The effects of reminiscence on self-health perception and self-esteem of institutionalized elderly in Taiwan.

This study examined the effects of reminiscence on self-health perception and self-esteem of elderly people residing in long-term care facilities. A longitudinal quasi-experimental design was conducted, using two equivalent groups for pre-post test and purposive sampling. Each subject was administered pre- and post-tests at a 4-month interval. Subjects in the experimental group underwent weekly intervention. Forty-eight subjects participated in the study, with 24 in the control group and 24 in the experimental group. The findings indicated that the experimental group demonstrated better self-health perception. In summary, Reminiscence not only supports self-health perception of the elderly but also empower nurses to become proactive in their daily nursing care activities. Efforts to improve health perceptions of the elderly may also reach consequences on their quality of life.

Special anesthetic concerns in mentally handicapped institutionalized patients undergoing gynecological procedures in an outpatient setting.

STUDY OBJECTIVE: To evaluate the anesthesia issues involved in caring for mentally handicapped outpatients. DESIGN: Retrospective chart review. SETTING: University-affiliated outpatient ambulatory center. PATIENTS: Twenty adult patients scheduled for gynecological procedures. INTERVENTIONS: None. MEASUREMENTS: Data collection sheet was used to record patients' age, ASA status, procedure, premedication, intravenous placement, degree of agitation, airway control, induction (method and drugs), intraoperative anesthesia care, postoperative medications, total time in hospital, postanesthesia care unit time and disposition. MAIN RESULTS: Agitation was present in 100% of the patients. A significant number of these patients were ASA III, needing oral or intramuscular sedation (35%) or mask induction prior to placement of an intravenous line. Severely agitated patients had the longest stays in the postanesthesia care unit (PACU). CONCLUSIONS: Agitation was the main reason why 90% of the patients required intubation for relatively minor procedures. Agitation was the main factor leading to prolonged recovery room time.

[Activities programs for institutionalized elderly patients with dementia].

Formal caregivers often play an important role in leading activities programs and arranging appropriate therapeutic activities in institutions. Studies have shown that, when elderly patients with dementia have participated in more structured activities, agitated behaviors have been significantly decreased. Also, structured activities have helped in the performance of ADL, as well as improved quality of sleep, decreased anxiety, improved self-expression and social interaction, enhanced caregiver's morale, and resulted in a more positive caring attitude on the part of caregivers. This article will discuss the categories and contents of activities programs for elderly patients with dementia and can be applied as a reference to assist formal caregivers in their efforts to arrange appropriate activities programs in institutions.

[Anthropometric characterization of an institutionalized elderly population from Caracas, Venezuela].

Most of the anthropometric standards are derived from adult populations but not from older subjects, so their use to evaluate the nutritional status in the elderly may not be the most appropriate. In this sense, the anthropometric characteristics and their relationship to sex and age in a cross-sectional sample of 809 Venezuelan subjects (370 males and 439 females), aged 60 to 102 years old, randomly selected from a elderly institution-alised population were studied. The results indicate that anthropometric variables show different degrees and signs of sexual dimorphism. Males presented higher stature, weight, waist and calf circumferences and higher bone diameters, while females have higher hip and thigh perimeters, and bigger trunk and extremities skinfolds. Independently of the changes observed in these variables with age, the difference spread between the sexes tends to stay and even to increase with age in the height and weight, in hip perimeter and in thigh and calf skinfolds; while, on the contrary, they attenuate in waist perimeter and in triceps, subescapular and suprailiac skinfolds. Males and females appear to be more similar for these variables in the advanced ages. In general, these results could be used as reference elements for similar researches in Venezuela, due to the low number of studies developed in the country in older populations

On the etiology of hearing loss in a population of 155 institutionalized children.

In this paper we present the results of an etiological study in 155 hearing-impaired children and young adults, born between 1960 and 1984, inmates of the IRSA institute (Royal Institute for Deaf and Blind children). Special reference to genetic factors was given. One third of the inmates of this institute were children of immigrants, the majority Italians and North-Africans. The study revealed the following classification for the Belgian pupils: hereditary deafness 22.9%; acquired deafness 44.9% and deafness of unknown origin 32.1%; in the group of immigrants, it was 39.1%, 45.6% and 15.2%, respectively. Six of the 45 patients with a genetic form of deafness presented a distinct syndrome, Usher syndrome (2 patients) being the most frequent. Autosomal dominant inheritance was present more often in the deaf population than in the hearing-impaired population (hearing loss below 80 db), but the difference was not statistically significant. Even after 1980 congenital rubella remained the most frequent exogenous factor causing congenital deafness and, furthermore, we documented for the first time in a retrospective study cytomegalovirus infection as a cause of hearing loss. Atypical retinal abnormalities not related to congenital rubella syndrome were found in 10 inmates by systematic ophthalmological examination: 7 in the unknown group, 1 in the genetic group and 2 in the acquired (meningitis) group.

[Assistance work in Romania. A multidisciplinary study of institutionalized children].

In October/November 1990 a health team worked in an institution for mentally retarded children in Rumania. The team diagnosed 154 children and encountered children with undernourishment, retarded growth, skin infections, injuries and untreated physical deformities. 11 children were in need of acute medical attention. Cerebral palsy and motoric, auditive and visual handicaps were not attended to. The children showed signs of deprivation, anxiety and behavioural maladjustment. They were mentally retarded, but receptive to stimulation. To conclude, the children suffered from physical, psychological, pedagogic and social neglect. The article provides information on the needs for further assistance to children in institutions in Rumania.

[Social needs of the institutionalized elderly in long-term care and their perception of self and others].

In this study social needs of the elderly are examined using the closeness needs of Yura and Walsh (1988). With a little material it is described how the elderly and his relative experience appreciation, acceptance, feeling of togetherness and the needs of closeness of the elderly and his body image and sexual integrity. The study was implemented in a geriatric hospital as half structured theme interviews. Ten elderly and one relative of each elderly were selected to interview. The interviewed elderly experienced that the relatives and nurses were appreciated them being matter-of-fact and helpful. The elderly described their own health better than that of the others in the same room. The closest relatives formed the network of their social support. The elderly had to become adjusted to their incompetence to move and their needs of help. The relatives thought with the same way.
