The kinematics and intra- and inter-therapist consistencies of lower cervical rotational manipulation.

The aim of this study was to determine the in vivo kinematics of cervical rotational manipulation in normal subjects and examine the consistency of this technique within and between therapists. A four camera motion analysis system operating at 120 Hz was used to measure the head on trunk angular displacements during manipulation performed by three therapists on eight subjects. One of the therapists performed the manipulation twice for each subject over separate sessions. A consistent pattern of de-rotation prior to thrust was found with little motion other than axial rotation during de-rotation and thrust. The pooled mean de-rotation displacement was 4.8 degrees and the pooled mean thrust angle was 11.3 degrees , but these varied widely, and none of the kinematic parameters recorded proved to be consistent within or between therapists. Most of the kinematic parameters were correlated with the exception of thrust duration. Qualitative analysis shows a consistent technique in cervical rotational manipulation. Pre-manipulation positioning of the head relative to the trunk was fairly consistent for a single therapist over separate sessions, but other than this, the kinematic parameters in cervical rotational manipulation are generally inconsistent within and between therapists.

The lower eyelid retractor consists of definite double layers.

PURPOSE: To examine whether the lower eyelid retractor consists of a single layer or multiple layers. DESIGN: Retrospective clinical case series and dissectional study. PARTICIPANTS: Fifty-one lower eyelid retractors (31 right, 20 left) of 44 patients (ages, 63-95 years) during entropion surgeries and 10 lower eyelids (5 right, 5 left) of 5 Oriental cadavers (73-91 years old at death) were observed macroscopically. Specimens from 20 postmortem lower eyelids of 12 Orientals (11 right, 9 left; 66-96 years old at death) were used for microscopic observations. METHODS: Macroscopically, we bluntly or, in parts, sharply dissected lower eyelid retractors into 2 layers during entropion surgeries. Cadaveric lower eyelids also were used to investigate relationships between the lower eyelid retractor and the Lockwood ligament. Cadaveric lower eyelids with sagittal full-thickness sections of the central part were examined microscopically using Masson trichrome staining. MAIN OUTCOME MEASURES: Anatomical findings in the lower eyelid retractor. RESULTS: Lower eyelid retractors easily were detached into 2 layers. Macroscopically, the anterior layer was defined as a coarse tissue continuing from the Lockwood ligament, which joined with the lower orbital septum, and constituted the lower conjoined fascia. The posterior layer appeared to be a dense fibrous sheet with a lustrous surface. Microscopically, the lower eyelid retractor consisted of a definite double layer. The anterior layer, a coarse fibrous tissue, consisted of the suborbicularis fascial layer, orbital septum, and superficial part of the capsulopalpebral fascia, which continued to the anterior lamellae of the lower eyelids. The posterior layer consisted of dense fibers of the capsulopalpebral fascia with smooth muscle continuing to the tarsus. CONCLUSIONS: The lower eyelid retractor consists of a definite double layer. The posterior dense layer containing smooth muscle is the main tractional component of the lower eyelid retractor.

Shedding consistency of strongyle-type eggs in Dutch boarding horses.

Faeces of 484 horses were sampled twice with an interval of 6 weeks while anthelmintic therapy was halted. Faecal eggs counts revealed that 267 (55.2%) horses had consistently low numbers of eggs per gram faeces (EPG) (EPG < 100 or = 100), 155 (32.0%) horses had consistently high EPGs (EPG > 100). Horses with consistently high EPGs were more often mares with access to pasture, aged less than 6 or more than 23 years, that were dewormed at intervals longer than 6 months, and were treated for the last time more than 3 months before the start of the study. Horses with consistently low EPGs were more often male horses with no or limited access to pasture, that were dewormed at maximally 6-month intervals, and were aged between 6 and 23 years. The results are an indication that some horses have consistently low EPGs and perhaps could be used as non-treated animals in a selective anthelmintic treatment scheme aimed at the prevention of the development of anthelmintic resistance.

