A method for combined dacryocystorhinostomy with external ethmoidectomy.

Severe polypoid rhinosinusitis requiring external ethmoid surgery may be seen in conjunction with partial or complete nasolacrimal obstruction. When this obstruction is associated with persistent, unresolved lacrimal symptoms, it is sometimes appropriate to combine surgical procedures, thereby avoiding separate lacrimal surgery. A method combining dacryocystorhinostomy (DCR) with external ethmoidectomy is described. The anatomy involved in this procedure and the indications for it are discussed.

Effect of nasolacrimal occlusion on timolol concentrations in the aqueous humor of the human eye.

We studied the effect of manual nasolacrimal occlusion on the concentration of timolol in the aqueous humor of eyes of patients undergoing cataract extraction. Aqueous humor samples were obtained at various times after timolol maleate instillation from patients with or without 5 min of nasolacrimal occlusion; aliquots were assayed by HPLC. In patients receiving occlusion treatment, average timolol concentrations were statistically greater than those in control patients both between 15 and 90 min after instillation and also at 180 min. Pharmacokinetic analysis indicated that occlusion increased the concentration of timolol in the aqueous humor 1.7 times. In both groups, timolol concentrations were highest approximately 1 h after instillation. The decline in aqueous humor timolol concentrations occurred at similar rates in both groups.

Anatomical analysis of the human lacrimal drainage pathway under an operating microscope.

Mesoscopic structures of the lacrimal drainage pathway were observed under an operating microscope in six cases of 5 adult cadavers. In all these cases the upper and lower canaliculi fused to form the common canaliculus. Two cases with a large common canaliculus showed large projections of mucosa which reduced the inner space of the common canaliculus. The remaining 4 cases with a small common canaliculus did not show any projections. These findings indicate that the human lacrimal system can function physiologically through a small narrow space of the common canaliculus.

The velo-cardio-facial (Shprintzen) syndrome. Clinical variability in eight patients.

Eight patients (three sporadic, five from two families) with the velo-cardio-facial syndrome (VCFS) or Shprintzen syndrome are reported. Major clinical findings of this syndrome include a characteristic pattern of facial dysmorphisms, cleft palate, cardio-vascular malformations, and (mostly mild-to-moderate) mental retardation or learning difficulties. The syndrome probably is caused by a dominant gene with very variable expression. From previous reports mostly ascertained from cardio-vascular or cleft palate clinics, the incidence of cleft palate and heart defects was calculated to be 98% and 82%, respectively. Out of eight patients of this study who were diagnosed mainly through their pattern of facial dysmorphisms, only two and four had clefts and heart defects, respectively, further demonstrating the variability in the expression of this gene. Similarly, mental retardation, noted in 100% of previous publications, was not present in all of our patients. In two instances, examination of the mother revealed that she probably carried the mutant gene, but that she showed a milder clinical expression than the index patient. It is suggested that careful family investigations should be performed following detection of an index patient, and that the rate of fresh mutations might be not as high as previously assumed.

MRI of childhood dacryocystocele.

We report an unusual presentation of a dacryocystocele in a 3-day-old child, with clinical and novel MRI findings. The literature contains extensive CT documentation, but descriptions of the MRI appearances of dacryocystoceles are lacking. Although CT is the standard initial imaging modality, MRI can be helpful if there are questions as to the content of the lesion or a need for further anatomical characterization.

Fusiform enlargement of the nasolacrimal canal: a rare anatomic variation.

To determine the standard width and shape of the nasolacrimal canal (NLC) in the coronal plane, a retrospective review of 950 coronal paranasal CT sections was undertaken. The coronal diameter of NLC was measured at its upper and lower ends and anomalies in its configuration noted. The mean diameters of the NLC were 4.8 +/- 1.2 mm and 4.7 +/- 1.1 mm on the right and left sides, respectively. There was no difference in the mean NLC diameter between the right and left sides, upper and lower portions or males and females. In two cases, an abnormal dilation of the NLC at its inferior portion was encountered. A marked enlargement of the NLC can occur in the absence of a neoplastic or obstructive process and may be detected as an isolated finding representing idiopathic or congenital enlargement. Segmental NLC dilatation is not always an indicator of pathology, particularly in asymptomatic cases, and should be considered a rare anatomic variation.

