[Evaluation and epidemiologic surveillance of nutritional deficiencies. Principles and application to elementary health units].

The authors briefly describe some anthropometric, biologic and clinical nutrition indicators used to assess the severity of malnutrition. Problems related to selection of population samples and framework for possible action against malnutrition are also indicated. For transverse evaluation, they suggest to utilize the cluster sampling method recommended by W.H.O. for studying vaccine coverage. The main indicator of nutritional status is then the brachial perimeter qualitatively evaluated with Shakir's strip. To monitor epidemiological importance of malnutrition, they suggest to take into account: the figures indicated by the maternities (birth weight), the figures indicated by M.C.H. and school health (ratio of malnourished children--weight average by age and sex), the figures indicated by specialized hospital departments such as pediatrics, nurseries... They insist upon seasonal factors and evaluation of the rate of coverage of the target population by maternities, M.C.H. and school health, in order to correctly read the declared figures.

Molecular techniques for the investigation of meningococcal disease epidemiology.

Meningococcal disease remains a major cause of childhood morbidity and mortality world wide and no comprehensive vaccine is available against the causative organism, Neisseria meningitidis. Molecular studies of the diversity of this bacterium have provided a number of key insights into its biology, which have implications for control of meningococcal disease. These have included the identification of hyperinvasive lineages and the correlation of genetic type with antigenic type and disease epidemiology. In practical terms, such studies have enabled the application of DNA-based technologies in the development of improved methods for diagnosis and epidemiological monitoring. These data are of especial importance with the current, and ongoing, development and introduction of new meningococcal vaccines.

[Epidemiological surveillance of infection by the human immunodeficiency virus (HIV) and of the acquired immunodeficiency syndrome (AIDS) in French Polynesia in 1991].

The authors make out a statement about HIV infection in French Polynesia at the date of 1991 December 31. 96 cases all together of seropositive and AIDS infected people were recorded. These patients are young generally (78 p.c. between 21 and 40 years old) sexually contaminated (72 out of 96) and live in Tahiti island (94 p.c.). Sex-ratio is 2.8 male/1 female. Among them, we noted 55 p.c. of Europeans, 38 p.c. of Polynesians and 7 p.c. of Asiatic people. Epidemiological monitoring of the infection was made easy because of a set of laws and possibilities of detection highly favourable. Progress of the infection is constant, with 20 new cases detected each year with a prevalence of 150 cases of AIDS per 1 million of inhabitants, French Polynesia could be classified as the 5th or 6th region of France as far as the importance of the disease. Clinical, biological and epidemiological taking of charge of patients is detailed as well as the prevention campaign.

Surveillance: the foundation for control and elimination of dracunculiasis in Africa.

The International Drinking Water Supply and Sanitation Decade (1981-1990) has stimulated a movement to eradicate human infection with the helminthic parasite Dracunculus medinensis (dracunculiasis), whose victims are disabled for weeks or months during the painful emergence of one or more worms from beneath the skin. Each year, millions of people acquire this infection by drinking unclean water. Among the critical activities that are necessary for the elimination of dracunculiasis, one of the most fundamental is that of epidemiological surveillance. Surveillance activities play a key role in the strategy to target affected villages for improved water supplies and other control activities. Accurate surveillance data also stimulate interest and support for national eradication programmes. Dracunculiasis is a condition with excellent characteristics for reporting through passive surveillance systems. However, active surveillance, as well as other innovative surveillance strategies, should be used to establish baseline information in those villages where cases occur, and later to monitor epidemiologically important indices needed to evaluate the progress of elimination efforts.

Pneumococcal carriage in children in The Netherlands: a molecular epidemiological study.

In 1999, Engelen and coworkers investigated colonization in Amsterdam among 259 children attending 16 day-care centers (DCCs) and among 276 children who did not attend day-care centers (NDCCs). A 1.6- to 3.4-fold increased risk for nasopharyngeal colonization was observed in children attending DCCs compared with NDCC children, while no difference in antibiotic resistance was found between groups. The serotype and genotype distributions of 305 nasopharyngeal Streptococcus pneumoniae isolates of the latter study were investigated. The predominant serotypes in both the DCC and the NDCC groups included 19F (19 and 18%, respectively), 6B (14 and 16%, respectively), 6A (13 and 7%, respectively), 23F (9 and 7%, respectively), and 9V (7 and 7%, respectively). The theoretical vaccine coverage of the 7-valent conjugate vaccine was 59% for the DCC children and 56% for the NDCC group. Genetic analysis of the pneumococcal isolates revealed 75% clustering among pneumococci isolated from DCC attendees versus 50% among the NDCC children. The average pneumococcal cluster size in the DCC group was 3.8 and 4.6 isolates for two respective sample dates (range, 2 to 13 isolates per cluster), while the average cluster size for the NDCC group was 3.0 (range, 2 to 6 isolates per cluster). Similar to observations made in other countries, these results indicate a higher risk for horizontal spread of pneumococci in Dutch DCCs than in the general population. This study emphasizes the importance of molecular epidemiological monitoring before, during, and after implementation of pneumococcal conjugate vaccination in national vaccination programs for children.