The fascial planes of the temple and face: an en-bloc anatomical study and a plea for consistency.

Many investigators have detailed the soft tissue anatomy of the face. Despite the broad reference base, confusion remains about the consistent nature of the fascial anatomy of the craniofacial soft tissue envelope in relation to the muscular, neurovascular and specialised structures. This confusion is compounded by the lack of consistent terminology. This study presents a coherent account of the fascial planes of the temple and midface. Ten fresh cadaveric facial halves were dissected, in a level-by-level approach, to display the fascial anatomy of the midface and temporal region. The contralateral 10 facial halves were coronally sectioned through the zygomatic arch at a consistent point anterior to the tragus. These sections were histologically prepared to demonstrate the fascial anatomy en-bloc with the skeletal and specialised soft tissues. Three generic subcutaneous fascial layers consistently characterise the face and temporal regions, and remain in continuity across the zygomatic arch. These three layers are the superficial musculo-aponeurotic system (SMAS), the innominate fascia, and the muscular fasciae. The many inconsistent names previously given to these layers reflect their regional specialisation in the temple, zygomatic area, and midface. Appreciation of the consistency of these layers, which are in continuity with the layers of the scalp, greatly facilitates an understanding of applied craniofacial soft tissue anatomy.

The handedness of rhesus monkeys. III. Consistency within and across activities.

The results of this study of the hand preferences of rhesus monkeys on three different tasks are threefold: (1) When retested on the same task at intervals exceeding one month virtually all individuals prefer the same hand as they did during the original test, (2) When retested on the same task, the strength of hand preference displayed by each individual is increased. (3) When tested on differing tasks, monkeys display little consistency in the laterality of hand preference or the strength of handedness expressed during different tasks. Many authors have concluded that the lack of obvious intertask consistency in the laterality of hand preference expressed by lower primates constitutes evidence for a corresponding lack of consistent laterality in the cerebral control of this behavior. This has led to them to conclude that cerebral dominance probably does not exist in these animals (Deuel, 1975; Warren, 1977). However, where data is available from the literature, including the present study, all reports show monkeys to more frequently prefer the same hand on all of the unimanual tasks they were given than would be expected by chance alone. This finding suggests that there is a weak tendency for consistent lateralization of hand usage in the monkey. Presumably, there is a corresponding predominance of the cerebral hemisphere contralateral to the preferred hand over its mate. Other studies consistent with the concept of cerebral predominance in the monkey were reviewed. These findings do not constitute evidence for cerebral dominance in the monkey akin to that found in man. They do suggest that when performing certain activities, monkeys may have one hemisphere predominant over the other even though the degree and laterality of predominance may vary greatly from one individual and task to another.

Consistency of remission and outcome in bipolar and unipolar mood disorders: a 10-year prospective follow-up.

