Bromocriptine holiday: effects on dopamine receptors and turning behavior in rats.

In experimental Parkinson's disease, we studied the effects of chronic administration (30 days), withdrawal, and reinstitution of bromocriptine. Dopamine (DA) receptor supersensitivity was produced by 6-hydroxydopamine and reversed by chronic bromocriptine administration. Drug holiday reestablished sensitivity of DA receptors, but reinstitution of bromocriptine again downregulated receptors. The effects of a drug holiday on DA receptors are short-lived, and long-term clinical improvement cannot be expected. Apomorphine-induced contralateral turning produced by the lesion was unaffected by any drug regimen.

[The results of drug holiday in Parkinson's disease].

The transitory suppression of L-dopa in patients with advanced Parkinson's disease improves motor response upon reintroduction of the drug although this improvement is not constant. The degree and duration of clinical improvement obtained following L-dopa drug holiday were studied and the factors which may intervene in the result of 32 patients with Parkinson's disease were analyzed. Improvement was found in 100% (24.9 +/- 6.7%) with significant differences being observed in the degree of disability (p < 0.05) and the final decrease of the doses of L-dopa (p < 0.05). Improvement was maintained during 10.3 +/- 7.9 weeks. Linear regression studies did not show any relation between the length of the drug holiday, longer than 4 days, and the degree and maintenance of improvement or final decrease of the doses of L-dopa. The authors consider this procedure useful in patients with important fluctuations in treatment response or with important refractory side effects to other measures.

[Summer holiday camps for asthmatic children--an attempted assessment (author's transl)].

118 schoolchildred (males:females = 2.5 : 1) with bronchial asthma--defined as recurrent allergic or non-allergic expiratory dyspnoea--were investigated as regards their clinical course, frequency of asthma attacks, exercise tolerance, anti-asthmatic drug requirements and spirometric changes during and after participation in specialized holiday programmes during the summer months in 1974, 1975, and 1976. Clear-cut improvement was observed with regard to asthma attack frequency (up to nearly 100%) and drug requirements (up to 76%). Steroid dependence was observed in most cases. No significant spirometric improvement was demonstrable. All data require critical evaluation and interpretation in view of seasonal variations in asthmatic manifestation and the influence of environmental factors. On reviewing the results it is concluded that treatment of the asthmatic child by supervised high altitude holidays (1400 m above seal level), combined with physical training and a respirator re-education programme, is still a justified therapeutic approach, but it cannot replace specific therapy laid down on the basis of careful diagnostic testing. Particpation of the asthmatic child in school gym classes is recommended. Abstention from physical exercis due to a misplaced protective attitude is not beneficial in most cases.

[Sun protection during holidays].

Ultraviolet radiation is causally involved in induction of skin cancer, premature skin aging and photodermatoses. The longing of our western society for a "healthy tanning" as well as the unbroken trend to spend the holidays in sunny regions lead to the fact that human skin is increasingly exposed to ultraviolet radiation and its detrimental effects. Because of the socio-political importance of the vacation period as the "most beautiful and most important time of the year", effective prevention of these unwanted UV effects has an enormous importance to the general population. In this article the most important methods for effective sun protection are critically discussed.

A holiday break for adolescents provided by the Malcolm Sargent cancer fund for children.

The paper describes a holiday for a group of adolescents with cancer (from the Young Oncology Unit at the Christie Hospital, Manchester, UK) at Malcolm Sargent House, Prestwick, Scotland. The aim was to provide an opportunity for young people with cancer to build and to develop therapeutic relationships away from the pressures of hospital, home and treatment. The nature and range of care given by the nurses and social worker who accompanied them is described and includes accounts of individual progress which are demonstrated by brief case studies.

Holiday depression, both fact and fiction.

In the whirl of the holiday season, the maintenance of normal mood is no mean task. Demoralization and despair are no paradox, no mystery, no fashionable topic. Nevertheless, they are also not depression, not a disorder, not a disease, but a predictable, genuine human reaction to an annual social ritual.

Utilization of a time-limited holiday hotline by older adults.

Lonely older adults are frequently hesitant to reach out for help because they fear forced institutionalization or other loss of control over their lives. The utilization of a time-limited holiday hotline for outreach to lonely older persons is examined. There were 306 calls from aged persons. Grief and bereavement were the primary themes of these calls. Trained professionals and supervised students staffed the telephone bank and provided such services as responsive listening and community referrals.

Holiday hazards.

Presented is a selective review of the toxicities of various plants, decorations, and miscellaneous items popularly used during the holiday season. Particularly hazardous agents include mistletoe, holly, bubble lights, fireplace flame colors, alkaline batteries, and mothballs. Specific questions regarding management of exposure to these items should be referred to regional poison control centers. Avoidance is the most effective treatment.

Dying to go on holiday.

