Intraforaminal neurinoma in the lumbosacral region.

A series of 12 patients with intraforaminal neurinomas in the lumbosacral space was reviewed. Our classification according to tumor extension relative to the affected root into intradural intra-arachnoid, intraforaminal extra-arachnoid, and extraforaminal types was useful in determining the best surgical approach to achieve root preservation and minimal postoperative deficits. Where sacrifice of roots was unavoidable, surprisingly few neurological deficits occurred.

[Anatomical study of the cutaneous innervation of the lumbosacral region. Application to the physiopathology of certain lumbalgias].

30 dissections have confirmed that the skin of the gluteal area is innervated by the posterior branches of T12 and L1, 19 times out of 30 (64 p. cent) or T12, L1 and L2, in 8 instances (26 p. cent) or T12, L1 and L2 receiving an anastomosis from L3, in 3 instances. This explains certain lumbo-gluteal pain coming from the dorso-lumbar area ("low lumbalgias of high origin"). In addition, the posterior branch, the most medial (most of the time L1, sometimes L2) goes through an osteo-aponeurotic channel formed by the iliac crest below and the dorso-lumbar fascia above. This channel, located at 7 or 8 cm of the spinal process, may be at the origin of a neuropathy. It projects at the same level in relation to the mid-line as the iliac insertion of the ilio-lumbar ligament. The latter seems impossible to feel as it is too deep and hidden by the superficial aspect of the iliac crest. Lumbalgias, sometimes related to a strain of this ligament, seem to be, most of the time, the result of an irritation of the posterior branch of L1 or L2.

[Complaints in the lumbosacral region and their management with Dolo-Neurobion].

Many patients complaining of acute pain in the lumbosacral area suffer from affections of intrapelvic organs (urinary bladder, prostate glands, female genital-organs). The routine diagnosis in such cases is described. In 53 own patients the analgesic and anti-inflammatory effect of Dolo-Neurobion has been evaluated. It is a combination drug consisting of the neurotropic vitamins B1, B6 and B12 and the analgesic metamizole. The treatment was started parenterally in the recommended doses and continued orally. If there were definite signs of infection in the pelvic area, additional antibiotics were administered after bacteriological tests. Dolo-Neurobion showed good or excellent results in 77,4% and moderate effects in 15,1% of the patients. There were no major side-effects or intolerance.

[Measurement data of the lateral lumbosacral region in healthy subjects during the growth period].

The following measurement data were obtained to characterise the healthy lumbosacral profile, via lateral x-rays taken of 90 female and 90 male patients between 8 and 17 years of age: Degree of slip 0%, slip angle -21.3 degrees, sacral inclination 45.5 degrees, posterior lumbosacral angle 140.5 degrees. These measurement data are independent of age and sex. If an adolescent suffers from lumbosacral spondylolisthesis, they will quantify the process of slip that has taken place, more accurately than the conventional method after Meyerding. If the shape of the sacral vertebra is considered at the same time, these parameters will also enable to assess the risk of progression of spondylolisthesis to a degree that is satisfactorily accurate in clinical practice.

[The morphology of the CNS in carbon tetrachloride poisoning. 6. Histology of spinal ganglia in the lumbosacric region (experimental studies) (author's transl)].

As a further report on our investigations concerning the effects of carbon tetrachloride intoxication the histological findings in lumbosacric ganglia of 48 rabbits are presented. The substance was applied perorally as well as a subcutaneously to 24 animals in each group. Detectable alterations were weak and consisted of pathobioses and pathonecroses of the ganglion cells and, partly as a consequence of these lesions, of an activation of glial elements. In most of the test animals as well as in 10 of the 11 controls cytoplasmic vascuoles were found. From the pathogenetic point of view the impairment of elementary cell functions by anoxemia both with primary and secondary hepatorenally caused lesions may be of major importance.

[Biomechanical aspects of surgical treatment of secondary stenosis of the spinal canal in the lumbosacral region].