Comparison of ribotyping, randomly amplified polymorphic DNA, and pulsed-field gel electrophoresis for molecular typing of Vibrio tapetis.

Brown ring disease, caused by Vibrio tapetis, is an important pathological problem in different species of cultured clams. In order to evaluate the genetic diversity of the pathogen, twenty-seven isolates of V tapetis with different origin were screened by ribotyping (RT), pulsed-field gel electrophoresis (PFGE) and randomly amplified polymorphic DNA PCR (RAPD). Restriction with PvuII, SalI, and SmaI gave 2 RT patterns, differentiating in all cases the strain 0202RD isolated from carpet-shell clams (Ruditapes decussatus) from the other strains tested. The use of NotI generated strain specific PFGE profiles, which could be grouped in two main clusters. Cluster 1 grouped all but one strain and was subdivided into six PFGE subtypes (1a to 1f) which joined at a similarity level of 75.6%. Cluster 2 included again only the isolate 0202RD. RAPD analysis yielded the same results with three different primers, this method being able to differentiate the isolates from R. decussatus from those isolated from other clam species. Of the three techniques evaluated, PFGE was the most discriminating of the three techniques evaluated, followed in discriminating power by RAPD and RT tests. On the basis of the results obtained, we conclude that the RAPD procedure, which is more rapid and easier to perform than the other techniques, shows to be very useful to analyze large amounts of strain collections from an epidemiological monitoring stanpoint. In addition, PFGE is of great utility to evaluate the genetic diversity of strains involved in an outbreak and to study the spreading of a specific clone.

[Estimate of seroprevalence of HIV infection in Belgium using the back calculation method].

An estimation of HIV seroprevalence in Belgium in June 1990 was made by the "back calculation method". The theoretical calculation of the number of virus carriers gave a range of values from 5,750 to 16,800 carriers. The upper part of this range seems to be more consistent with data from epidemiologic monitoring (range of values from 8,900 to 16,800 carriers). HIV prevalence rate in Belgium in June 1990 would is estimated between 0.9/1000 and 1.7/1000. The detection rate of the infection among HIV positives individuals should be at least 34%.

Use of Medicaid data for pharmacoepidemiology.

Because of the high prevalence of prescription drug use and the incomplete understanding of drug effects at the time of licensing, ongoing epidemiologic monitoring is required to provide information for clinical and regulatory decisions. Data produced through the administration of Medicaid programs have been considered for this purpose because the computerized files include prescription and diagnostic information for large, defined populations. However, the limited amount of data available in the computerized files and the atypical demographic characteristics of Medicaid populations create formidable difficulties in the use of these data for pharmacoepidemiology. This paper reviews these methodological problems and describes pragmatic solutions that have been developed through the ongoing use of these data bases for epidemiologic studies.

Epidemiological concepts regarding disease monitoring and surveillance.

Definitions of epidemiological concepts regarding disease monitoring and surveillance can be found in textbooks on veterinary epidemiology. This paper gives a review of how the concepts: monitoring, surveillance, and disease control strategies are defined. Monitoring and surveillance systems (MO&SS) involve measurements of disease occurrence, and the design of the monitoring determines which types of disease occurrence measures can be applied. However, the knowledge of the performance of diagnostic tests (sensitivity and specificity) is essential to estimate the true occurrence of the disease. The terms, disease control programme (DCP) or disease eradication programme (DEP), are defined, and the steps of DCP/DEP are described to illustrate that they are a process rather than a static MO&SS.

Scalp hair as a monitor of community exposure to lead.

