Endoscopic Nd:YAG laser coagulation of bleeding from radiation proctopathy.

Hematochezia from mucosal vascular lesions usually confined to the rectum represents an uncommon but problematic late complication of pelvic radiotherapy. We studied 47 patients with medically refractory hematochezia resulting from radiation-induced rectosigmoid mucosal vascular lesions. All lesions were endoscopically coagulated with Nd:YAG laser. Median duration of hematochezia before laser therapy was 11 months, despite previous medical treatment (98%) or bypass colostomy (6%). Within 3 to 6 months after laser treatment, the number of patients with daily hematochezia fell from 40 (85%) to 5 (11%; p < 0.001), and the median hemoglobin level increased from 9.7 gm/dl to 11.7 gm/dl (p < 0.001). Complications occurred in three patients (6%); no deaths occurred. The condition in six patients (12.8%) was not improved by laser treatment. Two patients (4%) ultimately required surgical treatment for bleeding control. On the basis of symptomatic, hematologic, and endoscopic responses, Nd:YAG laser photocoagulation controlled bleeding from radiation proctopathy in most patients with an acceptably low morbidity. Patients with sigmoid colon involvement responded less favorably. Endoscopic laser photocoagulation should be considered before surgical intervention for treatment of hematochezia from radiation proctopathy.

-Interruption of umbilical blood flow in an acardiac twin by endoscopic laser coagulation-.

We report on the successful intrauterine surgical treatment of a twin pregnancy with an acardiac fetus. At 18 weeks of gestation the patient presented with polyhydramnios and a hydropic acardius acephalus and the donor twin showed signs of congestive heart failure. Colour Doppler ultrasound allowed localisation of the communicating vessels running on the placental surface towards the umbilicus of the acardiac twin. At 20 weeks we performed endoscopic laser coagulation of the umbilical vessels of the acardiac twin. A sheath (9.8 Charriere) with a 1.9 mm diameter rigid fetoscope (field of vision 60 degrees) was introduced percutaneously under local anesthesia into the amniotic cavity of the "pump" twin. Under sonographic control the fetoscope was directed towards the communicating vessels on the placental surface. A Nd-YAG laser (0.4 mm diameter fiber) was used to coagulate two vessels, artery and vein, until interruption of the reversed arterial perfusion was accomplished. Tricuspid regurgitation of the normal twin disappeared throughout the following two weeks and no further complications occurred throughout pregnancy. At 39 weeks a healthy girl was delivered vaginally. No at the age of 3 months she is developing normally. Minimal invasive endoscopic laser coagulation of the umbilical vessels of the acardiac twin appears to be the optimal currently available treatment for the normal twin, for which otherwise a high mortality ( > 50%) and morbidity must be expected.

[Interstitial laser coagulation of the prostate in patients with prostatic hypertrophy: preliminary results with medium-term follow-up].

Although transurethral resection of the prostate (TURP) is the gold standard for treatment of BPH, a new, minimally invasive technique, called Interstitial Laser Coagulation (ILC), introduced by Muschter and Hofstetter, has been developed to treat BPH. ILC is base on the coagulation of prostatic tissue by means of diffused laser light using a diode laser system with a temperature feed-back (Indigo Medical 830e TM). We report our results in treating 20 patients with BPH and Bladder Outlet Obstruction (BOO), after a mean follow-up of 7 months. To define the indication for treatment and to monitor the success, urinary flow-rate (Omax), I-PSS symptoms score, residual urinary volume and prostate volume determined by ultrasound were recorded before treatment and after 3, 6, 12 months. Transurethrally, under spinal or regional anaesthesia, 2 or 3 insertions of the laser fibre in each prostatic lobe were performed beginning at the apex directly proximal to the bladder neck, made at 1 cm intervals, using a modified model Olympus cystoscope. After 6 months, Qmax increased from 6, 7 ml/sec to 11, 3 ml/sec; I-PSS score decreased from 18 to 8; residual urinary volume from 130 ml to 40 ml, prostate volume decreased from 60 to 48 ml. In selected cases, urodynamic evaluation was performed before and after treatment. Urodynamic findings were classified according Shafer diagram to grade B.O.O.: the score was 4 before treatment and 2 after 6 months. We observed UTI in 3 cases, retrograde ejaculation and acute urinary retention in 1 case, transitory irritative and obstructive syndrome in 8 cases after treatment. ILC is a simple, safe and effective treatment for BPH with significant improvements in both objective and subjective parameters. It is cheaper than others mini-invasive therapies and is able to treat any prostate volume, saving urethra and sexual function.

