Tibial component fixation in knee arthroplasty.

Using roentgen stereophotogrammetric analysis (RSA), the integrity of the bond between the tibial component and the tibia was studied in seven knees, implanted with a cemented Kinematic total knee prosthesis for gonarthrosis. Migration ranging from 0.3 to 1.9 mm over a two-year period occurred in all seven cases. Displacement, induced by external forces, ranging from 0.2 to 1.0 mm was observed in five cases. Measurable migration and inducible displacement may be the rule rather than an exception in total knee arthroplasty (TKA). Accordingly, absolute rigid fixation would not be necessary for successful function of a TKA.

[Attachment for stereoroentgenogrammetric photography].

Construction and operation principles of an automated stereoradiographic set and a stereoradiocomparator with a portable digital computer are described. This technique, when used for radiotopometric purposes, allows rapid determination of precise quantitative data in clinical conditions about internal structure of organism.

Measurement of the shape of the surface of the back in patients with scoliosis. The standing and forward-bending positions.

In order to determine if the configuration of the trunk is altered when a patient changes from an upright to a forward-bending position, the shape of the surface of the back of fifty-six patients who had adolescent idiopathic scoliosis was recorded, by means of Raster stereophotography, with each patient in three postures: standing erect, bending forward with the hands between the knees (forward bend 1), and bending forward with the hands touching the toes (forward bend 2). The effect of placing one foot on a block to produce a limb-length difference was also studied in the standing position (thirty patients) and in the forward-bending position (eighteen patients). The degree of rotation of the surface of the back and of kyphosis and lordosis of the surface of the trunk was measured from sections in the sagittal plane that were plotted from the computerized measurements of the surface of the back. Qualitatively similar rotation of the surface of the back was found in both the standing position and the forward-bending position. When the patient was in the forward-bending position, the degree of rotation of the surface of the back was minimally changed in the thoracic region but increased in the lumbar region. The amount of rotation of the surface of the back was similar in both forward bending with the hands to the knees and forward bending with the hands to the toes.(ABSTRACT TRUNCATED AT 250 WORDS)

Oral orthopedics and movement of maxillary segments. A roentgen stereophotogrammetric study.

An infant with a complete unilateral cleft of the lip and palate underwent maxillary expansion treatment using an oral orthopedic appliance. Movement of the maxillary bone segments was studied by means of metallic implants and roentgen stereophotogrammetry, and intra-oral changes were recorded by measuring transverse dimensions on casts. Expansion treatment had almost no influence on the positions of the maxillary segments, and movements of the segments showed little agreement with measures on casts. The findings suggest that the appropriate use of the term oral orthopedics and the evaluation of treatment effects would benefit from evaluating the movement of the segments by methods other than measurements of casts.
