Boron-neutron capture therapy in relation to immunotherapy.

The essential feature of tumour therapy rests upon host-tumour interaction. To achieve therapeutic effects, a prerequisite to immunotherapy is the reduction of tumour cells in the host's body. Such measures should not be immunosuppressive. Cytotoxic chemotherapy is not appropriate in this regard. Supraradical surgery and non-specific radiotherapy are not desirable for preservation of nervous function, if their immunosuppression is not as severe as cytotoxic substances. Boron-neutron capture therapy is a highly specific and least immunosuppressive means of reducing tumour cells of the central nervous system. A brief introductory review of basic research is presented. The interim clinical results are: (i) Treatment of recurrent glioblastoma: Survival extension obtained by neutron capture therapy is 21.9 +/- 7.2 mos in contrast to that obtained by conventional treatments of 6.7 +/- 0.6 mos (p less than 0.001), (Total survival 26.3 +/- 6.7 mos); and (ii) only three patients including two glioblastoma cases were treated with neutron by the same surgeon who, by performing the first tumour operation, had the advantage in topographic knowledge for determining the radiation field. They survived 4, 5, and 6 years in almost fully active conditions. The new Musashi Institute of Technology Reactor Thermal Neutron Therapy Facility and the increased domestic production of boron-10 isotope have enlarged the therapeutic capacity to two dozen patients a year.

Evaluation of a novel immunoglobulin A capture enzyme immunoassay for diagnosis of cytomegalovirus infection in renal and heart transplant recipients.

In a retrospective cohort study of 68 organ transplant patients, the usefulness of a new commercial immunoglobulin A (IgA) antibody capture enzyme immunoassay (EIA) specific to human cytomegalovirus (CMV) for the early diagnosis of CMV disease was investigated. The results were compared with those obtained with the CMV pp65 antigen assay in peripheral blood leukocytes, an IgM immunoblot assay, and six other commercial EIAs. In 21 of 28 patients with CMV disease, the pp65 antigen assay and the immunoblot assay identified patients before the onset of disease more frequently than any other serological test method (17 and 13 patients, respectively; p = 0.0029). In patients at risk for primary CMV infection, the pp65 antigen assay was the only method that identified all patients prior to the onset of CMV disease (p = 0.008). For the other patients who were at risk for CMV infection, the pp65 antigen assay and the immunoblot assay detected infection before CMV disease more frequently than any other test system (p = 0.026). With respect to CMV disease, both immunoblotting and the pp65 antigen assay showed excellent sensitivity (100% and 89%, respectively). However, the specificity of the immunoblot was poor (41%), while the specificity of the pp65 antigen assay was reasonably good (68%). The IgA capture EIA showed moderate sensitivity (61%) and reasonable specificity (76%). In conclusion, the pp65 antigen assay, which detects pp65 antigen in leukocytes, is the method of choice for diagnosis of either primary or recurrent CMV infection. The specificity of the pp65 antigen assay was improved by additional testing for specific IgA and IgM antibodies (95% vs. 68%). The IgA assay is of limited value in renal and heart transplant patients, since it detected IgA antibodies only sporadically, and even then, too late for a timely therapy.

Chemical capture of free-ranging cattle: immobilization with xylazine or medetomidine, and reversal with atipamezole.

Twenty-nine free-ranging Norwegian cattle were captured with xylazine (n = 20) or medetomidine (n = 9) using a tranquilizing gun, and the time from darting to recumbency (induction time) was recorded. Twenty-eight animals were given atipamezole IV 15-100 min after darting, and the effects of the antagonist were evaluated. Blood samples (n = 19) for haematology and serum chemistry were collected within 10 min after immobilization was induced. Xylazine (0.55 +/- 0.18 mg/kg; mean +/- SD; n = 18) or medetomidine-HCl (0.039 +/- 0.10 mg/kg; n = 8) induced complete immobilization after a single darting with sternal or lateral recumbency, the induction times being 9.6 +/- 3.8 and 12.0 +/- 6.8 min, respectively. No difference in the clinical effects of the two drugs was observed. Rapid reversal was achieved with 0.057 +/- 0.017 and 0.077 +/- 0.019 mg/kg of atipamezole-HCl in xylazine- and medetomidine-treated animals, respectively. All the animals stood within 2 min after IV administration of the antagonist. Seven animals showed signs of excitement shortly after reversal, but these side-effects were of brief duration. Heavy resedation with relapse into recumbency was seen 3-4 h after reversal in two cows captured with xylazine, while moderate resedation was observed in two medetomidine-treated animals 2 h after reversal. Except for the plasma glucose concentration, which was elevated in both xylazine- and medetomidine-treated animals, the mean values of the haematological and plasma chemical parameters were within the reference ranges established for Norwegian cattle.(ABSTRACT TRUNCATED AT 250 WORDS)

Diagnosis of Mycoplasma pneumoniae infection by microparticle agglutination and antibody-capture enzyme-immunoassay.