Lead was measured by photon activation analysis in scalp hair from three population groups with varied types of environmental exposure. Concentrations of lead in hair increased from rural to urban to smelter areas with medians of 9.1, 15.3, and 48.5 ppm, respectively. Boys under 16 residing near smelters showed consistently higher lead levels than girls of the same age group and from the same area. A history of exposure to lead was deduced from the distribution of concentration along the hair length by analyzing 1-or 2-cm segments of hair strands. A reasonably good blood lead-hair lead correlation was obtained for individuals who appeared to be in a steady state with respect to intake and excretion of lead. The analytical method for the photonuclear determination of lead in hair, the hair washing procedure, and the advantages of using hair as an epidemiologic monitor are described.

[Hepatic hydatidosis in Puglia: analysis of the hospital discharge cards in the years 1996-2000].

The authors valueted the distribution of Hepatic Hydatidosis in the Region Puglia (Italy) by hospital dimission cards (schede di dimissione ospedaliera). At present, the SDO represent the most important information tool to monitor hospital activity. The authors examined 468 cases of hepatic Hydatidosis admitted in regional hospital during 1996-2000. 54% were men and 46% women, 75% of whom aged between 40-60 anni. The median hospitalization time was of 12 days and 80% were surgical departments. All the cases examined were uniformedly distributed in surgical hospitals. No hospital acts as a pole of attraction. The periodal prevalence was 6.4/100.000. 40% of cases had surgical DRG with quadriennal sanitary cost of 4 miliardi liras. The Surgical procedure was 53% hepatic lesion demolition, 14% partial epatectomy, 7.2% lobectomy, 5.2% marsupializzazione, 2.6% hepatectomy, and 19% others procedures. At present, due to physiopathological considerations, radical surgery is preferred to conservative surgery. Hepatic hydatidosis is considered a public health problem whose epidemiologic monitoring and precautionary measures could be oriented to control the related sanitary costs.

Emergence and spread of two distinct clonal groups of multidrug-resistant Escherichia coli in a veterinary teaching hospital in Australia.

Multidrug-resistant Escherichia coli (MDREC) expressing AmpC beta-lactamases have emerged as a cause of opportunistic infections in dogs. Following a cluster of extraintestinal infections caused by two distinct clonal groups (CGs) of bla(CMY)-producing MDREC, a 12-month infection control study was undertaken at a veterinary teaching hospital in Brisbane, Australia. Swabs from the rectum of hospitalized dogs (n=780), hospital staff (n=16) and the hospital environment (n=220) were plated onto selective agar to obtain multidrug-resistant (MDR) coliforms. These were then tested by multiplex PCR for E. coli uspA, bla(CMY) and the class 1 integron-associated dfrA17-aadA5 gene cassette for rapid identification of MDREC CG 1 (positive for all three genes) and CG 2 (positive for uspA and bla(CMY) only). A total of 16.5 % of the dog rectal swabs and 4.1% of the hospital environmental swabs yielded MDREC, and on the basis of multiplex PCR, PFGE and plasmid profiling, these were confirmed to belong to either CG 1 or CG 2. Both CG 1 and CG 2 isolates were obtained from clinical cases of extraintestinal infection and rectal swabs from hospitalized dogs over the same period of time, whereas only CG 1 isolates were obtained from the hospital environment. Both CGs were prevalent during the first 6 months, but only CG 2 was isolated during the second 6 months of the study. Two isolates obtained from rectal swabs of staff working in the hospital belonged to CG 2, with one of the isolates possessing the same REDP as nine isolates from dogs, including six isolates associated with cases of extraintestinal infection. CG 1 isolates belonged to E. coli serotypes O162 : H-, OR : H- or Ont : H-, whereas CG 2 isolates belonged to O153 : HR, OR : HR or OR : H34. These results confirm that in this particular outbreak, canine MDREC were highly clonal and CG 2 MDREC may colonize both humans and dogs.

Data on height and weight from school health records as a national public health surveillance tool: the case of Sweden.

AIM: To investigate the suitability of using routine height and weight data from records within a school health service system, for population monitoring of child and adolescent growth on a national level. METHODS: A longitudinal study from age 7 to 18 years of all children born on the 15th of any month in 1973 and living in Sweden on 31 December 1989 (1,724 girls and 1,855 boys). Data were collected from school health records. RESULTS: Descriptive statistics for height and weight calculated from data obtained from school health records are summarized and compared with previous Swedish studies. The effect of exclusion of children with chronic diseases involving major growth impairment and/or exclusion of children born outside Sweden have a minor impact on the results owing to the relatively small number of excluded children. CONCLUSION: Longitudinal data for somatic growth (height and weight) from age 7 to 18 years from a nationally representative sample of children in Sweden, collected from school health records, can be used for epidemiological monitoring of growth with fewer missing individuals and at lower costs compared with other dedicated studies. Data quality is comparable to similar national surveys. The data are suitable for descriptive analysis of growth and other forms of observational study.

