[Relationship between various syndrome-types and defect of erythrocyte insulin receptors in non-insulin dependent diabetes mellitus].

Fifty-seven patients of non-insulin dependent diabetes mellitus (NIDDM) were divided into various groups by Syndrome Differentiation in TCM and 18 cases of non-diabetes mellitus were taken as control. The relationship of Syndrome-types and defect of the erythrocyte insulin receptors were studied with the percentage of specific insulin binding to erythrocyte, number and average affinity of erythrocyte insulin receptors. The results showed: (1) The patients of NIDDM had defect of erythrocyte insulin receptors specified with decreased number of insulin receptors; (2)Mild defect of erythrocyte insulin receptors was found in Syndrome of Yin Deficiency and hyperactivity of Heat, but serious defect in the Syndrome of both Qi and Yin Deficiency, in the Syndrome of both Yin and Yang Deficiency. Above data suggested that the defect of erythrocyte insulin receptors was correlated with Syndrome-types of patients with NIDDM, the more serious Deficiency Syndrome, the more serious defect of erythrocyte insulin receptors.

[Relationship between thymectomy and syndrome differentiation-typing treatment of myasthenia gravis].

According to the theory of traditional Chinese medicine, 37 patients suffering from myasthenia gravis were Syndrome-differentiated into following types preoperationally: (1) Both Qi-Yin Deficiency of Kidney-Spleen, 21 cases. (2) Yang Deficiency of Kidney-Spleen, 15 cases. (3) Qi Deficiency of Spleen only, one case. Thymectomy were done in all cases. Among them, 19 cases had thymoma. The results showed the relationship between the types and the resectionability rate and post-operative crisis rate. On the first two types, they were 95.2% and 9.5%, 46.7% and 73.3% respectively. It was considered that Syndrome differentiation-typing in TCM in the myasthenia gravis patients would provide an positive and reliable index for predicting the resectionability and prognosis of the patient.

[Primary discussion on the classification of the TCM patterns and measurable diagnosis of 760 cases of protracted abstinent syndrome by heroin-addiction].

OBJECTIVE: To discuss the TCM pattern classification and measurable diagnosis criterion of the protracted symptoms of heroin-addiction abstinence (PSHAA). METHODS: Through literature review and clinical study, the concept of TCM patterns and diagnostic standard were established, and a TCM pattern scale for quantitatively analysis of PSHAA was formulated. The scale were used on the clinical investigation on the abuser for 5 times on day 15, 30, 60, 90 and 120 after abstinence. Then the TCM patterns of PSHAA was classified using methods of DME and quantitative diagnosis, to create a corresponding scale of diagnostic indexes referring maximum likelihood method. RESULTS: (1) The TCM patterns of PSHAA commonly seen were Toxin-stasis accumulation (TSA) with Qi-blood insufficiency, TSA with Qi-yin deficiency, TSA with Yin-deficiency and Fire-excess and TSA with Yin-Yang deficiency. (2) The retrospective and prospective verification of the scale of diagnostic indexes showed it has high sensitivity and speciality, with low rates of misdiagnosis and of missed diagnosis. CONCLUSION: It is feasible to develop measurable diagnosis on the TCM patterns of PASAA using DME method and measurable diagnostic methods. The scale of diagnostic indexes formulated with the maximum likelihood method of quantified diagnosis has a certain clinical practicability.

A study of the immunofunction in patients of coronary heart disease with the fundamental deficiency and outward excess syndrome.

The fundamental deficiency and outward excess syndrome of coronary heart disease can be classified into Qi Yang deficiency with blood stasis (QYD) and Yin deficiency with blood stasis (YD). The patients showed disturbances in immunofunction manifested as marked increase of serum IgG, CIC, IC-IgG and IC-C3 levels and the percentage of B cells in the peripheral blood, while the percentages of OKT3+, OKT4+ and OKT8+ cells markedly decreased, especially the percentage of OKT8+ cells, resulting in an increase of the ratio of T4/8 and imbalance between TS and TH. The authors deem that the fundamental deficiency of coronary heart disease is related to the low cellular immunity, especially the imbalance between TS and TH cells, while outward excess of coronary heart disease is related to hyperactivity of humoral immunity.

[15 years' observation and related research on diagnosis and treatment according to traditional Chinese medicine of elevated serum lipids in 54 cases of coronary disease].