BACKGROUND: Previous studies of the natural course of bipolar disorder have suggested that relapse and psychosocial impairment are more common than many would expect during treatment under routine conditions. The present research sought to identify patterns of consistency in longitudinal recovery after an index manic or depressive episode in patients formerly hospitalized for major affective disorders. The data extend prior findings from the Chicago Follow-up Study involving four successive assessments of course and outcome for bipolar and unipolar patients over a 10-year follow-up interval. METHODS: Thirty-four RDC bipolar I manic, 17 psychotic unipolar depressed, and 72 nonpsychotic unipolar depressed patients were assessed at index hospitalization and prospectively followed-up at 2, 4.5, 7.5 and 10 years. Psychosocial functioning, rehospitalization, and overall outcome were rated by standardized instruments. Patterns of consistent remission or impairment in functioning were compared across follow-up periods. RESULTS: Consistently good functioning was observed more often among patients with unipolar nonpsychotic depression than bipolar disorder or unipolar psychotic depression. Psychosis at index hospitalization was not associated with poorer outcomes for either the bipolar or unipolar groups, although it significantly predicted psychosis at follow-up more robustly for unipolar than bipolar patients. Less than half of the bipolar cohort had good work performance at each follow-up, while unipolar nonpsychotic depressed patients had consistently better work functioning. Rehospitalization was more common for the bipolar than unipolar patients at the 4.5- and 7.5-year assessments. Sustained remission across follow-ups was associated with remission at subsequent follow-ups regardless of diagnostic polarity at index hospitalization. LIMITATIONS: The observational design, small sample size, and naturalistic treatment of subjects in this protocol prompts caution when interpreting treatment outcome findings. The availability of a bipolar cohort with relatively low attrition over a 10-year period may limit the generalizability of the current findings to patients who remain engaged in sustained contact with a long-term research program. CONCLUSIONS: Over a 10-year follow-up period, about half of bipolar patients show sustained remissions or patterns of improvement, while 30-40% experience some functional decline. The degree of consistency in remission patterns over time may hold greater prognostic significance than the polarity of an index affective episode in anticipating subsequent levels of psychosocial adjustment in severe mood disorders.

Panellist consistency in the assessment of medical appropriateness.

Where information about the appropriateness of a surgical procedure is lacking, expert panels have been used to establish guidelines for medical practitioners. Such a panel was convened to assess the appropriateness of percutaneous transluminal coronary angioplasty and coronary artery bypass graft surgery in the Netherlands. The panel, consisting of interventional cardiologists and cardiothoracic surgeons, used a modified Delphi process to rate 1126 clinical indications over two rounds. This article describes the degree of change in both agreement amongst members and in the appropriateness ratings over the two rounds, and examines the internal consistency of the ratings of individual panellists. Over the two rounds, agreement increased. Although most appropriateness ratings remained unchanged, there was significant movement from equivocal ratings to determinate ratings. While individual members showed some degree of inconsistency in their scoring, the panel as a whole scored very consistently. The observed changes in appropriateness were consistent with expectations, showing that the appropriateness method is used logically and consistently by panellists.

Patterns of contraceptive use among female adolescents: method consistency in a clinic setting.

Major changes in contraceptive use among adolescents in the United States have been documented through surveys and analysis of family planning program service statistics. While we know of the changes in the adolescent population as a whole and those served by organized family planning programs, little is known about the patterns of contraceptive use by individuals. This study examines patterns of contraceptive use among 547 clients of a new contraceptive and counseling service for adolescents in New York City. The principal concern is consistency of contraceptive method use as determined from clinic statistics. Findings show generally high degrees of method consistency, with new and current users displaying more consistency than prior users. Among clients who switch methods, two major patterns emerged: switching among the more effective methods and from less effective to more effective methods. Younger new users were the most consistent subgroup suggesting that a service designed for adolescents may have a positive influence on teenagers without prior contraceptive experience. Prior users were the least consistent subgroup suggesting that programs incorporate screening procedures to identify prior users and to develop new approaches to understanding their past contraceptive use patterns to improve their future use.

The evaluation of resin composite consistency.

OBJECTIVE: To evaluate a method for the assessment of the consistency of unset resin composite. METHODS: Fourteen commercial resin composites were selected, which had a range of consistencies as assessed subjectively. The material was placed in a cylindrical mold with 8 mm diameter and 8 mm depth, and a glass rod with 4.37 mm diameter driven into the composite at 24.4 mm min-1 for 6 s, i.e., to a depth of 2.44 mm. The maximum force reached during the 6 s period was recorded, and used as a measure of consistency. RESULTS: Consistency values ranged from 0.33 N to 31.3 N, with the highest values being found for some of the recently introduced 'packable' materials. The reliability of the test was high. SIGNIFICANCE: The test may be useful as a standard method of assessing resin composite consistency.

