive reagents. We conclude that enzyme-linked immunosorbent assay is a rapid, sensitive and specific means for rapidly diagnosing legionnaires' disease that can be performed in clinical laboratories unwilling or unable to use radioisotopes.

Fetoscopic visualization in second-trimester pregnancies.

Fetoscopic visualization may be required for the prenatal diagnosis of external fetal malformations which cannot be clearly defined by ultrasound. The synchronous use of real-time ultrasound guidance during the fetoscopic procedure has permitted successful visualization to be achieved in pregnancies of greater than 18 weeks' gestation. The chances of successful visualization and the influence of advancing gestational age have been critically evaluated. Between 18 and 23 weeks', when these techniques were used, advancing gestational age did not appear to exert a significant effect on the chances of successful visualization. Analysis of placental position indicated that an anterior placenta did not significantly affect bloodstaining of the fluid or preclude access to placental vessels for blood sampling.

Limitations of antenatal fetal heart rate monitors.

Erroneous or doubtful decelerations in fetal heart traces were present in 111 of 1000 consecutive antenatal clinical records obtained by monitors with autocorrelation. The incidence was 20% in fetuses less than 30 weeks of gestational age. Their elimination reduced the number of "decelerative" records by 42%. Erroneous or doubtful accelerations were also present in 11% of records. These errors are caused by the fetal heart rate monitor and may contribute to the high intraobserver and interobserver variation on visual analysis. They can be detected by computer analysis.

A malachite green procedure for orthophosphate determination and its use in alkaline phosphatase-based enzyme immunoassay.

An improved procedure for phosphate determination based on a highly colored complex of phosphomolybdate and malachite green is described. All necessary reagents are combined in one concentrated solution, making the assay sensitive and convenient. The procedure is based on the finding that the dye is easily soluble and stable in the presence of 6 N acid. The addition of Tween 20 is required to stabilize the dye-phosphomolybdate complex at phosphate concentrations above 10 microM. The time of color development at 25 degrees C is about 3 min. The procedure was adopted to measure alkaline phosphate activity in heterogeneous enzyme immunoassay with rho-nitrophenyl phosphate and pyrophosphate as substrates. In both cases, a 4-fold increase in sensitivity in terms of absorbance readings was obtained compared to the standard method based on rho-nitrophenol measurement. In visual analysis, the gain in sensitivity was as high as 20-fold, due to contrast color change (yellow to greenish blue).

Inorganic pyrophosphatase as a label in heterogeneous enzyme immunoassay.

Inorganic pyrophosphatase from Escherichia coli has been employed as a label in heterogeneous enzyme immunoassays. Enzyme-antibody conjugates were prepared with the use of glutaraldehyde and purified by gel permeation chromatography. Enzyme activity was measured by means of a sensitive one-step color reaction between phosphate, molybdate, and malachite green. The sensitivity in terms of absorbance readings was four to eight times higher than that of peroxidase-based assays. The color change (yellow to greenish blue) inherent in the use of pyrophosphatase as the labeling agent is highly suitable for visual analysis. Other merits of pyrophosphatase include the remarkable stability of the enzyme and its substrate, its compatibility with bacteriostatic agents, and its low Michaelis constant. Examples of the use of phosphatase in the assay of human alpha-fetoprotein and immunoglobulin G are presented.

Topographical evolution of spike-wave complexes.

Computer-generated 3-dimensional field potential maps of spike-wave complexes from two 4 X 4 electrode grids on the scalp were studied. A visual analysis of these field maps throughout the spike-wave evolution permitted quantification of the spike, trough and slow wave components in terms of distribution, origin and propagation. In addition, a more objective morphological analyzer also quantified the discriptive parameters of distribution, origin and propagation for the spike component of the patients' spike-wave complexes. We found that field distributions of spikes differed from that of waves. Succeeding positive troughs evolved more symmetrically than did spikes but less than ensuing negative waves. Negative waves were more diffuse, more symmetrical in evolution, and more posteriorly centred than either spikes or troughs. Unlike the troughs and slow waves whose fields tended to remain stationary during their evolution, spikes always moved from their points of origin. Spikes originated at the most lateral points of the grids and propagated laterally and anteriorly in one of two ways: a simultaneous origin at both lateral positions, then propagation toward the midline, and then usually anteriorly, a clearly unilateral origin with spread contralaterally to the homologous electrode position of the contralateral hemisphere followed again by anterior propagation. Interhemispheric lag times of spikes ranged from 0 to 25 ms with an average of 10.5 ms. Inter- and intrapatient variability was considerable. This type of analysis reveals properties of spike-wave complexes which may not be appreciated by standard paper writeout.