Prognostic value of ambulatory blood pressure measurements for the diagnosis of hypertension in pregnancy.

Several studies have indicated that the use of the 24 h mean blood pressure, mainly using reference thresholds derived from general nonpregnancy practice, does not provide an effective test for an individualized early diagnosis of hypertension in pregnancy, thus concluding that ambulatory blood pressure monitoring is not a valid approach in pregnancy. With the use of ambulatory blood pressure monitoring, epidemiologic studies have reported gender differences in the circadian variability of blood pressure and heart rate. Typically, men exhibit a lower heart rate and higher blood pressure than women, the differences being larger for systolic than for diastolic blood pressure. Moreover, normotensive and hypertensive pregnant women are characterized by differing but predictable patterns of blood pressure variability throughout gestation. However, the diminished blood pressure in nongravid women as compared with men, the added decrease in blood pressure during the second trimester of gestation in normotensive but not in hypertensive pregnant women and the large amplitude of the circadian pattern that characterizes the blood pressure of healthy pregnant women at all gestational ages, have not been taken into account when establishing reference thresholds for the diagnosis of hypertension in pregnancy. This review will describe these issues, summarize previous results from independent groups on the prognostic value of ambulatory blood pressure monitoring in pregnancy, propose answers as to an accurate reference threshold for blood pressure at different stages of gestation and suggest how this information should be used in order to identify those women at a higher risk of hypertension, who will also be more suitable for prophylactic and/or therapeutic intervention in the early stages of pregnancy.

Trichinellosis surveillance--United States, 1997-2001.

PROBLEM/CONDITION: Trichinellosis is a parasitic disease caused by tissue-dwelling roundworms of the species Trichinella spiralis. The organism is acquired by eating Trichinella-infected meat products. The disease has variable clinical manifestations, ranging from asymptomatic to fatal. In the United States, trichinellosis has caused hundreds of preventable cases of illness and occasional deaths. The national trichinellosis surveillance system has documented a steady decline in the reported incidence of this disease, as well as a change in its epidemiology. REPORTING PERIOD COVERED: This report summarizes surveillance data for trichinellosis in the United States for 1997-2001. DESCRIPTION OF SYSTEM: Trichinellosis became a nationally reportable disease in 1966, but statistics have been kept on the disease since 1947. The national trichinellosis surveillance system is a passive system that relies on existing resources at the local, state, and federal levels. Cases are diagnosed based on clinical history with laboratory confirmation. Cases are reported weekly to CDC through the National Electronic Telecommunications System for Surveillance (NETSS). Detailed data regarding signs and symptoms, diagnostic tests, and food consumption are gathered by using a supplementary standardized surveillance form and are reported to CDC by fax or mail. This information is compared with NETSS data several times a year by CDC staff. Discrepancies are reviewed with the state health departments. The purpose of the surveillance system is to determine the incidence of trichinellosis, to maintain awareness of the disease, to monitor epidemiologic changes, to identify outbreaks, to guide prevention efforts, and to measure the effectiveness of those efforts. RESULTS: Although trichinellosis was associated historically with eating Trichinella-infected pork from domesticated sources, wild game meat was the most common source of infection during 1997-2001. During this 5-year period, 72 cases were reported to CDC. Of these, 31 (43%) cases were associated with eating wild game: 29 with bear meat, one with cougar meat, and one with wild boar meat. In comparison, only 12 (17%) cases were associated with eating commercial pork products, including four cases traced to a foreign source. Nine (13%) cases were associated with eating noncommercial pork from home-raised or direct-from-farm swine where U.S. commercial pork production industry standards and Regulations do not apply. INTERPRETATIONS: The majority of the decline in reported trichinellosis cases is a result of improved observance of standards and regulations in the U.S. commercial pork industry, which has altered animal husbandry practices resulting in reduced Trichinella prevalence among swine. PUBLIC HEALTH ACTIONS: Because of the change in epidemiology of trichinellosis and the continued occurrence of cases among consumers of wild game meat and noncommercial pork, more targeted public education is needed to further reduce the incidence of this disease.

From decline to resurgence: current perspectives on Mycoplasma pneumoniae.

