[Incidence rate of late complications in legally induced abortion in the Federal Republic of Germany (author's transl)].

The article goes into the question as to whether and to what extent legally induced abortion can exercise a detrimental influence on later pregnancies in the form of late complication; this problem was investigated by means of the stratified sample technique. A few statistically significant observations were made leading to the conclusion that legally induced abortion does involve late complications in subsequent pregnancies. The study was performed in 650 women demonstrating certain disturbances during pregnancy or birth, and an equal number of women in whom parturition occurred without disturbances after uncomplicated pregnancy; the study period extended from March 1980 to July 1980 and was conducted in various large obstetrico-gynaecological departments in Federal German hospitals. It was found that legally induced abortion was a statistically more frequent precedent to abdominal pregnancy, retarded abortion, cervical insufficiency or imminent premature births, than among the women of the control group (p less than 0.001). The method employed to induce abortion (suction abrasio, etc.) less important for the occurrence of complications than the length of gestation at the time abortion was induced. In induced abortions beyond the 8th week, late complications are significantly more frequent than in women in whom abortion had been induced before the end of the 8th week (p less than 0.01).

[Poliomyelitis in the German Federal Republic 1974-1975 (author's transl)].

The virological surveillance of poliomyelitis in 1974-1975 led to the detection of specific characteristics of wild viruses in 57 out of 221 cases of poliovirus isolation. The disease symptoms were typical for poliomyelitis in 36 of these cases, less characteristic in 13. Polio wildvirus was isolated 8 times from the surroundings of the patients. In 36 foreign patients and one German the causative agent was imported from an endemic area. In these cases and in a further 7 German patients the disease was sporadic. In contrast 5 cases in German children in late autumn 1975 constitute a local epidemic of poliomyelitis.

[Neonatal screening for hypothyroidism in the Federal Republic of Germany (author's transl)].

Screening for neonatal hypothyroidism was introduced in 1979 in nearly all West German provinces on a regional basis as an additional preventive investigation. In February 1980 provincial control was replaced by uniform financing of screening for the whole country by public sick funds. Eleven out of 14 regionally organised screening laboratories took part in an enquiry into the incidence, clinical symptoms and causes of inborn hypothyroidism. Among 427 430 newborn infants tested 137 children with inborn hypothyroidism were detected in 1979/80. The incidence of 1:3100 corresponds to European figures. On average treatment could be instigated by the 12th day of life. Clinical symptoms of hypothyroid newborns differed in part markedly from classical signs of hypothyroidism. About 30% of the mothers had goitre. The cost-benefit relationship of newborn hypothyroidism screening is 1:10.

[Notifiable and non-notifiable infectious diseases in the federal republic of germany (author's transl)].

Owing to the growing density of population, urbanization, and the use of all kinds of publicly accessible facilities, the interruption of the infection chain for air-borne diseases proves particularly inadequate if no efficient prophylactic inoculations are available to combat viral infections or if no efficient chemotherapeutical measures can be adopted against bacterial infections. Equally problematic is the struggle against salmonellosis associated with the mechanized mass production and distribution of foodstuffs as well as the forms of infectious hospitalism, especially hepatitis B, which are favoured by the technical advances in the medical field. Venereal diseases prove highly resistant because the human patterns of sexual behaviour are difficult to influence. Thus the infectious diseases caused by deficient hygiene have virtually disappeared in our country, while those diseases survive or occur which are capable of profiting from the sociological changes and the technical progress of civilisation. On the whole, the mortality rate has drastically dropped while morbidity has remained more or less the same (with, however, a change in the spectrum of the infectious diseases); this is due mainly to mass inoculation and specific therapy.

[Statistical survey on laparoscopy in gynaecology up to 1977 in the Federal Republic of Germany (author's transl)].

1385 questionaires were sent to gynaecologists in West Germany. 682 (49.2%) were answered anonymously. 302 answers (44.2%) revealed that the gynaecologists did not do endoscopy. 265.900 pelviscopies or laparoscopies were included in the statistics. The laparoscopies or pelviscopies were done from 1949--1977 by 380 gynaecologic surgeons or gynaecological departments. In Germany to the end of 1977 unipolar high frequency current was used for hemostasis in 52%, bipolar high frequency current in 42%, and modern endocoaglation in 25% of the departments. In 240 departments (63%) the five to 8 mm. pelviscope was used. In 124 department (33%) the 11 mm. laparoscope was used. In the total of the cases 949 (3.56/1000) complications occurred. These required 690 laparotomies (2.59/1000). The cause of the complications were injuries to vessels or high frequency current burns. A total of 24 (0.09/1000) deaths or 1 death per 11,000 pelviscopies was reported. The operative mortality was 13 deaths (0.09/1000) in 140,977 (53%) purely diagnostic pelviscopies or laparoscopies, 3 deaths (0.08/1000) in 37,639 (14.2%) combined diagnostic and operative pelviscopies and 8 deaths (0.09/1000) in 87,284 (32.8%) laparoscopic tubal ligations. Since 1974 the operative mortality is less than 1:100,000. Since 1973 approximately 30--35,000 pelviscopies are done yearly in West Germany.

[Human larva migrans visceralis infections with toxocara in the German Federal Republic. Serological investigations (author's transl)].

The microprecipitation test on second generation larvae of Toxocara canis has proved to be highly specific for the diagnosis of human larva migrans visceralis infection caused by Toxocara species. Out of a total of 2864 samples examined from 1969 to 1976, 248 were found to be positive. Besides a very frequently occurring eosinophilia eye affections, lung changes and involvement of the CNS are prominent clinical symptoms. The investigations show that larva migrans visceralis infections also occur in West Germany, even if more rarely than in other countries. The diagnosis is only seldom made because of the lack of uniformity of the clinical symptomatology.

[Epidemiological observations on the typhoid fever outbreak 1974 in the south-west of the Federal Republic of Germany (author's transl)].

A typhoid fever epidemic broke out in Baden-Württemberg during October and December 1974, 436 cases could be analyzed statistically. The interval between the date of earliest symptoms and the registration at the health-office was 18 days. This was the double respectively the triple of previous typhoid outbreaks. This interval results from a prolonged stay at home and a hospitalisation time with an average of 5 days from the hospitalisation to the registration. The interval between hospitalisation and the definitive diagnosis was 19 hours. The analysis of the age and sex incidence shows a high percentage of 15 to 30 years old women, which is due to the occupational exposition. The low morbidity of the 50 to 65 years old men points at a pre-exposition during World War II.
