Measurement of right ventricular volumes using 131I-MAA.

A method has been presented for determining the right ventricular residual ratio, that is, the ratio of the end-systolic volume to the end-diastolic volume during each cardiac cycle. 131I-MAA was injected as a bolus into the right ventricle, and the ratio of isotope remaining in the chamber during the succeeding cardiac cycles was determined with a collimated scintillation counter placed over the right ventricle. Since the counter detected the radioactivity from the entire right ventricular cavity, potential errors from incomplete mixing were minimized. The washout curve from the ventricle was distorted somewhat by the accumulation of isotope in intervening lung tissue. This distortion was eliminated by subtracting the build-up curve of radioactivity in the lung recorded simultaneously with a second scintillation counter positioned over the lateral chest wall. In 14 dogs anesthetized with chloralose, the right ventricular residual ratio was relatively constant at 40.4 plus or minus 3.1 per cent. Duplicate measurements differed by less than 3 per cent indicating the good reproducibility of the method. Right ventricular stroke volume was determined from cardiac output (dye dilution) and heart rate. With this and the simultaneously determined residual ratio (131I-MAA), end-diastolic volume could be calculated. Stroke volume and stroke work were highly correlated with end-diastolic volume, in keeping with the Frank-Starling mechanism.

Measurement of right ventricular ejection fraction and volume by the thermodilution technique in the unsedated calf.

This study was designed to test the applicability and the reproducibility of the thermodilution method in the measurement of right ventricular ejection fraction (EF), end-diastolic volume (EDV) and end-systolic volume (ESV) in unsedated cattle. In the 16 healthy calves studied, it was possible, using a fast response thermodilution catheter, to obtain characteristic exponential steplike washout curves, allowing the calculation of right ventricular EF, EDV and ESV. Five to 10 successive thermodilution curves were recorded within a few minutes and two to five similar sets of measurements were performed throughout the same day to test the reproducibility of the technique. The same protocol was repeated the following day. Right ventricular EF, EDV and ESV were reproducible successively, throughout a given day and on following days. The mean intraset, intraday and interdays coefficients of variation ranged from a mean of 4.0 (SD 4.1) per cent to a mean of 18.2 (SD 7.9) per cent. Values of right ventricular volumes and EF agreed with those previously reported in various animal species. Thermodilution was, therefore, shown to be a useful method for measuring with a satisfactory reproducibility, right ventricular EF, EDV and ESV in the unsedated healthy calf.

Hemodynamic efficacy of rapid saline infusion and dobutamine versus saline infusion alone in a model of cardiac rupture.

Despite recent reports describing survival after cardiac rupture, the effectiveness of circulatory support while awaiting definitive surgical treatment is controversial. To assess the efficacy of volume expansion and pharmacologic support in cardiac tamponade due to cardiac rupture, a model of hemorrhagic cardiac tamponade was developed and treatment with rapid saline infusion and dobutamine was compared with rapid saline infusion alone in 15 closed chest dogs. A right ventricular wound of reproducible size was produced by deflating an aortic valvuloplasty balloon that had previously been passed by way of the internal jugular vein into the pericardial space and through a stab wound in the right ventricular free wall. Hemodynamic values were compared at baseline, during tamponade and after a rapid infusion (1 liter at 100 ml/min) of either saline solution alone or saline solution plus dobutamine (20 micrograms/kg per min). Atrial and pericardial pressures increased significantly in both groups. Mean arterial pressure, cardiac output and stroke volume increased with combined saline and dobutamine infusion to values similar to those at baseline (91 +/- 19%, 114 +/- 43% and 94 +/- 37% of baseline, respectively). In contrast, saline infusion alone caused a small increase in cardiac output but failed to significantly increase mean arterial pressure or stroke volume (76.8 +/- 14.2%, 55 +/- 18% and 51 +/- 17% of baseline, respectively). Combined rapid infusion of saline solution and dobutamine infusion has a more beneficial hemodynamic effect and may be more effective than rapid saline infusion alone in resuscitating patients with hemorrhagic cardiac tamponade due to cardiac rupture.

