[Sudden arythmogenic death in hospitals after heart surgery].

29 cases of sudden heart death (SHD) which occurred in hospital after heart surgery within 1989-1998 are reported. More than 50% were SHD after aortocoronary shunting, it occurred in stenosing atherosclerosis of more than 3 arteries. An important role in tanatogenesis belongs to ischemic heart disease (IHD) with complications (chronic aneurysm of the left ventricle, postinfarction failure of the mitral valve) and combination of IHD with aortal heart deficiency which is followed by pronounced hypertrophy of the left ventricle which is an important factor of SHD risk. Interventricular defects and Fallot's tetrad are most frequent among heart malformations with SHD after heart surgery. Cardiomegaly, myocardial fibrosis, dilatation of the heart cavities, fibroelastosis of ventricular endocardium, anomalous chordal arrangement in the left ventricle, right ventricular lipomatosis are main factors of SHD of the arrhythmogenic type after heart surgery. Introduction of the new notion "sudden heart death at the hospital stage after heart surgery" is suggested.

[Ultrasound angiography: new perspectives in diagnosis of renal mass lesions].

AIM: To evaluate diagnostic efficacy of advanced ultrasound (US) techniques in early and differential diagnosis of malignant tumors of the kidneys. MATERIAL AND METHODS: A total of 394 patients with renal mass lesions were studied. They had: renal cell carcinoma (n = 96), renal parenchyma pseudotumors (n = 22), lipoma (n = 48), angiomyolipoma (n = 19), renal abscess (n = 2), renal sinus lipomatosis (n = 1), parenchymal cysts (n = 205), retroperitoneal leiomyosarcoma with invasion in the kidney (n = 1). The following examinations were made: gray scale study with tissue harmonic imaging, panoramic scanning, US-angiography (color doppler imaging--CDI, power doppler imaging--PDI, 3D-reconstruction of the kidney vessels, 3D dynamic contrast enhanced angiography (DCEA) in the second harmonic mode with an echo-contrast agent levovist. Pulse Doppler with qualitative and quantitative evaluation of the doppler spectrum was used as an additional method. Verification of the diagnosis was made by conventional angiography, computer tomography, MR-tomography, histomorphological analysis after surgery in 91, 310, 6 and 88 patients, respectively. RESULTS: New US techniques allow differentiation of malignant tumors with renal lipomas and angiomyolipomas, focuses of inflammation, pseudotumors of the renal parenchyma and atypical renal cysts. CDI has revealed big pathologic vessels with arterio-venous and arterio-arterial shunts in renal cancer in 63.5% of the cases. PDI visualized a net of pathologically changed vessels in 74.8% of the cases. 3D-reconstruction in PDI was highly sensitive (90%) in visualization of neoplastic vessels. US-angiography was used for pre- and postoperative monitoring in 18 patients subjected to conservative renal surgery. In 23 patients DCEA with levovist allowed monitoring of all phases of contrast enhancement of renal vessels. Injection of contrast agent revealed tumor microvasculature in an early arterial phase (20-30 seconds). CONCLUSION: US-angiographic techniques in diagnosis of renal cancer increases sensitivity and specificity of the US method compared to conventional B-mode US from 55 and 60% to 98 and 95%, positive and negative predictive value from 58 and 57% to 95 and 98%, respectively.

Lipoma excision.

Lipomas are adipose tumors that are often located in the subcutaneous tissues of the head, neck, shoulders, and back. Lipomas have been identified in all age groups but usually first appear between 40 and 60 years of age. These slow-growing, nearly always benign, tumors usually present as nonpainful, round, mobile masses with a characteristic soft, doughy feel. Rarely, lipomas can be associated with syndromes such as hereditary multiple lipomatosis, adiposis dolorosa, Gardner's syndrome, and Madelung's disease. There are also variants such as angiolipomas, neomorphic lipomas, spindle cell lipomas, and adenolipomas. Most lipomas are best left alone, but rapidly growing or painful lipomas can be treated with a variety of procedures ranging from steroid injections to excision of the tumor. Lipomas must be distinguished from liposarcoma, which can have a similar appearance.

MR imaging of the spine in epidermal nevus syndrome.

