Suicide in Lithuania during the years of 1990 to 2002.

Lithuania, among other Eastern European countries, has the highest suicide rates in the world. While the mid 1980's experienced a sharp decline in suicidality, the year of 1991 began a new increase in suicide rates, reaching an estimated 44.7 per 100,000 persons in the year of 2002. The ratio of male to female rates was 4.5 to 6.1 in the years of 1990 to 2002. The suicide mortality of rural inhabitants, especially men, has been growing even more intensely, occurring twice as often as among their urban counterparts. When looking at the age factor, the highest suicide risk is for middle-aged men. Among the males aged 45 to 54 years suicide rates reach 154.6. The most common method of suicide remains hanging, both for males and females. The underlying reasons of the suicide spread in Lithuania are multi determined.

The sociodemographic patterning of health in Estonia, Latvia, Lithuania and Finland.

BACKGROUND: Public health problems in the Baltic countries are typical of Eastern European transition economies. A common assumption is that the economic transition has been particularly difficult for previously disadvantaged groups, and comparative research on the health differences between sociodemographic groups in the Baltic countries is therefore needed. This study compared associations of health with gender, age, education, level of urbanization and marital status in three Baltic countries and Finland. METHODS: The data were gathered from cross-sectional postal surveys conducted in 1994, 1996, 1998 and 2000 on adult populations (aged 20-64 years) in Estonia (n = 5052), Latvia (n = 4290), Lithuania (n = 7945) and Finland (n = 12796). Three self-reported health indicators were used: (i) perceived health, (ii) diagnosed diseases and (iii) symptoms. RESULTS: The prevalence of less-than-good perceived health (average, rather poor or poor) was higher in the Baltic countries (men 66-56%, women 68-64%) than in Finland (men 35%, women 31%). The odds ratios (with 95% confidence intervals) of less-than-good perceived health among the low educated compared to the highly educated in Estonia, Latvia, Lithuania and Finland were 2.03 (1.49-2.77), 2.00 (1.45-2.76), 2.27 (1.78-2.89) and 1.89 (1.61-2.20) among men, and 3.32 (2.43-4.55), 2.77 (2.04-3.77), 2.07 (1.61-2.66) and 1.89 (1.63-2.20) among women, respectively. Diseases and symptoms were also more common among the lower educated men and women in all four countries. However, urbanization and marital status were not consistently related to the health indicators. CONCLUSIONS: The Baltic countries share a similar sociodemographic patterning of health with most European countries, i.e. the lower educated have worse health. The methodological considerations of this study point out, however, that further research is needed to support public health policies aimed at the most vulnerable population groups.

Antimicrobial susceptibility and molecular analysis of Enterococcus faecalis originating from endodontic infections in Finland and Lithuania.

BACKGROUND/AIMS: Enterococcus faecalis strains with multiple antibiotic resistances can cause infections that are difficult to treat. The microbial flora in treatment-resistant apical periodontitis is dominated by E. faecalis, and is a potential source of infections at other sites. MATERIAL AND METHODS: Sensitivities to a range of antibiotics were determined for 59 endodontic E. faecalis isolates from Finland and Lithuania. The DNA sequence of the gene responsible for the species' intrinsic quinupristin-dalfopristin resistance, lsa, was determined from two isolates with diminished resistance. Four pairs of isolates from the same root canal were typed by pulsed-field gel electrophoresis. RESULTS: A high prevalence of resistance to rifampicin was found, whereas all isolates were susceptible or showed intermediate susceptibility to penicillin and ampicillin and four isolates were unusually susceptible to cefotaxime. No vancomycin or high-level gentamicin resistance was detected. Nine of 59 isolates were susceptible to quinupristin-dalfopristin. A fully quinupristin-dalfopristin-susceptible isolate also susceptible to clindamycin produced a truncated Lsa polypeptide, and an isolate with borderline quinupristin-dalfopristin-susceptibility had mutations proximal to the predicted ribosomal binding site. Pulsed-field gel electrophoresis showed that the same root canal could harbor two different strains of E. faecalis during the course of the same infection. CONCLUSION: Despite the differing antibiotic usage in Finland and Lithuania, E. faecalis from endodontic infections in these countries showed similar susceptibility patterns with levels of resistance considered typical for the species, and decreased resistance to clindamycin and quinupristin-dalfopristin as well as lesions in the lsa gene which were similar to those described in other clinical isolates.

The prevalence of common acquired melanocytic nevi and the relationship with skin type characteristics and sun exposure among children in Lithuania.

