A radiographic review of bone levels around Calcitek dental implants supporting mandibular over-dentures. Preliminary results at 3 to 6 years.

This retrospective study aimed to measure marginal bone resorption around cylindrical hydroxyapatite-coated implants retaining mandibular overdentures and compares this between two- and four- implant supported overdentures. Twenty three edentulous patients received 66 implants in their anterior mandible and were restored with overdentures. Panoramic radiographs one week post-operatively and at last recall were compared and marginal bone loss calculated. The mean recall interval was 51.22 months. The mean cumulative bone loss was 1.08 mm and the mean annual bone loss 0.263 mm. There was no significant difference between two- and four- implant overdentures. The results were comparable with other reports.

Tissue integration of Al2O3-ceramic dental implants: an experimental study in monkeys.

The effect of two different treatment modalities on the tissue integration of Frialit implants was compared in an experimental study in 10 monkeys. The maxillary central incisors were extracted. In one extraction site, chosen by lot, a Frialit implant was immediately inserted, allowing the implant to penetrate the oral mucosa. In the contralateral extraction site, a modified Frialit implant was placed and allowed to heal under a mucoperiosteal flap. None of the implants used in the study was loaded during the observation periods. After experimental periods of 1, 3, 6, and 8 months, the implants were evaluated and monkeys were sacrificed. In situ and histological evaluation showed no significant difference between the two treatment modalities under nonloading conditions. In both cases the healing around the implants was uneventful and good tissue integration was achieved.

Dental implants following radical oral cancer surgery and adjuvant radiotherapy.

Following radical oral cancer surgery and postoperative adjuvant radiotherapy with a total dose of 60 Gy, 71 IMZ and 150 Brånemark implants were placed in the mandibles and 28 Brånemark implants were placed in the maxillas of 60 patients between 1985 and 1995. Adjunctive hyperbaric oxygen therapy was not used. Osteoradionecrosis of the mandible occurred in two patients (3.4%), and necrosis of soft tissue in the floor of the mouth region occurred in three patients (5.2%). Twenty-one implants (18 in the mandible and 2 in the maxilla) were not osseointegrated when surgically exposed. In subsequent follow-up, 17 mandibular implants and 5 maxillary implants lost their osseointegration. The life table method indicated that 5-year actuarial implant success rates in the irradiated mandible were 77.5% for the IMZ system and 83.6% for the Brånemark system. These differences were not statistically significant. Retrospective analysis indicated that the success of implants in the irradiated mandible is determined after an interval of 18 to 24 months. For a small number of Brånemark implants in the irradiated maxilla, an actuarial success rate of 85.5% was found.

Influence of the prosthetic restoration modality on bone loss around dental implants placed in vascularized iliac bone grafts for mandibular reconstruction.

OBJECTIVES: Is bone loss associated with implants placed in vascularized iliac crest bone grafts for mandibular reconstruction dependent on the prosthetic restoration modalities used? STUDY DESIGN: Eleven vascularized iliac crest bone grafts for mandibular reconstruction were examined after ablative tumor surgery. Forty implants loaded for at least 1 year were observed for up to 6 years. Horizontal and peri-implant bone loss was assessed for 2 groups: 6 patients with implant-supported bridges and 5 patients with implant-retained overdentures. RESULTS: Horizontal bone loss reached a steady state around 2 mm after an observation time of 2 years. Bony pockets had a depth of less than 1 mm. The described pattern was nearly independent of the prosthetic restoration modality used. The implant survival rate was 97.6%. CONCLUSIONS: Vascularized iliac crest bone grafts are safe implant-bearing areas. Implant-retained overdentures can be used with as good a longterm efficacy as the more complicated implant-supported bridges.