The increasing rate of cholesterol (CH) of 54 cases with coronary heart disease was 33.33%. According to CH level, the authors divided them into two groups: the normal one (I) and the high one (II). By fifteen-year observation, CH of Group I kept normal, only with its triglyceride (TG) a little higher than ordinary level. But the CH and TG of Group II, which had been treated with anticholesteremic drugs, was obviously higher than Group I (P less than 0.05). In connection with identification according to TCM, both CH and TG of deficiency of Yang, and deficiency of both Yin and Yang were higher, especially TG (P less than 0.01). TG of the type of blood stasis was higher (P less than 0.01) except anticholesteremic drugs for Group II. The two groups were treated with the same combined treatment of TCM and WM. The long term effect of fifteen years in Group I was more obvious than in Group II. This showed that the treatment of high serum lipid of CHD was more difficult.

[Relation of hypothyroidism and deficiency of kidney yang].

32 cases of hypothyroidism caused by various factors were treated for one year with Chinese medicinal herbs preparation "Shen Lu tablet" (SLT) to warm and reinforce the Kidney Yang. 34 normal persons were studied as a control group. After treatment with SLT, the clinical symptoms of hypothyroidism were markedly improved. Average serum concentration of total T3, T4 increased significantly from 67.06 +/- 4.81 and 3.29 +/- 0.42 before treatment to 120.50 +/- 6.34 ng/dl and 6.31 +/- 0.38 micrograms/dl, respectively (P < 0.001). Serum levels of TSH decreased evidently from 20.81 +/- 2.78 before therapy to 3.32 +/- 0.61 ng/ml (P < 0.001). Before treatment with SLT, hypothyroidism group had higher erythrocyte sodium content ([Na]Rbc) than that of normal group. The permeability of the cell membrane oMNaos and the activity of sodium pump oKNaos in intact erythrocytes were markedly lower in the treated group than that in the normal group. In hypothyroidism patients treated by SLT the [Na]Rbc significantly dropped, and the oNNaos and oKNaos were significantly raised when compared with those before treatment, P < 0.001. It is suggested that hypothyroidism was closely related with Deficiency of Kidney Yang and energy metabolism.

[Clinical study on "liver yang exuberance syndrome"].

In order to study the nature of Liver Yang Exuberance Syndrome (LYES), and find new effective drug prescription to treat diseases with the syndrome, the authors chose 97 cases with LYES of hypertension, climacteric syndrome and hyperthyroidism. A clinical study was carried out on different diseases with the syndrome including the stagnancy of Liver Qi, excess of Liver Wind and rising of Liver Fire with a new prescription according to TCM theory. While another 83 cases with Yin Deficiency Yang Excess Syndrome (YDYES) of the same three diseases were employed as control group. The results showed that their total effective rates were 81.4% (in lowering blood pressure), 100% and 88.9% respectively. The marked effective rate of LYES of hypertension group was significantly higher than that of the control group (P less than 0.05), which showed the special characteristics of the TCM syndrome. Before and after treatment, the patients with LYES of the three diseases were examined in certain laboratory tests including function of autonomic nervous system, urinary MHPG-SO4, CA, plasma cAMP and TXB2, 6-keto-PGF1 alpha etc. The similar changes were found in different diseases with the same syndrome. The laboratory results of the cases got markedly improved with the improvement of their symptoms, which showed that these tests had active significance for comprehending the nature of LYES.

Effects of Kanlijian on exercise tolerance, quality of life, and frequency of heart failure aggravation in patients with chronic heart failure.

OBJECTIVE: To observe the effects of conventional therapy combined with Kanlijian (KLJ) on exercise tolerance, quality of life and frequency of heart failure aggravation in patients with chronic heart failure (CHF). METHODS: Sixty CHF patients differentiated as sufferring from the syndrome of Xin-Shen Yang deficiency were included in the study and randomly assigned at the ratio of 2:1 into the KLJ group (n = 39) and the control group (n = 21). All the patients were treated with conventional therapy of Western medicine, but to those in the KLJ group, KLJ was medicated additionally one dose daily with 24 wks as one therapeutic course. The efficacy on TCM syndrome and changes of scores on TCM syndrome were observed after treatment. The indexes, including 6-minute walking distance (6MWD), quality of life (QOL, accessed by LHFQ scoring), NYHA grade, hemodynamic indexes and reducing/withdrawal rate of diuretic and digoxin before and after treatment were recorded and compared. Also the frequency of re-admission due to aggravation of heart failure in one year's time were observed. RESULTS: (1) The efficacy on TCM syndrome, improvement on scores of TCM syndrome, therapeutic effects on 6MWD, QOL, and NYHA grade in the KLJ group were superior to those in the control group. (2) Hemodynamic indexes after treatment, left ventricular fractional shortening (LVFS) and E peak/A peak (E/A), between the two groups had no significant difference, while left ventricular ejection fraction (LVEF) was increased significantly in the KLJ group, but with no obvious change in the control group. (3) The reducing/withdrawal rate of diuretic and digoxin in the KLJ group was significantly higher than that in the control group. (4) The 1-year frequency of re-admission significantly decreased in the KLJ group. CONCLUSION: The adjuvant treatment of KLJ on the basis of Western conventional therapy can significantly improve CHF patients' exercise tolerance, quality of life and cardiac function, reduce the dosage of diuretic and digoxin needed, and decrease the re-admission frequency due to aggravation of heart failure.