Endothelin infusion reduces hypoxic pulmonary hypertension in pigs in vivo.

Previous work has shown that the plasma levels of the potent vasoactive peptide endothelin (ET) are increased in pathophysiological conditions with increased pulmonary vascular resistance and it has been speculated that ET may play some part in hypoxic pulmonary hypertension. We have therefore evaluated the effects of ET-infusion in the porcine pulmonary circulation after hypoxia-induced hypertension. Pits under general anaesthesia were artificially ventilated through an endotracheal tube and hypoxia was induced by decreasing the fraction inhaled O2 from 0.21 to 0.10. Haemodynamic parameters were continuously recorded using a Swan-Ganz catheter in combination with thermodilution for cardiac output measurements. ET-1 or ET-3 was given as an i.v. infusion through the Swan-Ganz catheter in the right ventricle. Hypoxia induced a reproducible increase in pulmonary vascular resistance (PVR), mean pulmonary artery pressure (MPAP) and right ventricular stroke work (RVSW) while the systemic vascular resistance (SVR) slightly decreased. Cumulative infusion of ET-1 (10, 25 and 50 ng kg-1 min-1) dose-dependently decreased MPAP and PVR; at a higher dose (100 ng kg-1 min-1), the PVR returned to the level observed at hypoxia. ET-infusions at 50 and 100 ng kg-1 min-1 evoked an increase in SVR and a decrease in cardiac output (CO) and stroke volume (SV). RVSW also gradually decreased during ET-1 infusion. Infusion of ET-3 evoked effects similar to those of ET-1 infusions, although the response to ET-3 was not that rapid in onset. In a second series of animals, repeated 15 min periods of hypoxia evoked a stable, reproducible response with a consistent increase in PVR, MPAP and RVSW which returned to baseline values during normoxia. Infusion of ET-1 (25 ng kg-1 min-1) evoked a rapidly developing decrease in PVR and MPAP which was quickly normalized upon cessation of the ET-infusion. ET-1 infusion at this concentration did not per se influence the haemodynamic parameters during normoxia. It is concluded that in the pig, short-term ET-infusion reduces the pulmonary hypertension associated with acute hypoxia.

[Echocardiography online quantification of left and right ventricular function by automatic boundary detection: reference values and reproducibility in healthy probands].

UNLABELLED: Automated border detection (ABD) is a new on-line technique that instantaneously calculates cavity areas from automatic tracking of the endocardial-blood interface with a modified ultrasonic integrated backscatter imaging system. After validation of this new method in comparison with off-line echocardiographic, Doppler- and angiographic analyses, we studied dynamic systolic and diastolic function of the left (LV) and right ventricle (RV) in 50 normal volunteers (31 +/- 9 years) in order to establish ranges of normality for the ABD-parameter. The averaged areas of the LV (apical chamber view) were 25.7 +/- 4.9 sq cm in end-diastole and 14.7 +/- 3.3 sq cm in end-systole, resulting in a fractional area change (FAC) of 43.2 +/- 4.8%. The peak filling (PFR) and peak ejection rate (PER) were 69.3 +/- 11.2 and -61.5 +/- 11.1 sq cm/s. Normalization for end-diastolic area (EDA) yielded 2.7 +/- 0.28 and -2.4 +/- 0.42 EDA/s. The areas of the RV (apical chamber view) were 17.1 +/- 3.8 sq cm in end-diastole and 9.0 +/- 2.0 sq cm in end-systole, resulting in a FAC of 47.3 +/- 9.2%. PFR and PER were 58.2 +/- 13.7 and -51.6 +/- 10.1 sq cm/s. Normalization for EDA yielded 3.4 +/- 0.74 and -2.9 +/- 0.62 EDA/s. The interobserver- and day-to-day-variability for all measured values was less than 10%. CONCLUSION: ABD permits reproducible on-line quantification of systolic and diastolic ventricular function and offers a non-invasive approach for longitudinal monitoring of cardiac patients.

Occipital neuralgia evoked by facial herpes zoster infection.