The performance of two new commercial assays for the serological diagnosis of Mycoplasma pneumoniae infection (microparticle agglutination and antibody-capture enzyme-immunoassay) was studied using a panel of 169 serum samples from patients with Mycoplasma pneumoniae pneumonia and a control group. Both assays were shown to be sensitive and specific for diagnosis. The performance of the capture immunoassay, however, decreased in older patients, probably due to its inability to detect cases of reinfection without IgM antibody response.

Application of a capture enzyme immunoassay in an outbreak of waterborne giardiasis in the United Kingdom.

A capture enzyme immunoassay (EIA) for the detection of Giardia lamblia antigen was used to examine 136 fecal samples collected during an outbreak of waterborne giardiasis in a city in the UK. Six cases of Giardia lamblia infection were detected that had previously not been diagnosed by microscopy. The capture EIA provides an efficient means of processing large numbers of samples for prompt and accurate assessment of an epidemic. It may also facilitate rapid tracing of epidemic sources.

Qualitative and quantitative evaluation of charge capture by a radiology information system.

Installation of a radiology information management system (RIS) is usually justified on the basis of improved departmental efficiency and improved charge capture. However, evaluation of the success of these expected improvements is often difficult. The installation and operation of such a system in a medium-sized tertiary care hospital has permitted the effects of the RIS on the operation of the department to be studied and the improvements in charge capture provided by the system to be quantitatively assessed. As a result of a side-by-side comparison with a conventional check-sheet manual billing system, it is apparent that the RIS reduces the errors inherent in manual systems. Subjectively, it is also apparent that personnel prefer the computerized system to the manual charge sheets.

A comparison of diagnosis capture from medical records, self-reports, and drug registrations: a study in individuals 80 years and older.

BACKGROUND AND AIMS: Reports of diseases and health problems vary as a function of the information source. In the present study we compared the capture of information on morbidity using medical records, self-reports, and drug registrations. METHODS: A concurrent review of medical records, self-reports, and registration of marker drugs was conducted to determine diagnosis for 44 common diseases. Diagnoses from the various sources were uniformly classified according to ICD-10. The study included a sample of 702 individuals, aged 80 and older, enrolled in population-based longitudinal studies. RESULTS: The morbidity rates differed considerably across the used sources. Although medical records captured most of the morbidity, self-reports offered supplemental information especially for less objective health problems. Marker drugs typically confirmed information in the records, but only for a limited number of diseases. DISCUSSION: In studies of aging and health, a thorough review of medical records and a concurrent evaluation of self-reports and marker drugs represent a valuable strategy for portraying morbidity. This strategy goes beyond the use of a single source like self-reports, and provides better estimates of health conditions in the elderly.

Determination of methylmercury fluxes across the air-water and air-soil interfaces by gas chromatography with electron capture detection.

A method for the determination of methylmercury (MeHg) fluxes across the air-water and air-soil interfaces was developed using an in situ chamber. The MeHg in the air coming out of the chamber was captured by a column containing sulfhydryl cotton fiber adsorbent. MeHg was then desorbed from the column by using 2 mol L(-1) HCl. The MeHg in the effluent was extracted with benzene, and determined by gas chromatography with electron capture detection. Finally, the MeHg flux was calculated using the chamber. The method was applied to simulated experiments, and the results showed that the MeHg fluxes in the air-water system were higher than those in the air-soil-water system. The method was also successfully applied to the field measurements of an environment polluted by a chemical factory, and the results showed that the MeHg fluxes across the air-soil and air-water interfaces were 0.21-3.09 and 0.14-0.79 ng m(-2) h(-1), respectively. The method will be a useful tool in the environmental study of MeHg.

Molecule capture by olfactory antennules: mantis shrimp.