Interstitial laser coagulation in patients with lower urinary tract symptoms from benign prostatic obstruction: treatment under sedoanalgesia with pressure-flow evaluation.

OBJECTIVES: To evaluate the effects on lower urinary tract symptoms and pressure-flow variables after interstitial laser coagulation (ILC) of the prostate using the Indigo diode laser system (Indigo, Palo Alto, USA). PATIENTS AND METHODS: Forty-nine men (median age 68 years, range 52-80) were assessed using symptom scores and voiding variables before and at 3 and 12 months after ILC. A subset of 26 men (median age 68 years, range 63-72) underwent pressure-flow measurements before and at 6 months after ILC. All treatments were performed in the outpatient department using sedoanalgesia. RESULTS: The International Prostate Symptom Score decreased from 22 to 11 at 12 months after ILC. The peak urinary flow (Qmax ) was 8.6 mL/s at baseline and increased to 9.9 mL/s at 12 months. Residual urine volumes were unchanged. The median duration of urinary retention after ILC was 3 days. From pressure-flow recordings, 17 patients were categorized as obstructed and seven as equivocally obstructed before ILC (using the International Continence Society definition). Their Qmax increased from 7.7 to 9.0 mL/s after 6 months, the detrusor pressure at Qmax decreased from 68 to 51 cmH2O and the Abrams-Griffiths number decreased from 54 to 29 (P<0.01). Patients with moderate to equivocal obstruction had a greater relief of symptoms than those who were clearly obstructed. Patients with prostate volumes of >40 mL had a greater decrease in the Abrams-Griffiths number than had patients with smaller prostates. Postoperative perineal pain was reported by 72% of patients; the pain subsided after 1-2 weeks. The re-treatment rate was 15% within the first year. CONCLUSION: Treatment with ILC produced substantial effects on symptoms and moderate to small changes in urodynamic variables. Patients with moderate or equivocal bladder outlet obstruction or large prostates seem to be the best candidates for this treatment. However, treatment was followed by perineal pain for 1-2 weeks in most cases. A long-term follow-up is necessary to determine the role of ILC.

Impact of laser coagulation in severe twin-twin transfusion syndrome on fetal Doppler indices and venous blood flow volume.

