Intervertebral disc collagen. Usage of the short form of the alpha1(IX) chain in bovine nucleus pulposus.

Nucleus pulposus, the central zone of the intervertebral disc, is gel-like and has a similar collagen phenotype to that of hyaline cartilage. Amino-terminal protein sequence analysis of the alpha1(IX)COL3 domain purified from bovine nucleus pulposus gave a different sequence to that of the long alpha1(IX) transcript expressed in hyaline cartilage and matched the predicted sequence of short alpha1(IX). The findings indicate that the matrix of bovine nucleus pulposus contains only the short form of alpha1(IX) that lacks the NC4 domain. The sequence encoded by exon 7, predicted from human COL9A1, is absent from both short and long forms of alpha1(IX) from bovine nucleus pulposus and articular cartilage. A structural analysis of the cross-linking sites occupied in type IX collagen from nucleus pulposus showed that usage of the short alpha1(IX) transcript in disc tissue had no apparent effect on cross-linking behavior. As in cartilage, type IX collagen of nucleus pulposus was heavily cross-linked to type II collagen and to other molecules of type IX collagen with a similar site occupancy.

[The clinical effects of percutaneous lumbar discectomy combined with sodium hyaluronate in the treatment of lumbar intervertebral disc herniation].

OBJECTIVE: To investigate the therapeutic effect of percutaneous lumbar discectomy (PLD) combined with sodium hyaluronate (SH) injection in the treatment of lumbar intervertebral disc herniation. METHODS: Forty-eight patients suffered from lumbar disc herniation were divided into two groups and treated by PLD combined with SH injection into epidural cavity (treatment group) or single PLD (control group) respectively. All patients were followed up for 24 months. The therapeutic effects in both groups were assessed and compared according to Macnab's criterion. RESULTS: The patients in the treatment group got much more significant improvement than those in the control group, with shorter therapeutic course and more safety. CONCLUSION: PLD combined with SH injection into epidural cavity is more effective and safety in the treatment of lumbar disc herniation than of pure PLD.

A comparison of the high buoyant density proteoglycans isolated from the intervertebral discs of chondrodystrophoid and non-chondrodystrophoid dogs.

Groups of purebred beagles and greyhounds of similar ages (1.5-2.5 years) were used for the study. Intervertebral disc proteoglycans (PGs) were radiolabelled in vivo (with [35SO4(2-)], 24 hours and 60 days prior to euthanasia, when lumbar discs were dissected into nucleus pulposus (NP) and annulus fibrosus (AF). Aliquots of each disc region were separately analysed for total PG content as hexuronate. The remaining tissue was subjected to extraction with 4.0 M GuHCl. High buoyant density PGs were isolated from these extracts by CsCl density gradient ultracentrifugation. The hydrodynamic size and aggregatability of the 24-hour, 60-day-old, and resident PG populations were determined by Sepharose CL2B chromatography in the presence or absence of excess hyaluronic acid. While the hydrodynamic sizes of the newly synthesized (24-hour) disc PG preparations appeared to be similar, the 60-day-old greyhound disc PGs were found to be larger than the corresponding beagle disc PG populations. However, the keratan sulphate-core protein complexes prepared by chondroitinase ABC digestion of the newly synthesized (24 hour) disc PGs showed that the greyhound disc preparations were also larger than those from beagle discs. Approximately 80% of the newly synthesized PGs from beagle and greyhound discs were capable of aggregating with hyaluronic acid, however, this was reduced to 55% for the 60 day-old PGs in NP and AF and even less for the resident PG populations (as determined by hexuronate analysis). Significantly, PG aggregation was lower in the greyhound NP and AF preparations than in the corresponding PGs isolated from the beagle disc.(ABSTRACT TRUNCATED AT 250 WORDS)

The significance of motor-point tenderness in patients with lumbar intervertebral disc protrusions.

The relationship between tender motor-points and physical signs was investigated in 48 patients with low back pain caused by intervertebral disc protrusions. There were 42 controls with no history of back pain. Motor-point tenderness was quantified on a four-point scale. All patients (except one) showed a variable degree of tenderness in the motor-points related to the relevant myotomes as determined by clinical examination. The greater the tenderness, the longer was the period of disability. The degree of tenderness seemed to have a prognostic value related to recovery from pain and the length of disability as judged by absence from work.

The effect of chondroitinase ABC on rabbit intervertebral disc. Radiological, histological and electron microscopic findings.

Seventy-eight rabbit lumbar discs were evaluated by radiographs and histology after the injection of chondroitinase ABC (40 U/ml for each disc) and compared with injection with phosphate buffer, and also with a control group who were not injected. There was considerable narrowing of the disc space after chondroitinase ABC injection. Safranin-0 depletion was present in the anterior part of the annulus fibrosus near to the nucleus pulposus in all the treated discs, indicating loss of proteoglycan. Electron microscopy showed collapse of the chondrocytes and notochordal cells. These findings suggest that chondroitinase ABC may be another chemonucleolytic agent which decreases disc volume and consequently decompresses the spinal cord or nerve roots; its effects were confined to the tissues within the intervertebral disc.

Isolation and characterisation of a neutral proteinase from the canine intervertebral disc.