A critical step in the process of olfaction is the movement of odorant molecules from the environment to the surface of a chemosensory structure. Many marine crustaceans capture odorant molecules with arrays of chemosensory sensilla (aesthetascs) on antennules that they flick through the water. We developed a model to calculate molecule flux to the surfaces of aesthetascs in order to study how the size, aesthetasc spacing, and flick kinematics of olfactory antennules affect their performance in capturing molecules from the surrounding water. Since the three-dimensional geometry of an aesthetasc-bearing antennule is complex, dynamically-scaled physical models can often provide an efficient method of determining the fluid velocity field through the array. Here we present a method to optimize the incorporation of such measured velocity vector fields into a numerical simulation of the advection and diffusion of odorants to aesthetasc surfaces. Furthermore, unlike earlier models of odorant interception by antennae, our model incorporates odorant concentration distributions that have been measured in turbulent ambient flows. By applying our model to the example of the olfactory antennules of mantis shrimp, we learned that flicking velocity can have profound effects on odorant flux to the aesthetascs if they operate in the speed range in which the leakiness of the gaps between the aesthetascs to fluid movement is sensitive to velocity. This sensitivity creates an asymmetry in molecule fluxes between outstroke and return stroke, which results in an antennule taking discrete samples in space and time, i.e. "sniffing". As stomatopods grow and their aesthetasc Reynolds number increases, the aesthetasc arrangement on the antennule changes in a way that maintains these asymmetries in leakiness and molecule flux between the outstroke and return stroke, allowing the individual to continue to take discrete samples as it develops.

The manifestation of attentional capture: facilitation or IOR depending on task demands.

Orienting attention exogenously to a location can have two different consequences on processing subsequent stimuli appearing at that location: positive (facilitation) at short intervals and negative (inhibition of return) at long ones. In the present experiments, we manipulated the frequency of targets and responses associated with them. Results showed that, even at long SOAs, where IOR is usually observed, facilitation was observed for infrequent targets at the same time that IOR was measured for frequent targets. These results are difficult to explain on the basis of either task set modulation of attentional capture or task set modulation of subsequent orienting processes. In contrast, we offer an explanation by which the different cuing effects can be considered as different manifestations of attentional capture on target processing, depending on the task set.

Haplotype-based analysis of alpha 2A, 2B, and 2C adrenergic receptor genes captures information on common functional loci at each gene.

The alpha 2-adrenergic receptors (alpha2-AR) mediate physiological effects of epinephrine and norepinephrine. Three genes encode alpha2-AR subtypes carrying common functional polymorphisms (ADRA2A Asn251Lys, ADRA2B Ins/Del301-303 and ADRA2C Ins/Del322-325). We genotyped these functional markers plus a panel of single nucleotide polymorphisms evenly spaced over the gene regions to identify gene haplotype block structure. A total of 24 markers were genotyped in 96 Caucasians and 96 African Americans. ADRA2A and ADRA2B each had a single haplotype block at least 11 and 16 kb in size, respectively, in both populations. ADRA2C had one haplotype block of 10 kb in Caucasians only. For the three genes, haplotype diversity and the number of common haplotypes were highest in African Americans, but a similar number of markers (3-6) per block was sufficient to capture maximum diversity in either population. For each of the three genes, the haplotype was capable of capturing the information content of the known functional locus even when that locus was not genotyped. The alpha2-AR haplotype maps and marker panels are useful tools for genetic linkage studies to detect effects of known and unknown alpha2-AR functional loci.

Discrimination between Atlantic and Pacific Subspecies of Northern Bluefin Tuna (Thunnus thynnus) by Magnetic-Capture Hybridization Using Bacterial Magnetic Particles.

The previously developed magnetic-capture hybridization technique employing bacterial magnetic particles was applied to discriminate between Atlantic and Pacific subspecies of the northern bluefin tuna (Thunnus thynnus) using specific DNA sequences. Nucleotide sequences of a 925-bp fragment (ATCO) flanking the mitochondrial ATPase and cytochrome oxidase subunit III genes in these two subspecies were compared. Two regions having single-nucleotide and three-nucleotide differences between the subspecies were adopted to design DNA probes (NR1, 21-mer; NR2, 29-mer), and two internal primer sets were designed to amplify DNA fragments containing these regions. The DNA probes were immobilized on bacterial magnetic particles via streptavidin-biotin conjugation and subjected to magnetic-capture hybridization with the digoxigenin-labeled fragments amplified using the internal primers. The luminescence intensities of DNA on bacterial magnetic particles obtained by hybridization between the probes and the complementary fragments were higher than those obtained by hybridization with noncomplementary fragments. These data suggest that this system employing DNA on bacterial magnetic particles may be useful for discrimination of these two subspecies by recognizing a single-nucleotide difference.

Visualizing the laboratory mouse: capturing phenotype information.