Pulsatile pattern of thyrotropin-release in normal men.

To evaluate the existence of a pulsatile secretion pattern of thyrotropin (TSH) the 24 h release profile of the hormone was investigated in 7 healthy male subjects by taking blood via an indwelling venous catheter at 10-min intervals. Thyrotropin levels were analysed with high precision by a recently developed immunoradiometric assay (IRMA) able to distinguish euthroid and hyperthyroid TSH-levels. Visual analysis (n = 10.4 peaks/24 h) and the computer assisted evaluation by the Santenprogramme (n = 9.9 peaks/24 h) as well as the Pulsar-programme (n = 7.7 peaks/24 h) revealed an episodic release pattern with a low amplitude of pulses. These preliminary results are highly suggestive of a pulsatile TSH secretion similar to other pituitary hormones.

Auditory evoked potentials compared to performance measures and EEG in assessing excessive daytime sleepiness in narcolepsy-cataplexy.

The AEP to the repetitive stimuli of the Wilkinson auditory vigilance task was compared between untreated patients with narcolepsy-cataplexy and matched controls for periods during which the tones were preceded by 13 sec or more of wakefulness (defined by EEG-polygraphic criteria). During these periods it had been found previously that narcoleptics did not perform worse than did controls. The AEP, nevertheless, showed significant differences for narcoleptics. These were similar to changes described for drowsiness in normals. The AEP, therefore, showed changes related to excessive day time drowsines in narcolepsy-cataplexy when a sensitive performane measure and visual analysis of EEG-polygraphic recordings did not.

A surface electrode array for detecting action potential trains of single motor units.

Action potentials of single motor units were detected by a linear surface electrode array placed perpendicular to the longitudinal axis of the biceps brachii. Twelve myoelectric signals were derived simultaneously from a voluntarily contracting muscle. Using a visual feedback control, 3 subjects produced spike trains of single motor unit action potentials (MUAPs) at a weak contraction. When the myoelectric signals showed an interference pattern at a moderate contraction, several MUAPs were isolated by a visual analysis. MUAPs occurring at about fixed intervals with constant amplitudes and with identical wave forms were presumed to be the action potential trains of single motor units. Reliable estimates of single MUAP wave forms were obtained by averaging and superimposing the detected signals at a timing of characteristic potential peaks. Then not only the firing rate of the spikes but also the territory and the wave form of single MUAPs were investigated. Most MUAPs had a sharp and symmetrical distribution of potentials on a skin surface along the muscle circumference, while some MUAPs showed complex wave forms with some separate potential peaks. The possible arrangement of muscle fibers belonging to the motor units was estimated from the MUAP wave forms.

A large scale, high resolution, automated system for rat sleep staging. II. Validation and application.

An automated rat sleep staging system was used to describe sleep-waking behaviour in a large number of rats. The computer scorings were validated by visual analysis of a limited set of data by two human raters and the agreement varied between 82 and 100% for homogeneous segments of the different sleep-waking stages. The automated system proved to be very consistent in view of the small variation in placebo values over 110 experiments. The data show that classification of rat sleep waking behaviour into 6 different stages is both advisable and feasible. The experiments further show that rat age (over a range from 0.5 to 2 years) does not greatly affect rat sleep-waking behaviour.

Reliability, validity and follow-up of the EEG in senile dementia: sequelae of sequential measurement.

