[Clinico-immunologic monitoring of HIV-infected patients: comparative analysis of indicators, characterizing development of the disease].

The paper contains the results of a 15-year monitoring (since 1986 to 2001) of HIV-infected patients followed up at the clinic of the Institute of Immunology, Federal Department "Medbioextrem", Russia's Health Ministry. An analysis of a category of HIV-infected persons, who practiced the intravenous consumption of drugs since 1997 to 2001, revealed not only concomitant multi-virus infections (hepatitis C--68%, hepatitis B--48%, herpes 41%, fungal infections--66% and bacterial infections--46%) but also lesions (up to 70%) in the gastrointestinal tract and in the hepatic-cellular and hepatic-biliary systems. It was established that, when only immunological indices are taken account of in presence of multi-virus infections, it does not ensure an adequate evaluation of an HIV-patient condition. The below biochemical parameters must be necessarily taken into consideration: activity of alanine aminotransferase, aspartate aminotransferase and of alkaline phosphatase. The changes in the biochemical parameters were shown to be essentially higher in HIV patients with hepatitis C virus (HCV) even during the 2 initial follow-up years, when the immunological parameters are in the norm or even insignificantly below it. A comparative analysis of dynamic clinical, immunological and biochemical indices was made within the case study for the purpose of defining the prognostic criteria for HIV progression scenarios.

Xenogeneic and allogeneic canine heart transplantation: a model for cytologic and immunologic monitoring of rejection mechanisms.

In the model of heterotopic intrathoracic heart transplantation, rejections of the graft are not lethal for the recipient animal. Therefore it is possible to follow the immunologic rejection mechanism to the final stage. Allogeneic dog hearts (n = 6) and xenogeneic fox hearts (n = 6) were transplanted. Immunosuppression therapy included cyclosporine and methylprednisolone. Cytoimmunologic monitoring was done every second day. Due to fluctuations of lymphocyte subpopulations and their immature forms, endomyocardial biopsies were performed. The transplanted allogeneic hearts survived for 53.2 +/- 14.8 days if treated with cyclosporine (historical control group without immunosuppression 6.8 +/- 0.8 days). Xenogeneic fox hearts stopped beating after 20.2 +/- 4.1 days (historical control 8.4 +/- 1.9 days). The allogeneic grafts were all rejected in an acute cellular fashion, whereas xenogeneic transplanted hearts showed humoral and cellular rejection mechanisms. Monitoring of circulating inflammatory cells allowed differentiation between humoral and cellular rejection, which was confirmed by histology. Both types of rejection were accompanied by an increase of lymphocytes and their activated forms. Differentiation of lymphocyte subpopulations revealed a significant increase of surface IgG-positive B-lymphocytes under humoral rejection, whereas acute cellular rejection episodes showed a substantial increase of surface IgG-negative lymphocytes. Humoral rejection that developed even with cyclosporine administration was not influenced, whereas cellular rejections were controlled by increasing methylprednisolone to 250 mg/day for 3 days.

Immunological monitoring of patients affected by anaplastic glioma concerning in effects of surgery, radio-, and chemotherapy. A preliminary report.

The authors studied 24 patients affected by anaplastic gliomas, as regards immunology. In all of them the authors evaluated the lymphocyte subpopulation (B and T), firstly by simple lymphocyte count, secondly by studying the rosettes E-total and EAC, thirdly by stimulating the lymphocytes with mitogens phyto-haemoagglutinin-P (PHA), concanavalin A (Con A), and pokeweed mitogen (PWM), and lastly by counting the release of Cr51 in Chang liver cells culture in order to obtain antibody dependent cellular cytotoxicity (ADCC). The parameters were also evaluated after surgery and during conventional radio-chemotherapy with BCNU. Whereas the so-called B-pool seems to be unaffected, the preliminary results show that the T-pool (identified by the E-t rosettes and by responses to PHA, PWM, and ConA) is depressed to a statistically significant degree, if compared with a control group. This depression seems to be related to the tumoral mass, and it is not increased by radio-chemotherapy. In addition, ADCC also seems to be depressed in our glioma patients in comparison with a control group and witha group of bladder cancer patients.

[Hematic radio-immunologic monitoring of clonazepam: clinical-pharmacological evaluations in a group of epileptic patients].

In a group of 22 epileptic patients under treatment with clonazepam (CNP) it has been evaluated the serum levels of CNP in a seried way for a period of several months, by a radio-immunological technique. Except for one patient, all the others were under politherapy. On the basis of the data obtained from this study we tried to evaluate the pharmachological action of CNP. Even if we found a good clinical efficacy of CNP with therapeutic ranges (from 19,57 +/- 10,98 ng/ml up to 60,16 +/- 31,18 ng/ml) we didn't find any correlation between the oral dose and serum level, taking into account also other parameters such as sex, pound and age. The Authors discuss the possible factors which may be thought involved in this lacking correlation.