A concerted effort to develop myriad new phenotypic alleles through mutagenesis programs presents new challenges for the biomedical community and for the informatics infrastructure needed to support this work. To handle and co-ordinate large programs of treatment, breeding, and sequential or longitudinal testing for a variety of obvious and subtle traits requires sophisticated data management software. Further, trait analyses, heritability testing, and animal availability and status must be captured and disseminated to the wider community. The Mouse Genome Database (MGD) will serve as the central integration point for the various mutagenesis programs, registering new alleles, providing accession identifiers, and capturing phenotypic descriptions. In addition, MGD will provide public access to unified searches over all alleles with links to the centres of origin for detailed testing data.

Market capture of inpatient perioperative services using DEA.

We develop and validate a method to measure "market capture" of inpatient, elective surgery. Data envelopment analysis (DEA) is used to measure the efficiency of the market capture for Perioperative Services at 53 non-metropolitan Pennsylvania hospitals. Eight procedures are studied, representing a wide spectrum of elective, scheduled, inpatient surgery (e.g., abdominal aortic aneurysm resection and hip replacement). Our results address issues in operating room management, such as: How should additional resources be allocated to each surgical specialty? Given existing market conditions, for which specialties can we expect to be able to increase our current workloads? Our results demonstrate DEA's potential as a valuable tool for operating room managers' strategic decision-making.

Failure of automatic capture verification in an epicardial pacemaker system.

Epicardial pacing is frequently used in pediatric patients with congenital heart disease. Automatic threshold determination has been reported to be safe in epicardial lead systems. We report a case of falsely low ventricular thresholds determined by automatic capture verification in a patient with complete loss of capture due to a lead fracture.

Capture of female Hylotrupes bajulus as influenced by trap type and pheromone blend.

Three different types of traps were evaluated for a semiochemical-based trapping method for Hylotrupes bajulus (L.) (Cerambycidae). One, designated the ground trap, was the most efficient under both laboratory and natural conditions and had an active space of about 3.5 m. Significantly more beetles were captured in traps baited with a mixture of synthetic pheromones, (3R)-3-hydroxy-2-hexanone [(3R)-ketol] + 1-butanol, than in those with either single compound or with racemic mixtures. Furthermore, the synthetic lure captured more females than two virgin males in a laboratory bioassay. In addition, doubling the concentration of the synthetic pheromone significantly increased trap catches. The present findings have potential value of the management of this pest.

Quantitative methodology for corticosteroids based on chemical oxidation to electrophilic products for electron capture-negative chemical ionization using capillary gas chromatography-mass spectrometry. I. Assessment of feasibility in the analysis of horse urine for dexamethasone.

Sensitive and specific methodology based on capillary column gas chromatography-electron capture-negative chemical ionization-mass spectrometry has been developed for the quantitative analysis of corticosteroids from biological fluids. The feasibility of this method is demonstrated in the quantitative analysis of dexamethasone in horse urine following administration of the drug. A structurally similar compound, 6 alpha-methylprednisolone, is added to the urine as an internal standard. The free dexamethasone and the internal standard are extracted and oxidized to high-electron-affinity 1,4-androstadiene-3,11,17-trione structural analogs and then analyzed by capillary column gas chromatography-electron capture-negative chemical ionization-mass spectrometry.

Labeled antigen capture assay: a method for detecting monoclonal antibodies to cloned gene products.

We report here a relatively easy and highly sensitive assay for detecting monoclonal antibodies to the product of virtually any cloned gene. The protocol, termed labeled antigen capture assay (LACA), is a solid-phase type radioimmunoassay which uses a bacteriophage T7 expression system to generate exclusively radiolabeled antigen. Thus, to generate radiolabeled antigen for screening, the gene encoding the protein of interest need only be subcloned downstream of a T7 promoter, and the new construct transformed into an Escherichia coli strain harboring a compatible plasmid which encodes a thermal inducible copy of the T7 RNA polymerase. Expression of the T7-promoted gene in the presence of rifampicin and [35S]cysteine (or methionine) yields labeled antigen, which is then "captured" by specific monoclonal antibody and detected by autoradiography. Our results indicate that as little as 30 ng of specific monoclonal antibody can be detected using the LACA protocol. The protocol is applicable to the product of any cloned gene but is particularly useful in the case where the biochemical properties of the gene product of interest are unavailable. In this report we use the LACA protocol to screen a hybridoma library for monoclonal antibodies to the STb heat-stable enterotoxin (STb) of E. coli. Mice were immunized with a genetically constructed, affinity-purified Protein A-STb hybrid protein, and following spleen cell fusion and HAT selection, hybridomas were screened by the described LACA protocol for production of STb-specific monoclonal antibody. Of over 1500 hybridomas tested 138 were positive, by primary LACA screening, for STb-specific IgG monoclonal antibodies.