A new phase consistency criterion and its application in electron crystallography

In this work, we present the principles and potential advantages of a methodology to assess Fourier components in terms of phase consistency. We define a new phase consistency criterion among sets of spatially translated images based upon a novel conception of the spatial shift property of the Fourier transform. The article shows how this criterion can be used in the alignment stage of the 3D reconstruction process with a two-fold objective: Assessment of the frequency components and robustness in the alignment. In that sense, the article shows and analyzes the results obtained from the application of the new index of quality in the context of projection image alignment. We have focussed our attention on the electron crystallography field, by applying such a phase consistency definition over image reflections. The results that have been obtained show that the new phase-consistency definition may complement the traditional SNR-based index of quality (commonly known as IQ) of reflections. As a consequence, the reliability of the alignment may be improved by discarding those reflections judged as non-reliable according to the phase-consistency criterion.

Comparisons of the five analytic reference lines of the horizontal lip position: their consistency and sensitivity.

One-hundred ten lateral facial profiles were judged to be attractive and were selected from 1000 Taiwanese pupils by a panel of four men and three women from different educational backgrounds. The 110 in the attractive group were analyzed by using five analytic reference lines: Ricketts E line, Holdaway's H line, Steiner's S1 line, Burstone's B line, and Sushner's S2 line. Comparisons of the "consistency" (the smaller coefficient of variation the better the consistency) and the "sensitivity" (the power to differentiate the attractive lateral facial profiles from the unattractive ones) of the five analytic lines were performed. The B line was found to be best in terms of both consistency and sensitivity. From the point of view of convenience, the E line is of great value because its anterior location makes it convenient for the clinician to use. However, the B line appears best from the perspective of the value of reference. This study found that those analytic reference lines that do not transverse an anatomic landmark of the nose most likely have poor consistency and sensitivity. This finding is consistent with the idea that the nose should be taken into consideration when a line is to be used as a reference for beauty on the lateral facial profile.

Consistency of inhalatory breathing patterns in professional operatic singers.

Respiratory behavior is generally considered important to voice function, the assumption being that it affects the voice source. Accurate and consistent control of the voice source is particularly important in professional operatic singing. An erratic behavior of a factor influencing voice production is incompatible with a well-controlled vocal behavior. We analyzed the consistency of inhalatory breathing patterns during singing in five professional operatic singers, using the same material as in a previous investigation of phonatory breathing patterns. Rib cage (RC) and abdominal wall (AW) movements were recorded by respiratory inductive plethysmography. Consistency was analyzed in terms of the mean correlation between three takes of ten musical phrases. Results revealed a high consistency in lung volume (LV) change and RC movements in all singers and in AW movements in three singers. Consistency across different phrases was slightly lower. The results are compatible with the idea that inhalatory behavior is important to voice source in singing. A high correlation between LV change and RC movement was found in all singers and between LV change and AW movement in three. The contribution to LV change from RC was greater than that from AW in all singers.

Consistency of inter-trial activation using single-trial fMRI: assessment of regional differences.

Recently, the technique of single-trial fMRI was introduced, which allows the assessment of hemodynamic responses to single task executions (e.g. sensory, motor, or cognitive). In this study, single-trial fMRI was used to examine regional differences in the inter-trial consistency (ITC) of brain activity related to the processing of a dynamic visuo-spatial imagery task. For every single trial, a t-statistic assessing task-related activation was calculated and thresholded at a p-value of P < or =l0.05 (uncorrected). The percentage of trials with t-values above the threshold was used to assess differences in the consistency of brain activity in occipital, parietal, premotor and prefrontal regions of interest. While most of these regions showed activation which was highly consistent across trials, the consistency of activation was significantly reduced in the dorsolateral prefrontal cortex. We hypothesize that the consistency of activation across trials may be interpreted as an indicator of the functional relevance of a brain region for processing and solving a cognitive task. Thus, the analysis concept presented here has the potential to provide new insights into the neuro-cognitive mechanisms of human information processing. In addition, the results of this study confirm that averaging across trials might result in a significant loss of information about functional neuroanatomy. Regions which are active in some trials only, which show only weak activity increases, or whose activity is not constant and time-locked, might not show up in averaged activation maps and could thus erroneously be interpreted as irrelevant for task processing.

