[Immediate reaction in native tests with red chironomid larvae].

Growing numbers of pet animals in households go along with increasing sensitisation hazards by various, partially highly aggressive allergens. Even running an aquarium can be risky for atopic humans, if the fishes are fed chironomid larvae. This is illustrated by a case of inhalation allergy in a patient who showed severe reactions to skin testing with native chironomidae.

[ImmunoComb--reliability and reproducibility].

ImmunoComb is a solid phase in vitro immunological test for the measurement of serum or plasma total IgE. The technique was compared with PRIST for measurement of total IgE. The study also included reproducibility, on a test serum dilution both on a complete plate and plate sections. Statistically the conclusion is that ImmunoComb is a colourimetric test that gives the exact total IgE titre with good reproducibility. Use of a photometer greatly improves the sensitivity of assessing the results.

Variation in the measurement of serum IgE levels.

Three assays for immunoglobulin E (IgE) are compared using seven serum samples covering a wide range of values. The assays examined are the double antibody radioimmunoassay, single antibody/ammonium sulfate precipitation method and the paper radioimmunosorbent test (PRIST). In addition, values from the double antibody and single antibody/ammonium sulfate assays are compared using two different antisera to IgE. PRIST results in consistently lower values but the three assays produce results that would not have been clinically significantly different. The range of values obtained over a six-month period with aliquots of the same two samples run in duplicate twenty-three times in the double antibody assay was 1080 to 1440 I.U./ml for the Hi sample and 5.4 to 9.0 I.U./ml for the LO. Results are compared to those of a national study in which serum samples were supplied blind to research laboratories for analysis by two methods. Coefficients of variation from 35% to 73% are unacceptably large and indicate the need to establish international standards for serum IgE quantitation.

Dietary allergy evaluated by PRIST and RAST in inflammatory bowel disease.

The role of allergic responses to dietary antigens in the pathogenesis of inflammatory Bowel Disease (IBD) remains speculative. We studied 50 patients with Ulcerative Colitis (UC), 50 patients with Crohn's Disease (CD) and 100 healthy controls (HC) matched for sex and age. In these patients total serum IgE and specific IgE to ten selected foods were estimated using Phadebas PRIST and RAST. There was no significant difference in the total serum IgE level between UC, CD and HC. The percentage of positive reaction to specific IgE was significantly lower in HC (score 1-2: 7%; score 2: 3%) compared with UC (score 1-2: 24%, p less than 0.004; score 2: 8%, n.s.) and CD (score 1-2: 16%, n.s.; score 2: 12%, p less than 0.03). In CD with colic or ileocolic involvement, the percentage of patients with a positive response to RAST was significantly greater (score 1-2: 26%; score 2: 21%) than in CD with ileal involvement. The considerable increase in positive results to RAST in IBD may be due to a greater absorption of antigens through the diseased wall.

[Behavior of eosinophilic granulocytes, total IgE and allergen-specific IgE antibodies in atopic neurodermatitis during hospital treatment in the North Sea climate].

Absolute number of eosinophilic leucocytes, total IgE level, and specific IgE antibodies against the environmental antigens cock's foot, house-dust, and house-dust mite (dermatophagoides pteronyssinus) were obtained from 25 patients suffering from atopic dermatitis at the beginning as well as towards the end of hospital treatment in the maritime climate of the North Sea. Initially the absolute eosinophilic leucocytes were increased. According to the improvement of the atopic skin lesions, there was a significant decrease up to normal rates in several cases at the end of hospital treatment. The increased number of eosinophilic leucocytes observed at the beginning of therapy was correlated to the total IgE level. On an average, pathologic initial rates of total IgE going along with acute lesions decreased during hospital treatment; but this observation may be statistically accidental, the cutaneous lesions, however, completely disappeared. Elevated initial rate of antigen-specific IgE antibodies, as well, showed obvious reduction, but they did not reach normal values in any case. According to update knowledge, the IgE level in serum seems to be predominantly controlled by a multifactorial genetic system.

[Glutarladehyde-pollen-tyrosine hyposensitization. Course control of therapy using RIST and RAST].

The efficiency of the glutaraldehyd-pollen-tyrosin vaccine was controled by determination of total IgE and IgE antibodies against grass-pollens every two weeks. In addition the clinical state was checked. Patients who suffered from an allergy to grass-pollens only were free of symptoms after therapy. To proof this, a test is necessary since the case history is unreliable in 10% of persons.

