[Effect of ethacizine on the heart conduction system and the indicators of central hemodynamics].

A trial of the antiarrhythmic drug ethacizine made in the Soviet Union which is an analogue of ethmozine was performed in 38 patients suffering from coronary heart disease. Automatism, conduction and excitation of the myocardium, hemodynamics and the antiarrhythmic effect were studied using electrophysiological methods, echo-cardiography and Holter monitoring. Ethacizine was found effective in arrhythmia. It reduces conduction in small changes of hemodynamics and myocardial contractility.

[Heart conduction system in the human in light of personal research].

A new model of the atrioventricular junction was developed by Anderson et al, of which each element has clinical significance and requires more specific research into its morphology and topography. Observations were carried out on materials consisting of 200 human hearts of both sexes of various ages (from 16 weeks of fetal life to 97 years of age). Histological slides were stained by Masson's method in Goldner's modification. It was concluded that the atrioventricular node consisted of two layers in all groups of examined hearts: cells of the compact area and transitional cells. With age, these cells change their morphology, especially in the transitional layer. Morphologically, within the atrioventricular bundle were differentiated. The topography of the atrioventricular node is dependent on the age of the examined heart. As much as in human adults it is found entirely within the triangle of Koch, in fetal and children's hearts its elements of its left branch were found in the false chordae tendinae of the left ventricle. The above observations suggest that in the case of younger hearts the triangle of Koch does not represent an absolute margination for the elements of the conductive system of the heart, which in the case of cardiac surgical procedures and ablations can in some percent, be the cause of iatrogenic complications.

[Topical diagnosis of disorders of the heart conduction system and surgical treatment of patients with supraventricular tachycardia].

Questions of preoperative diagnosis of the place of the disturbance of the normal heart excitation, spread of this excitation and distribution of places of the most frequent disposition of these foci in the heart depending on the pathology of the process are discussed. Sixty six patients with different kinds of tachicardia were examined, 40 patients were operated upon under conditions of artificial blood circulation. The examination of patients was performed by both routine and special methods (such as electrophysiological examination with the registration of 9 points of different parts of the heart simultaneously).

[Evaluation of disorders of the heart conduction system as risk factors in the surgical treatment of cholecystitis].

The author examined 1,144 patients with cholecystitis and found that 144 (12.6%) had heart block (HB)--atrioventricular block (AVB) and intraventricular block (IVB) were found in 11 and 133 patients, respectively. In some cases of cholecystitis, HB was combined with hypertensive disease, coronary heart disease, rheumatic heart disease. Patients with cholecystitis and HB underwent an exercise test (stepwise test) which was one of the principal criteria for the decision concerning the possibility of operating on the bile tract. A total of 864 (75.5%) patients with cholecystitis were operated on, 104 of whom had HB. No unfavourable outcomes were seen. Stable Stages I and III AVB (with an implanted cardiac pacemaker) and localized and complete block in the system of the right bundle branch without hemodynamic disorders were considered not to be a reason for rejecting surgery of cholecystitis. Surgery is inadvisable in block of some branches and complete (bilateral branch) block in the system of the left bundle branch, as well as in combination of Stage I AVB with IVB without performance of a graded physical exercise whose power should be at least 60% of the normal exercise, or 123 Wt, 750 kgm/min (the normal exercise being 100%, 176 Wt, and 1080 kgm/min).

[Features of the heart conduction system in alcoholic lesions of the myocardium].

An electrophysiologic functional study of the atrial conductive structure, the atrioventricular junction, the His-Purkinje system in 33 patients with second-stage chronic alcoholism showed impaired cardiac conduction through the atrioventricular node and, less commonly, the His-Purkinje system, to be the principal manifestation of alcoholic heart damage.
