[Endoscopy and sonography change a specialty: exemplified by gynecology].

Pelviscopy, a surgical technique employing new apparatuses and instruments, has replaced the classic laparotomy in gynecology in over 75% of cases. According to an organ-oriented surgical catalogue, pelviscopic surgery has proven to be successful in the treatment of ectopic pregnancy (nearly 100%), ovarian cyst enucleation, oophorectomy, and myoma enucleation. In cases of extensive bowel and omental adhesiolysis, pain-free status is achieved in 60%. Second-look and repelviscopy enhance the success rate of adhesiolysis. The results achieved in Kiel have been statistically proven in over 15,000 peliviscopies.

[On postoperative azotemia in geriatric surgery: significance, risk groups, possibilities of prophylaxis (author's transl)].

1032 Interventions on geriatric patients beeing more than 70 years old between 1966 and 1975 demonstrate the significance of azotemia in the postoperativ stage of the disease appears as well as its dependence upon prehospital factors, type and localisation also on the extension and duration on the intervention in each single case. Since it is hardly possible to exert an influence on the preliminary stress due to the old age and on the dysfunction determined by involution of the parenchymatose organs important for the course of the postoperative stage, a particular emphasis should be laid upon the prophylactic aspekt to the purpose of an appropriate screening against vegetativ-visceral, renally effective shock reflexes (hepatorenal syndrome) and on the other, usually extra-renal factors (PH 203R, splanchnic block). Further possibilities: considerate elimination of pain and general anesthesia, purpuseful and quick operative technique, improvement of the apparatus and instruments, after-treatment specific for the given case so as one consisting of a prophylaxis against pneumonia through a rapid reactivation of the operated person. However, regardless to all the professional advance made in this field, the primary human approach to the aged patient and to his life expectancy should be made due allowance for.

[Development of motoricity as functional-neurologic diagnosis].

Movement is one of the characteristic features of a living organism. The movements of human beings are controlled by the brain via the nervous system. Disturbances in the development of the brain therefore also manifest themselves in the organization and course of motoricity. The normal and pathological development of the child--from birth through to maturation--therefore also shows itself in the differentiation of motoricity and psychomotoricity. In the first three years of life the organization of motor coordination is even the most reliable indicator of normal somatic and psychic development and of the functional capacity of the central nervous system. The author discuss at length: The development of tonicity. The diagnostically most significant reflex mechanisms of neonates; time of occurrence and disappearance within the framework of the integration of voluntary movements. Motometric studies for determining the various coordinative capacities, the discussion centering on: The motor functions of the maturation test. Examinations of small children using the methods developed by Griffith, Brunet-Lézine. Metric scale according to Oseretzky, mimic scale according to Kwint. Disturbances of writing motoricity. Coordinative examinations on juveniles and adolescents. The motor diagnosis may be carried out without the need for using a larger number of apparatus and instruments and--provided broad methods of examination and longitudinal-section controls are employed--permits to obtain results, the reliability of which is not at present surpassed by those obtained using any other method.

[Lamellar keratoplasty with the excimer laser. Initial clinical results].

If excimer laser techniques are used to prepare the bed of the recipient eye and to prepare the donor lenticule diameter and thickness, congruence of the cut surfaces will be optimal. MATERIALS AND METHODS. The preliminary in vitro investigations have already been reported. We now present the clinical results of three patients with circumscript corneal opacities, whom we treated with the lamellar keratoplasty in accordance with the calibration method we developed (follow-up 4-12 months). The lenticule thickness required varied between 320 and 415 microns, depending on the extent of the opacity. The lenticule was fixed with a suture for 9 months. RESULTS. All transplants healed without complications. The parts of the cornea treated with the laser did not show opacities in the "interface" or adjacent corneal layers. CONCLUSION. Besides optimal fit of the transplant, the absence of intrastromal opacities makes the instrument and apparatus requirement worthwhile. The clinical results are superior to those of conventional dissection methods. The absence of intrastromal opacities indicates that our method will be suitable for treating high-grade myopia. If possible, intrastromal treatment should be carried out after lamellation (LASIK).

[Kinetics stability of liposomes].

Liposomes find extensive use as drug carriers. The surface characteristics of vesicles (particle size, charge, composition, hydrophobicity etc.) and also, the kinetic stability of liposomes are all key factors for controlling fundamental carrier properties, such as the encapsulation or adsorption efficiency, the organ distribution of the carrier or the RES clearance etc. Small unilamellar (SUV) vesicles of dimyristoyl-phosphatidylcholine (DMPC) were prepared under standardized conditions. The liposomes were prepared both in the absence and the presence of an uncharged polymer [polyvinyl alcohol (PVA), polyvinyl acetal (PVA1) and polyvinyl pyrrolidone (PVP) respectively] and simultaneously, the long-term (kinetic) stability of the aqueous lipid dispersions were studied. The aggregation behavior of colloidal dispersions of DMPC liposomes were tested in physiological salt solution and polymer solutions, respectively. Particle aggregation, which may take place on storage or when the stability is lowered, are well reflected in the shift of the mean size and the distribution function towards higher values. The particle sizes, the size distribution and their change in time were measured by photon correlation spectroscopy using a Malvern ZETASIZER 4 apparatus (Malvern Instruments, UK).

Electronic core level microanalyses and microcopies in multipurpose apparatus.

Using an instrument equipped with two electron guns, an electron analyzer, and a Si(Li) diode detector, we developed microanalytical techniques based on inner-shell electron excitations by incident electrons and X-rays, that is, electron energy-loss spectroscopy (EELS) in the reflection mode; electron probe microanalysis (EPMA) and X-ray appearance potential spectroscopy (XAPS); electron-induced Auger electron spectroscopy (e-AES); X-ray photoelectron spectroscopy (XPS), X-ray absorption spectroscopy (XAS); X-ray induced AES (XAES), X-ray fluorescence analysis (XRF), and scanning X-ray radiography (SXR). The corresponding characteristic images (including X-ray microradiography and X-ray photoelectron microscopy) were obtained in the scanning mode. The principle of the apparatus is described. Each spectroscopy and microscopy is illustrated by an example. Their performance and limits are discussed.

An examiner-induced visual vigilance apparatus.

A film/record instrument is described that could measure attending behavior, distractibility, hyperkinetic movement, and selected aspects of visual perception by recording the subject's response to significant visual stimuli.

The hematocrit-electrolyte-concentration-apparatus (HECA).

An instrument was developed which can simultaneously determine the hematocrit and the total electrolyte concentration in blood and plasma respectively. It is possible to correct the hematocrit value for fluctuations of the total electrolyte concentration. Pilot studies of this device have been conducted on physiological and rigid erythrocyte suspensions and on plasma with various total electrolyte concentration. In the future, this device will be employed in analyzing blood and plasma of patients undergoing hemodialysis.