OBJECTIVE: To evaluate the impact of fetoscopic laser coagulation of placental anastomoses in severe twin-twin transfusion syndrome on fetal Doppler indices and umbilical vein blood flow volume as calculated with Doppler and two-dimensional ultrasound. METHODS: Thirty-two cases of second-trimester severe twin-twin transfusion undergoing laser therapy were examined with serial ultrasound before and 1, 3 and 5 days after therapy. Pulsatility indices in the umbilical artery and ductus venosus were measured. Blood volume flow at the level of the intra-abdominal umbilical vein was calculated by means of Doppler and two-dimensional ultrasound. The development of hydropic signs in donors was recorded. Perinatal outcome in terms of neonatal survival was recorded for all cases. RESULTS: In recipients, ductus venosus pulsatility index decreased progressively after therapy and, by day 5, median pulsatility index was significantly lower than that before therapy (0.97 vs. 0.82, P < 0.0001). Umbilical vein blood flow volume in recipient twins showed no significant variations before and after laser. In donors, umbilical artery pulsatility index decreased significantly by the first day following therapy (2.1 vs. 1.6, P < 0.0001). Previously absent or reverse umbilical end-diastolic flow reappeared after therapy in 46% (7/15) of donors. Ductus venosus pulsatility index in donors increased significantly by day 1 after therapy (0.99 vs. 1.35, P < 0.0001) but, over days 3 and 5, it returned towards preoperative values. Umbilical vein flow volume measurements (mL/min/kg) in the donor increased by approximately 50% the day after treatment (151 vs. 232, P < 0.0001) and remained elevated. Umbilical vein flow volume before laser therapy was significantly lower in donors compared to recipients (151 vs. 260, P < 0.0001), but the difference was non-significant after treatment (240 vs. 267). One or more hydropic signs developed in eight (25%) donors during the 5 days' observation after therapy. CONCLUSIONS: Laser therapy induced important changes in fetal hemodynamic parameters, resulting in a reversion of the disturbances associated with severe twin-twin transfusion syndrome. The recipient twin showed a progressive improvement of previous signs of right cardiac overload. The donor experienced a substantial increase in umbilical vein blood volume flow accompanied by a transitory state of relative right overload, which may explain the development of transient hydropic signs in a proportion of donors.

[Refraction and visual acuity after laser coagulation treatment for retinopathy of prematurity in stage 3+].

PURPOSE: Correlations between myopia < or =-4.0 D, birth weight, gestation age at birth, extension of laser photocoagulation and mild posterior pole changes were assessed in eyes of 1-year-old children who underwent laser treatment for stage 3+ retinopathy of prematurity (ROP). In addition the relationship between best-corrected visual acuity (VA) and mild posterior pole alterations was evaluated at 3 years of age. PATIENTS AND METHODS: Patients who underwent argon or diode laser photocoagulation between 1996 and 2000 for ROP 3+ disease were analyzed in this retrospective study. Cycloplegic refraction was determined in 72 eyes of 1-year old patients ( n=39). At 3 years of age cycloplegic refraction was measured in 36 eyes of 19 patients, and visual acuity was determined in 25 eyes of 13 children. RESULTS: Myopia of < or =-4.0 D significantly correlated only with mild posterior pole changes (i.e. clinically significant dragging of the temporal vessels of the retina or macular heterotopia, p=0.001). The correlation was significant between a VA reduction to <0.8 and mild posterior pole alterations ( p=0.014). CONCLUSIONS: The prevalence of myopia < or =-4,0 D and visual acuity <0.8 appears to be increased even with mild posterior pole changes. Birth weight does not seem to be significant factor in the prevalence of myopia < or =-4.0 D following laser coagulation of stage 3 threshold ROP.

Interstitial laser coagulation versus transurethral prostate resection for treating benign prostatic obstruction: a randomized trial with 2-year follow-up.

OBJECTIVES: To investigate whether interstitial laser coagulation (ILC) is as effective and as safe as transurethral resection of the prostate (TURP). The treatment of choice for bladder outflow obstruction secondary to benign prostatic hyperplasia is TURP. However, ILC is a less invasive outpatient procedure that may be as effective and safe as TURP. METHODS: In a multicenter randomized trial at six U.S. tertiary care hospitals, we treated 72 men with bladder outflow obstruction secondary to benign prostatic hyperplasia with either TURP (n = 35) or ILC (n = 37). The outcome measures were peak flow rate, postvoid residual urine volume, prostate volume, prostate-specific antigen levels, symptom and quality-of-life indexes, sexual function, and adverse event rates. Measurements were taken at baseline and at 3, 6, 12, and 24 months. RESULTS: At 2 years, the TURP patients had better median peak flow rates, but not significantly so (range 16.5 to 13.9 mL/s, 95% confidence interval for the 2.6 mL/s difference of -0.4 to 7.6). The median scores on the symptom indexes and quality-of-life measures were similarly improved in both groups. Of 37 ILC patients, 6 (16%) were retreated with TURP in the first year. Sexual function declined in the TURP group but remained stable in the ILC group. The adverse event rates were similar, although the events were more serious in the TURP group. CONCLUSIONS: ILC compares respectably with TURP. Given the advantages of an outpatient procedure, similar results in symptom reduction and quality-of-life measures, and less severe adverse effects, ILC can be an acceptable alternative to TURP.