Objective evaluation of liver consistency to estimate hepatic fibrosis and functional reserve for hepatectomy.

BACKGROUND: The empiric evaluation of liver consistency is currently used to plan the surgical strategy. The aim of this study was to verify the feasibility of the objective measurement of liver consistency and to check its correlation with liver fibrosis and liver functional reserve. STUDY DESIGN: Fifty-two consecutive patients who underwent hepatic resections in our department were enrolled. The indications for liver resection were hepatocellular carcinoma in 36 patients, metastatic liver tumors in 12 patients, and other conditions in 4 patients. Liver consistency was measured with a new tactile sensor. A fibrosis index was calculated as an expression of the percentage of fibrotic tissue. Liver consistency was compared with the degree of liver fibrosis observed in histologic specimens (fibrosis index) and with liver function parameters. RESULTS: Liver stiffness showed a significant positive correlation with fibrosis index (r = 0.887, p < 0.0001). Liver stiffness also showed significant positive correlation with the indocyanine green test (r = 0.631, p < 0.0001) by a univariate analysis. The indocyanine green test and platelet count were independently and significantly associated with liver stiffness by a multiple regression analysis. In five patients, the liver stiffness values measured intraoperatively differed markedly from those expected from the indocyanine green test values. In these patients, the operative procedures were finally selected based on the liver stiffness measured with the tactile sensor and good clinical outcomes were obtained. CONCLUSIONS: These results show for the first time that liver stiffness can be clinically assessed quantitatively by means of the tactile sensor. The tactile sensor adequately estimates liver stiffness and this estimation is well correlated with liver fibrosis and functional reserve. Liver consistency determined objectively in this manner may be useful for optimizing surgical decision making.

Consistency of the efficacy of cetirizine and ebastine on skin reactivity.

BACKGROUND: At therapeutic dosage, cetirizine and ebastine induce significant inhibition of skin reactivity to histamine. The consistency of their efficacy, that is, efficacy with the least interindividual variability among subjects, has not been carefully assessed, however. OBJECTIVE: To compare the consistency and efficacy of these antihistamines on skin reactivity. METHODS: Twenty-four healthy volunteers participated in a randomized double-blind crossover study. The areas of wheals and flares induced by increasing (0, 5, 10, 50, 100, 200, and 300 mg/mL) histamine concentrations, administered by prick tests, were measured before and four hours after intake of 10 mg of each antihistamine, allowing concentration-response curves to be established. The threshold histamine concentrations inducing wheal areas of 3 mm2 (positivity) were calculated by interpolation. The coefficient of variation (SD/mean %) was used to evaluate the consistency of the response. RESULTS: Pretreatment concentration-response curves were similar, and threshold concentrations identical (0.29 mg/mL and 0.34 mg/mL for cetirizine and ebastine, respectively). For both, curves were lower after treatment than before. After cetirizine, the threshold concentration was significantly higher (217 mg/mL) than after ebastine (0.82 mg/mL) (P < .001), and total inhibition of the wheal reaction was observed in 21 of 24 patients at the lowest histamine concentration and in 17 of 24 at the highest. Ebastine never totally inhibited reaction, even to 5 mg/mL of histamine. Over the entire concentration-response curve, the coefficient of variation for the wheal reaction was 6.3% for cetirizine and 72.6% for ebastine, and, for flares, 11.0% and 83.7%, respectively. Hence, variability was much lower after cetirizine. CONCLUSION: Our study shows clearly that the efficacy of a single therapeutic dosage of cetirizine is consistently good for suppression of cutaneous reactivity to histamine in healthy volunteers. The need for ebastine to metabolize into the active carebastine might explain its lesser consistency.