Occipital neuralgia is a pain syndrome which may usually be induced by spasms of the cervical muscles or trauma to the greater or lesser occipital nerves. We report a patient with occipital neuralgia followed by facial herpes lesion. A 74-year-old male experienced sudden-onset severe headache in the occipital area. The pain was localized to the distribution of the right side of the greater occipital nerve, and palpation of the right greater occipital nerve reproduces the pain. He was diagnosed with occipital neuralgia according to ICHD-II criteria. A few days later, the occipital pain was followed by reddening of the skin and the appearance, of varying size, of vesicles on the right side of his face (the maxillary nerve and the mandibular nerve region). This was diagnosed as herpes zoster. This case represents a combination of facial herpes lesions and pain in the C2 and C3 regions. The pain syndromes can be confusing, and the classic herpes zoster infection should be considered even when no skin lesions are established.

[Myocardial catecholamine content in heart failure--II: Measurement in endomyocardial biopsies, reference systems, normal values].

To enable the assessment of a possible gradual loss of myocardial catecholamines in heart failure, we determined control values in endomyocardial biopsies from normal human right ventricular myocardium. The reproducibility of the determinations and its dependence on the reference system, wet weight (wwt) or non-collagen-protein (NCP), was investigated in explanted hearts. Parallel determinations of norepinephrine in several samples from 1-2 mg in the same heart yielded a variability of about 20%. To obtain reproducible values, catecholamine concentrations had to be related to non-collagen-protein. Non-collagen-protein content was higher in the ventricles (138 +/- 16 micrograms/mg wwt) than in the atria (102 +/- 15 micrograms/mg wwt). Norepinephrine levels in normal human myocardium, measured in right ventricular endomyocardial biopsies were 10.3 +/- 3 pg/micrograms NCP. If they were compared with norepinephrine levels in right atrial samples from 11 patients without heart failure, obtained at open heart surgery (17.6 +/- 6 pg/micrograms NCP), an atrioventricular gradient, with ventricular norepinephrine content being 58% of right atrial levels was calculated for healthy human hearts. This gradient was almost identical with that found in heart failure, patients, where right ventricular norepinephrine amounted to 60% of right atrial levels. This implicates a percentually homogeneous loss of norepinephrine in heart failure, which, however, does not equalize ventricular and atrial levels. Thus, to interpret myocardial catecholamine content in cardiac diseases, normal values in corresponding areas are mandatory.

Intraventricular Wenckebach conduction and localized reentry in a case of right ventricular dysplasia with recurrent ventricular tachycardia.

Serial electrophysiologic studies were performed in a patient with recurrent ventricular tachycardia of left bundle branch configuration whose course and noninvasive evaluation are consistent with a diagnosis of right ventricular dysplasia. The localization of slowed conduction to the right ventricular apex, the reproducible initiation and termination of tachycardia with programmed extrastimuli and the presence of continuous electrical activity during tachycardia suggest a reentrant mechanism for this arrhythmia. Antiarrhythmic drugs further prolong the already fragmented apical electrogram and result in Wenckebach conduction within the abnormal portion of the right ventricle during pacing and tachycardia. Electrophysiologic findings are remarkably similar in studies performed 32 months apart.

Reference values and reproducibility of Doppler echocardiography in the assessment of the tricuspid valve and right ventricular diastolic function in normal subjects.