OBJECTIVE: To evaluate the prevalence of common acquired melanocytic nevi and its relationship with pigmentary characteristics and severe sunburns in children. DESIGN: Cross-sectional study. SETTING: Kaunas city, Lithuania (latitude of 55 degrees 55 minutes). PATIENTS: A random sample of 484 children aged 1 to 2 years, 4 to 5 years, and 9 to 10 years and adolescents aged 14 to 15 years. INTERVENTIONS: A questionnaire that provided information about the history of sunbathing and skin type. MAIN OUTCOME MEASURES: Atypical melanocytic nevi were defined according to the clinical criteria of the ABCDE rule. RESULTS: The median number of all common acquired melanocytic nevi was 12 in boys and 11 in girls; the median number of melanocytic nevi 2 mm or larger in boys and girls was 4. Twenty-seven percent of children experienced severe sunburns more than once. After adjustment for age and sex, it was found that children who had severe sunburns in summer and skin type I had a higher density of all melanocytic nevi and melanocytic nevi 2 mm or larger. The prevalence of atypical melanocytic nevi was 7% in all children and was age dependent (age 4-5 years, 1%; 9-10 years, 4%; 14-15 years, 16%). Three percent of children had congenital melanocytic nevi. CONCLUSIONS: The total number of common acquired melanocytic nevi in children increased with age. There was a positive association between severe sunburns, the tendency of the skin to burn, and the number of all melanocytic nevi and nevi 2 mm or larger.

Time trends in lung-cancer mortality rates among men in Lithuania, 1965-1994.

This study examines time trends in lung-cancer mortality rates among men in Lithuania during the period 1965-1994. Age-standardized mortality rates increased from 32.5 per 100,000 in 1964-1969 to 62.9 per 100,000 in 1990-1994. Regression analysis indicates that net drift (sum of cohort and period slopes) was positive. Cohort effect was dominant in the trend observed. Mortality in cohorts born before 1945 showed an increase. In younger generations, born after 1945, the risk declined in each subsequent cohort. Higher prevalence of cigarettes with filters and lower tar content of cigarettes may be of importance. This analysis suggests that mortality will rise until cohorts born around 1950 reach middle age or old age. After that some stabilization can be expected if other conditions remain unchanged and previous trends continue.

Lung cancer near an industrial site in Lithuania with major emissions of airway irritants.

To assess the relation between air pollution with airway irritants, including sulfuric acid and lung cancer, a case-control study was performed near an industry producing sulfuric acid and fertilizers in Kedainiai county, central Lithuania. The county had the highest lung cancer rates of the country among men. Between 1967 and 1973, the levels of sulfuric acid exceeded 500 microg/m(3) within 2 km of the industry and 100 microg/m(3) more than 5 km away. A total of 277 men who were diagnosed as having lung cancer during 1981-1991 in Kedainiai county were included as well as 1,108 population controls. Information on residential history since 1960, smoking habits, occupations and workplaces during lifetime was obtained from questionnaires mailed to next-of-kin. The relative risk of lung cancer associated with living within approximately 5 km from the plant was 1.02 (95% CI: 0.76-1.38) compared to those who had never lived in this area. No relation with distance or duration of residence was observed. Furthermore, workers at the plant did not have an increased lung cancer risk. The relative risk of lung cancer associated with smoking was 21.2 (95% CI: 7.51; 60.1) for current smokers and 14.0 (95% CI: 4.88; 40.3) for exsmokers. The duration of smoking, a low age at start and amount of cigarettes smoked daily were positively associated with lung cancer risk. Smoking levels appeared more pronounced among study controls than in the rest of the country. Our study could not confirm earlier evidence of an association between exposure to airway irritants, such as sulfuric acid and lung cancer. Smoking is the major determinant of the risk of lung cancer in men in Kedainiai county. It is probable that higher smoking rates constitute the main reason for the increased lung cancer risk among men in this area.

Factors affecting N and P losses from small catchments (Lithuania).

Most of the important factors causing differences in nutrient losses and their interaction were analysed in three small catchments that are located in partially different geographic and climatic conditions in Lithuania. The investigation revealed that climatic factors change the amount and pattern of water discharge over year (larger water discharge during winter in the catchment located closer to the sea), but nutrient leaching is more dependent on land use. Agricultural factors, such as larger cultivated area and excessive fertilisation in one catchment cause larger nitrogen losses (15 kg N ha(-1) year(-1)). Large area of non-intensively used grassland leads to very small nitrogen losses (5.7 kg N ha(-1) year(-1)) in another catchment. However, larger water discharge combined with loamy sandy soils leads to comparatively high nitrogen losses (12 kg N ha(-1) year(-1)). The highest P losses (0.318 kg P ha(-1) year(-1)) occurred in the catchment with hilly relief and clay soil texture. In summary, extensive agriculture in the post-Soviet countries has reduced the importance of agricultural activity for the extent of nutrient losses and agricultural factors (cultivation, fertilisation and livestock density) are 'responsible' for the losses only in the region of sufficient agricultural activity (N input--71.5 kg N ha(-1), livestock density--0.87 LU ha(-1)).