Polymer Brushes: From Self-Consistent Field Theory to Classical Theory.

We consider planar brushes formed by end-grafted polymers with moderate to strong excluded-volume interactions. We first rederive the mean-field theory and solve the resulting self-consistent equations numerically. In the continuum limit, the results depend sensitively on a single parameter, beta, whose square is the ratio of the scaling prediction for the brush height to the unperturbed polymer radius of gyration, and which measures therefore the degree to which the polymers are stretched. For large values of beta, the density profile is close to parabolic, as predicted by the infinite-stretching theory of Zhulina et al. and of Milner et al. As beta decreases, the profile deviates strongly from a parabolic one. By calculating the most probable paths and comparing their contribution to various properties with those obtained from the full self-consistent theory, we determine the effect of the fluctuations about such paths. At large values of beta, these effects are very small everywhere. As beta decreases, fluctuation effects on the density profile become increasingly important near the grafting surface, but remain small far from it. For all values of beta, we find that polymer paths which begin far from the grafting surface are strongly, and almost uniformly, stretched throughout their length, including their free end points. Paths which begin close to the grafting surface are also stretched, but they initially move away from the grafting surface before reaching a maximum height and then returning to it. The classical theory is then derived from the self-consistent field equations by retaining, for each end point location, only that single trajectory which minimizes the free energy of the system. This free energy contains an entropy, of relative weight beta-1, which arises from the distribution of end points. Even for brushes which are only moderately stretched, the results of the classical theory for the brush profile and polymer end point distribution agree well with those of the full self-consistent theory except near the grafting surface itself. There the density profile as calculated in the self-consistent theory shows a characteristic decrease which is not captured by the classical theory. However it does capture the fact that the individual polymer paths are stretched in general throughout their length, including the end points, and yields nonmonotonic paths for polymers whose end points are close to the grafting surface. In addition, it reproduces extremely well the form of the density distribution far from the grafting surface, which is essentially Gaussian. This results from the fact that the stretching energy dominates the interaction energy of those polymers which extend far from the grafting surface, so that their behavior is nearly ideal. In the limit of infinite stretching, beta --> infinity, the theory reduces to that of Zhulina et al. and of Milner et al.

Amount of information about the attitude object and attitude-behavior consistency.

The synthesis of two separate lines of inquiry--research on information integration and longitudinal studies of attitudes--prompted the hypothesis that the degree of consistency between attitudes and behavior will increase as a function of the amount of information available about the attitude object. The hypothesis was tested in three separate longitudinal studies, ranging in length from 4 days to 4 months, that investigated the following behaviors: voting for candidates for political office, voting for two social policy election initiatives, and having an influenza vaccination. In support of the hypothesis, in each study, amount of information moderated the consistency between attitudes and behavior; and the significance of this relation remained even after controlling for the effects of a number of other potential moderators, including prior direct behavioral experience with the attitude object and attitude certainty. Consistent with previous research, direct behavioral experience was also a determinant of attitude-behavior consistency, and for the behavior of having an influenza vaccination, this relation was independent of the effect of amount of information. The discussion focuses on the interrelation among moderators of attitude-behavior consistency and on the theoretical implications of the findings.

Explorations in behavioral consistency: properties of persons, situations, and behaviors.

In a study exploring the cross-situational consistency of behavior, 140 undergraduate Ss were video-taped in each of 3 laboratory settings, and personality descriptions of these Ss were obtained from friends and acquaintances. Analyses focused on the degree to which Ss maintained consistent patterns of behavior across laboratory settings and between these settings and daily life. The following conclusions were reached: (a) Behavior can exhibit impressive consistency at the level of psychological meaning, (b) psychological properties of situations can be detected from their behavioral effects, (c) cross-situational consistency and discriminativeness are independent, and (d) some behaviors are more consistent than others.