In experimental studies and on clinical material, behaviour was checked of intervertebral discs and vertebrae under the axial pressure: which is the being exerted by the trunk weight, as well as the behaviour of the adjacent mobile segments after "diskoplasty" operation or vertebral interbody stiffening. The material pertaining to the experimental part comprises 6 undamaged spine preparations of the lumbosacral segment, while the clinical part consists of 60 patients out of 143 subjects having been operated on by "diskoplasty" or vertebral interbody stiffening from the approach through fenestration, hemilaminectomy, or plastic laminectomy. The results of performed experimental studies and spodylometric analysis of radiograms in patients treated operatively by decompressing the vertebral canal and by "diskoplastic" stabilization of the vertebrae, or vertebral interbody stiffening permit the following observations. The most favourable localization of the transplant is the medial and posterior parts of the vertebral interbody space, since in both segments L4/L5 and L3/L4 it reproduces the height of the vertebral interbody space, the height and width of intervertebral foramen, widens the intervertebral foraminis, and reduces the lumbar lordosis, which advantageously changes the biomechanic relations, transferring loads of the posterior to the anterior vertebral column. The localization of the transplant in the anterior part of the vertebral interbody space, in fact, increases the anterior vertebral interbody space, but decreases the height of posterior interbody space, the vertebral interbody angle alpha as well as the height and width of the intervertebral foramen. The retroshifting of vertebral L4 indicates mobile segment instability. By implementing a transplant of matching height and appropriate localization in vertebral interbody space, it is possible to increase or reduce the angle between the vertebral bodies at the operative level, widening the vertebral interbody space in anterior and posterior parts. The localization of the bony transplant in the anterior part of vertebral interbody space, involving the clinical material, caused in 65% constriction of intervertebral foraminis, retroshifting of vertebrae L4 and L3, and recurrence of algesic complaints, which may be elucidated by instability of the operated on or adjacent mobile segments.

Projections of the paraventricular nucleus of the hypothalamus to the sexually dimorphic lumbosacral region of the spinal cord.

Lumbar regions L5-L6 of the spinal cord of the male rat contain the sexually dimorphic motor nuclei, the spinal nucleus of the bulbocavernosus (SNB) and the dorsolateral nucleus (DLN), which innervate perineal muscles, the bulbocavernosus and the ischiocavernosus, respectively. This neuromuscular system controls penile reflexes which are essential to male reproductive success. Oxytocin has been shown to induce penile reflexes and the site of action for these effects is the PVN. Since PVN is known to project to cervical and thoracic levels of spinal cord, the present study examined projections of the PVN to the L5-L6 region of the spinal cord. WGA-HRP was injected into the region of L5-L6, aimed at the SNB and DLN and their dendritic extents, in intact male, castrated male and female rats. WGA-HRP-labelled cells bodies were found in the parvocellular subnuclei of PVN, as well as regions of the lateral hypothalamus and the dorsal area of the hypothalamus. These results demonstrate that the PVN projects to lumbar levels of the spinal cord that are sexually dimorphic and androgen-dependent. This suggests that PVN may modulate the activity of these motoneurons.

[Diagnostic importance of CT examination of the lumbosacral region of patients with lumbo-sciatica (author's transl].

In 160 patients with lumbo-sciatic complaints, CT examinations of the lumbo-sacral region were carried out. In 106 of these cases, which were controlled by myelography and/or operation, the CT diagnosis was correct in 74%. In 19%, the CT findings were uncertain, and in 7% they were wrong. A reliable CT diagnosis could be made mainly in the case of a medial or medio-lateral disc herniation of a size above 0.5 cm in diameter if the patient was not operated previously, and in extremely lateral prolapses into the intervertebral foramen without myelographic changes. Small medial protrusions, specially within a narrow spinal canal, or postoperative controls often gave rise to uncertain findings. However, osseous changes as compressions of the lateral recess or the narrow spine syndrome were demonstrated usually better by CT than on routine radiograms. Nevertheless, the indication for the performance of a lumbo-sacral CT should be kept narrow and should be limited to cases with clear-cut radicular signs.

[Usefulness of questionnaires in detection of early symptoms of sciatica and related pain syndromes of the lumbosacral region and determination of their incidence among workers of textile industry].

The study was aimed at the determination of sciatica case rate among cotton industry workers based on inquiry. Two questionnaires for detecting complaints typical of sciatica were developed. 2463 weavers and 3083 spinners were examined. A comparative group involved 978 shop attendants and 593 office workers. It was found out that pains characteristic of sciatica were experienced by 45% weavers, 43% shop attendants, 29.2% spinners, and 16% office workers. Results of physical examinations in randomly selected cases corresponded in 97% with the questionnaire data. In most cases pains resulted from overloading of the lower section of the vertebral column.