A survey comparing TCM diagnosis, health status and medical diagnosis in women undergoing assisted reproduction.

INTRODUCTION: For many women, undergoing assisted reproductive technology can be a difficult experience, and can result in changes in physical and emotional health and wellbeing. Recent research has suggested that acupuncture may be helpful for women undergoing assisted reproductive technology. To date, there is no information describing the traditional Chinese medicine (TCM) syndromes seen in women undergoing assisted reproductive technology, or relationships between TCM and Western medicine diagnoses. OBJECTIVE: To examine the health status of women undergoing assisted reproductive technology from both Western and Chinese medicine perspectives. METHODS: One hundred and eighty women were included in the study. All underwent a TCM diagnosis, 177 (98.3%) completed the quality of life questionnaire SF36. Information about Western medicine diagnosis was collected from case notes and was available for 176 (97.7%) women. RESULTS: Women in the trial reported poorer health on several domains of the SF36 compared with the South Australian population. The most common TCM diagnosis was 'Kidney Yang deficiency', diagnosed for 53.9% of patients. A TCM diagnosis of Qi or 'Blood stagnation' was associated with poorer quality of life on the mental health, emotional role function and social function domains of the SF36. No associations were found between TCM diagnosis and physical or general health components of the SF36. CONCLUSIONS: Associations between TCM and reproductive health diagnoses were demonstrated. Emotional health and wellbeing is an important aspect of patient care that needs to be addressed in clinical practice and research studies, as the findings suggest that this aspect of their health is often poorer during assisted reproductive technology.

[Clinical study of shengxue mixture in treating aplastic anemia].

OBJECTIVE: To observe the clinical effect of Shengxue mixture (SXM) in treating aplastic anemia, and studied its mechanism. METHODS: Eighty-four patients of the treated group, those with Spleen-Kidney Yang deficiency Syndrome and Spleen-Kidney Yin deficiency Syndrome were treated with SXM-A and SXM-B respectively, and 30 patients of the control group were treated with Stanozolol. The clinical effect and several experimental parameters were also observed. RESULTS: The basic cure rate, remission rate, total effective rate of the treated and control group were 21.43%, 27.38%, 86.90% and 3.33%, 20.00%, 56.67% respectively, the difference between 2 groups was significant (P < 0.05, P < 0.01). The effect of the treated group was obviously better than that of the control group. While the patients' symptoms were alleviated, the peripheral blood cells increased, the ratio of T lymphocyte subsets tended to balance, the level of natural killer cells activity increased, interleukin 2 reduced, and the reproduction of the bone marrow were markedly improved in most of the patients treated by SXM. CONCLUSION: SXM is an effective and safe drug for aplastic anemia. Its mechanism might be regulating the immune function, which facilitated the recovery of the bone marrow hematopoiesis function.

[Clinical study of yishen jianpi yangxue tongli therapy in treating polycystic ovary syndrome].

OBJECTIVE: To observe the effect of Yishen Jianpi Yangxue Tongli Therapy (YJYTT) in treating polycystic ovary syndrome (PCOS) and to explore the therapeutic mechanism of YJYTT. METHODS: One hundred and seven PCOS patients with syndrome differentiation of TCM as Pi-Shen Yang deficiency were randomly divided into two groups, the treated group (n = 76) was treated with Chinese herbal medicine, while the control group (n = 31) was treated by clomiphen. The therapeutic effect in both groups and the change of body mass index (BMI), Ferriman-Gallway (F-G) score, serum sex hormones and serum oral glucose tolerance test (OGTT) before and after treatment were observed. RESULTS: The comparison of total effective rate between two groups was insignificantly different, but the pregnancy rate of the treated group (65.7%) was markedly higher than that of control group (25.0%, P < 0.01). Also, significant difference was shown in the aspect of BMI, F-G score, serum testosterone (T), luteinizing hormone (LH) and OGTT, etc. between the two groups (P < 0.05). CONCLUSION: The YJYTT could markedly improve the clinical symptoms and pregnancy rate of PCOS patients, particularly showing good effect to the clomiphen resistant patients. The mechanism possibly was due to its regulation on hypothalamus-pituitary-ovarium axis and reducing the insulin resistance.