SUMMARYMycoplasma pneumoniae pneumonia (MPP) is an acute respiratory infection caused by Mycoplasma pneumoniae (MP) and constitutes the primary cause of community-acquired pneumonia (CAP) among children aged 5 years and older in China. The clinical manifestations of MP-associated respiratory disease in children are diverse, ranging from mild upper respiratory symptoms to life-threatening pneumonia. Notably, during the implementation of nonpharmaceutical interventions (NPIs) aimed at curbing SARS-CoV-2 transmission, there was a significant decline in MP infection rates. However, a pronounced global resurgence of MP infections was documented in 2023-2024. The increasing global prevalence of macrolide resistance, particularly in China, coupled with limited alternative antibiotic options for children, increasing rates of coinfections, and the absence of a preventive vaccine, collectively exacerbate treatment challenges and increase the disease burden. Furthermore, the insufficient availability of MP nucleic acid testing and antimicrobial resistance surveillance in primary healthcare settings, coupled with the absence of a comprehensive epidemiological monitoring network, results in a vicious public health cycle. Recent advances and emerging genomic data have provided new insights into its pathogenesis and clinical management. Therefore, in this narrative review, we aim to (i) synthesize the current understanding of the etiological characteristics of MP and the present status of MPP diagnosis and treatment; (ii) analyze recent advances and drivers behind the global resurgence of MP infections; and (iii) propose integrated prevention and control strategies to increase the recognition of MPP and prevent unanticipated outbreaks.

Molecular analysis of pseudorabies viral vaccines and their rapid differentiation from wild-type isolates using DNA-amplified glycoprotein I and thymidine kinase gene segment polymorphisms.

Several conventional and genetically recombinant modified-live viral (MLV) vaccines are used to control pseudorabies virus infections (Aujeszky's disease, PRV) in swine. Differentiating vaccinal PRV (V-PRV) from wild PRV (WT-PRV) is important for herd health, regulatory and forensic purposes, and for studies of PRV latency and epidemiology. All PRV vaccines used currently contain glycoprotein I (gI) and/or thymidine kinase (TK) gene deletions, whereas WT-PRV typically contain intact gI and TK genes. Utilizing these differences we developed an effective but simple differential polymerase chain reaction (PCR) approach based upon the amplification of gI and TK gene polymorphisms. The primary immunoreactive epitope-encoding region of the gI gene and nearly the entire TK gene were amplified and analyzed using nested PCR procedures. TK and gI PCR products were cleaved with Sal I and Sac I, and Nco I restriction enzymes respectively. PCR product and restriction fragment length polymorphisms enabled most V-PRV to be clearly distinguished from each other, and all of them, as a group, clearly differentiated from typical WT-PRV. Mixtures of V-PRV and WT-PRV could be identified as such. The uncommon but occasional occurrence of atypical WT-PRV containing altered gI and/or TK genes indicates the need for interpretive caution, particularly if aberrant gene segment polymorphisms are observed. This rapid and precise molecular approach will facilitate regulatory monitoring, epidemiological investigations, diagnostic differentiation, purity testing and latency/recrudescence studies with the class of biologicals and offers a model for similar analyses of other MLV biologicals as well.

Development and evaluation of a rapid flow-through immuno filtration test using recombinant filarial antigen for diagnosis of brugian and bancroftian filariasis.

There is an imperative need to develop a rapid antibody test that can be used for diagnosis of clinical cases in travelers and expatriates, primary surveillance in areas of unknown endemicity, detection of early infection in childhood and for monitoring chemotherapeutic programs. A rapid-format, simple and qualitative flow through immuno filtration test has been developed for the identification of total IgG antibodies to recombinant filarial antigen WbSXP-1. This test system employs colloidal gold-protein A reagent as the antibody capture reagent. The sensitivity and specificity of the test was evaluated in a total of 1,230 serum samples. The sensitivity of the test was found to be 90.8% with brugian (n = 70) and 91.4% with bancroftian (n = 140) microfilaraemic subjects. The test showed minimum reactivity (4/10) with Loa loa microfilaria (MF) positive sera and no reactivity (0/20) with Onchocerca MF positive sera. This rapid diagnosis is found to be non-reactive with individuals having other parasitic diseases including schistosomiasis (n = 10), soil-transmitted helminthiases (n = 34) and protozoan infections (n = 33) indicating the potential of this test as a prospective method of diagnosis for both brugian and bancroftian lymphatic filariasis. Stability kinetics was studied at different temperatures and different time periods. The rapid flow-through immuno filtration test is advantageous since it can be stored at room temperature, is user friendly and is particularly applicable in the field as an initial screening method, for epidemiological monitoring of filarial infections in bancroftian and brugian endemic regions of the world.