The Doppler echocardiographic indexes of the tricuspid and mitral valves were assessed in 74 normal subjects (35 women and 39 men, mean age 45 years). A reproducibility study was also performed to examine the various sources of technical and biological variability. There were significantly higher peak early and late flow velocities across the mitral valve than across the tricuspid valve (0.67 +/- 0.13 and 0.47 +/- 0.12 vs 0.51 +/- 0.08 and 0.35 +/- 0.09 m.s-1, respectively; all p less than 0.0001). There was no significant difference between the early:late (E:A) velocity ratios of the 2 valves (1.65 +/- 0.73 vs 1.75 +/- 0.67, p less than 0.01). There was a steeper mitral early deceleration slope (-3.59 +/- 1.07 vs -2.95 +/- 0.91 m.s-2) but no significant difference in pressure half-times across the 2 valves (47 +/- 7 vs 51 +/- 12 ms, p less than 0.1). No influence of gender or body surface area could be demonstrated. There was a weak but significant relation between mitral peak early, peak atrial velocity and E:A ratio and age (r = -0.39, p less than 0.001, r = 0.23, p less than 0.01, and r = -0.245, p less than 0.01, respectively). There was no significant correlation between any of the tricuspid flow parameters and age. Respiration caused pronounced variability in the tricuspid Doppler indexes and all tricuspid flows were sampled and analyzed only during inspiration. The intra- and interobserver variabilities were small for all of the Doppler indexes measured, but the day-to-day variability was quite significant especially for the pressure half-time, deceleration and acceleration slope values.(ABSTRACT TRUNCATED AT 250 WORDS)

[Assessment of right ventricular function using contrast echocardiography in patients with myocardial infarction].

OBJECTIVE: To evaluate right ventricular size, motility, and ejection fraction (RVEF) by contrast echocardiography. METHODS: We studied consecutive patients admitted to the coronary intensive care unit with acute inferior myocardial infarction, without prior infarction and with or without right ventricle involvement, according to accepted electrocardiographic findings. Polygelin, 3.5% solution, was used for contrast echocardiography. The reference standard was equilibrium radionuclide angiography. RESULTS: We studied 44 patients using contrast echocardiography, average patient age 60.8 +/- 10.6 years, 38 men and 6 women. Abnormal right ventricular size (more than 25 mm) yielded a sensitivity of 79%, specificity of 87%, positive and negative predictive values of 92 and 68%, respectively, and a likelihood ratio of 6. Abnormal right ventricular motility had a sensitivity of 70%, specificity of 94%, positive and negative predictive values of 95 and 67%, respectively, and a likelihood ratio of 11.6. Right ventricular ejection fraction < 30% with contrast echocardiography had a sensitivity of 69%, specificity of 97%, positive and negative predictive values of 90 and 88% respectively, and a likelihood ratio of 6.9. CONCLUSION: In patients with acute inferior myocardial infarction, contrast echocardiography with the area-length method is a valid and reproducible technique for evaluating right ventricular ejection fraction, which is easy to perform and can be done at the patient's bedside.

Right ventricular dysfunction in advanced heart failure.

For many years cardiologists were not interested in studying right ventricular function and the role of the right ventricle in heart failure and in other disease states has therefore been largely underestimated. More recently a consensus has grown on the critical role of right ventricular function in patients with advanced congestive heart failure. The estimation of right ventricular function is nowadays warranted in the standard evaluation of patients with heart failure either due to ischemic heart disease or to primary dilated cardiomyopathy, since it is helpful in the clinical assessment and in the prognostic stratification of such patients. Most information may be obtained non-invasively using sound but simple and reproducible echocardiographic indicators of right ventricular function.

Abdominal sonography in the evaluation of heterotaxy in children.

Heterotaxy is a rare congenital disorder characterised by abnormal viscero-vascular situs with either left or right isomerism that usually coincides with complex cardiac malformation. A precise diagnosis must be made for the selection of the most appropriate treatment. The diagnosis and evaluation of these conditions have been dependent on invasive studies such as angiocardiography, although more recently magnetic resonance imaging (MRI) has been advocated. In paediatric patients MRI is far from ideal because of the need for sedation and monitoring during the examination. The authors reviewed the modalities used in the evaluation of heterotaxy with emphasis on the use of ultrasonography, including pulse and colour flow Doppler imaging. A series of 14 children with heterotaxy is presented; ten with left abdominal isomerism (polysplenia) and four with right isomerism (asplenia). Results of abdominal sonography were compared with those of echo- and angiocardiography, surgery and autopsy. Of the ten children with left abdominal isomerism, two had no cardiac abnormalities. All those with right isomerism had a complex vitium. One patient with left abdominal isomerism proved to have right atrial isomerism. Some new reproducible patterns in viscero-vascular derangement are reported. The predictable patterns of left and right isomerism are outlined and atypical features discussed.