We report two cases of epidermal nevus syndrome (ENS) involving the spine. MR imaging of the spine demonstrated intraspinal lipomas in both cases. Abnormal, enhancing, enlarged cervical and lumbosacral nerve roots were present in one patient. Spinal imaging for patients with ENS may help in the diagnosis of subtle intracranial manifestations, as it did in both of our cases. ENS has features similar to those of other neurocutaneous syndromes, such as neurofibromatosis type 1 and encephalocraniocutaneous lipomatosis.

[Histomorphometric indices of the pancreas in intoxication with alcohol and narcotics].

The results of histomorphometric examinations of the pancreas in drug- and alcohol-addiction as well as in acute intoxications are described. They show that, in chronic alcoholic intoxication as compared with opiomania, there are more pronounced interlobular sclerosis and lipomatosis, which is manifested by an enlarged middle lobe of the stroma (containing fat and connective tissue fibers), by a higher density of islands of Langerhans and by their higher sectional area. Autolysis and edema of the stroma are more severe in an acute intoxication with drugs than in other cases of fatal outcome. A possibility is demonstrated to differentiate between various types of the so-called long-term exogenous intoxication and to discriminate between different types of thanatogenesis typical of various cases of acute intoxications. Data are presented that demonstrate the advantage of the quantitative analysis of histological preparations over the qualitative one.

[Role of ultrasonography in the follow-up of surgically treated patients. Upper urinary tract. Complications after conservative surgery].

The Authors describe the purposes of ultrasonographic follow-up after both conservative surgery and endoscopic or endourological operations of the upper urinary tract. In these cases ultrasonography evaluates the results of normal surgery, the presence of early or late complications, and of iatrogenic lesions. The echo-patterns of deformations in the outline of the kidneys, of hematic, urinary or lymphatic collections (both retroperitoneal and intraperitoneal), of fistulas, of sclero-lipomatosis and of post-cicatrization hydronephrosis are described. The importance of evaluating the degree of obstruction by a dynamic ultrasonographic study with furosemide-test is emphasized. The ultrasonographic monitoring of urethral and pyelostomy setting of catheters is also described. The echo-patterns of retroperitoneal and intraperitoneal fluid collections and renal hematomas, in relation also the kind of fluid contained are widely discussed.

[Sciatica or herniated disk].

Lumbar radiculalgia may be due, beside the disc-nerve root conflict, to stenosis of osteoarticular canals, wide dural sac, epidural lipomatosis, segmental arachnoiditis, malignant or benign tumours and meningoradiculitis. Extraspinal truncular or radicular sciatica is usually due to compression by an expansive process. Some types of pain referred from articular structures may mimic sciatica.

Nevus psiloliparus: report of two nonsyndromic cases.

A diagnosis of nevus psiloliparus was made both clinically and histopathologically in two otherwise healthy girls, one being 4 years and the other one being 1 year old. A congenital hairless patch with a round or oblong shape and a soft surface was noted on the scalp. In one case the lesion was yellowish and flat, whereas in the other case it was skin colored and somewhat elevated. In both cases, histopathological examination showed the absence of mature hair follicles and the presence of undeveloped follicular structures as well as orphaned arrector pili muscles in the dermis. As a new histopathological feature of this type of nevus, we found arrested anlagen of hair bulbs in both cases. The fatty tissue was abundant and also involved, in the form of aberrant lobules, the lower portion of the dermis. Clinical examination did not show any associated extracutaneous abnormality, and during a follow-up period of 2 years in either case, respectively, the children developed without any complication. Although neurological abnormalities could not be excluded by imaging techniques, such extracutaneous involvement is highly unlikely because the nevi psilolipari were of rather limited size. When clinicians and dermatohistopathologists have become familiar with this new entity, they will most likely recognize it as a nonsyndromic skin disorder more often than as a cutaneous sign of encephalocraniocutaneous lipomatosis.

[A new view of anatomic substrate, electrophysiological mechanisms of atrial fibrillation, and results of interventional treatment].

