Management of the difficult patient.

All physicians must care for some patients who are perceived as difficult because of behavioral or emotional aspects that affect their care. Difficulties may be traced to patient, physician, or health care system factors. Patient factors include psychiatric disorders, personality disorders, and subclinical behavior traits. Physician factors include overwork, poor communication skills, low level of experience, and discomfort with uncertainty. Health care system factors include productivity pressures, changes in health care financing, fragmentation of visits, and the availability of outside information sources that challenge the physician's authority. Patients should be assessed carefully for untreated psychopathology. Physicians should seek professional care or support from peers. Specific communication techniques and greater patient involvement in the process of care may enhance the relationship.

Self-poisoning in Auckland reconsidered.

Data is presented describing 345 self-poisoners presenting at the accident and emergency department and 211 referred to the psychiatric liaison service at Auckland Hospital during the 12 month period between August 1976 and July 1977. Both groups show the age and sex distribution commonly seen in self-poisoning, with a preponderance of young females. Referral rates increased with age and with the implication of anti-depressants, neuroleptics and anticonvulsants. Only 20 percent of the referred patients had a serious psychiatric disorder. Personality disorder, depressive neurosis and transient situational disturbance accounted for 80 percent. The benzodiazepine group of anxiolytics was implicated in 40 percent of incidents and the use of this class of medication is discussed in relation to the problem of self-poisoning. It is suggested that if all Auckland doctors made a decision not to prescribe oral benzodiazepines, the incidence of self-poisoning in the city could be reduced to a half to two thirds of its present proportions.

Personality disorders in a Dutch forensic psychiatric sample: convergence of interview and self-report measures.

Convergence of PDQ-R- and SIDP-R-derived personality disorder diagnoses was studied in a sample of 85 forensic psychiatric patients. For categorical diagnoses, the mean kappa was .34, but on a dimensional level convergence was somewhat higher. Paranoid, antisocial and borderline personality disorders had prevalence rates around 40%; the other personality disorders occurred with much lower frequency. The PDQ-R yielded more diagnoses, except for antisocial, histrionic, narcissistic, and sadistic personality disorder. Because the latter disorders are among the most prevalent in forensic settings, and because they have important risk and treatment implications, the PDQ-R is not suitable as a screening device in forensic populations. Semistructured interviews that make use of collateral information are recommended for diagnosing personality disorders in forensic subjects.

[Personality traits and psychiatric disorders in females with violent partner relationships: a correlational study].

UNLABELLED: The domestic violence (physical or psychological, uni or bidirectional) poses questions about female victims' personality traits or mental disorders that facilitate the development of violence. A sample of 32 females between 20 and 65 years old was studied with the International Psychiatric Interview - Modified (MINI) and the Minnesota Multiphasic Personality Inventory 2 (MMPI-2). ANOVA non parametric and Fischer's Exact Test, (p<0.05) were used. RESULTS: impulsivity was predominant in crossed violence, and victims showed depressive symptoms and low esteem. MINI showed general anxiety disorder and MMPI-2 showed paranoid symptoms in both groups.

Stability and course of personality disorders: the need to consider comorbidities and continuities between axis I psychiatric disorders and axis II personality disorders.

The literature pertaining to the stability and course of personality disorders is briefly reviewed. Available data suggest that PD diagnoses demonstrate only moderate stability and that--although generally associated with a plethora of negative outcomes--they can show improvement over time. This paper highlights the pervasiveness of diagnostic co-occurrence and its implications for continued investigation of the question of PD stability. In addition to examining the stability (and outcome) of PDs, longitudinal studies need to consider continuities between diagnoses. The Collaborative Longitudinal Personality Disorder Study (CLPS) is described briefly vis-a-vis some of these major issues.

Epidemiology of psychiatric disorders in Edmonton. Antisocial personality disorders.

3258 randomly selected adult household residents of Edmonton, Canada, were interviewed by trained lay interviewers using the Diagnostic Interview Schedule (DIS). 104 subjects fulfilled DSM-III antisocial personality disorder (ASP) criteria. Lifetime prevalence rates were found to be significantly higher in males and in the younger adult age groups. The age of onset (i.e. age at which conduct disorder symptoms first appeared) was found to be under 10 years in the majority of cases, with females lagging just slightly behind males. Symptom patterns and frequencies were examined and the relative risks for these symptoms were calculated. Comorbidity, calculated using full DSM-III severity criteria, but without exclusion criteria, revealed an increased prevalence of nearly every other psychiatric disorder in those with antisocial personality disorder, with 90.4% having at least one other lifetime psychiatric diagnosis.
