Total quality management in a health care organization: how are employees affected?

Since the late 1980s, health care managers have exhibited a rapidly growing interest in large-scale total quality management (TQM) programs. This quasi-experimental study examines the effects of one such TQM program on employee job satisfaction, perceptions of organizational climate, and general opinions concerning the work situation. Two years after the TQM program had been introduced, responses of participants and nonparticipants were compared. Participants in the program exhibited a higher level of job satisfaction and more favorable opinions regarding both the organization and their work.

Perceptions of VA chiefs of staff on total quality management implementation.

A national survey of United States Veterans Affairs (VA) Medical Centers (n = 167) chiefs of staff was conducted. The survey solicited feedback of the attitudes and perceptions of respondents toward planning, implementation, and monitoring of the Total Quality Management (TQM) process in their facilities. A Likert scale type of responses were collected and analyzed to measure the degree of agreement or disagreement of the respondents to survey statements on TQM. The survey included responses to three sections of the questionnaire: 25 statements on TQM, information on the TQM process status in the facility, and demographics of respondents and their facilities. The results of the survey indicate a considerable involvement of responding chiefs of staff in the TQM process and indicated their willingness to support it. Respondents agreed that TQM requires time and commitment of top management, and, again, they indicated their willingness to devote the time necessary for the process to succeed. They were, however, cautious in endorsing the statement that TQM will decrease health care costs (fiscal and human). The survey provides information that should prove valuable to administration in planning TQM and Continuous Quality Improvement efforts.

The implementation of the composite health care system into total quality management in military medical treatment facilities.

The Composite Health Care System (CHCS) is a new medical computer system which will integrate into Total Quality Management (TQM) in Department of Defense hospitals. Both are examples of integrated systems which mandate a complete reshaping of the way the business of patient care is conducted. As a medical information management system, CHCS is an effective component of TQM and follows W. Edward Deming's TQM principles. With proper training, patience, and full support from end user and senior staff, CHCS and TQM can work together to maintain high quality standards for patient care in today's rapidly changing military medical service.

Self-assessment of all the health centres of a public health service through the European Model of total quality management.

The Basque Country Public Health Service has moved in the last years from considering quality as an attribute of patient care to thinking that all management can be subject to improvement. Consequently, its general management team has promoted and supported a self-assessment experience of all their centres by means of the European Quality Model. This strategy has been facilitated by the Basque Country Government, which has strongly encouraged total quality management in companies, and has created the Basque Foundation for Quality Promotion, a key institution in this whole process. A total of 26 centres of the Public Health Service concluded a self-assessment process. As the main result of this, different improvement areas were detected, and various necessary actions were implemented in the centres assessed. Advantages, troubles and future work lines to extend and improve the use of the EFQM model in the health sector are discussed.

Macroergonomics and total quality management: how to improve quality of working life?

In this paper, we present a macroergonomic model of work design that is applied and tested to examine Total Quality Management (TQM) in the public sector. According to the model, TQM can influence different aspects of work design and quality of working life (QWL). Questionnaire data collected in 2 public sector organizations in the USA show that TQM can have both positive and negative impact on work design and QWL. The main positive impact of TQM was found on job content, job control and participation, and social relationships. The main negative impact of TQM was on workload, uncertainty, and clarity of job duties. The impact of TQM on QWL was mixed. Our results show that the impact of TQM on work design and QWL varied very much across the 6 participating departments, as well as within the departments. Further research is warranted to assess the human impact of TQM, in particular research on the linkage between various aspects of TQM, on one hand, and work design and QWL, on the other hand.

Total quality management and the reduction of inpatient violence and costs in a forensic psychiatric hospital.

OBJECTIVE: The study evaluated the effects of using the principles of total quality management (TQM) to reduce aggressive incidents in the dining rooms at a state forensic hospital. METHODS: A ten-member, multidisciplinary team, which included a patient representative and a private-sector mentor, used the TQM FADE method (focus, analyze, develop, and execute) to address the problem. The team analyzed violent mealtime incidents, reviewed mealtime policies and procedures, and conducted a patient survey. Five recommendations were made: substitute plastic utensils for silverware, play music selected by the hospital's music therapists, allow patients at the highest privilege levels to leave the dining room after eating, open the main courtyard and gym during meals, and train food service workers in therapeutic communication. RESULTS: One year after implementation of the recommendations, aggressive incidents in the dining rooms were reduced by 40 percent, assaults using silverware were eliminated, and a total of 70 nursing staff hours a day were saved by eliminating silverware control procedures in the dining rooms. The milieu in the dining rooms has been improved by the addition of music and more flexible procedures. CONCLUSIONS: TQM techniques can be effectively applied in public-sector institutions to analyze and solve problems such as mealtime violence.

Transformational leadership: moving total quality management to world-class organizations.

Transformational values and competencies will become critically important by the year 2001 if we are to achieve a health system that fosters community well-being and basic care for all, financed through a combined public-private partnership that is cost effective and uses treatments that unite body, mind and spirit. This article will focus on the use of transformational leadership as a strategy to move health-care organizations beyond traditional values and approaches by building upon the core values of total quality management (TQM). Learning organizations will emerge and finally world-class organizations will evolve, combining the characteristics of total quality and learning organizations, and more. A world-class organization can be described as being the best in its class or better than its competitors in the community, state, nation or world.

The role of total quality management in patient-centered restructuring.

The fundamental structure and organization of the American hospital has changed little in the past fifty years. The current and future environment requires hospitals to reconsider how basic patient care delivery systems are organized and managed. Organizations are discovering how the traditional structure limits a hospital's ability to achieve sustainable improvements in clinical and service quality. A number of pioneering hospitals have initiated efforts during the past several years to completely redesign their organizations to create systems that are patient-centered and customer responsive. This paper discusses the opportunities and obstacles of operational restructuring and how total quality management principles can assist in achieving an effective transition to a new system.

Total quality management in health services: historical development, conceptual meaning, and comprehensive model.

Today's health care environment is characterized by burgeoning costs and increasing concern about service quality. Not surprisingly, researchers and practitioners are turning to Total Quality Management (TQM) in their quest for creating high quality health care delivery systems. Despite the central importance of TQM, there is practically no common definition of total quality in medical care research. In this paper, we trace the historical development of the quality concept, offer a valid definition of "total quality," and detail a comprehensive model of TQM for health care research. Implications of our model for academicians and practitioners are discussed.

Total quality management in clinical virology laboratories.

The diagnostic laboratories in India are progressively promoting higher standards and are moving towards accreditation and international acceptance. Hence, the concept of "Quality" will need to be understood and implemented. Total quality management (TQM) in a laboratory is an integrated program involving all laboratory staff and management. TQM is a framework to operate and it is aiming for integration, consistency, increase in efficiency and a continuous drive for improvement. A well structured clinical virology service will include serology setup, cell culture facility and capacity for molecular diagnosis. The quality of results from the laboratory is significantly influenced by many pre-analytical and post-analytical factors which needed attention. The end goal of the TQM should be to provide the best care possible for the patient.