OBJECTIVE: The Christmas period road toll is a highly publicized 'summary' of the number of road deaths that occur each year during New Zealand's summer holiday period. Our aims were to identify significant changes in the Christmas road toll during 1968-98, highlight the ability to detect changes and determine if focusing on the changes in the Christmas road toll provided a useful insight into trends in fatal crashes. METHODS: Regression modelling of the number of fatalities was used to examine trends in the Christmas road toll in New Zealand over time. A power analysis was used to investigate the ability to detect significant reductions in the Christmas road toll. RESULTS: The Christmas road toll has not decreased significantly in the past 30 years and has not improved significantly in contrast to the rest of the year. The number of deaths in any given year can be expected to vary 'naturally' between 10 and 39. The statistical power to detect a hypothetical 19% reduction in the Christmas road toll is minimal. CONCLUSIONS: Stochastic fluctuations in the Christmas road toll make it extremely difficult to identify underlying trends, even if substantial. Little insight can be gained from yearly comparisons of road tolls considered over short periods. More focus should be placed on road tolls that are calculated over longer periods, e.g. the previous six months. IMPLICATIONS: Natural fluctuations arising in small counts will make it difficult to highlight real improvements in road tolls.

Sudden infant death syndrome during weekends and holidays in Norway in 1967-1985.

A few previous studies have revealed an increased risk for sudden infant death syndrome (SIDS) during weekends and holidays suggesting environmental factors as potential trigger mechanisms for death. In the present study, the weekend/holiday effect has been assessed on the basis of 1480 SIDS cases in 19 Norwegian counties during a 19-year period (1967-85). The phenomenon has been confirmed for 17 of the 19 counties. Special attention has been attached to differences in the geographical and seasonal distribution of the phenomenon. Deaths of other causes in infancy did not display such a dependency on the day of the week.

Summer holiday camps for gay and lesbian young adults: an evaluation of their impact on social support and mental well-being.

This study evaluates the effects of participating in a summer holiday camp for Flemish (Belgium) gay and lesbian young adults (N 197). Analysis showed to what extent participation affects the constitution and quality of friendships, the availability of confidant and appraisal support and reported levels of hopelessness, self-esteem, and depression. The study is based on panel data with a six-months interval. At follow-up, respondents reported a higher proportion of gays and lesbians in their friendship network, more satisfaction with these friendships, higher levels of confidant and appraisal support, less hopelessness, higher self-esteem, and lower levels of depression.

Expanding weekend and holiday clinical services: a financial perspective.

Many payers view weekend and holiday inpatient stays at hospitals that provide interventional and therapeutic services only on weekdays as unnecessary, and are denying payment for these days. Hospitals not only must absorb the incremental costs associated with maintaining patients during these days, but also may lose payment for days to which they legitimately are entitled. To reduce both costs and denied payments, hospitals should develop a means to identify unnecessary days; provide immediate feedback to physicians in a manner that helps reduce problematic days; and place a priority, when appropriate, on moving patients to lower-level care settings, such as skilled nursing, home health, and custodial care facilities.

Beach holiday sunburn: the sunscreen paradox and gender differences.

A survey about sunbathing practices was performed on a summer holiday weekend at a Galveston beach. The likelihood of sunburn increased with increasing duration of sun exposure, with 100% of subjects experiencing sunburn after exposure > or = 4.5 hours. Men exhibited a significantly higher frequency of sunburn, employed fewer sun-protective measures, and demonstrated less knowledge concerning sun safety information and skin cancer than women. This information suggests a need for greater educational efforts directed toward changing public attitudes about preventing sunburn, especially those of men, that currently lead to high-risk sunbathing behavior.

Risk of Pseudomonas aeruginosa cross-colonisation in patients with cystic fibrosis within a holiday camp--a molecular-epidemiological study.

OBJECTIVE: A study on the molecular epidemiology of Pseudomonas aeruginosa in patients with cystic fibrosis (CF) from Germany (N = 18) and Israel (N = 12) is presented. The aim is to provide an answer to the question as to whether or not social contact outside the hospital environment involves a potential risk for person-to-person spread of this pathogen. METHODS: Sputa from German and Israeli patients were obtained while these were attending a holiday camp in Israel. The sputum samples were analysed with regard to Pseudomonas aeruginosa. Strains dissimilar in macroscopic appearance and/or antibiotic resistance patterns were genotyped using pulsed-field gel electrophoresis after digestion of genomic DNA with restriction endonuclease Spel. The genetic polymorphism of DNA fragment patterns of all strains (N = 146) was studied for their overall relatedness using a fingerprint software system. RESULTS: Most of the German patients (77.7%) were colonised persistently by a unique clonal type during the four-week screening period. Isolates obtained from Israeli patients displayed a very close clonal relationship and a higher antibiotic resistance as a result of preceding epidemic spread of certain clones before the camp. Additionally, isolates showing identical PFGE patterns were demonstrated once in a single male Israeli patient and in one female German patient, suggesting previous cross-colonisation. CONCLUSION: The occurrence of person-to-person spread through social contact in patients with CF is supported by our findings, but remains a rare event outside the hospital environment, provided appropriate hygienic measures are applied.

Reducing the risk for injury while traveling for Thanksgiving holidays.

Each year in the United States, motor-vehicle crashes result in approximately 40,000 deaths and 3.2 million nonfatal injuries. In 2000 during the Thanksgiving holiday, motor-vehicle crashes killed approximately 500 persons (US Department of Transportation, National Highway Traffic Safety Administration, unpublished data, 2000), and resulted in >43,000 hospital emergency department visits.