Primary natural killer cell lymphoma of the lacrimal sac.

Primary lymphoid tumors of the lacrimal sac are quite rare, and all reported cases are of B-cell tumors with good prognosis. To our knowledge, this is the first case of primary natural killer (NK) cell lymphoma of the lacrimal sac. A 55-year-old woman presented with a lacrimal sac tumor, and histological diagnosis of NK cell lymphoma was made. Although disease was initially localized to the right lacrimal sac, it invaded into the adjacent ethmoidal sinus before chemotherapy was initiated (clinical stage IIE). Epstein-Barr virus (EBV)-encoded small RNA (EBER) was detected in lymphoma cells by in situ hybridization. Systemic chemotherapy combined with intrathecal chemotherapy followed by local radiotherapy was performed, and the patient achieved complete remission. However, shortly after completion of chemoradiotherapy, the lymphoma relapsed with rapid systemic dissemination. The disease was refractory to chemotherapy, and the patient eventually succumbed due to sepsis.

[Prognosis of conjunctivo-dacryocystorhinostomy. Modification with different implant materials].

BACKGROUND: In cases of total canalicular occlusion or the total absence of the lacrimal apparatus the functional restoration requires the repair of a newly created lacrimal system. The intraoperatively reestablisted lacrimal drainage from the conjunctival sac will be maintained by a plastic tube. Beside other criteria the influence of the used inserted materials polyethylene or silicone should be evaluated. PATIENTS AND METHODS: From 9/1978 to 2/1999 we have been treated and consecutive documented 37 cases of conjunctivodacryocystorhinostomy and its modifications in 36 patients (1mal both eyes). The patients (22 men, 14 women) ranged in age from 9-76 years (mean age of 35.2 years). In a retrospective review results were compared for the following two groups based on the type of used plastic tube: group I: (n = 23); insertion of a polyethylene tube (from 1976-1990); group II: (n = 14) insertion of a silicone tube (from 1991-1998). RESULTS: Causes of lacrimal drainage system obstruction were trauma (n = 21), malformation (n = 8), chronic inflammation (n = 5) and tumor (n = 2). The following different surgical techniques were performed: conjunctivodacryocystorhinostomy (cdr, n = 23), conjunctivodacryocystostomy (cd, n = 10) and conjunctivorhinostomy (cr, n = 4), (right eye: n = 21, left eye: n = 16). In group I 14 of 23 operations (61%) had successful functional results, in group II 10 of 14 operations (72%), respectively. The polyethylene tubes on the average were maintained for 15 months after surgery (4mal spontaneous removal) and the silicone tubes for 12 months, respectively (4mal spontaneous removal). CONCLUSION: Traumatic disturbance and congenital defects of canaliculi and surrounding tissue require in the case of any symptoms the surgical reconstruction of the lacrimal pathway. With the introduction of the silicone tube the success rate could be further advanced.

Endoscopy-guided transcaruncular Jones tube intubation without dacryocystorhinostomy.

PURPOSE: To evaluate the functional surgical success, complications, and degree of satisfaction after endoscopy-guided transcaruncular Jones tube intubation without dacryocystorhinostomy (DCR). METHODS: A pair of Westcott scissors was passed into the nose from the side of the caruncle, and the lacrimal bone was penetrated between the lacrimal sac and the nasal mucosa. After the proper length of Jones tube was decided, the tube was inserted into the nasal cavity and fixed at the caruncle with nonabsorbable suture material. This procedure was done without DCR. We retrospectively reviewed the 60 patients (70 eyes) with canalicular obstruction who underwent Jones tube intubation in this manner. RESULTS: The overall success rate was 91.4%. The length of Jones tube used ranged from 16 to 30 mm. Retrograde air blowing into the eye via the Jones tube was the most common complaint (25 of 70 eyes). Complications of this technique included tube problems (25.7%), in particular, downward displacement (22.9%), which was corrected easily in the outpatient clinic, and extrusion. Other complications were frequent inflammation and conjunctival growth over the tube opening. CONCLUSIONS: Endoscopy-guided Jones tube intubation without DCR is a simple and useful procedure for correcting canalicular obstruction.