Relationship between housing environment and allergic symptoms of children using ATS-DLD questionnaires.

Using pupils in three areas with different environmental factors (Suginami-ku, Tokyo: Aikawa-machi, Kanagawa prefecture: and Shimizu-shi, Shizuoka prefecture) as subjects, respiratory symptoms and domestic environment were surveyed by meams of the American Thoratic Society-Division of Lung Disease (ATS-DLD) questionnaires together with measurement of total immunoglobulin E (IgE) by the radioimmunosorbent test (RIST) and specific mite and cedar IgE by the radioallergosorbent test (RAST), as well as the relationship between "allergic symptoms" as designated in the ATS-DLD questionnaire and housing structure. The following knowledge was obtained. According to the ATS-DLD standardized questionnaires for prevalence of respiratory symptoms and allergic symptoms, Aikawa showed a significantly low value of 8.5% (p less than 0.001) compared with 15.8% for Suginami and 15.7% for Shimizu, referring to allergic rhinitis only. The positive rate of 300 U/ml and over of total IgE was 55.5% in Suginami, 49.1% in Shimizu and 40.4% in Aikawa. Aikawa showed the lowest the positive rate of total IgE in allergic symptoms. The housing was classified into three types according to the original structure each of which had characteristic ventilation rates. The positive rates according to the housing structure showed no differences by areas for Type A (wooden house) with good ventilation, but for Types B (mortar frame house) and C (steel reinforced frame house) with airtight structures, the rates increased in the order of Aikawa less than Shimizu less than Suginami, and a significant difference was observed between Aikawa and Suginami (p less than 0.01). However, in the same area no difference was noted between Type A and Types B and C. No difference in the positive rate of allergic experiences by housing structure, was noted among areas and housing structures in the same area for the group not experiencing allergic symptoms (A. Inexp. Group), while no difference was noted for Type A but the rates were high in the order of 60.0% in Aikawa, 65.2% in Shimizu and 75.3% in Suginami for Types B and C in the group experiencing allergic symptoms (A. Exp. Group). Furthermore, in the same area when the Type A was compared with Types B and C, the latter showed a higher positive rate than the formar in all three areas, with a significant difference (p less than 0.05) noted in Suginami.

How do you recognize allergy?

A suggested basic approach to the diagnosis of allergic diseases, which involves the characteristics of allergic diseases themselves, is presented. Using these characteristics as basic guidelines in investigation, not only should the condition be diagnosed but main cause or causes and triggering factors should be demonstrated in individual patients. Investigational procedures ranging from history, and including simple skin tests and more sophisticated immunological investigations, are briefly analyzed and it is stressed that if a really adequate allergic investigation of all patients with suspect allergic disease is carried out in an effort to demonstrate and subsequently control all causes and triggering factors, then development of chronic disease, sometimes intractible to our standard methods of therapy, could well be prevented in many cases.

[Objective immunological and clinical data observed during an altitude cure at Briançon in asthmatic children allergic to house dust and dermatophagoides (author's transl)].

In 42 asthmatic children with positive intradermal skin tests to Dermatophagoides pteronyssinus and domestic dust, assays of total and specific IgE were done at regular intervals during a prolonged (9 months) stay at high altitude (Briançon 1300 m). Geometric mean of total IgE (1047 UI/ml at the beginning) dropped to 40% (p less than 0,001) after the stay. In the same way, specific IgE to dermatophagoides, to domestic dust and to grass pollen dropped during the stay (p less than 0,001). On the other hand, other immunoglobulins (IgG, IgA and IgM) did not undergo any significant variations. Clinical improvement of asthma in these allergic children was observed at the first week of stay and could be maintained without drugs during all the 9 months in altitude. Rather than a consequence of an aspecific effect of altitude on the biosynthesis of reaginic antibodies, it would seem that the very consistent fall in total and specific IgE and the clinical improvement should be interpreted as the result of removal from antigenic provocation into a hypoallergenic atmosphere.

[Role of IgE in the study of the allergic risk of the young child (author's transl)].

Cord blood IgE were determined by using the PRIST and the RAST techniques in 123 cases. The RAST assay was never positive. Total IgE were detectable in 34% of the samples; statistically increased levels were found in babies born from allergic mothers and/or when the mothers were treated with progesterone during their gestation.