Temporal and spatial variation of nutrient levels in the Nemunas River (Lithuania and Belarus).

There are discordant results on trends in nutrient river water quality from the economical transition countries in Europe. The present study assessed the impact of these economical changes on the load and concentration at 17 monitoring stations along the Nemunas River and its major tributaries (Lithuania and Belarus). Three time periods were evaluated: the Soviet rule command system period 1986-1991, the transfer to market economy period 1992-1996 and the post reform period 1997-2002. The most surprising result in this study, was the increased area-specific load of NO(3)-N from the first to the third period at almost all the sampling sites. The increase was particularly large (43-78%) at the sites in the Lithuanian part of the river. The corresponding load increase in the Belarussian part of the river was only 1-15%. The statistical analyses of concentration data confirm the strong upward NO(3)-N trend at the Nemunas mouth and at 5 of the 6 tributaries in the lower part of Nemunas. Temporal and spatial analysis of nitrates transport in the Nemunas River and its main tributaries revealed that nitrates mainly originate from agricultural areas. The upward trends were most likely an effect of ploughing of pastures and unbalanced crop fertilisation in combination with large storage and accumulation of soil-nitrogen during the Soviet period. On contrary to nitrate-N, the area-specific load of PO(4)-P decreased significantly from the first to the third period at all sites along the Nemunas River (31-86%). Seasonal (SMK) and Partial (PMK) Mann-Kendall tests on PO(4)-P concentrations also showed significant downward trend at 14 of 16 investigated sites. The decrease of PO(4)-P levels was attributed to the reduction of municipal and industrial point source emissions and to the decreased livestock numbers. The NH(4)-N load showed the same pattern as PO(4)-P. At the river mouth the load was 90 kg km(-2) yr(-1) during the first period compared to only 20-30 kg km(-2) yr(-1) in the third period. The trend test on NH(4)-N concentrations detected significant downward trends at 5 out of 16 sites. The declines were explained by decreased emissions from cities and large animal breeding farms. This study showed that trend analysis at multiple sites in a river basin is crucial for the understanding of the variability in time and space. Such analysis is also important for our interpretation of underlying sources and fluxes in a drainage basin over time. This is particularly important for compounds that have different source origin.

Variations and trends of ground-level ozone and AOT40 in the rural areas of Lithuania.

The analysis of data of the ground-level ozone concentration and accumulated ozone exposure over a threshold of 40 ppb (AOT40) in the rural areas of Lithuania is presented. Trends in the annual ozone mean, 95th and 25th percentiles were determined as statistically not significant at Preila and Rugsteliskes sites during the 1994-2004 period. Trends in the ozone concentration and its percentiles in the air masses arriving to Preila station in "polluted" and "unpolluted" sectors have been examined. Statistically significant changes in the ozone annual mean and 25th percentile were found in air masses in both "polluted" and "unpolluted" sectors in the 1988-2002 period. The trend analysis in the ozone monthly mean and percentiles for each month of the year revealed the main changes in the ozone level in both sectors during January-May. Insignificant downward trends in monthly 95th percentile in "unpolluted" sector and upward trends in "polluted" sector were found during summer months. Values of AOT40 for the protection of forests as well as crops and semi-natural vegetation were determined during the 1994-2004 period. The estimated AOT40 values for the protection of forests were lower than the critical level at Lithuanian rural sites but AOT40 values for the protection of crops and semi-natural vegetation were found to be higher than the critical level at both sites.

Time trends of incidence, mortality, and relative survival of invasive skin melanoma in Lithuania.

