The structure, manpower and tasks of the Tuberculosis Programme (ELAT) in Mozambique are explained. The activities are organised on three levels (ministerial, provincial and district). From 1985 to 1990 (1st half) 59,339 cases were detected. Of them, 32,978 were new smear-positive, 17,772 new smear-negative, 5,664 relapses and 2,425 extra-pulmonary cases. Results achieved by 3 regimens used by the Programme varied on average from 55% for the standard, 77% for the short-course and 72% for the retreatment regimen. The Programme is run by paramedical staff and no specific personnel exists in the districts. To upgrade the few personnel involved in the ELAT, supervision and training are considered as backbones of the Programme. The country suffers from a devastating war, which has aggravated the poor transport network, reduced accessibility to remote areas, hampered regular supervision and makes distribution of supplies irregular and very expensive, since drugs, reagents, forms and spare parts have to be sent by air or sea. In spite of all these terrible misfortunes, results achieved show that it is possible to diagnose and treat cases under such condition.
Since 2002 some of the world's largest disease management programs have been launched in Germany--initially for type II diabetes and coronary heart disease--with approximately 5 million eligible insured persons and (in 2005) 2.04 million registered participants. The programs were not piloted prior to their start, and the prescribed statutory evaluation cannot be expected to result in an effectiveness validation. The article describes the development of a study protocol for a randomized controlled trial. It also discusses methodological problems, in particular the definition of primary target criteria, the guarantee of a 'naturalistic' intensity of intervention, and the creation of a control group during nation-wide implementation. Due to lacking support from the stakeholders the proposed randomized trial could not be implemented. Its design, however, allows the required standards for the program evaluation to be specified.
The paper contains an analysis of condition of resources and scope of activity of all Industrial Health Care Units in Poland in the period between 1981-1986. Special attention is given to the problems of medical staff, especially industrial physicians and different specialists working for these Units. The relationship was discussed between the existing resources and the Units. The relationship was discussed between the existing resources and the Units activity. The activity was understood as the number of consultations given both in cases of spontaneous attendance and prophylactic (including periodical) medical examination. Also, disproportions between these two types of consultations were discussed. The results were interpreted in detail, so that the cause-effect relationships could be found. The sense and potential advantages coming from this analysis were presented.

OBJECTIVE: --To analyze the health insurance status of physicians, other health personnel, and insurance industry personnel. DESIGN: --The study was based on data collected by the US Bureau of the Census in the March 1991 Current Population Survey for six groups of workers in health care occupations and three classifications of insurance employees. This survey included 6182 civilian health personnel and 1498 insurance workers under the age of 65 years. RESULTS: --Of civilian health personnel under the age of 65 years, 9% (90% confidence interval [CI], 8.2% to 9.8%) are uninsured, equivalent to 834,000 persons, including 15,000 (90% CI, 5000 to 25,000) physicians. Among insurance workers, 5.1% (90% CI, 3.9% to 6.2%) are uninsured. While 6% (90% CI, 4.2% to 7.9%) of those working in physicians' offices are uninsured, 52.2% (90% CI, 48.2% to 56.3%) receive no employer contribution toward their coverage. More than a fifth of nursing home employees lack insurance coverage, as do nearly a quarter of the 1.868 million health care workers with annual incomes less than $10,000. CONCLUSION: --Nearly a million health care and insurance workers are themselves uninsured and at high risk for being unable to obtain needed care.

Analysing the trends in obtaining specialization in occupational health during last 15 years an evident deterioration of formal qualifications of physicians employed in occupational health service (OHS) was observed. These data were confronted with the needs resulting both from the present model of workers health care as well as with those arising from ratification of ILO Convention No 161 of 1985. It was stated that discrepancy between the need for specialists and the actual possibilities of their training cannot be removed without radical reform of the Polish health care system in general and health service reform in particular.