[A pair of crude drugs used in Shang-Han-Lun, especially the ways of using roasted licorice].

The frequency and usage of "Yao-dui" (pair of two kinds of crude drugs, Yaku-tai used in Shang-Han-Lun, a famous formulary in traditional Chinese medicine, was examined. The best ten kinds of pairs of two crude drugs frequently mentioned in the formulary were those among five drugs contained in Guizhi-tang (Keishi-to in Japanese), which is a major formulation in Shang-Han-Lun. Radix Glycyrrhizae Preparata (Sha-kanzo in Japanese) and Ramulus Cinnamomi (Keishi in Japanese) were used in pairs very frequently, which is used for the treatment of palpitation caused by Xin-yang deficiency (Shin-yo-kyo in Japanese) and usually used with Poria. Radix Glycyrrhizae Preparata and Radix Paeoniae worked in pairs for the treatment of muscular spasm and pain due to Xue-deficiency-syndrome (Kekkyo in Japanese). Radix Glycyrrhizae Preparata is used together with Rhizoma Zingiberis (Kankyou in Japanese) for the treatment of diarrhea accompanied with cold pain in the lower abdomen and usually used with Radix Ginseng and Aconiti Preparata. The experimental knowledge of the usage of these crude drug pairs will be useful to create a new formulation of crude drugs for modern medical treatment.

[Analysis of 4 metal elements in 11 kidney tonic Chinese traditional medicines].

The contents of Zn, Cd, K and Na in 11 kidney tonic Chinese traditional medicines usually used in clinical practice were determined by FAAS. The results showed that element Zn, which is closely related to yang deficiency of the kidney, and the macro elements K and Na, which are essential to human-body, are very rich, but element Cd is lower. The relations between trace elements and the therapeutic efficacy were discussed, and it is concluded that the results are in agreement with clinical usage of medicines by herbalists.

[Cell-mediated immunity in chronic pyelonephritis].

The authors used monoclonal antibody (CD3, CD4, CD8) and indirect immunofluorescence technic to study peripheral blood lymphocyte surface markers in patients with chronic pyelonephritis (CPN) and in normal controls. In CPN patients, a significant decrease in CD3, a lower percentage of CD4 and significant higher percentage of CD8 leading to a decrease in the CD4/CD8 ratio were noted. No differences could be observed between Spleen-Kidney Yang deficiency and Kidney Yin deficiency. The overall picture shown by the CPN patients was compatible to a cell-mediated immune response defection. The CPN patients were then treated with Zi-Ling capsule (ZLC)/transfer factor (TF) and various antibiotics for three months, during which period the authors found a significant increase in CD3, CD4, but no significant change in CD8. Symptoms such as lumbago, fatigue, frequency of urination were improved. A significant decreased in CD4 after cessation of treatment with ZLC and TF was observed. These observations suggested that both ZLC and TF could also improve the immune function, however, they mainly affect CD4 but not CD8 cells, and the effect lasted for only a short period. In conclusion, ZLC/TF and antibiotics administered together would serve as a useful therapeutic measure to be recommended for patients with CPN.

[Experimental study of warming and recuperating kidney yang by you-gui-yin].

Yang deficiency animal models were induced by hydrocortisone 1 mg/day for 7-day to male mice. Then these model mice of experimental group were perfused by gastrogavage with 0.5 ml You-gui-yin (YGY) solution, the control group 0.5 ml normal saline 10-day. The plasma circular nucleotide and cortisol were radioimmunoassay by r-immune counter. The content of cAMP of experimental group was 144.24 +/- 33.35 pmol/ml, the control group 109.11 +/- 31.98 pmol/ml (P less than 0.05). The contents of cGMP were 28.39 +/- 10.22 and 45.39 +/- 15.33 pmol/ml respectively (P less than 0.05). The contents of cortisol were 12.42 +/- 2.21 and 8.96 +/- 1.19 micrograms/dl respectively (P less than 0.05). On the other hand YGY to the model mice could raised the living ability at low temperature. The results suggested that YGY had the effect of adjusting system circular nucleotide and promoting the secretion of adrenal cortex.