The aim of this study was to provide insights into the descriptive epidemiology of invasive skin melanoma in Lithuania by analyzing population-based incidence (1978-2002) and mortality (1990-2002) time trends, and relative survival based on 3485 skin melanoma. We calculated age-standardized incidence and mortality rates (cases per 100,000) using the European Standard Population and calculated period estimates of relative survival. The incidence rates increased from 1978 (men: 1.7, women: 2.3) to 2002 (men: 5.0, women: 7.0). The incidence increase over time is accompanied by cohort effects among both men and women. Mortality rates increased from 1990 (men: 1.2, women: 1.7) to 2002 (men: 2.3, women: 2.2). Relative 5-year survival rates among men were 10% lower than among women. The overall difference in survival was mainly due to a more favourable survival among women aged 60-74 years. Overall prognosis was less favourable among men, most likely due to diagnoses at later stages.

Long-term evaluation of the needle surface wax condition of Pinus sylvestris around different industries in Lithuania.

The aim of our study was to evaluate the annual dynamics of needle surface wax erosion and wettability in Scots pines exposed to a gradient of industrial pollutants emitted from the main factories of Lithuania: a nitrogen fertilizer factory, an oil refinery and a cement factory. Decreased emissions (in the case of the oil refinery and the cement factory) were reflected in the increased structural surface area (SSA, i.e. area covered by tubular waxes) on the needles. The nearly constant amount of emissions from the nitrogen fertilizer factory within the 1994-2000 period corresponded to negligible annual differences in SSA. Annual changes in the hydrophobicity of needles on the investigated transects were small. Despite the decreased pollution within the 7-year period, industrial emissions are still causing significantly accelerated wax erosion and increased wettability in needles sampled from the stands most heavily affected by pollutants.

Epiphytic lichen community dynamics in deciduous forests around a phosphorus fertiliser factory in Central Lithuania.

The detailed dynamics of epiphytic lichen communities were observed while studying permanent quadrats in the zone of influence of a phosphorus fertiliser factory in central Lithuania. The most significant changes were induced by several factors: changes in macroenvironment (increase of illumination), bark scaling, succession processes, individual growth characteristics of the community members, and influence of fungal infection and invertebrate grazing. None of these changes could be directly linked with air pollution. These observations have shown that in conditions of more or less stable pollution, epiphytic community dynamics should be evaluated with care, the best indicators of the characteristics of the communities being species richness and presence/absence and abundance of indicator (nitrophilous or acidophilous) species.

Radical scavenging activity and composition of raspberry (Rubus idaeus) leaves from different locations in Lithuania.

Raspberry (Rubus idaeus) leaves, collected in different locations of Lithuania were extracted with ethanol and the extracts were tested for their antioxidant activity (AA) by using ABTS(.)(+) decolourisation and DPPH(.) scavenging methods. All extracts were active, with radical scavenging capacity at the used concentrations from 20.5 to 82.5% in DPPH(.) reaction system and from 8.0 to 42.7% in ABTS(.)(+) reaction. The total amount of phenolic compounds in the leaves varied from 4.8 to 12.0 mg of gallic acid equivalents (GAE) in 1 g of plant extract. Quercetin glucuronide, quercetin-3-O-glucoside and rutin were identified in the extracts.

Factors associated with Listeria monocytogenes contamination of cold-smoked pork products produced in Latvia and Lithuania.

A total of 312 samples of sliced, vacuum packaged, cold-smoked pork from 15 meat processing plants in Latvia and Lithuania, obtained over a 15-month period from 2003 until 2004, were analyzed for the presence of Listeria monocytogenes at the end of their shelf-life. Overall, 120 samples (38%) tested positive for L. monocytogenes. Despite the long storing period, the levels of L. monocytogenes in cold-smoked pork products were low. Manufacturing processes were studied at seven meat processing plants. A new approach with a logistic multivariable regression model was applied to identify the main factors associated with L. monocytogenes contamination during the manufacturing of cold-smoked pork products. Brining by injection was a significant factor (odds ratio 10.66; P<0.05) for contamination of product with L. monocytogenes. Moreover, long cold-smoking times (> or = 12 h) had a significant predictive value (odds ratio 24.38; P<0.014) for a sample to test positive for L. monocytogenes. Pulsed-field gel electrophoresis results indicated that various sources of L. monocytogenes contamination existed over periods of time in several meat processing plants. In two meat processing plants, persistent L. monocytogenes strains belonging to serotypes 1/2a and 1/2c were found.

Reliability of cause-specific mortality rate statistics: case of Lithuania.