Ethical issues relating to reproduction control and women's health.

There are many ethical aspects which derive from the application of reproduction control in women's health. Women's health can be enhanced if women are given the opportunity to make their own reproduction choices about sex, contraception, abortion and application of reproductive technologies. The main issues that raise ethical dilemmas following the development of assisted reproduction techniques are: the right to procreate or reproduce; the process of in vitro fertilization itself-is it morally acceptable to interfere in the reproduction process?; the moral status of the embryo; the involvement of a third party in the reproductive process by genetic material donation; the practice of surrogacy, cryopreservation of pre-embryos; genetic manipulation; experiments on pre-embryos, etc. Induced abortion raises ethical issues related to the rights of the woman versus the rights of the fetus. For those who consider life to begin at conception abortion always equals murder and is therefore forbidden. Those who believe in the absolute autonomy of the woman over her body take the other extreme approach. The discussion surrounding abortion usually centers on whether it should be legal or illegal. Access to safe abortion is critical to the health of women and to their autonomy. The development of new effective contraceptive methods has a profound impact on women's lives. By the use of contraception it is possible to lessen maternal, infant and child mortality and to reduce the prevalence of sexually transmitted diseases. Research and development of new effective reversible contraceptives for women and men is needed. Dissemination of information about the safety and effectiveness of contraceptive methods is of great importance. Female genital mutilation is still practiced worldwide due to customs and tradition among various ethnic groups. The procedure is considered to be medically detrimental to the physical and mental health of women and girls, and is considered by many as oppression of women. The practice has to be stopped. Recognition of the fetus as a 'patient' has a potential effect on women's right for autonomy; they have no legal obligation to undergo invasive procedures and to risk their health for the sake of their fetuses. The woman carries ethical obligations toward her fetus. This obligation should not be enforced by the law. At present women bear most of the burden of reproductive health. All of them have a right of access to fertility regulation. Governments and society must ensure the women's equal rights to health care just as men have in the regulation of their fertility.

Doppler echocardiographic index for assessment of global right ventricular function.

Echocardiographic assessment of right ventricular function remains difficult and challenging. However, there is considerable clinical need for a simple, reproducible, and reliable parameter of right ventricular function in patients with right-sided heart disease. The purpose of this study was to assess the clinical value of a Doppler-derived index, combining systolic and diastolic intervals of the right cycle, in assessing global right ventricular function in patients with primary pulmonary hypertension. The study population comprised 26 consecutive patients with primary pulmonary hypertension and 37 age-matched normal subjects. The sum of right ventricular isovolumetric contraction time and isovolumetric relaxation time was obtained by subtracting right ventricular ejection time from the interval between cessation and onset of the tricuspid inflow velocities with pulsed-wave Doppler echocardiography. An index of combined right ventricular systolic and diastolic function was obtained by dividing the sum of both isovolumetric intervals by ejection time. The index was compared with available parameters of systolic or diastolic function, clinical symptoms, and survival. Right ventricular isovolumetric contraction time and isovolumetric relaxation time were prolonged significantly in patients with primary pulmonary hypertension (85 +/- 41 msec and 135 +/- 43 msec) compared with normal subjects (38 +/- 7 msec and 49 +/- 9 msec, respectively; p < 0.001). Ejection time was shortened significantly in patients with pulmonary hypertension (241 +/- 43 msec versus normal [322 +/- 21 msec]; p < 0.001). However, the index was the single most powerful variable to discriminate patients with primary pulmonary hypertension from normal subjects (0.93 +/- 0.34 versus 0.28 +/- 0.04; p < 0.001) and was the strongest predictor of clinical status and survival. The index was not significantly affected by heart rate, right ventricular pressure, right ventricular dilation, or tricuspid regurgitation. It is well known that right ventricular systolic and diastolic dysfunction coexist in patients with primary pulmonary hypertension. This article reports the use of an easily obtainable Doppler-derived index that combines elements of systolic and diastolic function. This index appears to be a useful noninvasive means that correlates with symptoms and survival in patients with primary pulmonary hypertension.

