From July 1971 to August 1981, 31 selected patients with T1, T2 tumours of the Tonsillar region were treated according to the following protocol: Telecobalt therapy to the primary site and to neck nodes to a dose of 45 Gy, Brachytherapy to the primary site to a dose of 25-30 Gy using iridium 192, Boost dose to involved neck nodes, with electrons, or radical neck dissection, wether N1, N2 or N3. The actuarial survival free of disease is 76% for the whole group and 86% for the NO group. The local control rate is 100% without any recurrences at the primary site having been observed. Disease control in the neck is 94% overall and 100% for the NO group. The effects on salivary function were reduced in comparison with telecobalt therapy alone. These favourable results favour the use of this protocol for superficial, minimally infiltrating tumours less than 4 cm in diameter without obvious extension to the base of the tongue or retromolar trigone.
We recently introduced the implantation of Iridium 192 as a method of local treatment of breast cancer in Austria. The afterloading technique is described. This modality should be used as a boost to the "high-risk" areas following conservative breast surgery and combined with megavoltage external irradiation. Interstitial implantation may also be used as a primary form of treatment. A report on 35 patients is presented, 25 of whom underwent a curative schedule for T1-2, N0-1 tumors. 10 patients were treated individually. The aesthetic results are very pleasing. There were no severe complications and no early local recurrences. The interpretation of the results can be only in the form of trends because of the short follow-up time of 1 year.

At first the causes of the renaissance of interstitial radiotherapy are cited. The physical and radiobiological aspects about modern radionuclides are presented and compared with conventional methods. The advantages of these modern treatment schedules are demonstrated, especially in terms of radiation protection. Indications and methods of modern interstitial radiotherapy with I-125 seeds and Ir-192 wires are pointed out. Finally published results are mentioned.
Radical or total prostatectomy and interstitial irradiation with concomitant pelvic lymphadenectomy are discussed. Indications, relative merits, and pitfalls of each technique are examined, and the treatments are compared for efficacy.
The presented paper gives a historic review of the change from the contact therapy to the percutaneous irradiation. The dominating role of radiotherapy compared to surgery is pointed out. With the help of therapeutic planning in Giessen an aimed pendulum irradiation, a high-volume percutaneous irradiation, a modern high-intensity afterloading system (Buchler), and an interstitial therapy are reported. Recently published important cure rates are shown in a table. The 5-year survival rate of 78 patients in Giessen from 1957-1975 was 42.3%. The discussion concentrates on the tolerance dose of the vagina according to the different dose rates and fractionations.
