Economic theory and international migration.

The modern literature on the economics of immigration focuses on 3 related issues: 1) what determines the size and skill composition of immigrants flows to any particular host country; 2) how do the immigrants adapt to the host country's economy; and 3) what is the impact of immigrants on the host country's economy? This article reviews the theoretical framework and empirical evidence provided by the economics literature on these questions. It demonstrates that the economic approach, using the assumption that migration behavior is guided by the search for better economic opportunities and that the exchanges among the various players are regulated by an immigration market, leads to substantial insights in to these issues.

[Stenosis of the external auditory canal: etiology, classification and management].

Stenosis of the external auditory canal is uncommon, poly-etiological pathology of the external auditory canal. The management of stenosis of the external auditory canal is difficult because of re-stenosis and danger of postoperative complications. The purpose of this article is to present modern classification, etiopathogenesis of the external auditory canal stenosis, indications for the surgical management, some aspects of the surgical management, using the modern literature and clinical cases.

[Adipose tissue as an endocrine organ: role in pathogenesis of ischemic heart disease and insulin independent diabetes mellitus].

Obesity is a risk factor for cardiovascular morbidity and mortality. The present review of the modern literature is devoted to the problem of regarding the adipose tissue as not only a repository of energy supplies but an active endocrine organ as well whose activity exerts a definite effect on the function of many bodily systems. Specific emphasis is directed toward aspects of the function of certain secretory proteins involved in the process of the arterial pressure regulation and/or organs injuring.

Pudendal nerve entrapment as source of intractable perineal pain.

Perineal pain caused by pudendal nerve entrapment is a rarely reported entity, with only a handful of cases in the modern literature. A 25-yr-old male medical student had refractory unilateral orchialgia for 32 mo and concomitant proctalgia for 14 mo. Pain was positional in nature, exacerbated by sitting and partially relieved when standing or recumbent. Pudendal nerve entrapment was diagnosed clinically, with computed tomography-guided nerve blocks providing temporary relief. A prolonged left pudendal nerve distal motor latency on electrodiagnostic testing later confirmed the diagnosis. At surgery, the left pudendal nerve was found flattened in the pudendal canal of Alcock and in contact with the sharp inferior border of the sacrospinous ligament. After surgical decompression and rehabilitation, the patient experienced significant relief of pain and returned to medical school. This case suggests pudendal nerve entrapment should be considered in the differential diagnosis of chronic urogenital or anorectal pain, particularly if the pain is aggravated by sitting or if there is a history of bicycle riding.

[Totally implantable systems for long-term chemotherapeutic treatment].

The authors report their own experience with completely implantable systems for the long-term chemotherapeutic treatment, compared with a review of modern literature. The experience even if low demonstrates a good methodology in reducing the number of complications associated with the use of percutaneous catheters.

[Role of the protein tyrosine phosphatase CD45 in signal transduction of hematopoietic cells].

This review deals with the analysis of modern literature about structure, catalytical activity, biological role and practical usage of protein tyrosine phosphatase CD45, which is widely distributed in the membranes of hematopoietic cells. The enzyme location makes it a highly sensitive test for estimation of development of such pathological states as immunological and neoplastic diseases, malignant tumors and transplantation responses of tissue tearing away. Unequal enzyme isoforms expressed in various leucocyte cell lines at different stages of development and differentiation are typical of those species. The existence of multiple isoforms is not used only to determine various cell lines, but also causes existence of variable adhesion requirements resulting in local restriction of activity and changes in the substrate binding. There is no doubt that definition of substrate molecules, influence of proteintyrosinephosphatase CD45 translocation and regulation of its activity by other ligands are important questions. Solution of these problems will be useful for searching the correctional means of dysfunctions of the tissues, enriched with protein tyrosine phosphatase CD45.

[Complementarity and Gestalt circle--Viktor von Weizsäcker and the importance of a general theory of illness].

Two developments have led to the elaboration of a complementarity theory in modern natural science--the Copenhagen interpretation of quantum mechanics and Viktor von Weizsäckers introduction of the subject into biology. The revolutionary implications of these developments are discernible in modern literature. The author seeks to show that a paradigm shift in positivistic clinical medicine is necessitated by the history and theory of science.

Reversal of poorly controlled glaucoma on diagnosis and treatment of hypothyroidism.