[Retinal hemangioma and laser coagulation in its treatment].

Four patients, aged 19 to 34, suffering from retinal hemangiomas, are described. Argon laser coagulation treatment consisted in destruction of the tumor proper, as a rule, without preliminary formation of a limiting barrier, stage-by-stage, in 2-4 sessions. The following coagulation regimens were used: light spot size 250-500 microns, exposure length 0.2-0.5 s, irradiation power 350-600 mWt.

[Argon laser coagulation of xanthelasmas].

For the therapy of palpebral xanthelasmas surgical, chemical and physical methods are used. We report on the first results of argon laser coagulation of 32 xanthelasma lesions with an average size of 34 mm2 in 11 women and 2 men. The lesions were treated with argon green, spot size 1000 microns, energy 250-500 mW and continuous mode. The therapy was well tolerated; all lesions responded to the therapy. There were no complications, and no functionally relevant scars developed. The cosmetic outcome was assessed as good or very good in 80%). Twelve recurrences were seen after 12-16 months. The method described represents an alternative treatment in selected cases. It is easy to perform and very well accepted by the patients.

Prospective evaluation of interstitial laser coagulation of the prostate: importance of surgical technique and patient selection.

PURPOSE: To evaluate the impact of improvements in surgical technique and patient selection on overall outcomes of interstitial laser coagulation (ILC) of the prostate and to describe our treatment algorithm. PATIENTS AND METHODS: During a 4-year period, 66 men with bothersome lower urinary-tract symptoms underwent ILC of the prostate using the Indigo 830e Diode Laser System (Ethicon Endo-Surgery). Patients were assessed preoperatively and followed prospectively. Parameters included American Urological Association Symptoms Index, Quality of Life Scale, Problems Due to Symptoms Score, postvoiding residual volume, maximum flow rate, number of treatment punctures, and adverse events. One-year follow-up data were compared with baseline data using the Wilcoxon signed rank test or Fisher's exact test. Patients were stratified into those treated during the first 2 years (group 1) and those treated in the latest 2 years (group 2), corresponding to changes in surgical technique and patient selection. RESULTS: Maximum flow rates improved by 47% and 85% in groups 1 and 2, respectively, at 12 months postoperatively compared with baseline (P = 0.04)l. The mean number of punctures differed significantly, with 6.4 in group 1 and 8.4 in group 2 (P = 0.03). Subjective measures were significantly improved from baseline in both groups and did not differ between groups. The incidence of adverse events was similar in the two groups. CONCLUSIONS: Improvements in surgical technique and patient selection described herein corresponded to significantly higher maximum flow rates without an increase in adverse events.

[Krypton laser coagulation in idiopathic macular holes].

Twenty-six eyes with acute idiopathic senile macular holes and visual acuity of between 0.02 and 0.5 were treated by laser coagulation with krypton red. The laser burns were placed in a ring around the hole, at the border of the retinal detachment. A few weeks after treatment marked regression of the detachment was seen. During the follow-up period (3-18 months) visual acuity improved in 18 eyes, deteriorated in 1 and was unchanged in 7.

Ileal atresia and thrombo-embolic liver calcifications diagnosed after treatment with intrauterine laser coagulation therapy for twin-to-twin transfusion syndrome: report of 2 cases.

Ileal atresia is considered to be an acquired anomaly. It is thought to be caused by a mesenteric vascular accident during fetal life which leads to necrosis of downstream tissues. We present a case in which ileal atresia occurred after intrauterine laser coagulation therapy for a twin-to-twin transfusion syndrome. The laser treatment might have been the etiological factor of the atresia. A second case in which thrombo-embolic calcifications were found in the liver is presented as well.