OBJECTIVES: To evaluate the reliability of cause-specific mortality rate statistics. STUDY DESIGN: The underlying causes of death among different demographic groups in a territorial unit of Lithuania were verified and the data were compared with the corresponding official statistics. METHODS: Community-based autopsy and expert analysis of medical records. RESULTS: The study contingent consisted of 1474 permanent residents aged 0-101 years [809 (54.9%) males and 665 (45.1%) females] who had died in hospital (n=546, 37%) and out of hospital (n=928, 63%) in 1989-1991. The underlying cause of death was verified in 98.6% of cases by full autopsy (69.9%) and expert analysis of medical records (28.7%). Circulatory system diseases were found to be implicated in 44.4% of all deaths (35.9% of males and 54.7% of females), malignant neoplasms were the cause of 19.4% of deaths (21.5 and 16.8%, respectively) and external causes were responsible for 19.4% of deaths (27.3 and 9.8%, respectively). Cause-specific mortality rates were sex and age dependent. CONCLUSIONS: The proportion of circulatory system diseases in the cause-specific mortality structure was found to be significantly lower, and that of external causes (injury and poisoning) was found to be higher than the corresponding proportions presented by official Lithuanian statistics. Verified cardiovascular death rates corresponded with those in the European Union as a whole.

Prevalence of childhood epilepsy in Kaunas, Lithuania.

The prevalence of epilepsy in children aged 0-15 years of Kaunas city, Lithuania, was evaluated on 1 January 1995. Multiple sources for case identification were used, i.e. medical records at the university hospital, regional outpatient clinics and consultation centres, institutions, schools and kindergartens for the handicapped. Active epilepsy was defined as two or more unprovoked epileptic seizures with at least one seizure occurring within the previous 5 years, regardless of the antiepileptic drug treatment. Prevalence was found to be 4.25 (3.42, if age-standardized) in 1000. The highest rate was found in the 10-14 years age group. The male/female ratio was 1.29. No possible causes could be determined in 60.3% of cases. Congenital causes were diagnosed in 18.8% of cases, perinatal causes in 15.3%, traumatic causes in 2.6% and neuroinfectious causes in 2.4%. Classification of epilepsies and epileptic syndromes [Commission on Classification and Prognosis of the International League Against Epilepsy. Proposal for revised classification of epilepsies and epileptic syndromes. Epilepsia 1989; 30:389-399] revealed that 50% of cases were localization-related epilepsies, 29.9% were generalized epilepsies, 15.9% were undetermined whether partial or generalized and 4.2% were unclassifiable. Rates for idiopathic, symptomatic and cryptogenic cases are presented.

Evaluation of intensive care unit performance in Lithuania using the SAPS II system.

BACKGROUND AND OBJECTIVE: Outcome prediction and evaluation of intensive care unit (ICU) performance using severity of illness scoring is a tool for the estimation of effectiveness and quality of intensive care. We used the simplified acute physiology score (SAPS) II system to evaluate ICU performance. METHODS: The present study is a prospective observational study in an ICU at Vilnius University Emergency Hospital, Lithuania. The observed death rate was compared with the predicted death rate calculated using SAPS II system. The ability of the SAPS II prognostic system to predict the probability of hospital mortality was assessed with discrimination and calibration measures. RESULTS: Two-thousand-and-sixty-seven patients consecutively admitted to the ICU were studied. The median SAPS II score on the first ICU day was 29. The SAPS II system showed a good ability to separate those patients predicted to live from those predicted to die (an area under the receiver operating characteristic curve was 0.883). The calibration curve demonstrated under-prediction of the actual death rate (Hosmer-Lemeshow goodness-of-fit test, chi2 = 56.98; df = 8; P < 0.001). The observed mortality was higher than predicted by the SAPS II equation (observed to predicted ratio is 1.28). CONCLUSIONS: The SAPS II system is a useful tool for the assessment of ICU performance. This system demonstrated a good ability of discrimination, but an under-prediction of the actual mortality rate, in Lithuanian ICUs.

Dental health patterns in young adults in Lithuania: an exploratory, analytical approach.

Patterns of variation in experience related to caries and treatment with regard to psychosocial, socio-economic, lifestyle, and oral healthcare-related characteristics were explored. A random sample of 382 (response rate 51%) subjects residing in 10 areas of Lithuania was examined (35 to 44-year-olds). Participants were given a dental examination and asked to complete a structured questionnaire. Experience related to caries and treatment differed with regard to residency and between genders. Analysis of self-reported data elucidated different aspects of psychosocial and socio-economic status and a variety of lifestyle factors. Interrelationships between the studied determinants were also analysed. Different patterns of dental health were revealed for men and women and for urban and rural participants. It can be concluded that factors influencing the dental status of Lithuanians are not only multiple, they seem also to influence dental health in complex ways. An exploratory approach should therefore be considered as a prerequisite to a causal approach of studying caries in populations.