Hypothyroidism was diagnosed in a 62-year-old woman with poorly controlled primary open-angle glaucoma, including a disc hemorrhage and a documented arcuate scotoma. After 1 year of thyroxine therapy the glaucoma was easily controlled, pilocarpine treatment was stopped and the visual fields were completely normal. To our knowledge this is the first report in the modern literature of reversal of open-angle glaucoma with treatment of hypothyroidism.

[Functioning of fructose-1,6-bisphosphatase--main enzyme of gluconeogenesis in microorganisms].

The modern literature data about common characteristics, genetic and molecular-biological properties of main enzyme of gluconeogenesis (fructose-1,6-bisphosphatase) were analyzed. Regulation of fructose-1,6-bisphosphatase activity (stimulation and inhibition) by fructose-1,6-bisphosphate, fructose-2,6-bisphosphate, phosphoenolpyruvate, AMP and by metal ions are discussed. It was concluded that apart from the fact that fructose-1,6-bisphosphatase was intensively investigated, this enzyme from Mollicutes failed to be studied sufficiently.

Grading Duke: "A" for acknowledgment.

The death of seventeen-year-old heart transplant patient Jesica Santillan at Duke University Hospital due to a mismatch of blood types sent shockwaves across the country and brought heightened attention to the issue of medical error. This Article explores the modern literature on medical error and proposes changes in medical culture to effectuate a system in which medical errors can be reported without fear. It argues that the way to achieve this goal is by placing less focus on the individual and more on systemic factors that provide the conditions in which errors occur. This new system would take a proactive approach to error, change the culture of medicine, and relieve physicians of the burden of impossible overachievement.

Stem revision: special implant versus primary device.

The purpose of hip revision surgery is to relieve pain and restore hip function, while improving hip biomechanics and bone stock. The entity of bone stock impairment due to lysis indicates to the surgeon which revision stem he would preferably choose. Modern literature has described several options for stem revision, ranging from primary cemented/less implants in the case of limited proximal bone defects, to special revision stems where a massive femoral bone stock impairment is present. There are many classifications of the femoral bone-stock loss in the literature but most authors seem to agree that the treatment of minimal proximal defects should be performed with primary implant stems. The need to bridge a bigger bony gap makes distal fixation a good deal. Two options are described for this purpose: the Wagner stem and modular stems. Wagner's concept consists of a distal fixation based on the interference of the straight tapered titanium alloy stem at the level of the femoral isthmus, while longitudinal fins provide rotational stability. A wide variety of modular stems was lately developed to prevent major complication associated to the Wagner's stem, but some new problems arose from proximal overfill of the canal, difficult in assembly and fretting corrosion. Early experience with the new ZMR taper (Zimmer, Warsaw, IN, USA) modular revision stem is reported.

Aprotinin and hemostasis monitoring concerns during cardiac surgery.

Aprotinin (Trasylol) is a serine protease inhibitor, isolated from bovine lung that initially was marketed for the treatment of pancreatitis. In the mid 1980s, reports of its ability to decrease hemorrhaging after cardiopulmonary bypass surgery introduced the drug to the realm of cardiac surgery. Unfortunately, its introduction into this arena was followed by the publication of multiple studies and case reports that blamed aprotinin for poor outcomes in the form of early graft closure. More than 17 years have passed since the initial article describing the use of aprotinin during cardiopulmonary bypass, and with time there has been a significant increase in scientific knowledge and clinical experience. Interestingly, modern literature does not support the dogma that aprotinin is a procoagulant. Aprotinin increases the activated partial thromboplastin time (aPTT), as well as the kaolin- and celite-activated clotting time (ACT), regardless of heparin. Aprotinin, because of its ability to inhibit kallikrein, has been found to decrease thrombin antithrombin III complexes, fibrin-split products, fibrinopeptide 1+2, prothrombin fragments, and all markers of thrombin formation. Some authors have suggested that it may have a synergistic effect with heparin to ensure graft patency. Anticoagulation monitoring during the use of aprotinin also has been developed based on early studies. Aprotinin administration does influence the results of various ACT tests, and consequently different methods of testing anticoagulation have been developed. Researchers have demonstrated that the celite ACT is not "artificially" prolonged in the presence of heparin and aprotinin, rather the kaolin ACT is "artificially" shortened. This article will review the scientific literature with regard to aprotinin's anticoagulatory effects and review the current recommendations for hemostasis monitoring during the use of aprotinin.

[Effectiveness of green tea catechines for the correction of the alimentary obesity in the experiment].