In a longitudinal study, slowing of the EEG in SDAT patients and elderly controls is investigated using both visual and power spectral analyses. For this a reliability study in 20 SDAT patients is performed and results for intra-rater, inter-rater and test-retest reliability (in 24 h) are used in the selection of parameters. Next, the diagnostic efficacy of the chosen parameters for SDAT is confirmed in a validity study in 43 SDAT patients, 41 elderly non-organic psychiatric patients and 51 elderly normals. In the longitudinal study 78 of these subjects could be retested after 20 months. The parameter for visual analysis appeared to be superior in actually showing a progress of slowing of the EEG in 20/21 SDAT patients over time. The parameter for power spectral analysis was superior in measuring the more subtle processes in the normal elderly. Results of these studies are used to discuss some problems in the measurement of EEG in SDAT over time.

Electroencephalogram and seizures in chronic alcoholism.

CNS complications of chronic alcoholism are frequently difficult to assess due to the variety of direct and secondary conditions which can result from alcoholic drinking and lifestyles. The influence of alcoholism and alcohol-related factors on the EEG of patients with chronic alcoholism was studied in 213 patients (15.4% of all adults who had EEGs) using visual analysis. The influence of a variety of alcohol-related factors - drinking history, clinical complications, traumatic head injuries, head CT findings and laboratory results - on the EEG and alcohol-related seizures was studied. The effect of EEG results on the decision to treat alcohol-related seizures was also assessed. 152 of the patients had seizures, mostly (90% of those with defined seizure types) generalized tonic-clonic seizures. 53% of all seizures occurred in the early withdrawal period (8 h to 7 days abstinence). A history of partial seizures was significantly associated with findings of focal EEG abnormalities, a history of head injuries and structural lesions on CT. The clinical significance of these findings was unclear, however, as the majority of patients who had focal EEG abnormalities or structural brain lesions still appeared to have generalized withdrawal seizures. The EEG and CT appeared to be complementary tests: for most patients, focal abnormalities were demonstrated on only one of the two tests. The majority of patients (56%) with normal EEGs had predominantly low voltage recordings (less than 25 muV), compared with 13.9% of 1167 patients without a history of alcoholism (P less than 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

A data compression algorithm for the electroencephalogram.

This paper describes a data compression algorithm for the EEG using a local error measure. The algorithm discards input signal samples provided they can be reconstructed from the stored samples with an error smaller than a given threshold, and according to simple reconstruction functions (the hold, the ramp, and the cosine). An equivalent (in terms of storage) sampling rate of 44 Hz is achieved without noticeable degradation of the signal quality for visual analysis. The method can be easily implemented for real time multichannel data compression/reconstruction.

Multisystem involvement of the central nervous system in Strümpell's disease. A neurophysiological and neuropsychological study.

The multisystem involvement of the central nervous system (CNS) in familial spastic paraplegia (FSP) has not been fully investigated by means of complete neurophysiological and neuropsychological examinations. The classification which distinguishes pure and complicated forms of FSP, is based on clinical features and does not take into account the possibility that clinically silent lesions of the CNS can be identified by means of adequate tests. The study was intended to assess the subclinical and multisystem involvement of the CNS in a group of 11 patients affected by FSP, clinically distinguished in 7 pure forms and 4 complicated forms. Neurophysiological tests included saccadic eye movements analysis, visual and auditory brain stem evoked responses. Neuropsychological examination was devised by means of a special purpose mental deterioration battery. Our results, showing a high incidence of multisystemic subclinical involvement of the CNS, confirm and extend the concept that FSP is a multisystemic degenerative disease of the CNS, and that the existence of "pure" forms should be reconsidered.

A rapid genotype assay for Pla haplotypes.

The method described here results in simple visual analysis of 1, 2, or 3 ethidium bromide-stained bands corresponding to the PlA1, PlA2 or PlA1/A2 phenotypes, respectively. The amplification primers, designed to yield a 711bp product which encompasses exon 2 and substantial intronic sequence both 5' and 3' of the exon enable exquisite resolution of the restriction enzyme digestion products which define the alleles. In addition to prediction of fetal risk of neonatal alloimmune thrombocytopenia using this method, an ethnically heterogeneous pool of platelet donors was screened for the platelet antigen PlA (HPA-1) and gave unequivocal results which should be useful for platelet matching.