Total extracardiac right heart bypass using a polytetrafluoroethylene graft.

BACKGROUND: With regard to hemodynamics and late arrhythmias, total cavopulmonary connection has been accepted as a superior technique as compared to Fontan type procedures. However, intra-atrial baffles for lateral tunnel or conduit remain construction retain some similar disadvantages. PATIENTS AND METHODS: As an alternative to total cavopulmonary connection, total extracardiac right heart bypass using a polytetrafluoroethylene tube for the inferior vena cava to pulmonary artery connection may obviate some problems. Five patients with complex heart disease necessitating one ventricle repair underwent this procedure successfully. RESULTS: Aortic cross-clamp time ranged from 0 to 24 minutes (mean = 15.8 min). No case required takedown or an additional step. Although the follow-up periods have been relatively short (mean = 19 months), all patients are well and no arrhythmic event or thromboembolic episode has occurred. CONCLUSIONS: As a simple, safe, and reproducible procedure, total extracardiac right heart bypass is an alternative to Fontan or total cavopulmonary connection procedure.

[Right ventricular function assessed by scintigraphy using continuous infusion of krypton-81 m].

Scintigraphy using continuous infusion of krypton-81m, (81mKr) is a reproducible, noninvasive method for determining right ventricular ejection fraction (RVEF). Radionuclide determinations of RVEF by 81mKr scintigraphy were compared with those by the technetium-99m (99mTc) first-pass technique in 25 patients; the results obtained by both methods correlated well (r = 0.86, p less than 0.005). In 20 additional patients, the RVEF determined by 81mKr was compared with the RVEF as measured by multislice ECG-gated cardiac X-ray computed tomography, and there was good correlation between RVEFs obtained by the two methods (r = 0.76, p less than 0.005). Furthermore, the RVEF by the 81mKr technique and the LVEF by the 99mTc gated blood pool technique were compared with hemodynamic measurements. There was a significant inverse correlation between RVEF and pulmonary artery pressure or total pulmonary resistance. Other determinants of right ventricular systolic function were the presence of proximal right coronary artery disease, reduced left ventricular function and associated tricuspid regurgitation.

Thermodilution measurement of the right ventricular ejection fraction.

This study addressed the clinical reproducibility and validity of the thermodilution (TD) measurement of the right ventricular ejection fraction (RVEF). Forty-one patients underwent right heart catheterization, including TD RVEF, within 2 h of gated first pass (GFP) and gated blood pool (GBP) radionuclide ventriculography; 21 had single plane contrast right ventricular angiography (ANGIO) during the same catheterization. Analysis of variance showed no difference among three successive TD RVEF measurements. (table; see text) Stroke volume by RV ANGIO correlated with Fick (n = 10, r = .86) and thermodilution stroke volume (n = 21, r = .88). It is concluded that although the thermodilution method is an accurate way to measure flow, it is not an accurate way to measure right ventricular ejection fraction, and by inference, ventricular volumes. The most likely explanation for this finding is incomplete mixing as in previous studies of indicator methods of measuring left ventricular volumes.

Right-heart echocontrast in the anesthetized dog after i.v. administration of a new standardized sonographic contrast agent. 3rd communication: comparison of various contrast agents employed in contrast echocardiography.

Right-ventricular contrast provided by the new standardized sonographic contrast agent SH U 454, which consists of pure galactose microparticles, with 300 mg microparticles/ml and by 5 other currently employed ultrasonic contrast media was examined by 2 D echocardiography in 10 anesthetized female beagle dogs (6.8-12.7 kg) following i.v. administration. With a 5-min interval between injections each animal was given 3 injections of 2 ml of each formulation in random order using an injection speed of approximately 2 ml/s. The echocardiographic investigation was recorded on videotape and the contrast assessed blind using a visual score system (rating 0-5) by two investigators working independently and also by a videodensitometer. Only SH U 454 caused an intense, homogeneous and reproducible opacification of the right heart chambers. By comparison with other formulations for right-heart echocontrast, SH U 454 was significantly superior (p less than 0.05) in every parameter assessed.