A neutral proteinase of 94 kDa capable of degrading gelatin, canine disc proteoglycan, and L-lysine and L-arginine peptide substrates has been isolated from the greyhound intervertebral disc. Strong inhibition of this proteinase with class-specific inhibitors, such as APMSF, TLCK and benzamidine indicated a 'serine'-type specificity. Metallo, aspartyl- and cysteine proteinase inhibitors were devoid of significant action. Degradation of the resident canine disc proteoglycan monomer by the disc proteinase was shown to occur at the hyaluronic acid binding region, thereby diminishing its ability to aggregate with hyaluronic acid. The hydrodynamic size of the proteoglycan degradation products was only slightly less than that of the intact disc proteoglycan subunits.

[Extensive bony condensation of the vertebrae in intervertebral disc degeneration. (Study of 25 cases)].

The authors report 25 cases of extensive bony condensation of the vertebrae due to degeneration of the intervertebral discs, giving on lateral XRay a dome-shaped or helmet-shaped appearance. 19 cases occurred in women. Two-thirds of the patients were aged between 35 and 55 years, with an average of 45 years. The lesions were lumbar in 21 patients. They affected in 20 cases two adjacent vertebrae. All the clinical, radiological, pathological and discographic findings, together with the course, confirmed the degenerative origin of these bony condensations, the pathogenesis of which is still not perfectly understood.

Late results of treatment of intervertebral disc disease with chymopapain.

The authors report a 12-year follow-up review of 33 patients treated with chymopapain (Discase) injection for intervertebral disc disease. This carefully controlled series of patients was treated under local anesthesia in the prone position. Only the single offending disc that correlated with the myelographic and clinical pathology (without the use of discography) was injected with Discase. Anaphylaxis is treated instantly at the first sign of disturbance, and no patients suffered shock. The percentage of patients who were completely cured or improved continues at about 70%. Aside from sensitivity, complications attributable to proper use of the enzyme remain at zero. Follow-up review of those patients over 10 years would indicate that there is no risk of delayed organ toxicity, no risk of carcinogenesis, and no apparent risk of first-generation teratogenesis.

Chemonucleolysis or discectomy? Results of a randomized multicentre trial in patients with a herniated lumbar intervertebral disc (a preliminary report).

A randomized trial was carried out to compare the results of discectomy and chemonucleolysis in 151 patients, suffering from lumbar disc herniation L4-L5 or L5-S1. All patients fulfilled strict entry criteria. Seventy eight patients underwent surgical discectomy, 73 were treated with chemonucleolysis. Within one year of follow-up 18 patients (25%) required surgery after chemonucleolysis, whereas 2 patients (3%) in the surgery group underwent re-operation. Increase of radicular pain within 30 days after treatment was encountered in 16 patients (22%) in the chemonucleolysis group, as compared to none in the surgery group. The results of open surgery after preceding chemonucleolysis was successful in 44% and unsuccessful in 56%. The final result of chemonucleolysis, including second treatment, was still slightly less favourable (73%) the result of primary surgery (79%).

Role of actin stress fibres in the development of the intervertebral disc: cytoskeletal control of extracellular matrix assembly.

Orientation of collagen fibrils is a key event in the development of many tissues. In the intervertebral disc, the outer annulus fibrosus comprises lamellae of parallel collagen fibres, the direction of orientation of the long axis of which alternates in angle between lamellae. In development, this organisation is preceded by the formation of sheets of oriented fibroblasts, which then deposit the oriented lamellae. Here, using fluorescent labelling, confocal and electron microscopic techniques on developmental series, we show that the orientation of cells in lamellae is associated with the formation of adherens junctions intercellularly, involving cadherins and vinculin, and longitudinal stress fibres (label for filamentous actin and tropomyosin) intracellularly. The stress fibres direct the initial elongation of cells and control the deposition of oriented extracellular matrix via junctional complexes with the matrix involving vinculin and alpha 5 beta 1 integrins, which in turn promote the formation of oriented fibronectin at the cell surface; oriented collagen is deposited between cells at the same stages. Shortly after birth, the stress fibres disappear, probably because cells now gain orientational cues from the matrix, and are undergoing differentiation-related changes to form fibrocartilage cells. Assessment of the cellular heterogeneity of the ovine intervertebral disc: comparison with synovial fibroblasts and articular chondrocytes.

The objective of this study was to compare and contrast the cellular heterogeneity of ovine intervertebral disc (IVD) annulus fibrosus (AF) and nucleus pulposus (NP) cells, synovial fibroblasts (SFs) and articular chondrocytes (ACs) grown in monolayer and alginate bead culture. Phase contrast microscopy of the monolayers indicated that the SF and AF cells, and the AC and NP cells had similar morphologies. Immunolocalisation of type I, II and VI collagen epitopes in the monolayers, however, demonstrated distinct quantitative and relative differences between the SF and AF, and the AC and NP cells. Immunolocalisation of bead-incorporated proteoglycans (PGs) also demonstrated quantitative and qualitative differences between the SF and AF cells. Quantitation of (35)S-bead PGs indicated that the AF cells synthesised significantly higher PG levels than the SFs, and AC cells synthesised significantly higher PG levels than the NP cells. These data were also consistent with the relative metabolic activities and cellular viabilities displayed by each cell type in bead culture. AF cells shared morphological traits with SFs in monolayer, but displayed distinctly different characteristics in bead culture. Similarly, NP cells shared similarities with ACs in monolayer and bead culture, but overall were less metabolically active. Data presented here support the proposal that the AF and NP cells of the ovine IVD should be considered as specialised fibrochondrocytic cells.