Obesity is one of the most important problems of the modern medicine. The immobile life-style and consumption of high-calorie food are the most remarkable risk-factors of the obesity, which in presence of genetic predisposition contribute to the increasing of the body weight. In the modern literature there are numerous data about the important role of free radical oxidation in the pathogenesis of the obesity. In this relation application of natural antioxidants for the purpose of the correction of excess weight has gained an active research interest. Numerous researches showed natural antioxidants to be effective in correction of elevated blood cholesterol, triglycerides and LDL levels. In this relation the purpose of our research was to investigate effectiveness of green tea catechizes on lipid metabolism disorder, antioxidant status and excess body weight during experimental alimentary obesity. Experiment was conducted on rats kept on high-calorie diet for 7 weeks. Simultaneously one group of animals had been administered catechines in addition to the high-calorie diet during last 4 weeks. The experiment established corrective effect of catechines on the parameters of lipid metabolism (blood cholesterol, triglyceride and LDL levels), epididymal fat mass and antioxidant enzymes activity. Obtained results may be important for the development of weight losing diets.

[Study on the shu-points of five zang (viscera) for treatment of insomnia based on the theory of the five zang (viscera) storing vitality].

According to Prof. DONG Gui-rong's law of syndrome differentiation of "viscera, yin and yang, qi and blood, deficiency and excess" in internal injury diseases and academic thought of syndrome differentiation of regulating five zang vitality for treatment of insomnia, the author consults a great number of ancient and modern literature of TCM, analyzes the controlling functions of five zang storing vitality on whole organism and the relation between heart vitality and brain vitality, and explain the basis of acupuncture at five-zang shu-points of the Gallbladder Channel to regulate vitality of five zang-organs for treatment of insomnia from intension and functions of five zang vitality, whole regulative mechanism of five zang vitality, the cause and pathogenesis of insomnia of over or too less five zang-vitality induced by pathogenic factors, and according to typing and syndrome differentiation of five zang vitality, theory and principle of acupoint selection, so as to establish and put forward the method and theory of acupuncture at five zang shu-points of the Gallbladder Channel for treatment of insomnia.

[Medical data security in medico-legal opinioning].

Medical data security can be approached in medico-legal opinioning in three main situations: security of medical data, on which the opinion should be based, opinioning itself and whether the medical data security was properly ensured and ensuring medical data security during medico-legal opinion giving. The importance of medical data security, during collecting, processing and storing, as well in medical as in legal institutions, is of major importance for the possibility of providing a proper medico-legal opinion. Theoretically speeking, it is possible to give a proper medico-legal opinion using incorrect data, but the possibility is low. When the expert is given improper, unreadable, incomplete or even bogus in part or in the whole medical data it is extremely possible, that he fails in giving his opinion. The term "medical data" was defined and subsequently there was a brief review of medical data storing methods made and specific threats bound with them, based on modern literature. The authors also pointed out possible methods of preventing the threats. They listed Polish as well as international regulations and laws concerning the problem, accenting the importance of preserving medical data for the purposes of medico-legal opinioning.

[Epizootics of "avian" influenza as the manifestation of pandemic].

In this review of literature modern notions on the role of birds in the evolution of the pathogenicity signs and immune system of the main and intermediate hosts of influenza viruses, as well as on the mechanisms of overcoming interspecific barriers, are analyzed. The chronology of the spread of "avian" influenza among humans, starting from 1997, the properties of the natural reservoir of this infection, and in particular influenza viruses A, the ways of their variability and evolution are presented. The conclusion has been made that the mixing, joint evolution, recombination and reassortment of viral genomes may be caused by global events in individual geographical regions.

[Advances of studies on relative evaluation indexes for acupuncture and moxibustion treatment of senile dementia].

The clinical and mechanism studies of acupuncture and moxibustion in treatment of dementia from modern literature in recent 15 years are reviewed. In clinical studies, changes of scores for relative dementia scale and daily life scale, relative laboratory indexes, imaging indexes and physical diagnostic indexes after acupuncture are explored. In studies of mechanisms, dementia models are established with different methods and changes of various indexes were observed. Studies find that acupuncture and moxibustion can improve intelligence and social activity ability, increase therapeutic effect and have good effects on important indexes of dementia. However, the criteria for diagnosis, therapeutic effects, syndrome standardization, evaluation of long-term therapeutic effects and prognosis for dementia still need to be further studied.

[Cytokines and antiviral immuniy].

In the review of modern literature and results of own research about role of immunoregulation cytokines imbalance system in pathogen of deep-rooted virus infections are analyzed. The question on molecular mechanisms of cytokine chain deregulation at persistent virus infections is discussed.