Endoscopic laser coagulation of placental anastomoses in 200 pregnancies with severe mid-trimester twin-to-twin transfusion syndrome.

OBJECTIVE: To investigate perinatal outcome after endoscopic laser coagulation of the placental vascular anastomoses in severe mid trimester twin-to-twin transfusion syndrome (TTTS). STUDY DESIGN: In a prospective study between January 1995 and September 1999, we performed laser therapy in 200 consecutive pregnancies with TTTS between 16 and 25 weeks of gestation. We compared outcome of the first group of 73 pregnancies whose outcome has been reported previously in a study comparing laser surgery and serial amniodrainages [Am J Obstet Gynecol 1999;180:717-24], with the following group of 127 patients. RESULTS: The overall survival rate increased from 61% (89/146) in group 1 to 68% (172/254) in group 2. The percentage of pregnancies with survival of both fetuses was 42% (31/73) in group 1 and increased to 54% (69/127) in group 2 (P=0.142). The survival rate for at least one fetus was 81% (103/127) in group 2. The median gestational age at delivery of liveborn babies was 33.7 weeks in group 1 and 34.4 weeks in group 2 with a median interval of 13 weeks between the intervention and delivery. CONCLUSION: This study of a large population of pregnancies with severe second trimester twin-to-twin transfusion syndrome confirms the improvements of outcome after laser therapy as compared to serial amniodrainages reported previously. Furthermore, it shows a trend towards an increase in survival rates with growing experience in this technique, most likely attributable to a more selective identification and efficient coagulation of the placental vascular anastomoses.

Techniques of coagulation laser prostatectomy for narrow divergence angle fibers.

Although laser prostatectomy has become an accepted technique for the treatment of obstructive prostatism, considerable debate remains about which laser prostatectomy method to use in various treatment situations. This article discusses the different methods of noncontact side-firing coagulation laser prostatectomy using narrow divergence angle fibers (< 30 degrees). Static treatment strategies that have been successfully employed with widely divergent beams are not appropriate for fibers producing narrow divergence angle beams. Narrow divergence angle beams produce a small diameter spot on the prostatic urethra and far larger power density. Additionally, neodymium/yttrium aluminum garnet light scatters relatively poorly within prostatic tissue. Most light continues along the path of through transmission until ultimate tissue absorption and conversion into heat. The width and total volume of the coagulated lesion is therefore limited when using a narrow divergence angle fiber to produce static lesions. Probe movement is essential. Rapid (> 2 mm/s) probe movement produces only superficial coagulation. An initial dwell period of approximately 3 s is also important to maximize coagulated tissue volume. Scanning strategies where the fiber is moved through the prostatic urethra in longitudinal and radial directions are discussed and compared. Radial and longitudinal scanning methods produce similar coagulation defects. Treatment using a rocking motion within a limited volume of tissue may increase coagulation depth. No technique is ideal for all clinical situations. Vaporization prostatectomy or contact laser transurethral incision of the prostate is appropriate for primary treatment of glands < 30 g or as adjunctive therapy to facilitate early catheter removal. Alternative treatment methods are compared to noncontact coagulation prostatectomy.

[The use of argon laser coagulation in the combination treatment of patients with tuberculous chorioretinitis].

The outcomes of argon laser coagulation (ALC) of active tuberculous chorioretinal foci (mainly of exudative and hemorrhagic nature, macular and paramacular localization) in 26 patients are illustrated. The coagulation was performed concurrently with a combined general and local antituberculosis therapy (with 48 patients as controls). The developed direct ALC technique was used together with antibacterial therapy of the total surface of the inflammation focus itself. The examination findings prove that the effectiveness of the above technique is high, since the disappearance of the clinical signs of the inflammatory activity is achieved in a relatively short time (45 days); acute conditions and relapses are prevented; complications are excluded; and an evident economic effect is produced (the shorter period of hospitalization, less duration of the main therapeutic regimen and lower frequency of relapses).
