[Structural and functional findings in experimental blind loop syndrome].

Proximal and distal the blindsack a villus and crypt prolongation connected with a decreasing absorption of octanoate could be pointed out. The structural changes are very likely a hyperplasia of the mucosa. Within the blindsack a hyperplasia in connection with an accelerated proliferation of cells and an almost three and a halffold enlargement of the surface of the villi could be found. Simultaneously, the mucosa was damaged. An artificial bile duct as well as a neomycin therapy caused a decrease of the structural and functional changes but did not prevent the mucosal hyperplasia. These results are explained with adaptive processes of the small intestinal mucosa in a sense of hyperregeneration alterations of sprue typus.

[Absorption of medium chain triglycerides in patients with blind loop syndrome].

Absorption of medium chain triglycerides (MCT) was estimated in 10 patients with stagnant loop syndrome (SLS). Six patients had steatorrhea due to gastrojejunal anastomosis and 4 due to diabetic neuropathy. The diagnosis of SLS was confirmed by antibiotic therapeutic test after exclusion of other causes of malabsorption syndrome. Each patient was submitted to 2 types of diet during one week each, differing only in the type of added lipid. In the diet with long chain triglycerides (LCT), soy oil and butter were added. In the diet, with medium chain triglycerides, only MCT oil from babaçu was added. The excretion of lipid was determined in the feces in the last 3 days of each diet. The results showed that the medium fecal lipid excretion was 48.6 (sd = 19.8) g/d in the LCT diet and 26.6 (sd = 13.8) g/d in the MCT [d = 22.0; s(d) = 3.10; t = 7.10; p less than 0.001], which indicates that in patients with SLS, MCT diet was better absorbed than LCT diet. It is suggested that MCT oil may be used as a caloric source during nutritional recovery in SLS.

Blind loop syndrome: multiple ileal ulcers following side-to-side anastomosis.

A 59 yr old woman who had multiple ileal ulcers following side-to-side anastomosis without bowel resection is reported. She had a surgical history of adhesive ileus 15 yrs earlier, and was admitted with a complaint of lower abdominal pain. A barium meal study showed a stagnant and dilated distal ileum. At laparotomy, a previously performed side-to-side ileal anastomosis was encountered, and a markedly dilated bypassed loop was recognized. The affected intestine was resected revealing multiple longitudinal ulcers and small shallow ulcers mostly located on the mesenteric side. This feature is similar to that of ischemic enteritis. This case further supports the fact that when side-to-side anastomosis is performed as a bypass operation, multiple ulcers may develop in a bypassed loop after a long period of time.

Kinetics of bile acid metabolism in experimental blind loop syndrome.

In an experimental model of the contaminated small bowel syndrome, Eriksson's washout technique was used to study bile salt metabolism. Bile salt secretion rate was increased from 204-7 +/- SD 36-5 mumol/100 g body weight/24 h to 329-9 +/- 146-6, pool size from 33-4 +/- 5-4 mumol/100 g BW to 42-5 +/- 7-2, and biliary flow from 6-6 +/- 1-72 ml/100 g BW/24 h to 8-4 +/- 2.1. Detailed analysis of the washout curves gave indirect evidence of a short-circuit jejunohepatic shunt in the model contaminated small bowel syndrome. Parameters of this shunt were calculated with 9-1 mumol/100 g BW for 'short-circuit' pool size and 13-8 for its circulation frequency.