Literature data on anatomic and electrophysiological properties of pulmonary veins (PVs) are very scarce. The study included an investigation of 20 human autopsy hearts with and without cardiovascular pathology. In addition, biopsy samples from the right superior PV were taken from 5 patients with atrial fibrillation (AF) for ultrastructural study. The investigation found myocardial sleeves (MS) in 86.3% of cases, an additional PV between right PVs--in 15%, a collector type of superior and inferior PV--in 25% of cases. MS were situated on the adventitial side of PVs; along the whole length of the PVs the MS myocyte fascicles ran in different directions. Local changes, such as productive myocarditis, fibrosis and fibro-lipomatosis, were observed in MS of PV of patients with AF. 106 patients (80 of whom were male; middle age 42.9 +/- 12.5 years) with symptomatic AF underwent clinical examination. Electrophysiological features of the PVs were evaluated in 30 patients. Effective and functional refractory periods of PVs were found to be shorter than those of the left atrium and non-arrhythmogenic PVs. Radiofrequency ablation (RA) with application of electrophysiological criteria proved to be effective in all patients with frequent ectopy from PVs. The use of electroanatomical mapping allowed successful RA in 82% of patients with persistent and chronic AF. The study demonstrates that the changes in MS of PV, discovered by means of light and electron microscopy, as well as their complex three-dimensional morphological structure may cause and maintain AF. Very distinct electrophysiological features, found in AF patients, probably play a major role in arrhythmogenicity.

[Clinicomorphological characteristics of the soft palate in patients with chronic ronchopathy].

Clinicomorphological study of soft palate structures in patients with chronic ronchopathy defines the following pathological processes: hypertrophy of the soft palate resultant from a marked stromal edema, overproduction of collagenic fibers, stromal lipomatosis, disturbance of lymphatic circulation, damage to myocytes affecting impairment of contractile and tonic processes in the soft palatine muscles. The above morphological alterations may be caused by previous inflammation in soft palate tissue and the tonsils. Hypertrophic, baggy, atonic soft palate narrows the airways at the level of the soft palate.

[Dercum's disease: a case report].

A case of Dercum's disease in a 51-year-old obese woman with a history of brain vasculitis, and painful subcutaneous multiple lipomas is described. This disease, included in the category of rare diseases by the World Health Organization, first described in 1892 by Francis Xavier Dercum, is characterized by its prevalence among women, its familiarity, by the presence of multiple painful subcutaneous lipomas and its association with obesity, hypercholesterolemia and asthenia. The disease has to be differentiated from Madelung syndrome, the multiple familiar lipomatosis and Proteus' syndrome. The quality of life of the patients is often poor and since treatments so far have not been conclusive a better knowledge of the pathogenesis of the disease is desirable.

[Pathomorphology of the mixed form of mycoviscidosis in an adult patient].

The paper describes one case of the mixed form of mucoviscidosis, with involvement of the lung (chronic obstructive bronchitis, bronchiectasis, emphysema, pneumosclerosis), liver (cirrhosis), pancreas (lipomatosis with islet atrophy), ileum (enteritis). There were the following complications: cor pulmonale, esophageal varicose veins, ascitis, splenomegaly, secondary diffuse renal amyloidosis of the kidney, adrenals, and spleen, and brain edema. Due to modern therapy the patient reached the age of 25 years, which is a rarity.

[Spontaneous urine extravasation during intravenous urography. Apropos of 21 cases. Review of the literature].

Twenty one cases of spontaneous urine extravasation during an intravenous urography have been analysed in order to determinate the radiological signs and the causes of this rare phenomenon. Related to the increased pressure of the excretory tract, these extravasations usually disappear without complications. No specific treatment is required except the treatment of the obstacle due generally to calculi. No infection nor development of retroperitoneal fibrosis nor sinusal lipomatosis were observed in the patients we studied.

[The adipose tissue of the pelvis in imaging technics].

Fat is a fundamental contrast agent in pelvic imaging. The features of fat are typical on CT and MRI but vary on ultrasonography according to the physical characteristics and histology. The study of pelvic fat is of great importance in pelvic exploration because fat outlines both the normal structures and their borders. Involvement or masking of the pelvic fat is a good marker of carcinologic spread. Changes in pelvic fat may also be observe in inflammatory diseases. Specific diseases of fatty pelvic connective tissue such as lipomatosis and liposarcomas, are classically described. Teratomas with a fatty component derived from pelvic structures (ovaries) are easily diagnosed using imaging methods.

[Thyrolipoma. A case report and review of the literature].

The authors report a case of thyrolipoma in a 50 year old woman, presenting with nodular goitre and tracheal compression. Microscopically, the coexistence of adipocytes and thyroid vesicles within the thyroid nodule is characteristic of this rare tumor. The relationship between this tumor and diffuse thyroid lipomatosis and amyloid goitre are discussed.
